Abstract

Pendahuluan:Penurunan aktivitas fisik dialami oleh 60-70% pasien yang menjalani hemodialisis dan berdampak pada penurunan kebugaran tubuh dan kualitas hidup pasien. Tujuan penelitian ini adalah menjelaskan hubungan kualitas tidur dengan aktivitas fisik pasien yang menjalani hemodialisis di waktu senggang.Metode:Desain penelitian ini adalah deskriptif korelasional dengan 103 pasien hemodialisis di RSUD Dr. Soedirman Kab. Kebumen yang dipilih menggunakan teknik purposive sampling. Data diperoleh dari kuisioner Pittsburgh Quality Sleep Index (PSQI)dan International Physical Activity Questionnare (IPAQ). Variabel independen dalam penelitian yaitu kualitas tidur, variabel dependen yaitu aktivitas fisik.Hasil:Terdapat hubungan antara kualitas tidur dengan aktivitas fisik pasien yang menjalani hemodialisis di waktu senggang. Analisis menggunakan uji statistik Spearman rho menunjukkan nilai r=pengetahuan dengan aktivitas fisik pasien sebesar -0,664 dengan p value sebesar 0,000. Simpulan:Kualitas tidur pada pasien dengan penyakit ginjal kronis yang menjalani hemodialisis dapat mempengaruhi aktivitas keseharian individu. Intervensi yang tepat dalam meningkatkan kualitas tidur perlu dilakukan agar pasien yang mejalani hemodialisis memiliki cukup energi untuk melakukan aktivitas fisik.

Abstract

Introduction: Burnout syndrome is still a problem related to nurses' psychology, especially during the current COVID-19 pandemic. Burnout syndrome can be influenced by interpersonal and environmental factors of a person. The purpose of this study was to analyze the relationship between personality, workload and appreciation of burnout syndrome in nurses who treat COVID-19 patients at the Indrapura Field Hospital, Surabaya. Methods: This study uses a descriptive analytic design with a cross sectional approach. The number of samples was 42 respondents who were taken using a non-probability sampling technique, namely purposive sampling. Analysis using Spearmen Rho test. Result: The results showed that there was a relationship between workload (p = 0.034; r = 0.327) and rewards (p = 0.039; r = -0.320) with burnout syndrome in nurses treating COVID-19 patients at the Indrapura Field Hospital Surabaya. There was no relationship between personality (p=0.244;r=-0.184) and burnout syndrome in nurses who treated COVID-19 patients at the Indrapura Field Hospital, Surabaya. Conclusion: Burnout syndrome in nurses who treat COVID-19 patients at the Indrapura Field Hospital Surabaya is related to workload and rewards. Personality factors with positive attitudes are not necessarily able to prevent someone from experiencing burnout syndrome. So that this research can be used as an illustration for nurses during the current COVID-19 pandemic and situations in the future.

Abstract

Objective: This study aimed to develop a behavior model in controlling blood pressure of patients with hypertension by integrating the theory of Health Belief Model and adaption model theory by Roy. Method: This was a cross-sectional study conducted in January 2020. The population in this study was hypertension patients at the Community Health Center. The sampling technique used in this study was consecutive sampling. The instrument used in this study was a questionnaire. The data were analyzed by Partial Least Squares. Result: Modification factor, cues to action, individual beliefs and cognator significantly affected the behavior for blood pressure control with t-statistic value >1.96. Modification factors affected individual beliefs (t=10,648). Conclusion: This behavioral model can assist nursing in developing strategies to improve blood pressure control behavior in hypertensive patients. The behavior of the hypertensive patients was possibly improved by the involvement of the health worker by routinely educating the patients and their family related to hypertension and how to control blood pressure and also related to stress and how to manage it as well as with support from the family in self-care management and lifestyle modification.

Abstract

Diabetes Melitus (DM) memerlukan pemahaman secara komprehensif dan konsisten dalam untuk menghindari komplikasi penyakit. Tujuan dari kegiatan pengabdian kepada masyarakat (PKM) ini adalah untuk memberikan edukasi dan pelatihan Peer Health Coaching dalam upaya meningkatkan pengetahuan pada kader, keluarga dan pasien DM. PKM ini menggunakan pendekatan Community Based Research (CBR) dilakukan edukasi dan pelatihan dengan menerapkan peer health coaching di Kalidami, Kelurahan Mojo, Kecamatan Gubeng, Surabaya, Jawa Timur. Evaluasi PKM dengan membandingkan pengetahuan tentang penatalaksanaan DM dari Kuesioner DKQ-24 (Diabetes Knowledge Questionnaire). Peserta yang mengikuti kegiatan ini sebanyak 32 orang dimana pengukuran tingkat pengetahuan didapatkan p<0.001 (p<0,05) sehingga terdapat perbedaan yang bermakna antara sebelum dan sesudah diberikan pendidikan dan pelatihan dalam hal peningkatan pengetahuan terkait penatalaksanaan DM. Pendidikan dan pelatihan peer health coaching mampu meningkatkan pengetahuan terkait manajemen penyakit, pasien DM dan keluarganya untuk mampu menjalankan efikasi diri yang baik dalam manajemen DM.

Abstract

Stroke merupakan penyebab kematian nomor.dua serta penyebab kecacatan utama diseluruh dunia. Pengenalan dan pengambilan keputusan terhadap Stroke dapat ditingkatkan dengan menggunakan BEFAST (Balance, Eyes, Face, Arm, Speech, Time) sebagai instrumen penilaian awal pada tahap prehospital. Tujuan penelitian ini adalah untuk mengetahui efektivitas penggunaan BEFAST sebagai deteksi dini stroke di prehospital. Desain penelitian ini menggunakan Systematic Review, yang diambil dari PubMed, Proquest, Science Direct, dan Google Scholar dari tahun 2019 sampai dengan 2024. Pencarian artikel inimenggunakan kata kunci Stroke OR cerebra arterial disease AND prehospital detection AND BEFAST. Desain penelitian menggunakan Preferred Reporting Items for Systemic Review and Meta-Analysis (PRISMA) Guidelines. Dari 217 artikel yang ditemukan, 5 artikel yang di review dalam penilitian ini. 5 artikel tersebut (100%) mendukung penggunaan BEFAST sebagai pengenalan awal stroke prehospital dengan sensitivitas dan spesifisitas yang tinggi. Penggunaan BEFAST merupakan aspek objektif untuk deteksi dini stroke prehospital, persiapan aktivasi kode stroke di unit gawat darurat, sehingga dapat mengurangi risiko kematian dan kecacatan.

Abstract

The application of the ERAS protocol perioperatively is considered to be able to improve the quality of perioperative care by reducing the problems caused by surgical procedures. The aim of this systematic review was to synthesize the evidence for the use of ERAS in craniotomy surgery patients. Five databases spanning 2017-2022 were used in this review, namely SAGE, Science Direct, Web of Science, PubMed and Scopus. Only articles using the RCT and CT designs were included in the review, the quality assessment of the articles used the JBI critical appraisal tools, the selection and extraction of articles were visualized using the PRISMA guidelines. There were 99 articles found, but only 8 articles met the inclusion criteria with a total of 1146 respondents. The main result of this review is to report the benefits of implementing the ERAS protocol on length of stay, cost of care, pain, patient satisfaction and perioperative complications. This review concludes that the implementation of the ERAS protocol can significantly help improve the recovery process of post-operative craniotomy patients. The results of studies regarding indicators of patient satisfaction in the pre-operative to post-operative process are still minimal, so that it can be considered to conduct studies on this matter in the future.Keywords: enhanced recovery after surgery; ERAS; length of stay; perioperative craniotomy ABSTRAK Penerapan protokol ERAS pada perioperatif dinilai dapat meningkatkan kualitas perawatan perioperatif dengan mereduksi masalah yang ditimbulkan oleh prosedur operasi. Tujuan dari systematic review ini adalah untuk mensintesis bukti-bukti penggunaan ERAS pada pasien bedah kraniotomi. Lima database dengan rentang tahun 2017-2022 digunakan dalam tinjauan ini, yakni SAGE, Science Direct, Web of Science, PubMed dan Scopus. Hanya artikel yang menggunakan desain RCT dan CT saja yang diikutkan ke dalam tinjauan, penilaian kualitas artikel menggunakan tools critical appraisal JBI, seleksi dan ekstraksi artikel divisualisasikan menggunakan pedoman PRISMA. Ditemukan 99 artikel, namun hanya terdapat 8 artikel yang memenuhi kiteria inklusi dengan jumlah total responden 1146. Hasil utama dari tinjauan ini adalah melaporkan manfaat penerapan protokol ERAS terhadap length of stay, biaya perawatan, nyeri, kepuasan pasien dan komplikasi perioperatif. Tinjauan ini menyimpulkan bahwa penerapan protokol ERAS secara signifikan dapat membantu meningkatkan proses pemulihan pasien post-operatif kraniotomi. Hasil studi mengenai indikator kepuasan pasien pada proses pre-operatif sampai post-operatif masih minimal, sehingga dapat dipertimbangkan untuk melakukan studi akan hal ini di masa mendatang.Kata kunci: enhanced recovery after surgery; ERAS; lama rawat inap; perioperatif kraniotomi

Abstract

Background: Stroke-associated Pneumonia (SAP) is one of the complications that occur in stroke patients and is associated with worse clinical outcomes. Risk factors for SAP and its impact on discharge status remain unclear.Purpose: This study aimed to determine the factors influencing the incidence of stroke-related pneumonia in the hospital and the patient’s discharge status.Methods: A case-control study was conducted at a private hospital in Surabaya from January 2021 to 2022. Medical record data were reviewed to identify stroke patients with suspected or confirmed pneumonia, after which risk factors (age, gender, stroke type, length of stay, stroke frequency, dysphagia, diabetes mellitus, hypertension, COVID-19, and discharge status) associated with the occurrence of SAP were examined. We excluded diagnoses of diseases with clinical manifestations such as tuberculosis-like pneumonia, lung tumor, pulmonary edema, pulmonary embolism, pulmonary atelectasis, and incomplete or missing medical record data. Data analysis used the Chi-square test to identify the risk factors for stroke-associated pneumonia.Results: One hundred sixty-three stroke patients treated in all inpatient rooms were diagnosed with pneumonia; 52.8% of them had confirmed pneumonia, while the rest were still suspected of pneumonia. Most pneumonia occurs in ischemic stroke patients (93.9%), first-time stroke patients (87.1%), experienced dysphagia (55.2%), length of stay less than 7 days (55.2%), hypertension (55.2%), diabetes mellitus (57.1%), and discharged home (84%). We also found as much as 12.9% inpatient mortality due to SAP. Dysphagia, DM, and COVID-19 were significantly correlated with the incidence of stroke-associated pneumonia (p=0.000).Conclusion: Stroke-associated pneumonia is a substantial medical complication among stroke patients affected by dysphagia, DM, and COVID-19. Screening for dysphagia is necessary for nurses to prevent SAP in stroke patients.

Abstract

The Early Warning Score (EWS) is a crucial tool for detecting early signs of clinical deterioration in hemodialysis (HD) patients. However, previous research has primarily focused on the intra-dialysis phase, necessitating a systematic review to explore the effectiveness of EWS across all phases (pre, intra, and post-dialysis). Objective: This study aims to assess the effectiveness of EWS in detecting complications in hemodialysis patients across these three phases and to evaluate its impact on morbidity, mortality, length of hospital stays, and readmission rates. Methods: This systematic review was conducted following PRISMA guidelines. Literature searches were performed in databases such as PubMed, Scopus, ScienceDirect, ProQuest, and Google Scholar. The keywords used in the search were “Early Warning Score” OR “EWS” AND “Hemodialysis” OR “Renal Dialysis” OR “Dialysis” AND “Pre-dialysis” OR “Intradialysis” OR “Post-dialysis” and can utilize Boolean logic (AND, OR, or NOT) to maximize search results. The screening of articles with respect to limitations including year 2016 - 2024. Results: Out of 1,246 identified articles, 15 studies met the inclusion criteria. The findings indicate that EWS is effective in detecting complications across all hemodialysis phases, with significant improvements in clinical management and reductions in morbidity and mortality rates. Conclusion: The comprehensive application of EWS in the pre, intra, and post-dialysis phases can enhance the safety of hemodialysis patients.

Abstract

Risiko bencana alam yang dapat merusak infrastruktur, mengganggu perekonomian, dan mengancam keselamatan sering kali diperburuk oleh kurangnya kapasitas masyarakat serta minimnya kesadaran mitigasi. Upaya peningkatan kapasitas melalui edukasi dan pelatihan partisipatif diperlukan untuk menciptakan masyarakat yang lebih siap dan tangguh dalam menghadapi bencana. Kegiatan ini diharapkan dapat meningkatkan kemampuan teknis dan kesadaran relawan, memperkuat jejaring lintas sektor, serta menyusun panduan operasional untuk meningkatkan kesiapsiagaan dalam menghadapi berbagai ancaman bencana. Kegiatan dilakukan secara partisipatif, mencakup persiapan, pelaksanaan, simulasi, dan evaluasi. Materi pelatihan meliputi mitigasi bencana, bantuan hidup dasar, evakuasi, dan dukungan psikologis. Evaluasi menunjukkan peningkatan signifikan dalam pengetahuan dan keterampilan relawan (p<0,05), termasuk pemahaman psychological first aid untuk mengelola trauma pascabencana. Abstract

Based on Calista Roy's adaptation theory and self-determination theory, this study aims to develop a psychosocial adaptation model for hemodialysis patients, affecting their quality of life. The method used was a non-probability sampling technique through purposive sampling, with a sample size of 119 hemodialysis patients at the Bina Husada Cipta Canthi Clinic (BHCC) and the Semesta Mandiri Clinic (KSM), calculated using the rule of thumb. Statistical analysis used SEM-PLS. The results showed that stimulus factors, interdependence, self-concept-resilience, and role function influenced quality of life. At the same time, basic psychological needs did not affect the quality of life of hemodialysis patients. This study concludes that the psychosocial adaptation model for hemodialysis patients' quality of life is valuable. Therefore, dialysis unit staff are expected to use this psychosocial adaptation development guideline to improve the quality of comprehensive hemodialysis unit services for hemodialysis patients.   Keywords: Adaptation, Hemodialysis, Quality of Life, Psychological, Social
