Abstract

Introduction: Acute Decompensated Heart Failure (ADHF) is a sudden worsening of heart failure symptoms. In 2019, WHO reported 17.9 million deaths worldwide from heart disease, with ADHF affecting 1.5% of Indonesia’s population. Shortness of breath is the most common symptom, occurring in 80% of hospitalized ADHF patients. Deep Diaphragmatic Breathing can help improve breathing by strengthening the diaphragm. This case study aimed to evaluate the effectiveness of Deep Diaphragmatic Breathing in improving respiratory status in patients with Acute Decompensated Heart Failure (ADHF).   Objective: This research used a descriptive case study design with a single case study approach 7 days, assessing and evaluating the effects of Deep Diaphragmatic Breathing exercises to reduces ineffective breathing patterns in Acute Decompensated Heart Failure. Data collection through interviews, observation, measurement, physical examination, and medical records.  Case: A 46-year-old patient with Acute Decompensated Heart Failure (ADHF) was admitted with shortness of breath, nausea, vomiting, and loss of appetite for one day. On examination, the patient was conscious (GCS 456), BP 95/66 mmHg, temperature 36.2°C, pulse 68/min, RR 22/min, SpO₂ 98% (with nasal cannula), weight 60 kg, and height 167 cm. The main complaint was dyspnea, especially when lying down, with wheezing and irregular breathing. Blood test (B2) showed weak palpable pulse, warm moist extremities, and transient chest pain. Nursing care included respiratory monitoring, deep diaphragmatic breathing, and positioning in Fowler or semi-Fowler. By day 6, breathing improved (RR 18/min, SpO₂ 99%) without oxygen support. Conclusion: Deep Diaphragmatic Breathing reduced ineffective breathing patterns in ADHF patients. Future research should involve larger sample sizes and explore long-term outcomes to strengthen evidence for its clinical application.

Abstract

Introduction: Autism is a disruption verbal, nonverbal, and social interaction growth in children. Children with autism spectrum disorder have a different parenting program with normal children. A mother who has autism spectrum disorder children need for more space up her time to treat their children right, and sometimes they will have their stress moments because of what they think about their autism spectrum disorder children. This study aims to explain the interventions to enhace the coping of parents of children with autism.Method: The literature was searched using electronic database: Scopus, CINAHL, Science Direct, Sage, Google Schoolar with inclusion criteria include: parents of children with Autism Spectrum Disorder, articles published in 2011-2021 using experimental studies in English and Indonesian. Ten articles obtained for review.Result: Ten articels that fulfill a searching requirement and chosen to be analyzed. From ten articels there are divided into two interventions on it, Cognitive Based Intervention wich are Parent Education Program, Coping Effectiveness Training, Mindfulness-Based Interventions, Problem-Solving Skills Training, and Psychological Based Intervention wich are Virtual Resilience Intervention.Conclusion: This research shows that some interventions or programs that have been examined effectively can increase coping at parents who has autism spectrum disorder children. The intervention that is considered the most effective and often used is the parent education program.

Abstract

Background: Prediabetes is a condition that can be controlled and managed to prevent the occurrence of type 2 diabetes mellitus (T2DM). This condition can occur at all ages, especially in young adults. However, little is known about what factors increase the risk of prediabetes in young adults in East Java, Indonesia.Purpose: This study aimed to estimate the prevalence and the influential risk factors of prediabetes among young adults in East Java, Indonesia.Methods: This study used a cross-sectional design. The purposive sampling technique was used to recruit young adults in East Java Province, Indonesia. International physical activity questionnaire short-form version questionnaire (IPAQ-SF) and physical indicators for anthropometry were used to obtain data on sociodemographic characteristics, prediabetes knowledge, and physical activity. In addition, blood pressure, impaired fasting glucose (IFG), and body mass index (BMI) were measured. Multivariable logistic regression was employed in the analysis to determine risk factors associated with prediabetes.Results: There were 126 participants recruited, with 69 (54.8%) having prediabetes based on IFG levels. Age (p=0.035), regular exercise (p=0.015), activity level (p=0.026), body weight (p=<0.001), waist circumference (p=0.002), BMI (p=<0.001) and obesity (p=<0.001) were significant factors associated with prediabetes.Conclusion: The high prevalence of prediabetes in young adults is associated with age, routine exercise, activity level, body weight, waist circumference, BMI and obesity. It is crucial to implement strategies, such as regular IFG testing, to identify young adults with these risk factors for prediabetes screening.

Abstract

Diabetes Melitus (DM) memerlukan pemahaman secara komprehensif dan konsisten karena sebagian besar mengalami kondisi distress diabetes, manajemen diri yang buruk dan kualitas hidup yang rendah. Tujuan dari kegiatan pengabdian kepada masyarakat (PKM) ini adalah untuk memberikan edukasi dan pelatihan mindfulness pernafasan dan suportif edukatif dapat menurunkan risiko distres diabetes Pasien Diabetes Mellitus dan meningkatkan pengetahuan kader, keluarga dan pasien DM. PKM ini menggunakan pendekatan Community Based Research (CBR) dilakukan edukasi dan pelatihan dengan menerapkan peer health coaching di RW 05, Kelurahan Mojo, Kecamatan Gubeng, Surabaya, Jawa Timur. Evaluasi PKM dengan membandingkan pengetahuan dan skrining risiko distress diabetes. Peserta yang mengikuti kegiatan ini sebanyak 45 orang dimana pengukuran tingkat pengetahuan diperoleh p<0.001 (p<0,05) sehingga terdapat perbedaan yang bermakna antara sebelum dan sesudah diberikan pendidikan dan pelatihan dalam hal peningkatan pengetahuan terkait penatalaksanaan DM. Pendidikan dan pelatihan mindfulness pernafasan dan suportif edukatif mampu meningkatkan pengetahuan terkait penyakit diabetes.

Abstract

Diabetes Mellitus is a chronic disease that has bad complications for the sufferer's health, so it requires non-pharmacological treatment. The aim of this study is to explain the effectiveness of non-pharmacological therapies such as heliotherapy and walking therapy in reducing random glucose levels (RBS) in patients with type 2 diabetes mellitus (DM). This study used a Pre-Post Test With Two Group Design research design. The population for this study consisted of 32 respondents, divided into two groups: the heliotherapy intervention group and the walking intervention group, each with 16 respondents. The research subjects were selected based on the criteria for diabetes mellitus patients. The independent variables were heliotherapy and walking therapy, while the dependent variable was the reduction in RBS levels. Research instruments included heliotherapy standard operating procedures (SOP), walking therapy SOP, Glucometer, and observation sheets. Data analysis was performed using paired t-tests and independent t-tests. The statistical test results showed a p-value of 0.353 (α>0.05), indicating that there is no significant difference in effectiveness between heliotherapy and walking therapy in reducing RBS levels in patients with type 2 diabetes mellitus (DM). It can be concluded that the reduction in RBS levels is higher with walking therapy compared to heliotherapy in patients with type 2 DM.

Abstract

Diabetic retinopathy (DR) represents a leading cause of blindness worldwide, early detection is critical to prevent vision loss. However, traditional screening methods, which rely on human experts, prove to be costly and time-consuming. The systematic review aims to assess the validity of artificial intelligence (AI) as a screening tool for detecting DR among diabetic patients. A systematic literature search was performed of the following databases: PubMed, Scopus, CINAHL, and Web of Science. The last date of our search was January 31, 2024. We included all observational studies, including cohort, case–control and cross-sectional studies and evaluated their quality using the Joanna Briggs Institute tool. We included diagnostic test accuracy studies evaluating the use of AI algorithms for DR screening in patients with diabetes. Studies were excluded if they exclusively assessed diagnostic accuracy for DR that did not use AI algorithms as a diagnostic tool and studies with incomplete or inaccessible data. Thirteen studies with sample sizes ranging from 69 to 1378 participants, reported good sensitivity of AI for detecting visually threatening DR (VTDR). The lowest sensitivity was 89.2%, and the highest was 100%. In terms of specificity, Any DR exhibited higher specificity compared to RDR and VTDR, ranging from 80.2% to 100%. The sensitivity and specificity of the Artificial Intelligence (AI)-based tools available for DR screening was considered acceptable, especially in detecting VTDR and Any DR, was regarded as good. These results implied the potential usefulness of these tools for DR screening in settings with limited resources. However, further high-quality comparative studies were deemed necessary to evaluate their effectiveness in real-world clinical settings.

Abstract

Objective: This study aimed to develop a behavior model in controlling blood pressure of patients with hypertension by integrating the theory of Health Belief Model and adaption model theory by Roy. Method: This was a cross-sectional study conducted in January 2020. The population in this study was hypertension patients at the Community Health Center. The sampling technique used in this study was consecutive sampling. The instrument used in this study was a questionnaire. The data were analyzed by Partial Least Squares. Result: Modification factor, cues to action, individual beliefs and cognator significantly affected the behavior for blood pressure control with t-statistic value >1.96. Modification factors affected individual beliefs (t=10,648). Conclusion: This behavioral model can assist nursing in developing strategies to improve blood pressure control behavior in hypertensive patients. The behavior of the hypertensive patients was possibly improved by the involvement of the health worker by routinely educating the patients and their family related to hypertension and how to control blood pressure and also related to stress and how to manage it as well as with support from the family in self-care management and lifestyle modification.

Abstract

Introduction: Burnout syndrome is still a problem related to nurses' psychology, especially during the current COVID-19 pandemic. Burnout syndrome can be influenced by interpersonal and environmental factors of a person. The purpose of this study was to analyze the relationship between personality, workload and appreciation of burnout syndrome in nurses who treat COVID-19 patients at the Indrapura Field Hospital, Surabaya. Methods: This study uses a descriptive analytic design with a cross sectional approach. The number of samples was 42 respondents who were taken using a non-probability sampling technique, namely purposive sampling. Analysis using Spearmen Rho test. Result: The results showed that there was a relationship between workload (p = 0.034; r = 0.327) and rewards (p = 0.039; r = -0.320) with burnout syndrome in nurses treating COVID-19 patients at the Indrapura Field Hospital Surabaya. There was no relationship between personality (p=0.244;r=-0.184) and burnout syndrome in nurses who treated COVID-19 patients at the Indrapura Field Hospital, Surabaya. Conclusion: Burnout syndrome in nurses who treat COVID-19 patients at the Indrapura Field Hospital Surabaya is related to workload and rewards. Personality factors with positive attitudes are not necessarily able to prevent someone from experiencing burnout syndrome. So that this research can be used as an illustration for nurses during the current COVID-19 pandemic and situations in the future.

Abstract

Urological surgery is a medical procedure in treating disorders of the urinary tract, reproductive organs, and other urogenital systems that have the most common risk of complications in the form of urinary incontinence. This systematic review aimed to assess the effectiveness of pelvic floor exercises are in reducing the occurrence of urinary incontinence among patients who have undergone urological surgery. Four databases, including Scopus, PubMed, ProQuest and Sciencedirect, were reviewed for relevant articles published from 2020 to 2025. The literature search was conducted using keywords such as "Pelvic-Floor-Exercises AND Urinary-Incontinence AND Post-Urology-Surgery". Guidelines from the Joanna Briggs Institute were used to assess the quality of journals reviewed. Articles were analyzed by extracting data in the form of country, type of study, sample, age, gender, intervention (type and duration), instrument, and outcome. Seven randomised controlled clinical trials (RCTs) involving 422 articles were included. All studies reported a significant decrease in urinary incontinence after Kegel exercises (p 0.05). Research indicates that engaging in pelvic floor exercises can enhance the strength of pelvic floor muscles, leading to better bladder control and a decrease in the occurrence of urinary incontinence. Pelvic Floor Exercises can be part of a urological post-operative recovery program that supports the patient's overall recovery and quality of life. Nurses can teach and motivate patients to perform pelvic floor exercises according to the patient's progress and condition. The results lay a strong groundwork for application in clinical settings, and further studies are necessary to establish a standardized protocol and investigate its long-term effects.

Abstract

ABSTRACT Introduction:. Hemodialysis patients with end-stage renal disease (ESRD) often experience low self-efficacy and poor adherence to treatment regimens, which negatively affect their health outcomes and quality of life. Conventional education methods have shown limited effectiveness in sustaining behavioral change. Nurse-led mobile health (mHealth) interventions provide an innovative approach that combines nursing expertise, behavioral theory, and digital technology to enhance self-efficacy and long-term adherence. Methods: This systematic review followed the PRISMA 2020 guidelines and included studies published between 2019 and 2025 from four databases: Scopus, PubMed, ProQuest, and Web of Science. Eligible studies examined nurse-led mHealth interventions aimed at improving self-efficacy and adherence among adult hemodialysis patients. The methodological quality of the included studies was evaluated using the JBI critical appraisal tools. Results: Fourteen studies met the inclusion criteria, comprising randomized controlled trials and quasi-experimental designs. Nurse-led mHealth interventions were delivered through mobile applications, telehealth platforms, and instant messaging systems, demonstrating consistent improvements in self-efficacy, adherence, and clinical indicators. Interventions guided by the FITT (Frequency, Intensity, Time, Type) framework and behavioral theories such as the Health Belief Model, Self-Efficacy Theory, and Transtheoretical Model achieved the most sustainable outcomes. Conclusions: Nurse-led mHealth interventions effectively improve self-efficacy and adherence among hemodialysis patients. Integrating behavioral theory, optimized digital dosage, and nursing leadership represents a scalable strategy to strengthen chronic disease management and patient empowerment in contemporary nursing practice.
