Abstract

Introduction: Individuals with Chronic Kidney Disease (CKD) may experience symptoms of uremic syndrome. Intradialytic stretching exercises have been proposed as a method to increase activity levels while reducing uremic symptoms. Muscle cramps are a common complication of hemodialysis treatment and often lead to early termination of sessions, therefore, timely intervention to address muscle cramps in hemodialysis patients is a critical priority. Objective: To systematically analyse the effectiveness of intradialytic stretching exercises on muscle cramps among individuals undergoing hemodialysis Methods: A systematic review was conducted through five major databases: Scopus, PubMed, Science Direct, ProQuest, and Wiley, for the last five years publication (2020 to 2025). Eligible studies included those that implemented intradialytic stretching exercises interventions focused on muscle cramps for individuals undergoing hemodialysis. Data extracted included study characteristics, sample demographics, design, and measured outcomes. Study quality was assessed using the Joanna Briggs Institute (JBI) critical appraisal tool, and findings were synthesized through narrative analysis. Result: Eleven studies (n=11) that obtained JBI quality assessment scores ranging from 77% to 92% were included in the final analysis, involving 809 participants from 7 countries, with intervention durations ranging from 4 to 24 weeks. The reviewed intradialytic stretching exercise interventions addressed aspects of muscle cramps, urea levels, and fatigue. Conclusion: Intradialytic stretching exercises demonstrate significant potential in preventing muscle cramps of individuals undergoing hemodialysis. Further research is needed to evaluate long-term outcomes and optimise intervention designs.

Abstract

Diabetic retinopathy (DR) represents a leading cause of blindness worldwide, early detection is critical to prevent vision loss. However, traditional screening methods, which rely on human experts, prove to be costly and time-consuming. The systematic review aims to assess the validity of artificial intelligence (AI) as a screening tool for detecting DR among diabetic patients. A systematic literature search was performed of the following databases: PubMed, Scopus, CINAHL, and Web of Science. The last date of our search was January 31, 2024. We included all observational studies, including cohort, case–control and cross-sectional studies and evaluated their quality using the Joanna Briggs Institute tool. We included diagnostic test accuracy studies evaluating the use of AI algorithms for DR screening in patients with diabetes. Studies were excluded if they exclusively assessed diagnostic accuracy for DR that did not use AI algorithms as a diagnostic tool and studies with incomplete or inaccessible data. Thirteen studies with sample sizes ranging from 69 to 1378 participants, reported good sensitivity of AI for detecting visually threatening DR (VTDR). The lowest sensitivity was 89.2%, and the highest was 100%. In terms of specificity, Any DR exhibited higher specificity compared to RDR and VTDR, ranging from 80.2% to 100%. The sensitivity and specificity of the Artificial Intelligence (AI)-based tools available for DR screening was considered acceptable, especially in detecting VTDR and Any DR, was regarded as good. These results implied the potential usefulness of these tools for DR screening in settings with limited resources. However, further high-quality comparative studies were deemed necessary to evaluate their effectiveness in real-world clinical settings.

Abstract

The Early Warning Score (EWS) is a crucial tool for detecting early signs of clinical deterioration in hemodialysis (HD) patients. However, previous research has primarily focused on the intra-dialysis phase, necessitating a systematic review to explore the effectiveness of EWS across all phases (pre, intra, and post-dialysis). Objective: This study aims to assess the effectiveness of EWS in detecting complications in hemodialysis patients across these three phases and to evaluate its impact on morbidity, mortality, length of hospital stays, and readmission rates. Methods: This systematic review was conducted following PRISMA guidelines. Literature searches were performed in databases such as PubMed, Scopus, ScienceDirect, ProQuest, and Google Scholar. The keywords used in the search were “Early Warning Score” OR “EWS” AND “Hemodialysis” OR “Renal Dialysis” OR “Dialysis” AND “Pre-dialysis” OR “Intradialysis” OR “Post-dialysis” and can utilize Boolean logic (AND, OR, or NOT) to maximize search results. The screening of articles with respect to limitations including year 2016 - 2024. Results: Out of 1,246 identified articles, 15 studies met the inclusion criteria. The findings indicate that EWS is effective in detecting complications across all hemodialysis phases, with significant improvements in clinical management and reductions in morbidity and mortality rates. Conclusion: The comprehensive application of EWS in the pre, intra, and post-dialysis phases can enhance the safety of hemodialysis patients.

Abstract

Perioperative assessment is essential to optimize surgical outcomes and reduce complications, especially in cancer patients who often have complex clinical conditions. This systematic review aims to provide an overview of existing domains and practices in perioperative assessment specific to cancer patients. A systematic search was conducted in databases including ScienceDirect, Scopus, PubMed, and Google Scholar, identifying 1,071 articles. After removing 384 duplicates, 687 articles underwent title and abstract screening, leading to the exclusion of 980 articles. Forty-five full-text articles were reviewed for eligibility; 16 were excluded due to inaccessibility, resulting in 12 articles included in the final analysis. Data were synthesized descriptively to identify common assessment domains and practices. The review found several key domains commonly assessed perioperatively in cancer patients, such as nutritional status, frailty, psychological health, cardiopulmonary function, and immunological parameters. However, there is notable variability in the instruments and methods used across studies, indicating a lack of standardized assessment protocols. Comprehensive perioperative evaluations that integrate these domains may enhance surgical planning and risk prediction in oncology patients. While multiple domains are recognized as important in perioperative cancer assessment, standardized and integrated approaches remain limited. Addressing these gaps could improve risk stratification and patient outcomes, although challenges like tool inconsistency, interdisciplinary coordination, and resource availability must be overcome.
