This study aims to describe several problems related to data processing of the top 10 outpatient diseases. The research method used is a qualitative research method with a descriptive approach and a waterfall software development method, while the data collection techniques used are observation and interviews with medical record staff. In the research, problems were found such as diagnoses that were difficult to read by poly officers, the existence of duplicate tasks for medical record officers and recording still using manuals filled in by poly officers which were then inputted into the Microsoft Office Excel program and only made worksheets to then be processed back into the top 10 outpatient disease reports. The suggestions given for problem solving are the coordination between officers and doctors regarding the writing of diagnoses, the addition of medical record officers to reduce the workload and the design of a software that can facilitate the process of processing data on the top 10 outpatient diseases using Visual Studio 2012. So that the final report on the top 10 diseases can be accurate and can improve the quality of service in the hospital.

Limited knowledge Diseases and medical conditions are constantly evolving, and sometimes physicians may not update their knowledge of recent changes in ICD-10 (International Statistical Classification of Diseases) code classifications or other coding systems in use. This may lead to errors or discrepancies in code assignment, the aim of which is to provide global standards in the documentation and reporting of health events. The ICD coding system is essential in the healthcare field because it allows medical professionals to effectively record, analyze, analyze patient data, as well as create global epidemiological statistics. Based on research, creating solutions to problems that occur. Therefore, researchers created an information system with the aim of making it easier for medical records officers to record and determine the 5 major diseases which will later be reported to the Head of Medical Records, such as manual searches, increasing officer workload, and entering data into Microsoft Excel to create a list. the top three diseases are still a long process two of the many drawbacks of indexing. The method proposed to overcome this problem is to apply the development method with Agile Software Development by involving key stakeholders. The features that can be implemented in this software include forms for collecting patient data. This system helps in efficient management of medical files by facilitating simple access to patient data and reducing the chances of human error and loss of information. The problem of duplicate immunization data often occurs because existing systems do not have adequate validation. This causes child immunization data to be entered more than once, either due to errors in entering information or discrepancies in existing data in various programs or networks. In addition, the use of separate systems to recapitulate child immunization data also causes duplicate data. In order to overcome this problem, a system is needed that is able to detect duplicate immunization data. The Agile Development method aims to be used to design and develop this system repetitively and continuously. The Agile Development method allows developers to adapt to changes that occur during the development process and get feedback from users. In applying the Agile Development method, a user needs analysis is carried out involving medical experts, immunization officers, and user systems. Then a system design is carried out which includes validation of immunization data using predetermined rules. The system is also equipped with accurate reporting features and provides data duplication prevention. The result of applying this method is an information system that is able to detect duplication of immunization data so that immunization data reports become more accurate. Users can easily enter child immunization data information and the system will provide feedback if there is duplicate data. This increases the efficiency and accuracy of child immunization reports, and reduces errors and unwanted data duplication.

The main problem faced today is the lack of structure in the reporting of family planning measures at RSAI Limijati. Reporting does not distinguish the type of family planning performed by the patient and does not provide information on whether the patient is performing a new family planning procedure or repeating the previous one. As a result, the resulting report is not well structured and does not provide a clear picture of patient data according to the type of family planning and the history of family planning actions that have been taken. To overcome this problem, this study proposes a qualitative approach with a descriptive approach developed using the agile method. This method aims to develop a medical record information system that is able to produce a well-structured final report for each patient's data, taking into account the type of birth control performed. The system will also provide a detailed explanation of whether the patient is taking a new family planning procedure or repeating the previous one, and will record this information along with other patient data such as demographic data and medical history. The result of applying this method is expected to produce a more structured and informative family planning reporting system. This improved system will make it easier to conduct family planning reporting and ensure that patient data is well organized. Information on family planning measures for each patient can also be obtained more easily according to the type of family planning performed, providing a more complete and accurate picture of the patient's condition related to family planning.

With a focus and in-depth analysis of the case findings from research conducted at Hospital X, it was found that in the process of submitting claims for national health insurance incomplete patient data often hinders the claim submission process. Delays in claim submission can impact the hospital's finances and the quality of services. Therefore, the completeness of patient data is a crucial factor in the claim submission process. The purpose of this research is to design a confirmation menu to support decision-making for national health insurance claims using the extreme programming method. The main focus of this research is to develop a confirmation menu in the system to assist staff in addressing incomplete patient data. This design is built using the extreme programming method, which is considered more flexible, simple, and easy to adapt to frequently changing requests. As a result of this research, the application of the extreme programming method in system design can assist the staff involved in the claim process to confirm and communicate when patient data is incomplete. Additionally, this system can improve performance indexes and minimize hospital losses, thereby continuously enhancing excellent service delivery.

Chronic renal failure is a serious kidney disorder that can lead to a high risk of death, chronic renal failure causes 850,000 deaths every year. This study aims to produce an electronic-based reporting information system on reporting the blood usage of hemodialysis patients. In kebonjati bandung hospital, there are problems in reporting the blood usage of hemodialysis patients, which until now still use manual reporting. This research uses qualitative descriptive research used to gain a better understanding of the research topic. Researchers use methods such as observation, interviews, and analysis of relevant documents to collect data. A software development method called the agile method is used in this research. The creation of the information system design model was made using visual studio 2010 with an access database system and the system was tested using black-box. The results of the information system created can help facilitate officers in minimizing the time of filling in the data needed to produce efficient reports.

Researchers conducted research aimed at creating an electronic-based death certificate information system application to manage patient death data accurately to increase work effectiveness and efficiency in medical records installations so that the source of relevant death report data is available. One of the problems found was providing information on patient death certificates at Kebonjati Hospital which was felt to be less efficient because it still used manual methods and handwritten media in the form of photocopy paper, which reduced the quality of medical records and resulted in them being inaccurate because the writing was not clear and blurry. In conducting this research, the author conducted research usingl qualitative researchl using al descriptive approachl including data collection, lobservation, and linterviews. The system development method uses agile methodology. The results of implementing the death certificate reporting system are designed using Visuall Studio 2012l and a Microsoftl Access databasel which isl expected to speed up and expedite the work of medical record officers in filling in patient identity, displaying time of death, and obtaining accurate and competent reports.

All of the patient service units at the Occupational Health Hospital of West Java Province are affected because the present asset management system in the medical record installation is still based on manually recorded information and then archived. As a result, the data collection for incoming goods, outgoing goods, and stock in the system is inaccurate and not yet well controlled. The purpose of this research is to develop a system for managing assets in medical record installations; it will use Visual Studio 2012 for programming, an Access 2013 database for secure data storage, and a development methodology to make data processing easier for officers. Agile approaches are used by the system. Therefore, the author hopes that this design can help medical record officers in monitoring assets in the medical record installation.

There has been a sea shift in health services brought about by the development of ICT, and not only in the way health information systems are run. The primary objective of this study was to gather requirements for a Microsoft Visual Studio 2010-based outpatient claims verification information system for BPJS health. We need a reporting information system that uses computerization to decrease service delays, and the existing reporting system implementation is the key cause for it. While the development technique employs the waterfall methodology, the research method applies a descriptive qualitative approach. The result of the applied system design is in the form of an outpatient BPJS file claim verification report. With the design of this system, it can be a proposal and can be implemented by Muhammadiyah Bandung Hospital to facilitate the process of reporting outpatient BPJS file claims.

The purpose of this research is to design an ineligible outpatien BPJS claim reporting information system implemented in Microsoft Visual Studio 2012. A qualitative methodology based on descriptive statistics was employed for the investigation. Observation and interviews with hospital claims officials were the methods utilized to gather data. We utilize the v-model, a variant of the waterfall approach, to build our software. The investigation revealed certain issues, such as the persistence of infeasible BPJS outpatient claims and the persistence of traditional reporting methods. Reports detailing the total number of BPJS claims that were deemed ineligible are generated by the intended information system. It is believed that health institutions will be able to use this design's findings to streamline the process of following up on accusations that BPJS has returned to hospitals.
