Abstract

Decreased consciousness is a clinical condition that requires rapid and accurate assessment. One method used is the application of nail bed pressure pain stimulation to elicit a response from the patient. This study aims to determine the consistency of responses from patients with decreased consciousness when given pain stimulation based on the Glasgow Coma Scale (GCS) score in the emergency department of Dr. M. Djamil Hospital in Padang. This was a quantitative descriptive study with 70 respondents selected using purposive sampling. Consciousness levels were measured twice. Data were collected in July 2025 using the GCS assessment format, then analyzed descriptively and cross-tabulated using frequency distribution, percentages, means, standard deviations, and correlations. The nail bed pressure technique produced an average GCS score of 6.01 ± 0.625 on the first assessment and 6.04 ± 0.711 on the second assessment. Most scores were in the range of 6–7. The nail bed pressure technique provided an inconsistent picture of changes in GCS responses in patients with decreased consciousness.

Abstract

The prevalence of coronary heart disease (CHD) in Indonesia continues to increase. Padang ranks 10th with the most CHD patients in Indonesia. The clinical manifestation of CHD is angina pectoris. One of the efforts in minimizing angina pectoris is self- management. This study aimed to determine the relationship between self-management and the degree of angina pectoris in coronary heart disease patients at the cardiac polyclinic of RSUD dr. Rasidin Padang. This study used a quantitative research design with a cross-sectional approach. Sampling in this study used purposive sampling technique with 106 respondents. The instruments used were CSMS (Coronary self-management scale) and Canadian Cardiovascular Society angina grade (CCS). The results of univariate analysis showed that the patient's self-management was in the good range and the highest degree was in the angina II category. The bivariate analysis test using the chi-square correlation test showed a significant relationship between self-management and the degree of angina pectoris with significance 0.000 (? <0.005), meaning that there was a significant relationship between self-management and the degree of angina pectoris and coefficient relation was 0.95 (strong relation). The limitation of this study was the cross-sectional design used where the measurement of the dependent variable and the independent variable was carried out simultaneously so that it cannot prove the existence of cause and effect between the two variables. It is hoped that the hospital through health workers can increase health education in an effort to increase adequate knowledge related to self-management of coronary heart disease patients.

Abstract

The declining number of end-stage renal disease (ESRD) patients who opt for Continuous Ambulatory Peritoneal Dialysis (CAPD) as a renal replacement therapy is influenced by several factors, including high infection rates, limited patient and family knowledge, and restricted access to CAPD services. However, research exploring the perspectives of healthcare professionals, such as dialysis nurses and nephrologists, on this phenomenon remains limited. This study aimed to explore the perspectives of dialysis nurses and nephrologists regarding barriers to CAPD utilization. A qualitative descriptive design was employed, using semi-structured interviews with 12 participants, including 10 dialysis nurses and 2 nephrologists at a tertiary hospital in West Sumatra, Indonesia. Data were analyzed using Colaizzi’s seven-step method. Five major themes emerged: (1) Weak coordination, unsupportive policies, and limited expertise; (2) Infection risk as the primary challenge in CAPD; (3) Ineffective patient and family education in CAPD care; (4) Psychological barriers to accepting CAPD therapy; and (5) Determinants of patient decision-making in choosing CAPD, and 15 Sub-themes. This study highlights the barriers that patients face when choosing CAPD as a renal replacement therapy. Unsupportive policies, increased infection risks, and inadequate educational systems were identified as key challenges that require urgent evaluation and improvement. Strengthening these aspects is crucial to ensure optimal care for patients who opt for CAPD. Keyword: Indonesia, Nephrologist, Nurse Perspective, Peritoneal Dialysis, Qualitative Study

Abstract

Hypertension remains a major public health concern worldwide, largely driven by unhealthy lifestyles such as poor diet, physical inactivity, and excess body weight. Early identification of modifiable risk factors, including cholesterol levels, body mass index (BMI), and physical activity, is crucial for preventing hypertension. This community-based screening activity was conducted on 7 August 2025, at SMA Negeri 9 Padang, targeting teachers, administrative staff, and food vendors. A total of 80 adults participated in the study using convenience sampling. The activity included health education followed by individual health screenings, which consisted of blood pressure measurement, BMI calculation, cholesterol testing using portable point-of-care devices, and physical activity assessment using structured questionnaires. The results showed that 45% of participants were classified as prehypertensive, 83.8% had high cholesterol levels, and 66.3% were overweight or obese. Most participants (72.5%) reported moderate physical activity levels, which fell short of the World Health Organization’s recommendations. These findings indicate a high risk of hypertension among adults in the school community, particularly related to elevated cholesterol, excess body weight, and insufficient physical activity. We identified hypertension risk factors and provided health education and individualized screening feedback to raise awareness of cardiovascular risks and emphasize the importance of regular screening.

Abstract

Fatigue is the most common complaint among patients with chronic kidney disease (CKD) undergoing hemodialysis. Energy conservation management involves modifying daily activities to reduce energy expenditure and alleviate fatigue. This study examines the impact of energy conservation management on fatigue levels in patients undergoing hemodialysis. This quasi-experimental study involved 52 hemodialysis patients who were randomly assigned to either the intervention or control group. The study was conducted in a hemodialysis unit at a public hospital in West Sumatra, Indonesia, from July to September 2024. The intervention group received energy conservation management training over a six-week period, comprising 12 sessions. Fatigue levels were assessed using the Functional Assessment of Chronic Illness Therapy-Fatigue (FACIT-F) questionnaire. Data analysis was performed using paired and independent t-tests. The results showed a significant reduction in fatigue in the intervention group (p < 0.001). The mean (SD) fatigue score decreased from 28.92 (6.14) at baseline to 22.04 (4.41) after the 12 intervention sessions. Furthermore, a statistically significant difference in fatigue scores was observed between the intervention and control groups (p < 0.001). Effective energy conservation management reduces fatigue in hemodialysis patients. Nurses can teach this strategy to help patients manage fatigue in daily life
