Literature Study: The Effect of Abdominal Massage to Reduce Gastric Residual Volume in Critically Ill Patients Adella Indri Afitasari; Dian Hudiyawati

Abstract

Gastrointestinal dysfunction is one of the most common problems experienced by patients in the ICU, such as abdominal distension, constipation, and increased gastric residual volume. Currently, non-pharmacological interventions have been developed to address these problems. Abdominal massage intervention is considered a method that can improve digestive function. The purpose of this literature study was to determine the effect of abdominal massage to reduce the gastric residual volume in critically ill patients. Selected articles were obtained through online databases such as PubMed, Google Scholar, and ScienceDirect. There were seven articles obtained after going through screening and identified for critical review. There were five study randomized controlled trials, three quasi-experimental designs, one study, one group pre-test post-test, and one article one-shot case study. The results of this study found that abdominal massage was effective in reducing gastric residual volume. Mechanism of abdominal massage with stimulation of peristalsis, changes in intra-abdominal pressure, mechanical and reflexive effects on the intestine, thereby shortening the transition time of food in the intestine, increasing bowel movements, and easier flow of food through the digestive tract. So that nurses in the ICU are expected to use abdominal massage as an intervention option at critical times, considering that this intervention can prevent gastrointestinal dysfunction with low risk and is cost-effective.

Abstract

Chronic Kidney Disease (CKD) is a progressive and irreversible decline in kidney function that results in the decreased ability of the body to retain fluids and electrolytes. Treatment methods in end-stage kidney disease are long-term treatment with hemodialysis and kidney transplantation. Hemodialysis therapy in the long term can cause anxiety. Aromatherapy can assist patients in coping with anxiety. The goal of this study was to determine the efficacy of aromatherapy in reducing anxiety in patients with hemodialysis. Articles were obtained through online databases such as PubMed, Google Scholar, and Science Direct. There are seven articles obtained after going through data screening and will be identified for critical review. The findings show that inhalation aromatherapy was effective in reducing anxiety levels among hemodialysis patients. Inhaled aromatherapy affects the central nervous system and has a balancing effect on the cerebral cortex and nerves in the brain. Inhaled aroma compounds will interact quickly through the central nervous system and olfactory nerves and then stimulate the nerves in the brain under the balance of the cerebral cortex to produce the release of the hormone melatonin, serotonin, which can cause a feeling of relaxation or sedative.  Future research should include a variety of aromatherapy variants that can be used based on patient preferences, as well as an assessment of possible side effects.

Abstract

Chronic kidney disease (CKD) is a disease of kidney function disorders that occurs when the body fails to maintain metabolism, fluid, and electrolyte balance which can cause retention of urea and nitrogen waste in the blood. One of the treatments for patients with kidney failure is hemodialysis, but it can also cause psychological issues such as anxiety. Benson relaxation is a nursing intervention that can reduce anxiety. The purpose of this literature is to determine the effectiveness of the Benson relaxation method in reducing anxiety in patients undergoing hemodialysis. The study is a literature review that was conducted using databases such as PubMed, Elsevier, ScienceDirect, Portal Garuda, and Google Scholar, yielding ten articles. The findings indicate that Benson's relaxation therapy is effective in reducing the anxiety level of hemodialysis patients. This exercise will stimulate the brain to release alpha waves that cause humans to feel happy, calm, and feel comfortable. Therefore, Benson relaxation is recommended as an additional intervention for reducing anxiety in hemodialysis patients.
