Abstract

This study is motivated by the complexity of the legislative system in Islamic governance, which integrates divine revelation with the dynamic needs of society. Unlike secular legal systems that rely solely on human rationality, Islamic legislation is rooted in the Qur'an and Sunnah, and further developed through ijtihad methods such as ijma', qiyas, and maqashid al-shari'ah. The objective of this research is to examine the fundamental principles of legislation in Islam, its relationship with public policy, and the challenges of its implementation in contemporary Muslim-majority states. The research applies a normative juridical method, supported by conceptual and sociological approaches, with qualitative descriptive analysis. The findings reveal that the Islamic legislative system is flexible and responsive to social change while maintaining its normative integrity through instruments such as maqashid al-shari’ah and siyasah syar’iyyah. Islamic legislation demonstrates the capacity to adapt to modern governance frameworks, as long as it remains grounded in the values of justice, public interest, and human rights protection.

Abstract

Background: Chronic kidney disease (CKD) is a global health problem with increasing prevalence and high mortality rates. Patients undergoing hemodialysis often struggle with non-adherence to fluid restriction, which can lead to serious complications such as hypertension, pulmonary edema, and heart failure. Purpose: To analyze the effectiveness of video-based health promotion media in improving adherence to fluid restriction among CKD patients undergoing hemodialysis. Method: This study employed a pre-experimental design with a one-group pretest-posttest approach. A total of 66 participants were selected using total sampling. Adherence was measured using Interdialytic Weight Gain (IDWG) with a digital scale, and data were analyzed with a paired t-test. Results: Findings revealed that prior to the intervention, all patients (100%) were non-adherent to fluid restriction. After receiving health education via video at least three times, adherence increased to 64%. The mean IDWG significantly decreased from 3.8 kg to 2.6 kg (p < 0.001). Conclusion: Video-based health promotion proved effective in improving adherence to fluid restriction among hemodialysis patients with CKD. These findings align with Bandura’s Social Learning Theory, which emphasizes the importance of observation and modeling in health behavior change. Suggestion: Healthcare facilities routinely integrate video-based education, involve families in support programs, and conduct further studies using quasi-experimental or randomized controlled trial (RCT) designs to strengthen scientific evidence.   Keywords: Chronic Kidney Disease; Hemodialysis; Health Promotion; Video Media.   Pendahuluan: Gagal ginjal kronik (GGK) merupakan masalah kesehatan global dengan prevalensi yang terus meningkat serta tingkat mortalitas yang tinggi. Pasien GGK yang menjalani hemodialisa kerap menghadapi tantangan ketidakpatuhan terhadap pembatasan cairan, yang berpotensi menimbulkan komplikasi serius seperti hipertensi, edema paru, hingga gagal jantung. Tujuan: Untuk menganalisis efektivitas media promosi kesehatan, video dalam meningkatkan kepatuhan pembatasan cairan pada pasien GGK yang menjalani hemodialisa. Metode: Penelitian ini menggunakan desain pre-experimental dengan pendekatan one group pretest-posttest. Sampel penelitian berjumlah 66 partisipan yang dipilih menggunakan teknik total sampling. Kepatuhan diukur melalui Interdialytic Weight Gain (IDWG) dengan timbangan digital. Data dianalisis menggunakan uji T berpasangan. Hasil: Sebelum intervensi, seluruh pasien tidak patuh terhadap pembatasan cairan. Setelah diberikan edukasi melalui media video sebanyak ≥ 3 kali, tingkat kepatuhan meningkat menjadi 64%. Rata-rata IDWG pasien menurun signifikan dari 3.8 kg menjadi 2.6 kg (p < 0.001). Simpulan: Media promosi kesehatan berbasis video terbukti efektif dalam meningkatkan kepatuhan pembatasan cairan pada pasien GGK yang menjalani hemodialisa. Temuan ini mendukung Social Learning Theory (Bandura) yang menekankan pentingnya observasi dan modeling dalam perubahan perilaku kesehatan. Saran: Edukasi berbasis video dapat diintegrasikan secara rutin dalam layanan kesehatan serta melibatkan keluarga dalam program pendampingan, dengan penelitian lanjutan menggunakan desain quasi experimental atau RCT untuk memperkuat bukti ilmiah.   Kata Kunci: Gagal Ginjal Kronik; Hemodialisa; Media Video; Promosi Kesehatan.
