Abstract

Penderita stroke akan mengalami gangguan keseimbangan, terbatas aktivitas hidup sehari-hari sehingga mengalami imobilitas dan gangguan supply darah sehingga resiko tinggi terjadi ulkus dekubitus. Resiko ulkus dekubitus dan kemunduran kemandirian fungsional yang dialami oleh pasien stroke dapat dibantu dengan melakukan mobilisasi segera. Penelitian ini bertujuan untuk menganalisis pengaruh mobilisasi segera setelah stroke terhadap kemandirian fungsional dan pencegahan resiko ulkus dekubitus. Jenis penelitian yang digunakan adalah design quasy experiment dengan menggunakan metode pre post test with control group design. Populasi dalam penelitian ini adalah seluruh pasien stroke iskemik di Rumah Sakit Haji Adam Malik Medan dan sampel berjumlah 37 orang untuk kelompok kontrol dan 37 orang untuk kelompok intervensi. Pengumpulan data dengan data primer dan sekunder dan dianalisis dengan uji statistik Wilcoxon Test dengan tingkat kepercayaan 95%. Hasil penelitian menunjukkan bahwa tidak ada pengaruh mobilisasi segera terhadap pencegahan resiko ulkus dekubitus pasien stroke kelompok kontrol (p=0,083), tidak ada pengaruh mobilisasi segera terhadap kemandirian fungsional pada kelompok kontrol pada pasien stroke (p=0,058), ada pengaruh mobilisasi segera terhadap pencegahan resiko ulkus dekubitus pasien stroke kelompok intervensi (p=0,000) dan ada pengaruh mobilisasi segera terhadap kemandirian fungsional pada kelompok intervensi pada pasien stroke di Rumah Sakit Haji Adam Malik Medan (p=0,000). Disarankan bagi keluarga pasien yang mengalami stroke pemberian mobilisasi segera dapat diterapkan sebagai terapi untuk dilakukan di rumah dan bagi perawat direkomendasikan menjadi masukan dalam meningkatkan frekuensi mobilisasi segera pada pasien stroke untuk mengantisipasi terjadinya luka dekubitus dan peningkatan kemandirian fungsional.

Abstract

Stroke patients generally experience daily self-care disorders due to old age (elderly), so that motor skills are low. One factor affecting the ability to take care of oneself every day and be motivated to be independent is self-efficacy. The purpose of this study was to analyze the effect of self-efficacy-based education on the daily self-care of stroke patients at the Haji Adam Malik General Hospital and its implications with counselling guidance. The type of research used is a quasi-experimental design using the nonequivalent control group pre- post-test only design method. The population in this study were all ischemic stroke patients at Haji Adam Malik Hospital Medan and a sample of 36 people. Researchers provided educational interventions based on self-efficacy using visual media, discussions, motor skills training, and occupational exercises for three meetings per week with a duration of 30 minutes each session for six weeks. Data collection with primary and secondary data and analyzed by statistical test Paired Sample t-Test. The results showed an effect of self-efficacy-based education on the daily care of stroke patients at Haji Adam Malik Hospital Medan (p-value = 0.000 <0.05) such as motor function and improving their daily activities.

Abstract

Hipertensi merupakan masalah kesehatan kronis yang umum ditemukan pada lansia dan dapat menimbulkan komplikasi serius apabila tidak ditangani dengan baik. Intervensi manajemen perawatan diri diyakini dapat meningkatkan perilaku kesehatan pada lansia yang menderita hipertensi. Penelitian ini bertujuan untuk mengevaluasi efektivitas self-care management terhadap perilaku pasien lansia dengan hipertensi. Penelitian ini menggunakan desain quasi experimental non-equivalent control group pretest-posttest design. Sebanyak 64 pasien lansia dengan hipertensi dibagi ke dalam kelompok intervensi dan kelompok kontrol. Kelompok intervensi menerima edukasi berupa video tentang self-care management sebanyak dua kali seminggu selama dua minggu. Pengumpulan data dilakukan menggunakan kuesioner yang mengukur pengetahuan, sikap, dan tindakan, serta dianalisis menggunakan uji paired t-test dan uji t-test independen. Hasil menunjukkan bahwa terdapat peningkatan yang signifikan pada skor pengetahuan, sikap dan tindakan juga meningkat secara signifikan (p < 0,05). Temuan ini menunjukkan bahwa self-care management berbasis video efektif dalam meningkatkan pengetahuan, sikap, dan tindakan dalam self-care management pada lansia dengan hipertensi.

Abstract

A urinary tract infection is an infection of the urinary tract both upper and or lower, where the number of bacteria is greater than 10⁵ bacterial colonies per unit per ml in a urine specimen. Several studies have shown that there are risk factors that can cause urinary tract infections such as age, gender, long lying down, and other predisposing factors. Objective: To identify the factors that influence the occurrence of urinary tract infections in H. Adam Malik Hospital Medan. Method: This study was designed cross-sectional approach. 59 patients were applied in this study using consecutive sampling. Data were collected using a questionnaire and analyzed using chi-square and Fisher. Results: the results showed that there was no relationship (p>0.05) among age (p=0.869; OR=0.9), gender (p=0.812; OR=1.1), predisposing factors with diabetes mellitus (p=0.146), human immunodeficiency virus (p=0,286), kidney disease (p=0,386), autoimmune (p=0,641) and there were significant relationship (p< 0.05) among prolonged lying down (p=0.04; OR=3,1) were the most significant factor related to urinary tract infection without urinary catheter. Conclusions: the results showed that there was no relationship among age, gender, or predisposing factors such as diabetes mellitus, human immunodeficiency virus, kidney disease, or autoimmune disease with the incidence of urinary tract infection. Long lying down is a relationship with urinary tract infection.

Abstract

Background: Patients with stroke suffer various nerve and muscle issues. The weakness and paralysis of patients with stroke affect their ability to carry out their daily activities.Purpose: To explore nurses' experiences in performing strength of muscle exercise on patients with stroke at Haji Adam Malik Central General Hospital, MedanMethod: This qualitative study with a phenomenological descriptive approach employs a purposive sampling technique to choose 10 nurses who have prior experience performing the strength of muscle exercise on stroke patients. Data are collected through in-depth interviews and then evaluated using Collaizzi method.Results: The five issues identified by the thematic analysis are the nurses’ understand about the strength of muscle exercise, challenges and solutions encountered while performing the strength of muscle exercise, how strength of muscle exercise is implemented, the need to enhance the skill in performing strength of muscle, and the nurses’ emotional reaction while performing the strength of muscle exercise on the stroke patients.Conclusion: Nurses’ knowledge of muscle strength enables them to demonstrate it to stroke patients and help them find the best solution to various challenges they may encounter.Suggestion: The need to improve nurses' skills in muscle strength exercise is related to efforts to improve the quality of nursing care for stroke patients.
