Abstract

The self-help group is one of the group therapies which provides social and psychological support for its members since they can share their problems, and information about nursing, prevention, and medication. The study was to identify the influence of self-help groups on the life quality of patients under chemotherapy treatment. The study was a quasi-experimental method with a pretest-posttest without a control group design. The samples were 35 patients under chemotherapy treatment using the purposive sampling technique. The data were gathered by using a questionnaire, the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC-C30) pre and post-intervention. The data were analyzed by using Wilcoxon Signed Rank Test. The study showed that there was a significant influence on the quality of life of those who were under chemotherapy treatment after self-help group intervention (p=0.000). It is recommended that the hospital management evaluate the life quality of the patients under chemotherapy treatment regularly. The nursing education department should make self-help groups the subject materials for the students as one of the nursing interventions. Abstrak: Self-help group adalah salah satu terapi kelompok yang memberikan dukungan sosial dan psikologis bagi anggotanya karena mereka dapat berbagi masalah, informasi tentang keperawatan, pencegahan, dan pengobatan. Penelitian ini bertujuan untuk mengidentifikasi pengaruh kelompok swadaya terhadap kualitas hidup pasien yang menjalani pengobatan kemoterapi. Penelitian ini merupakan penelitian eksperimen semu dengan metode pretest-posttest without control group design. Sampel penelitian adalah 35 pasien yang menjalani pengobatan kemoterapi dengan teknik purposive sampling. Data dikumpulkan dengan menggunakan kuesioner, European Organization for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC-C30) sebelum dan sesudah intervensi. Analisis data menggunakan Wilcoxon Signed Rank Test. Hasil penelitian menunjukkan bahwa ada pengaruh yang signifikan terhadap kualitas hidup mereka yang menjalani pengobatan kemoterapi setelah intervensi kelompok swadaya (p=0,000). Direkomendasikan agar manajemen rumah sakit mengevaluasi kualitas hidup pasien yang menjalani pengobatan kemoterapi secara teratur. Jurusan pendidikan keperawatan sebaiknya menjadikan kelompok swadaya sebagai bahan ajar mahasiswa sebagai salah satu intervensi keperawatan.

Abstract

Background: Incontinence Associated Dermatitis is an inflammation of the skin that occurs when urine or feces comes in contact with the perineum.  The use of a skin barrier that contains moisturizers can prevent skin inflammation.  The mechanism of action of moisturizers is found in aloe vera and olive oil, as well as accelerating wound healing, anti-inflammatory, and antimicrobial. Objective: To identify the effect of skin barrier aloe vera and olive oil on the prevention of Incontinence Associated Dermatitis in immobility patients. Methods: The study was a Quasi-Experiment post-test-only comparison group design.  The population was newly admitted immobility patients without Incontinence Associated Dermatitis. A sample of 66 people was divided into the control group and the intervention group, sampling using purposive sampling, analyzed using the Mann Whitney and Kruskal Wallis tests. Results: The results showed that there were differences in the values of the three groups, namely the aloe vera, olive oil, and control groups (p = 0.003), with the highest control mean value. Conclusion: This study revealed that immobility patients who were not given the skin barrier using aloe vera or olive oil experienced the most Incontinence Associated Dermatitis. Abstrak: Latar belakang: Incontinence Associated Dermatitis adalah peradangan kulit yang terjadi ketika urin atau tinja kontak dengan perineum. Penggunaan skin barrier yang mengandung pelembab dapat mencegah peradangan kulit. Mekanisme kerja pelembab terdapat pada aloe vera dan olive oil, sekaligus dapat mempercepat penyembuhan luka, antiinflamasi, dan antimikroba. Tujuan penelitian: Untuk mengidentifikasi pengaruh skin barrier aloe vera dan olive oil terhadap pencegahan Incontinence Associated Dermatitis pasien imobilitas. Metode: Jenis penelitian adalah quasi-eksperiment dengan post-test only comparison group design. Populasi adalah pasien imobilitas yang baru masuk tanpa incontinence associated dermatitis. Sampel sebanyak 66 orang dibagi dalam kelompok kontrol dan kelompok intervensi, pengambilan sampel menggunakan purposive sampling, dianalisa menggunakan uji Mann Whitney dan Kruskal Wallis. Hasil: Menunjukkan terdapat perbedaan nilai ketiga kelompok, yaitu kelompok aloe vera, olive oil dan kontrol (p=0,003), dengan nilai rata-rata kontrol paling tinggi. Kesimpulan: Penelitian ini mengungkapkan bahwa pasien imobilitas yang tidak diberikan skin barrier aloe vera maupun olive oil paling banyak mengalami incontinence associated dermatitis.

Abstract

Introduction: Stroke is the main cause of mortality and morbidity in Indonesia. Dysarthria is a symptom that can be seen from stroke. These symptoms occur in 15% of stroke events. Dysarthria is a motor speech disorder. Atypical lacunar stroke (ALS) is a stroke subtype that rarely occurs in one in 14 lacunar strokes or occurs in 6.8% of lacunar strokes, where isolated dysarthria or dysarthria facial paresis is the most common form of presentation. Method: This study is an observational analytic study with a case-control type of research with the aim of knowing the relationship between CT scan images of lacunar infarcts and dysarthria in ischemic stroke patients. The population in this study were all medical records of stroke patients who went to the Department of Neurology at the Haji Adam Malik General Hospital (RSUPHAM) Medan for a period of one year from January 1, 2019 to December 31, 2019 who underwent a head CT scan for 18 patients. Results: The subjects on the CT scan results of lacunar infarction with dysarthria 18 people (72%), and 7 people (28%) on the CT scan results there is no lacunar infarction. The same number and percentage were also found in the non-chivalrous control group. The test results obtained p value = 1,000 and odd ratio = 1.00, in other words there is no significant relationship between lacunar infarct and dysarthria. Conclusion: No significant relationship was found between the incidence of lacunar infarction and the incidence of dysarthria. The population with the most lacunar infarcts is men, hypertension, obesity, hypercholesterolemia, smoking and diabetes.

Abstract

Introduction: Sleep disorders are common in stroke patients and can affect 20% - 78% of patients. Stroke patients also often experience depression and anxiety associated with a decrease in the patient's sleep quality. The purpose of this study was to determine the relationship between depression and anxiety with sleep quality in post-stroke patients. Method: This study used a cross-sectional design. The research subjects were post-stroke patients at the neuro polyclinic at H. Adam Malik General Hospital Medan and Satellite Hospital during July - December 2022 who met the inclusion criteria. Assessment of depression, anxiety and sleep quality used Hamilton Depression Rating Scale (HDRS), Hamilton Anxiety Rating Scale (HARS), and Pittsburgh Sleep Quality Index (PSQI) Questionnaires, respectively. Data were analyzed using Spearman's correlation test. Results: Bivariate analysis found that there was a strong relationship between the HDRS score and the PSQI score (r = 0.708; p <0.001), and the HARS score and PSQI (r = 0.650; p <0.001). Conclusion: There was strong correlation between depression and anxiety with sleep quality in post-stroke patients.

Abstract

Introduction: The neuroophthalmic manifestations of HIV infection involve afferent and efferent visual pathways. Neuroophthalmic involvement is caused by direct infection and opportunistic infections. Screening for neuroophthalmic manifestations should be done even without vision complaints to identify neuroophthalmic signs and symptoms and refer to a neuroophthalmologist soon. The aim of this study was to determine the relationship between CD4 levels and neuroophthalmic manifestations in HIV positive patients Method: It was a cross-sectional study with primary data sources taken consecutively from HIV positive patients at Haji Adam Malik General Hospital in Medan. Subjects were examined for CD4 levels and also performed the neuroophthalmic and neurological physical examination such as visual acuity examination, visual field, pupil examination and funduscopy. Results: There were 45 subjects with demographic characteristics the most of subjects were male (71.1%), age group 31-40 years (55.6%) with an average age of 35.98 + 9.23 years, self-employed (46.7%), married (60% ) and Bataknese (62.8%). CD4 levels < 200 cells/µL were found at 55.6% with a median value of 162 cells/µL (30-878). The most of neuroophthalmic clinical manifestations were found in 52% of subjects with CD4 levels <200 cells/µL with symptoms of blurred vision (22.2%) and clinical signs of abnormalities on fundoscopy (20%). There was a significant relationship between CD4 levels and neuroophthalmic manifestations in HIV patients (p<0.005). Conclusion: There was a significant relationship between CD4 levels and neuroophthalmic manifestations in HIV patients (p<0.005).

Abstract

Introduction: Rabies is a zoonotic disease that can attack the central nervous system and can be fatal because it can cause death. Rabies is caused by an RNA virus of the genus Lyssavirus in the family Rhabdoviridae and can be transmitted to humans through contact with infected animals, particularly bites and scratches. Most cases (98%) were caused by dog   bites and the rest by other animals such as monkeys and cats. This disease has a very poor prognosis, but can be prevented by vaccination. Rabies virus is a neurotrophic virus that can infect humans and cause fatal encephalitis. Method: This was a retrospective case report, after analysis of patient clinical data. The patient’s family  provided written  informed  consent  to  publish  their  case  details  and  any accompanying images. Results: A 73-year-old man has clinical encephalitis with suspected viral EC with suspected rabies dd/non-specific viral dd bacterial. Patients complain of not being able to swallow and afraid  of water, restlessness, hypersalivation, patients are sensitive to  sound,  shortness  of  breath, difficulty  swallowing, fear  of  wind,  patients  want  to  bite  and  hydrophobia,  aerophobia  and hypersalivation. Conclusion: Rabies eradication is necessary because Indonesians are culturally close to animals, especially dogs, which are the main transmitters of rabies. In some areas, dogs are pets that are very close to their owners. Rabies is a neglected tropical disease. Most cases of rabies occur in Asia and Africa. There is no effective treatment to cure rabies but the disease can be prevented by handling rabies bite cases (GHPR) as early as possible.

Abstract

Background: Patients with HIV-AIDS are at increased risk for both opportunistic infections, such as cerebral toxoplasmosis, and cognitive impairment. IgG anti-Toxoplasmosis gondii (T.gondii) is a marker for latent infection. However, its role in cognition in HIV patients remains unclear. The study aims to determine the correlation between Ig-G anti-T.gondii and each domain cognitive function in HIV patients with cerebral toxoplasmosis. Method: This was a cross-sectional study involving 110 HIV patients with cerebral toxoplasmosis who met the inclusion and exclusion criteria. We assessed cognitive function using Montreal Cognitive Assessment Indonesian Version (MoCA-INA) and measured the IgG anti-T.Gondii using the ELISA method. The Spearman correlation test was used to determine the correlation between Ig-G anti-T.gondii with each cognitive domain. Results: There was a significant correlation between IgG anti-T.gondii and cognitive function. (p = 0.004, r = -0.275). There was also significant relation between IgG anti-T.gondii with attention (p = 0.046, r =-0.19), abstraction (p = 0.036, r = -0.2), and delayed recall (p = 0.047, r =-0.19). But there was no significant relation between Ig-G anti-T.gondii with visuospatial (p = 0.171), naming (p = 0.521), language (p = 0.810), and orientation (p = 0.11) Conclusion: Lower Level Of IgG Anti-Toxoplasmosis Gondii Antibodies Is Associated With Worse Cognitive Function In Human Immunodeficiency Virus-Acquired Immunodeficiency Syndrome (HIV-AIDS) Patients with Cerebral Toxoplasmosis

Abstract

Background: Cognitive function disorders often occur in individuals with epilepsy. The decline in cognitive function in epilepsy patients can be caused by various factors, including age of onset, seizure frequency, and duration of the disease. This study aims to investigate the relationship between the age of onset, seizure frequency, and disease duration on the cognitive function of epilepsy patients at the Neurology Outpatient Clinic of Haji Adam Malik General Hospital in Medan Method: The chosen research design is a cross-sectional analytic. The study was conducted at the neurology outpatient clinic of the Adam Malik Central General Hospital in Medan. The samples used were patients diagnosed with epilepsy at the Neurology Outpatient Clinic of Adam Malik Central General Hospital in Medan, aged 18-65 years, and without any other medical conditions aside from epilepsy. The study involved 35 subjects.Result: The majority of the research subjects are aged 18-45 years (94.3%), with more males (51.4%) than females. Most have a high school education background (48.6%), and the largest group is unemployed (62.9%). The most common age group at onset is 0-18 years (48.6%). The majority of subjects experience seizures in the rare category (less than 1 time per month) at 62.9%. Most subjects have a disease duration of less than 15 years (65.7%). The majority of epilepsy patients at the Neurology Outpatient Clinic of H. Adam Malik Hospital (52.1%) have abnormal cognitive function based on the MoCA-Ina test results. Statistical analysis was conducted using the chi-square test and multivariate analysis with logistic regression. The chi-square test results revealed a significant relationship between age of onset and cognitive function (p=0,008), a significant relationship between seizure frequency and cognitive function (p=0,004), and a significant relationship between disease duration and cognitive function (p=0,012). The multivariate analysis results indicated that disease duration had a significant relationship with cognitive function, with an odds ratio of 0,081 (p-value=0,037, 95% CI 0,008-0,862).Conclusion: There is a relationship between age of onset, seizure frequency, and disease duration with the cognitive function of epilepsy patients at the Neurology Outpatient Clinic of Haji Adam Malik General Hospital in Medan

Abstract

Patient non-adherence to antihypertensive medication is a significant risk factor for increased morbidity and mortality in individuals with hypertension. Hypertension, a prevalent cardiovascular condition, is classified as a degenerative disease. Consistent antihypertensive treatment is crucial for improving patient quality of life. This study employed a cross-sectional survey design. The sample consisted of hypertensive patients who presented at the Belawan Health Center between July 22 and 26, 2024. Consecutive sampling was used to recruit 75 participants. Data were collected via interviews regarding participant characteristics and the administration of the Morisky Medication Adherence Scale-8 (MMAS-8), Hypertension Fact Questionnaire (HFQ), and World Health Organization Quality of Life-BREF (WHOQOL-BREF) questionnaires to assess medication adherence, hypertension knowledge, and quality of life, respectively. Analysis revealed strong positive correlations between medication adherence and quality of life (ρ = 0.677, p < .001), and between hypertension knowledge and quality of life (ρ = 0.700, p < .001). Furthermore, both medication adherence and hypertension knowledge significantly predicted quality of life (F = 53.704, p < .001). Multiple linear regression analysis (Y = 30.488) indicated that a one-unit increase in adherence was associated with a 2.680-unit increase in quality of life (p < .001), and a one-unit increase in knowledge was associated with a 2.704-unit increase in quality of life (p < .001). In conclusion, there are strong positive correlations between both medication adherence and hypertension knowledge with the quality of life of hypertensive patients at the Belawan Health Center.

Abstract

This community service examines the empowerment and independence of Desa Binaan Bagan Kuala in the Tanjung Beringin Sub-district of Serdang Bedagai Regency, North Sumatra, through the implementation of ecotourism, ecoprint development activities, appropriate waste management, and malaria eradication initiatives. The objectives of this research are to assess the socio-economic impacts of these initiatives on the local community and evaluate their contributions to sustainable development. The research utilized a mixed-methods approach, collecting both qualitative and quantitative data from 150 households in the village through surveys and interviews. The findings indicate that ecotourism initiatives have generated a 30% increase in local income, attracting over 1,000 visitors annually and creating approximately 50 jobs. The ecoprint development activities have empowered 40 local artisans, resulting in a 25% increase in their income and the production of over 2,000 ecoprint items in the past year. Moreover, the implementation of appropriate waste management practices has led to a 40% reduction in local waste pollution, with 75% of households participating in waste segregation and recycling programs. Malaria eradication efforts have also shown significant results, with reported malaria cases decreasing by 60% over the last two years due to community engagement in health education and the distribution of insecticide-treated nets. This comprehensive approach to community empowerment not only enhances economic independence but also promotes environmental sustainability and public health. The success of these initiatives demonstrates the potential for rural communities in Indonesia to achieve sustainable development through participatory strategies that address both economic and health-related challenges.
