Abstract

Pharmaceutical supplies are one of the resource elements that must be controlled in the operational management of a hospital to support hospital services in order to provide maximum health services. The study aims to analyze the Implementation of Drug Inventory Control with the ABC-EOQ-ROP method at Sundari Hospital, Medan. The research method used was descriptive qualitative which was carried out at Sundari Hospital, Medan in October-November 2024. Informants were the head of the pharmacy installation, pharmacy staff, and head of the room. Data were analyzed using interactive analysis. The results of the study showed that the availability of drugs before the implementation of the EOQ and ROP methods based on the type and quantity of drugs was not sufficient to meet the needs where there were still some drugs that could not be met in a timely manner. The availability of drugs after the implementation of the EOQ and ROP methods in the pharmacy warehouse when viewed based on the type and quantity of drugs was sufficient to meet the needs of the pharmacy installation in a timely manner. It is recommended that drug inventory control in this study only emphasizes the calculation of drug inventory control that requires greater management (category I) quantitatively.

Abstract

Medical records are important records that contain all information related to services that have been provided to patients. The less than optimal process of implementing medical records can be due to a lack of human resources, training, or supporting facilities and infrastructure. This study aims to analyze the need for workers in the medical records department of the hospital with a theoretical approach (queueing theory) at Esthomihi Hospital Medan. This type of research is quantitative with an analytical observational approach. The research design used is cross sectional. This study was conducted at Esthomihi Hospital in December-January 2025. In this study were all health workers related to the implementation of medical records consisting of doctors, nurses, medical records officers and pharmacists at Esthomihi Hospital Medan as many as 252 people. The sample in this study were all doctors, nurses, medical records officers and pharmacists related to patient medical records at Esthomihi Hospital Medan as many as 252 people. Data analysis was carried out univariately, bivariate and multivariate. The results of the study indicate that there is an influence of the level of patient arrivals, the level of patient service, the number of patient servers, and the system capacity. The most influential variable on the need for manpower in hospital medical records affairs with a theoretical approach (queueing theory) at Esthomihi Hospital Medan is the variable number of patient servers with ap value of 0.000, OR = 125.571 (95% CI = 25.114-627.866) meaning that respondents who stated that the number of patient servers was lacking a 125.571 times less chance compared to respondents who stated that the number of patient servers was good n It is expected to be able to improve, expand, develop, experience and knowledge directly regarding the need for manpower in hospital medical records affairs with a theoretical approach (queueing theory) at Esthomihi Hospital Medan.

Abstract

Mutu Pelayanan Kesehatan adalah mutu yang di anggap sebagai kunci dalam direferensi dan keunggulan layanan merupakan sumber potensi berkelanjutan, sehingga pengukuran dan perbaikan tingkat mutu menjadi sangat penting. Penelitian ini dilakukan untuk menilai pengaruh faktor-faktor gaya kepemimpinan kepala ruangan terhadap mutu pelayanan keperawatan. Penelitian ini menggunakan metode cross sectional. Penelitian ini dilakukan pada perawat dan pasien di Unit Rawat Inap Rumah Sakit Royal Prima Jambi dengan metode total sampling. Analisa data menggunakan metode bivariat dan multivariat. Hasil penelitian menunjukkan semua faktor gaya kepemimpinan yaitu komunikasi, prilaku, disiplin dan kemampuan gaya kemampuan mempengaruhi mutu pelayanan keperawatan dengan nilai p value < 0,005 dan faktor yang paling dominan adalah faktor kemampuan gaya kepemimpinan.

Abstract

Latar belakang: Mutu pelayanan merupakan faktor determinan utama yang memengaruhi tingkat kepuasan pasien di institusi pelayanan kesehatan. Kualitas pelayanan tidak hanya ditentukan oleh aspek fisik, seperti ketersediaan fasilitas dan kelengkapan sarana, tetapi juga mencakup aspek non-fisik yang mencerminkan kualitas interaksi antara tenaga kesehatan dan pasien. Penelitian ini bertujuan untuk mengetahui hubungan antara kualitas pelayanan tenaga kesehatan dan kepuasan pasien.   Metode: Penelitian ini menggunakan desain cross-sectional yang dilaksanakan di wilayah kerja Puskesmas Cikampak, Kabupaten Labuhanbatu Selatan. Populasi penelitian mencakup seluruh pasien yang berkunjung ke Puskesmas Cikampak sepanjang tahun 2024, dengan total 200 orang. Seluruh populasi dijadikan sampel melalui penerapan teknik total sampling. Data dikumpulkan menggunakan kuesioner yang disebarkan kepada responden. Untuk menganalisis data, digunakan uji statistik Chi-Square untuk menguji hubungan antarvariabel yang diteliti. Hasil: Hasil penelitian menunjukkan bahwa tidak terdapat hubungan yang signifikan antara dimensi bukti fisik dan tingkat kepuasan pasien (p = 0,155). Sebaliknya, dimensi keandalan, daya tanggap, empati, dan jaminan memiliki hubungan yang signifikan dengan kepuasan pasien, dengan nilai signifikansi p < 0,001. Kesimpulan: Keandalan, daya tanggap, kepedulian, dan jaminan yang optimal memiliki pengaruh signifikan terhadap tingkat kepuasan pasien.

Abstract

Latar belakang: Anjungan Pendaftaran Mandiri (APM), yang berbasis aplikasi, merupakan sebuah platform digital berbasis web yang dirancang untuk membantu pasien rawat jalan dalam melakukan pendaftaran secara mandiri tanpa melalui loket pendaftaran. Penelitian ini bertujuan untuk mengevaluasi efektivitas penggunaan metode APM di Puskesmas Batugana, Kabupaten Padang Lawas Utara. Metode: Penelitian ini menggunakan metode kualitatif dengan pendekatan fenomenologi, yang dilaksanakan di Puskesmas Batugana pada periode September 2022–Desember 2023. Informan penelitian berjumlah empat orang yang terdiri atas Kepala Puskesmas, Kepala Bidang Pendaftaran Mandiri, petugas rawat jalan, dan seorang pasien, dengan teknik pengambilan sampel purposive sampling. Data dikumpulkan melalui wawancara mendalam dan observasi terhadap pasien yang memanfaatkan metode APM. Hasil: Penerapan APM di Puskesmas Batugana dinilai efektif dalam mengurangi antrean pendaftaran serta meningkatkan efisiensi layanan. Keberhasilan penerapannya dipengaruhi oleh faktor pendukung, seperti kompetensi SDM, ketersediaan SOP, serta sarana dan prasarana yang memadai. Kesimpulan: Efektivitas penerapan APM sangat bergantung pada kualitas sumber daya manusia, keberadaan standar operasional prosedur, dan ketersediaan sarana serta prasarana di Puskesmas Batugana.

Abstract

Community Health Centers are required to implement a Community Health Center Information System (SIP) in accordance with Minister of Health Regulation No. 31 of 2019. Medan Teladan Community Health Center has implemented various information systems, one of which is e-Puskesmas, but still found obstacles. This study aims to evaluate the implementation of the Management Information System (SIMPUS) at Medan Teladan Community Health Center using the Human Organization Technology (HOT)-Fit method. This type of research is descriptive qualitative research. The study was conducted at Medan Teladan Community Health Center starting from December 2024 - July 2025. Informants in this study were 5 people consisting of the head of the community health center and medical record officers as key informants, registration officers, SP2TP officers and pharmacy service officers as supporting informants. Data analysis was carried out by data reduction, data presentation and drawing conclusions to identify relevant finding patterns in each aspect of HOT-Fit. The results of the study obtained an evaluation of the implementation using the HOT-Fit method on the human aspect, the user side has problems with input errors, where around 60% of informants reported data input. Organizational aspects, the organizational structure is in place, monitoring and evaluation have been carried out and guidelines for using the information system are in place even though they only adopt those from the Ministry of Health. Technology aspects, the quality of the system is still not optimal because the system has not been integrated with all programs and frequent server disruptions or data bridging failures, differences were also found between drug data in the system and actual stock in the pharmacy warehouse. Net benefit aspects, this system has succeeded in reducing dependence on manual processes and increasing data accuracy, but data integration across all programs is still an obstacle. It is recommended to the Medan City Health Office to encourage the integration of information systems between programs into one platform and ensure server stability. Keywords: System evaluation, information system, HOT-Fit

Abstract

Latar Belakang: Tingginya angka kejadian Infeksi Saluran Pernapasan Akut (ISPA) di Kecamatan Medan Denai diduga kuat berkaitan dengan buruknya kualitas udara dalam ruangan, khususnya di kawasan padat penduduk. Penelitian ini bertujuan untuk menganalisis hubungan antara berbagai parameter kualitas udara dalam ruangan dengan kejadian ISPA di wilayah tersebut.Metode: Penelitian ini menggunakan desain observasional analitik dengan pendekatan cross-sectional. Sampel terdiri dari 300 responden dari rumah-rumah di area padat penduduk Kecamatan Medan Denai yang dipilih menggunakan teknik purposive sampling. Data mengenai parameter fisik dan kimia kualitas udara serta status ISPA dikumpulkan melalui pengukuran langsung, kuesioner, dan observasi. Data dianalisis menggunakan uji chi-square.Hasil: Ditemukan hubungan signifikan antara kejadian ISPA dengan suhu udara, kelembapan, laju ventilasi, kadar SO2, NO2, CO, dan paparan asap rokok (p<0,05). Hasil analisis menunjukkan bahwa laju ventilasi yang buruk meningkatkan risiko ISPA sebesar 26,4 kali (RR=26,430), kelembapan yang tidak sesuai standar meningkatkan risiko 43,8 kali (RR=43,84), dan paparan asap rokok meningkatkan risiko 5,5 kali (RR=5,550). Sementara itu, tingkat pencahayaan, kadar timbal (Pb), formaldehida (HCHO), dan volatile organic compounds (VOC) tidak menunjukkan hubungan yang signifikan.Simpulan: Kualitas udara dalam ruangan yang tidak memenuhi standar, terutama laju ventilasi yang buruk, secara signifikan meningkatkan risiko kejadian ISPA di Kecamatan Medan Denai. Pemilik rumah direkomendasikan untuk secara aktif meningkatkan sirkulasi udara dengan cara membuka jendela dan pintu secara teratur untuk memastikan pertukaran udara yang memadai. Selain itu, penting untuk mengendalikan sumber polusi di dalam rumah, seperti tidak merokok di dalam ruangan, serta menjaga suhu dan kelembapan pada tingkat yang ideal untuk meminimalkan risiko penyakit pernapasan. Edukasi dan pemantauan berkala oleh pemerintah juga diperlukan untuk meningkatkan kesadaran masyarakat. ABSTRACT Title: The Impact of Indoor Air Quality on the Incidence of Acute Respiratory Infections (ARIs) in Medan Denai Sub-district, Medan CityBackground: The high incidence of Acute Respiratory Infections (ARIs) in the Medan Denai Sub-district is strongly suspected to be associated with poor indoor air quality, particularly in densely populated areas. This study aimed to analyze the relationship between various indoor air quality parameters and the incidence of ARIs in the region.Method: This research employed an analytical observational design with a cross-sectional approach. The sample comprised 300 respondents from households in densely populated areas of Medan Denai Sub-district, selected using purposive sampling. Data on physical and chemical air quality parameters and ARI status were collected through direct measurements, questionnaires, and observations. The data were analyzed using the chi-square test.Result: A significant relationship was found between the incidence of ARIs and air temperature, humidity, ventilation rate, levels of SO₂, NO₂, CO, and exposure to tobacco smoke (p < 0.05). The results indicate that poor ventilation increases the risk of ARIs by 26.4 times (RR = 26.43), non-standard humidity increases the risk by 43.8 times (RR = 43.84), and exposure to tobacco smoke increases the risk by 5.5 times (RR = 5.55). Conversely, lighting levels, lead (Pb), formaldehyde (HCHO), and volatile organic compounds (VOCs) showed no significant association.Conclusion: Substandard indoor air quality, especially poor ventilation, significantly increases the risk of ARIs in Medan Denai Sub-district. Homeowners are advised to actively improve air circulation by regularly opening windows and doors to ensure adequate air exchange. Furthermore, controlling indoor pollution sources, such as refraining from smoking indoors, and maintaining optimal temperature and humidity levels are crucial to minimizing the risk of respiratory diseases. Public education and regular monitoring by government agencies are essential to raise awareness and promote healthier indoor environments.

Abstract

This study evaluates the effectiveness of internal verifiers at Muhammadiyah Hospital North Sumatra in minimizing delayed inpatient claims submitted to BPJS Kesehatan, Indonesia’s national health insurance agency. A mixed-methods sequential explanatory design was employed, combining quantitative data from 2020 (prior to verifier recruitment) and 2022 (post-recruitment) with qualitative insights from interviews with internal verifiers, coders, and other stakeholders, as well as a review of claim files processed by the hospital’s BPJS Unit. Interview protocols were adapted from a 2018 study at Koja Hospital, North Jakarta, and data were analyzed using the Miles and Huberman approach, with findings integrated across methods. Source triangulation ensured validity and reliability. Results showed that internal verifiers significantly reduced the proportion of delayed claims despite an overall increase in submissions, with the highest rate of pending claims dropping from 25.6% in 2020 to 17.9% in 2022. Of the 2022 delays, 31.2% were due to BPJS Kesehatan system errors or confirmation processes, indicating that only two-thirds stemmed from hospital-related issues. Qualitative analysis identified key contributors to delays, including diagnostic coding errors, inconsistent understanding among BPJS verifiers, incomplete documentation, limited staff knowledge, and human resource inaccuracies. The presence of internal verifiers improved claim management and hospital revenue by increasing eligibility and accelerating approvals. To further optimize claim processing, strategies such as enhancing coding accuracy, improving staff competency, ensuring documentation completeness, and adopting electronic systems with supportive incentives are recommended. Keyword:  Internal verifier, pending claims, performance

Abstract

Background: Indonesia is still ranked 4th in the world for having low DPT3 immunization coverage among children. Complete basic immunization coverage is targeted at 95% according to the strategic plan of Indonesian Ministry of Health, but it still achieves at 91.1%. This study aims to analyze leader behavior and health center management on immunization program implementation.Method: Quantitative research using a cross-sectional design was conducted at the Puskesmas Biru-Biru Deli Primary Healthcare Center, Serdang District. The research population and samples consist of 122 respondents which include all 121 midwives in 17 villages and one person in charge of immunization services. The survey data were analyzed using the chi-square test and logistic regression test.Results: The chi-square test showed an influence of leadership behavior and management process on immunization program implementation (p = 0.000). In the last stage of logistic regression analysis, one variable that most dominantly affected the program implementation is management process (p = 0.0000; OR = 25,497; 95% CI = 6.475-100.406). The primary healthcare center needs to improve leader’s behavior and management process.

Abstract

Patient loyalty is a critical factor in ensuring the sustainability and competitiveness of hospitals, particularly within health services under the National Health Insurance (JKN) program such as BPJS Kesehatan. According to the 2022 report by the Indonesian Hospital Association (PERSI), the patient retention rate in Indonesian hospitals is only 56%, while the remaining 44% switch to other health facilities due to dissatisfaction with service quality, costs, and accessibility. This phenomenon reflects relatively low patient loyalty, especially among BPJS participants. This study employed a quantitative approach with a cross-sectional design and was conducted at Bandung General Hospital Medan, involving 260 inpatients purposively selected from a total population of 341 patients. Data analysis was performed in three stages: univariate, bivariate using the Chi-Square test, and multivariate using multiple logistic regression. The results revealed four factors that significantly influenced BPJS patient loyalty: satisfaction drivers, emotional response, health worker performance, and relationship quality. Among these, satisfaction drivers emerged as the most dominant factor (OR = 7.611). In contrast, perceived value, expectations, service recovery, and brand image did not show a significant effect. These findings suggest that BPJS patient loyalty is more strongly shaped by direct experiences and emotional interactions during the service process than by external attributes such as brand image or initial expectations.
