Abstract

This study aimed to evaluate the effect of diabetes medication adjustment during Ramadan fasting according to the recommendation of International Diabetes Federation-Diabetes and Ramadan International Alliance (IDF-DAR) to HbA1c values in type-2 diabetes mellitus patients. The study was conducted at a hospital in Jakarta, Indonesia with a cross-sectional study design involving 80 ambulatory patients with type-2 DM who fasted during Ramadan in 2016. About 60% of patients used oral antidiabetes drug (OAD), mostly the combination of biguanide + sulfonylurea drugs (27.5%). The adjustments were made in which 56.2% was in accordance with IDF-DAR recommendations. The study showed that the HbA1c values decreased insignificantly (p = 0.082) from 8.75 ± 1.90 to 8.63 ± 1.82 after the adjustment. The appropriate drug adjustment based on IDF-DAR had a significant effect on the HbA1c value of DM patient with p-value 0.030 (p < 0.05).The discrepancy of drug use based on IDF-DAR 3.222 times greater caused uncontrolled HbA1c. The most influential variable to HbA1c value was drug type with p-value 0.006 (p < 0.05). Improper adjustment of Insulin or combination insulin-OAD caused HbA1c value 5 times greater to be uncontrolled than OAD.

Abstract

Household surveys are one of the methods to obtain accurate information on medicine utilization in society. The study was carried out to identify the access of medicine and medicine utilization profile. A survey using convenient sampling method was conducted in 30 households with national health insurance (JKN) diagnosed with chronic diseases in East Jakarta. Each family was observed once a week for 8 weeks to analyse their diseases, medication used, and medicine access. About 19 (63.3%) respondents enrolled in this study were male. The mean ± SD of age was 55.87±12.486 years old. About 23(76.7%) respondent had access of medicine through hospital and 7(23.3%) by pharmacy directly. The most common chronic diseases identified were cardiovascular and endocrine disorder. Cardiovascular, alimentary tract and metabolism, and nervous system were medications most commonly used. Most of respondents used about 1-3 kind of medications in a month. About 101 kind of drugs used, 74 kinds of drug among of them were generic and 27 were non generic. About 12 (40%) respondents used vitamin and 8 (26.67%) used supplement. This study highlights respondent access of medicine through hospital and cardiovascular medicines were the most commonly used.

Abstract

Senyawa bioaktif peptida laut saat ini menjadi fokus penelitian karena memiliki sifat yang unik. Salah satu peran biologis penting dari senyawa peptida tersebut adalah sebagai agen antihipertensi terhadap aktivitas Angiotensin-I Converting Enzyme (ACE). Terdapat beberapa senyawa peptida yang telah terbukti mampu menghambat reseptor ACE, seperti senyawa peptida yang dihasilkan oleh teripang (Acaudina molpadioides), kerang biru (Mytilus edulis), dan ikan tuna (Thunnini). Dalam penelitian ini dilakukan identifikasi dan evaluasi terhadap interaksi yang terjadi antara senyawa peptida dengan reseptor ACE menggunakan motode penambatan molekuler berbasis protein-peptida. Sequencing senyawa peptida dimodelkan terlebih dahulu menggunakan server PEP-FOLD. Konformasi terbaik dipilih untuk dilakukan studi interaksi terhadap makromolekul reseptor ACE menggunakan software PatchDock. Interaksi yang terjadi diamati lebih lanjut menggunakan software BIOVIA Discovery Studio 2020. Berdasarkan hasil dari penambatan molekuler berbasis protein-peptida, senyawa peptida kerang biru dan ikan tuna memiliki afinitas yang baik terhadap reseptor ACE, yaitu dengan ACE score masing-masing adalah −391,62 kJ/mol dan −516,56 kJ/mol. Dengan demikian, senyawa bioaktif peptida laut tersebut diprediksi dapat dipilih sebagai kandidat inhibitor reseptor ACE berbasis peptida

Abstract

The diterpene lactone fraction of Andrographis paniculata Nees (sambiloto) (FDTLS) main compound is andrographolide. Andrographolide has poor bioavailability, high lipophilicity, and low solubility. Entrapping poorly soluble substances into a hydrophilic polymer, such as chitosan, could improve their dissolution, bioavailability, and activity. The purposes of this study were to improve the FDTLS dissolution and to compare the effect of different amounts of chitosan on physical characteristics, entrapment efficiency, and FDTLS release from nanoparticles, which was made by the ionic gelation-spray drying method using tripolyphosphate as a crosslinker. The nanoparticles were prepared with different FDTLS-chitosan ratios = 1:2 (FK1), 1:2.5 (FK2) and 1:3 (FK3). The nanoparticles were evaluated for its morphology, physical state using differential thermal analysis (DTA), Fourier-transform infrared spectroscopy (FTIR), X-ray powder diffraction (XRD), entrapment efficiency, and drug release. All the nanoparticles' formulas had a spherical shape and FK2 showed the smoothest surface with the highest enthalpy. FDTLS-chitosan nanoparticles diffractogram showed a more amorphous form. The FDTLS-chitosan nanoparticles entrapment efficiency was 29.01-32.69%. The FDTLS-chitosan nanoparticles showed an increased dissolution rate by 1.6 times compared to the FDTLS substance. The study concludes that the formation of nanoparticles using chitosan by ionic gelation-spray dry method could improve the dissolution of FDTLS.

Abstract

This research is a continuation study in the inventory of lichens of the genus Stereocaulon which is currently focused on the Stereocaulon massartianum Hue collected in the rocks of Diatas Lake, West Sumatera, Indonesia. Reports on phytochemical studies and pharmacological activities of this species are still limited, based on literature studies that have been carried out. This research was conducted to determine their secondary metabolites and potential antibacterial activity. The air-dried thallus of lichen S. massartianum was macerated successively using n-hexane, ethyl acetate and methanol solvents. The compounds were separated by chromatography and recrystallization methods, then analyzed by spectroscopy (UV-Vis, FTIR, 1H and 13C-NMR). Furthermore, the antibacterial activity assay was performed by agar diffusion method on ethyl acetate extract, and TLC-Bioautography for the isolated compound. Three compounds have been isolated from ethyl acetate extract, i.e. atranorin (1), stictic acid (2) and norstictic acid (3). The results of antibacterial assay from the extract showed antagonistic activity against pathogenic bacteria S. aureus, E. faecalis, E. coli, and P. aeruginosa at concentrations of 10 and 20%, while TLC-bioautography of compound 3 exhibited growth inhibition area in all test bacteria.

Abstract

Diabetes mellitus is a chronic metabolic condition frequently associated with complications and comorbidities that often require hospitalization for effective management. Such patients are often prescribed multiple medications, which elevate the risk of potential drug-drug interactions (pDDIs). Hence, this study aims to determine the prevalence, severity, common interacting pairs, and factors associated with pDDIs among hospitalized diabetes patients. The study used a retrospective cross-sectional study design conducted at Universitas Indonesia Hospital. Lexicomp® Lexi-InteractTM software was used to analyze and classify possible drug interactions. Logistic regression analysis was used to identify different factors associated with potential DDIs. Of the 200 patients, 89.0% were observed to have pDDIs. A total of 966 interactions were found, of which 75.6% were moderate, 16.2% were minor, and 8.1% were of major severity. Meanwhile, in the risk rating C category, 71.0% were predominant, followed by B and D, 15.0% and 11.0%, respectively. Multivariate regression analysis showed a statistically significant association of pDDIs with 5–8 prescribed medicines (OR=22.8; 95% CI=5.5-94.7; p<0.001), >8 prescribed medicines (OR=64.4; 95% CI=11.3-336.5; p<0.001). The findings of this study revealed that pDDIs are highly prevalent in adult inpatients with diabetes. This emphasizes the critical need for appropriate monitoring and management strategies to reduce pDDIs and their adverse consequences.

Abstract

UV radiation can generate ROS, thereby damaging neutrophils, leading to the uncontrolled release of elastase, and causing aging. This study intends to examine the antioxidants and anti-elastase activity of the skin of Garcinia dulcis (Roxb.) Kurz fruit in serum gel extract preparation. The methods used in this research are microwave-assisted extraction and antioxidants using the ABTS (2,2’-azino-bis(3-ethylbenzothiazoline-6-sulfonic acid) method. Three formulas were made with 100 x IC50, 200 x IC50, and 300 x IC50. The test results for the three formulas showed the best antioxidant and anti-elastase activity values in formula 3, with IC50 values ​​of 66.26 ppm and 111.48 ppm. This indicates that the higher the extract concentration in the formulation provides better activity in the serum gel preparation. In the third formulation, evaluation observations were made on the preparation, and a pH value close to the skin’s pH was produced, namely in formula 3. Therefore, it can be concluded that the Mundu fruit peel extract has antioxidant and anti-elastase activity. The extract is applied to cosmetic preparations and still provides good antioxidant and anti-elastase activity results.

Abstract

The outer skin of robusta coffee bean (Coffea canephora) was a processing waste containing phenolic compounds with antioxidant role. This activity is relevant when used in cosmetic preparations such as peel-off masks, which form a thin elastic layer on the facial surface and are easy to remove. One of the important ingredients of peel-off masks is plasticizer such as sorbitol characterized by flexibility, not easily fragile, and elasticity. Therefore, this research aimed to develop peel-off formula from robusta coffee skin by influencing sorbitol variations on the characteristics of masks in the form of gel. A total of six gel masks formulas were made with sorbitol variations of 10%, 12.5%, and 15%. The two types of gel base used were Carbopol 940 and HPMC (Hydroxy Propyl Methyl Cellulose), at concentrations of 0.25% and 4%, respectively. The results showed that sorbitol variations affected the physical characteristics (p > 0.05) but did not influence antioxidant activity of robusta coffee skin extract peel-off masks.

Abstract

The interaction that occurs between warfarin and antibiotics is a pharmacokinetic interaction which cause bleeding. This study aims to determine whether there is a relationship between the use of cephalosporins and an increase in APTT and PT in cardiovascular patients. This retrospective observational study employs case-control analysis to examine APTT and PT features in cardiovascular patients receiving single anticoagulant or anticoagulant + cephalosporin therapy for the period JulyDecember 2020. The sampling method used in this study is a convenient sampling method that meets the inclusion and exclusion criteria. Analysis of the data used is Logistic Regression. The study found among 65 male and 15 female, 40 patients used anticoagulant, and another 40 patients used anticoagulant and cephalosporine. Among 40 patients who used combination anticoagulant and cephalosporin, 22 of them had experienced of increasing APTT and PT. Cephalosporins showed a significant correlation with APTT and PT, with a P-value of 0.015. From this study, it can be concluded that the use of cephalosporins increases in APTT and PT values in cardiovascular patients who are given anticoagulant drugs and cephalosporins

Abstract

This study aims to investigate the potential of chalcone derivatives as antimalarial agents. The structure of the  chalcone derivatives was designed by inserting amino substituents on acetophenone and methoxy variants on benzaldehyde to  produce three amino chalcone derivatives (C1, C2, and C3). The synthesis was carried out by carrying out the Claisen-Schmidt  condensation reaction with NaOH 40% as catalyst, resulting in compound yields ranging from 66% - 83%. The structure of the  three compounds was determined by FTIR, MS, and 1H-NMR spectroscopy techniques, which confirmed that the compounds  had structures that were in line with the desired molecular structure. The antimalarial activity test was carried out by inhibiting  the heme polymerization process into hemozoin (β-hematin) using hydroxychloroquine sulfate as a positive control. Absorption  measurements were carried out at two different wavelengths, namely 415 nm and 630 nm. The results of the IC50 antimalarial  activity of the three compounds (C1, C2, C3) were obtained respectively at 227.61; 115.18; 260.01 µg/mL and positive control of  184.98 µg/mL. From these results, it was found that compound C2 showed better antimalarial activity compared to the other two  compounds and positive control.
