Abstract

Latar belakang: Ketokonazol merupakan agen antifungi yang tergolong dalam klasifikasi BCS (Biopharmatical Classification System) kelas 2 yang menunjukkan kelarutan rendah dan permeabilitasnya tinggi. Penggunaan matriks polimer seperti alginate dan gum akasia dapat meningkatkan solubilitasnya. Metode Ultra High Performance Liquid Chromatography (UHPLC) merupakan metode analisis dengan efisiensi tinggi, cepat, dan sensitive sehingga sesuai untuk pengujian mikrodisolusi tablet ketokonazol dalam matriks alginate-gum akasia. Validasi metode dengan parameter linearitas, akurasi, presisi, LOD, LOQ, selektivitas diperlukan untuk memastikan keandalan metode dalam pengujian mikrodisolusi menggunakan formulasi tersebut. Metode: Analisis dilakukan menggunakan sistem Reverse Phase-Ultra High Performance Liquid Chromatography (RP-UHPLC). UHPLC menggunakan  kolom C18 Phenomen ex Luna (250 x 4,6 mm, 5 μm) dengan fase gerak terdiri dari kombinasi asetonitril:WFI dengan TEA 0,15% pH 3,3 dengan perbandingan (50:50), laju alir 1 mL/menit, volume injeksi 20 μL, dan panjang gelombang 265 nm. Hasil: Nilai koefisien korelasi diperoleh pada r2 = 0,9993 pada rentang 10-250 µg/mL. Nilai perolehan kembali dalam rentang 98-101%, uji presisi keterulangan diperoleh %RSD dari intraday dalam rentang 0,54-129 dan interday dalam rentang 0,25-1,95. Hasil dari LOD adalah 6,51 μg/mL, dan LOQ adalah 19,73 μg/mL. Kesimpulan: Metode analisis divalidasi sesuai dengan pedoman ICH, termasuk parameter seperti linearitas, akurasi, presisi, batas deteksi (LOD), batas kuantitasi (LOQ), dan selektivitas. Metode yang divalidasi sesuai untuk analisis mikrodisolusi beads ketokonazol dalam medium HCl pH 1,2 menggunakan matriks alginat-gum akasia

Abstract

Urolithiasis is a chronic and increasingly prevalent condition worldwide, characterized by kidney stone formation due to an imbalance between crystallization promoters and inhibitors in the urinary tract. Strobilanthes crispus (kejibeling) is ethnopharmacologically recognized for its diuretic and anti-urolithiasis effects, yet its molecular mechanisms remain poorly explored. This study aimed to screen bioactive compounds of S. crispus with potential diuretic activity, evaluate their binding affinity to diuretic-related protein targets, and compare them to furosemide as a control. Metabolites were identified through the KNApSAcK database and AI-assisted literature search (ChatGPT 4.0 and Gemini 2.5), followed by drug-likeness evaluation using Lipinski’s rule of five. The diuretic-related targets—CA1, aquaporin, vasopressin receptor, and epithelial sodium channel (ENaC)—were selected based on SwissTargetPrediction, PASS Online, and literature evidence. Molecular docking was performed using PyRx, visualized with Discovery Studio, and validated through analysis of key interacting residues and RMSD. Six main compounds (caffeic acid, apigenin, gallic acid, naringenin, (-)-epicatechin, kaempferol) complied with Lipinski’s criteria. Among these, naringenin, apigenin, (-)-epicatechin, and kaempferol exhibited comparable or superior binding affinities to furosemide at critical residues of the target proteins, with binding energies ranging from -7.3 to -7.9 kcal/mol. Docking visualizations confirmed interactions at residues similar to those of furosemide, supporting the predicted binding sites. RMSD values (<2.5 Å) indicated dynamic stability of the ligand-protein complexes. These findings suggest that S. crispus bioactive compounds, particularly naringenin, apigenin, (-)-epicatechin, and kaempferol, have potential as natural diuretic agents with mechanisms resembling furosemide, supporting their use as herbal preventive therapy for kidney stones.

Abstract

Persistent and uncontrolled hypertension can cause damage to other organs, including the kidneys. In Indonesia, hypertension is the third leading cause of kidney disease, contributing 25.8% of all kidney disease. Hypertensive patients with chronic kidney failure generally receive different antihypertensive medications from one patient to another due to the individual use of medications and the varying effects of the drugs among individuals. Rational drug use is one of the important factors in providing healthcare services for patients according to the standards. The evaluation of drug utilization serves to make sure proper, safe, and effective use of medications. This study aims to determine the patient demographics, regimen of the medicines, and the incidence of Drug Related Problem (DRP) in hypertensive patients with chronic kidney disease at RSUD Ibnu Sina from January to June 2021. This study is an observational research using a descriptive method. Data was collected from a retrospective analysis of patient medical records, and 18 patients who met the inclusion criteria were identified. The most commonly used is single antihypertensive medications (38.9%) from the diuretic group (furosemide) and the ARB group (candesartan and losartan). In the profile of additional medications, vitamins and minerals (88.9%), gastric ulcer prophylactics (66.7%), and antiemetics (61.1%) were found. DRP (P1.2) (50%) was caused by several factors, namely (C3.3) (25%), (C3.2) (15%), (C3.1) (5%), and (C4.1) (5%).

Abstract

Obat merupakan bahan yang sangat berpotensi jika digunakan dengan tepat sesuai manfaat yang diharapkan. Dengan mematuhi semua informasi yang tercantum pada kemasan maka akan terhindar dari efek merugikan. Penelitian ini bertujuan mengetahui hubungan faktor sosiodemografi terhadap tingkat pengetahuan masyarakat tentang pemahaman obat tradisional berdasarkan informasi pada kemasan di Desa Taman. Jenis penelitian ini menggunakan metode cross-sectional dengan melibatkan 100 responden yang diambil dengan teknik purposive sampling dengan alat bantu kuesioner. Berdasarkan hasil penelitian yang diperoleh tingkat pengetahuan masyarakat baik 29%, Cukup 36%, kurang 34% sedangkan sikap masyarakat cenderung positif yaitu 71% dengan perbandingan negatif 29%. Pada hasil uji hubungan menggunakan uji spearman didapatkan hasil uji sebesar r=0,087 dimana nilai r tersebut lebih besar dari nilai p=<0.05. yang menyatakan tidak adanya hubungan yang signifikan pada kedua variabel tersebut. Kesimpulan pada penelitian ini adalah tidak terdapat hubungan antara tingkat pengetahuan dan sikap masyarakat mengenai pemahaman obat tradisional berdasarkan informasi pada kemasan di Desa Taman, Kecamatan Abiansemal, Kabupaten Badung.

Abstract

Hipertensi merupakan salah satu masalah kesehatan utama yang memicu penyakit kardiovaskular. Enzim pengonversi angiotensin (ACE) berperan penting dalam regulasi tekanan darah sehingga menjadi target utama pengembangan obat antihipertensi. Kulit ubi jalar ungu (Ipomoea batatas var. Ayamurasaki) diketahui mengandung metabolit sekunder, seperti flavonoid, antosianin, dan asam fenolik, yang berpotensi sebagai inhibitor ACE. Penelitian ini bertujuan memancarkan potensi 10 senyawa aktif kulit ubi jalar ungu sebagai penghambat ACE melalui pendekatan in silico. Metode yang digunakan meliputi penambatan molekul (Gnina), serta prediksi farmakokinetik (SwissADME) dan toksisitas (ProTox-III). Hasil docking menunjukkan bahwa senyawa uji yaitu myricetin (-9,07), quercetin (-10,75), cyanidin (-9,95), pelargonidin (-9,39), peonidin (-9,36), 3CQA (-9,33), apigenin (-9,22), 4CQA (-9,19), dan 5CQA (-8,88) memiliki nilai ikatan bebas rendah dan memiliki afinitas baik, Senyawa berinteraksi melalui hidrogen hidrogen, hidrofobik, dan elektrostatik. Prediksi toksisitas menunjukkan sebagian besar senyawa berada pada tingkat 5 (tidak beracun), kecuali myricetin dan quercetin (Tingkat 3). Seluruh senyawa uji memiliki profil farmakokinetik yang baik, sehingga mendukung potensinya sebagai kandidat antihipertensi alami.

Abstract

Tuberculosis remains a major infectious disease worldwide, with medication non-adherence a critical barrier to successful treatment. Patients with visual impairments face additional challenges in medication management due to their inability to read medication labels, increasing the risk of medication errors and non-adherence. This systematic review analyzed 23 studies on the application of artificial intelligence (AI) to improve medication adherence in tuberculosis (TB) patients, with a particular focus on the potential of AI voice assistance for visually impaired patients. Results showed that AI such as machine learning (ML), natural language processing (NLP), and computer vision effectively improved the accuracy of drug resistance prediction (>90%) and adherence by up to 32%, although challenges such as algorithm bias, data privacy, and geographic accessibility remained significant. For visually impaired TB patients, voice technologies such as Google Assistant or Alexa offer non-visual solutions through personalized reminders, text-to-speech pill recognition (85-95% accuracy), and intake confirmation, overcoming the limitations of visual-dependent video-observed therapy (VDOT). Integrating voice AI with digital therapies (DTx) has the potential to improve autonomy and clinical outcomes in resource-limited settings, but requires empirical randomized controlled trials (RCTs) and user training. Abstract

Proton pump inhibitors (PPIs) are widely used for the management of acid-related gastrointestinal disorders. Although their clinical efficacy is generally comparable, differences in acid-suppressive potency, duration of action, and acquisition costs may influence prescribing patterns and healthcare expenditures. This study aims to evaluate PPI utilization patterns and costs among hospitalized patients at a tertiary hospital in Surabaya, Indonesia, during 2023–2024 and to identify potential cost-control measures. This retrospective descriptive study analyzed PPI utilization data from the hospital information system for 2023–2024 and pharmacy billing records of National Health Insurance (JKN) patients from October to December 2024. Four oral and five injectable PPI formulations were utilized during the study period. Generic omeprazole, in both oral and injectable forms, accounted for the largest proportion of PPI use. Although injectable lansoprazole was prescribed less frequently, it contributed substantially to overall PPI expenditures. Omeprazole, lansoprazole, and pantoprazole were the most commonly prescribed PPIs among JKN patients. Pantoprazole, despite not being included in the hospital formulary, continued to be used and showed an increasing utilization trend. A shift in prescribing patterns from omeprazole to lansoprazole and pantoprazole in December 2024 was associated with increased costs. Omeprazole represented the highest utilization volume, whereas lansoprazole accounted for the largest share of PPI-related expenditures. Cost-control strategies should focus on optimizing formulary compliance and ensuring the appropriate use of higher-cost PPIs. Further studies evaluating clinical outcomes and the impact of formulary restrictions on lansoprazole use are warranted.

Abstract

Stress and anxiety are mental disorders that can reduce quality of life. Aromatherapy using essential oils is an alternative non-pharmacological therapy to help reduce stress and anxiety. Essential oils from dlingo rhizome (Acorus calamus L.) and bangle rhizome (Zingiber purpureum Roxb.) are known to have relaxing activity, so they have the potential to be developed into aromatherapy balm preparations. This study aims to formulate and evaluate the physical characteristics of aromatherapy balm combined with dlingo rhizome and bangle rhizome essential oils. The preparation was made in five formulas with varying total concentrations of essential oil combinations of 0%, 5%, 10%, 15%, and 20%. Physical evaluation included organoleptic tests, pH, homogeneity, spreadability, and adhesiveness. The results showed that all formulas produced semi-solid, homogeneous preparations, with a white to yellowish color and a distinctive aroma of essential oils that became stronger with increasing concentration. The pH value of all formulas was in the range of 6.13-6.45. The adhesive strength values ​​of Formulas A, B, C, D, and E were 5.00; 2.00; 1.74; 1.37; and 0.66 seconds, while the spreadability values ​​were 2.82; 2.83; 2.94; 2.59; and 2.64 cm. Based on the results of the physical evaluation, all formulas met the organoleptic, homogeneity, and pH requirements. Based on the results of the adhesion test, only Formula A met the adhesive strength requirements for topical preparations (>4 seconds). Based on the results of the spreadability test, all formulas did not meet the reference spreadability for topical preparations used in the study.

Abstract

Rumah sakit memiliki berbagai permasalahan dalam proses pelayanan, seperti pemborosan, waktu tunggu, non-valueadded activities, dan penggunaan sumber daya yang belum optimal. Lean management merupakan pendekatan perbaikan berkelanjutan yang digunakan untuk mengidentifikasi dan mengurangi pemborosan guna meningkatkan efisiensi dan efektivitas pelayanan. Penelitian mengenai penerapan Lean management telah dilakukan di Indonesia dan luar negeri dengan fokus dan outcome yang beragam, sehingga diperlukan sintesis untuk memperoleh gambaran yang lebih komprehensif. Tujuan dari penelitian ini ialah untuk mengidentifikasi dan membandingkan penerapan Lean management dalam meningkatkan efisiensi dan efektivitas pelayanan rumah sakit di Indonesia dan luar negeri serta mendeskripsikan outcome yang dihasilkan. Artikel yang memenuhi kriteria inklusi dan eksklusi dari sumber referensi PubMed, Google Scholar dan ScienceDirect. Penerapan Lean dilakukan pada pelayanan rawat inap, instalasi gawat darurat, pelayanan farmasi, pengelolaan obat, dan proses pelayanan rumah sakit. Metode yang digunakan meliputi Value Stream Mapping, 5 Why, BAS/BASICS, observasi, wawancara, dan analisis value-added activities. Outcome menunjukkan perbaikan efisiensi melalui pengurangan waste, waktu tunggu, cycle time, dan lead time. Pada aspek efektivitas, penerapan Lean dikaitkan dengan peningkatan kualitas pelayanan dan kepuasan pasien. Lean Management berpotensi meningkatkan efisiensi dan efektivitas pelayanan rumah sakit melalui pengurangan pemborosan dan perbaikan proses. Outcome penerapan bervariasi sesuai dengan jenis pelayanan rumah sakit.

Abstract

Skin hyperpigmentation is a dermatological problem associated with increased melanin production, in which tyrosinase plays an important role in melanogenesis. Halophilic and marine-derived fungi can adapt to extreme environmental conditions and may produce secondary metabolites with biological activities, including tyrosinase inhibition. This review aimed to evaluate the potential of halophilic and marine-derived fungi as sources of tyrosinase inhibitors by examining fungal origin, species, sample type, active metabolites, assay methods, and reported inhibitory activity. A literature search was conducted using Google Scholar with keywords related to marine fungi, endophytic fungi, halophilic fungi, and tyrosinase. Original research articles published in English that reported tyrosinase inhibitory activity of halophilic or marine-derived endophytic fungi were considered. Eleven articles were selected for further analysis. The findings showed that fungi associated with sponges, algae, sediments, and marine environments produced metabolites with varying tyrosinase inhibitory activities. Several isolated compounds exhibited strong activity, with lower IC50 values than kojic acid, whereas crude extracts generally showed lower activity. Azaphilone, polyketide, and related metabolites were reported among several active isolates. However, most studies were limited to in vitro enzyme assays, with insufficient validation of mechanisms, toxicity, and efficacy in cellular or in vivo models. Halophilic and marine-derived fungi have potential as sources of novel tyrosinase inhibitors, but standardized assays, mechanistic characterization, and safety and efficacy validation are needed to support further development as potential skin-lightening agents.
