Abstract

Prehospital care has historically provided medical care with the aim of saving lives and preventing disability. Prehospital services that are accommodated by ambulances have complex and unpredictable care arrangements, so ambulance personnel, namely nurses, must prepare themselves to deal with all situations and conditions. This study aims to describe the competencies possessed by nurses while on duty in prehospital ambulances. This research is a literature study from several databases, and we obtained 20 articles for analysis. The results of the article analysis show that nurses who work in prehospital ambulances must have the competence to be able to save patients' lives, which is formed from knowledge, attitudes, and skills. Knowledge that must be possessed by ambulance nurses includes assessment, triage, and situation analysis. The positive attitudes of ambulance nurses include decision-making, communication, collaboration, caring, ethics, and culture. The skills possessed by ambulance nurses are BHD, intubation, examination of vital signs, interpretation of supporting examinations, and drug management. Ambulance nurses can improve their emergency care competencies through continuous education and training.

Abstract

Diabetes Mellitus (DM) is a group of metabolic diseases characterized by hyperglycemia that occurs due to abnormalities in insulin secretion, insulin action or both and is often accompanied by increased blood pressure. One of the drug-related problems that are frequently encountered is patient non-compliance in taking drugs. This can occur due to a lack of knowledge about managing pharmacological and non-pharmacological therapies, making therapeutic outcomes difficult. This study aims to determine the effect of visual media education on managing pharmacological and non-pharmacological treatment on the level of knowledge of type 2 DM patients with comorbid hypertension. This type of research is quasi-experimental, with pretest and posttest designs without a control group design. The research sample is the age group that has the age of 18-68 years, as many as 123 people using Purposive Sampling. The results showed that the level of knowledge before being given education was in a good category (4.81%), sufficient (42.29%), and less (52.90%). After being given education visual media, there was an increase in knowledge results for the good (74.25%), sufficient (18.14%) and poor (7.61%) categories. The results of the Wilcoxon test analysis obtained a p-value (0.000) < (0.005), which means that there is an effect of visual media education on the level of knowledge of type 2 DM patients with comorbid hypertension. Through the results of this study, it is hoped that pharmacists can provide ongoing educational counselling to chronic disease patients receiving polypharmacy to prevent drug-related problems.

Abstract

The bite of the Aedes sp mosquito causes nearly 0 million people to be infected with the dengue virus annually, one of areas declared as an endemic area is East Nusa Tenggara Province. 5669 cases and 58 deaths due to dengue hemorrhagic fever (DHF) were found in East Nusa Tenggara Province in 2020 (CFR 1.02%). Highly influential external factors include the availability of clean water, the crisis of drinking water and clean water, and poor condition of water container that do not meet the requirements. This study aims to manage Larvitrap installation movement and abatezation of the water containers in the community/households in Kupang City. This was a observational study with survey design regarding the installation of Larvitrap tool. The density of mosquito larvae was measured based on the House Index (HI), Container Index (CI), and Breteau index (BI). 383 families were involved as the study samples who were selected using the accidental sampling technique. The results showed that there were 866 units of water resevoir (47.8%). Fatululi Village had the highest percentage of indoor water container by 15.7% (140 units), and Oesapa Village had the highest percentage of outdoor water container by 15.5% (143 units). The highest percentage of positive indoor water container was found in TDM and Kelapa Lima by 30.4%, while the highest percentage of positive outdoor water container was found in TDM village by 48.4%. Aedes aegypti larvae and Aedes albopictus larvae were identified in the indoor and outdoor water containers in TDM Village, while that were only aedes aegypti larvae found in six other villages. The positive larvitrap larvae found in Oebufu Village was 15%, 10% was found in Oesapa Village, while there were no larvae found in 5 other villages. The flick density index obtained the highest House Index (HI) in Kelapa Lima village by 58.33%, the highest Container Index (CI) was found in TDM village by 44.03% and the highest Breteau Index (BI) was found in TDM village by 218.75%. It can be concluded that it is necessary to conduct education on mosquito larvae control that involves active community participation and supervision towards weekly draining of community water containers. In addition, abate sprinkling and dengue vector control activities should be carried out simultaneously throughout Kupang City at the beginning of the rainy season, at the peak of case incidence and at the end of the rainy season.

Abstract

The main contributor to cardiovascular disease is atherosclerosis. The Liver X Receptor is one of the unexplored signaling pathways in atherosclerosis that contributes to cholesterol efflux and inhibitory inflammation (LXR). Catechin, as an LXR agonist, influences the expression of the NPC1L1 protein transporter, which inhibits cholesterol absorption. The objective of this study is to predict the NPC1L1 inhibitor of Catechins from Green Tea. The role of NPC1L1 inhibitors is to prevent atherogenesis. Molecular docking is the research method used. Pyrx's Open Babel was used for analysis. Autodock vina in Pyrx was employed for docking, and Chimera v1.8 was administered for visualization. The result of molecular interaction was assigned. Pose view was used in this study. Catechins have the potential to be an NPC1L1 inhibitor, according to the findings. The main parameters used to predict the biological effect were energy bonds, hydrogen bonds, and hydrophobic interactions of molecules with NPC1L1. All Catechins isolates had low affinity energy and a strong affinity for NPC1L1. Epigallocatechin gallate (EGCG) is the most effective inhibitor because it has the lowest binding energy and the most active sites, including Gln 200, Tyr 192, Trp 202, Cys 189, Gly 207, Asp 217, Gly 190, Phe 205, Asp 208. There are hydrogen bonds at Thr 219, Ile 218, Asn 204, Asn 211, Arg 201, and Asn 204. The interaction energy between NPC1L1 and EGCG is -7.5 kCal/mol. Based on the results of the in-silico analysis, the researchers concluded that Catechins have the potential to be an NPC1L1 inhibitor. Further research into molecular dynamic simulation and in vivo analysis is required to demonstrate the synergistic effect of Catechins as an inhibitor of atherogenesis.

Abstract

Globally, people are experiencing increased feelings of stress, anxiety, and depression as a consequence of the situation that has arisen due to the current pandemic. Pregnant women also often feel anxious and afraid of being exposed to COVID-19, which has an impact on anxiety and mood disorders. Yoga is an ancient way of living in harmony with oneself (body, emotions, and mind) and nature and can improve individual health and reduce stress. This study aims to determine the average difference in anxiety in pregnant women after being given "prenatal tayo". This research is a quasi-experiment using a pretest-Posttest Control Group Design, with 25 respondents fulfilling the inclusion and exclusion criteria. The inclusion criteria in this study were pregnant women with gestational ages between 14 and ≤ 30 weeks, being able to read and write, and owning a smartphone. Exclusion criteria in this study were pregnant women with pregnancy complications and pregnant women with a history of previous miscarriages caused by a weak uterus or heart disease. The sampling technique is purposive sampling. Statistical tests using independent T test and Paired T-Test. There is a difference in the anxiety of pregnant women before and after being given "Tayo Prenatal". More research is needed regarding changes in anxiety in pregnant women measured every week to determine the effect of the intervention of giving "Tayo Prenatal" compared to giving Yoga.

Abstract

Moringa (MO) leaves contain several chemical substances of high value, including vitamin C, flavonoids, phenolic compounds, carotenoids, tannins, and triterpenoids, among which one functions as an antioxidant. Previous research observed that MO leaves have already been developed into various formulations; unfortunately, these formulas could not completely obscure the aroma and taste problems that rendered MO leaves less acceptable to consumers. In this research, Moringa leaf powder (MLP) and red ginger (RG) are combined to cover the unpleasant taste and aroma of MO leaves. The purpose of this study assess the quality of three tea bag formulations, including their water content, ash content, hedonic and antioxidant activities, by quantitative tests; F0 (3 Gr of MLP), F1 (2.25 Gr of MLP, and 0.75 Gr of RG), and F2 (the amount of MLP and RG is equal to 1.5 Gr). This experimental study employs a completely randomized design, three special treatments, and three repetitions. Statistical analysis of quality test results and antioxidant activity uses One-Way ANOVA followed by a post hoc test utilizing the Least Significant Difference (LSD). The water content tests revealed that all three formulations qualified the Indonesian National Standard (SNI), ranging from 3.86% to 4.29%. Adding ginger did not influence the water content (p > 0.05). Otherwise, it increases the ash level from 8.65% to 9.98%. Based on the results of the hedonic test, the panelists preferred F0 over F1 and F2 concerning color by a score of 86%. In contrast, they preferred F2 regarding aroma and taste by a score of 82%. The addition of RG in F1 and F2 was significant to the preference level of panelists ('p <0.05). The IC50 value for the antioxidant activity test was highest for F2 at 59.31 ± 0.13 ppm. The addition of RG boosted the antioxidant activity of MO tea products considerably (sig <0.05). Consequently, developing MO tea products containing RG in F2 could be potentially produced as herbal tea beverages with antioxidant properties.

Abstract

Dengue hemorrhagic fever remains a public health concern in West Nusa Tenggara Province, with the majority of cases occurring in Mataram City and the Pesinggahan Environment of West Pagesangan Village. The density of Aedes aegypti larvae as disease vectors has a strong influence on the high incidence of cases and the spread of dengue hemorrhagic fever. The objective of this study was to identify if there was a relation between the density of Aedes aegypti larvae and the rate of dengue hemorrhagic fever in the Pesinggahan Environment of Pagesangan Barat Village. This study is an analytic observational study with a cross-sectional design. This study's sample size was 56 houses. The data collected included information on the density of Aedes aegypti larvae and the incidence rate of dengue hemorrhagic fever in the Pesinggahan Environment of Pagesangan Barat Village over the previous year. Descriptive analysis was used to examine data on the density of Aedes aegypti larvae and the incidence rate of dengue hemorrhagic fever in the Pesinggahan environment. The findings revealed that the density of Aedes aegypti larvae was high (Density Figure (DF) = 6), as was the incidence rate of dengue hemorrhagic fever (0.39%). The statistical analysis between the density of A. aegypti larvae and the incidence rate of dengue hemorrhagic fever in the Pesinggahan Environment with the chi square test demonstrated indicate that p-value = 0.000 < alpha = 0.05. Based on the results of the study, there was a relationship between the density of A. aegypti larvae and the incidence rate of dengue hemorrhagic fever in Pesinggahan, Pagesangan Barat Village. Further research is required regarding the detection of the dengue virus transmitted by A. aegypti mosquitoes to their offspring (transovarial).

Abstract

Aphasia is a communication disorder that occurs after brain damage. Delays in diagnosing aphasia can lead to depression, decrease quality of life, and raise hospital stays and costs. Ischemic stroke aphasia screening instruments have been widely developed but are still difficult to interpret.  Nowadays, the Language Screening Test (LAST) is reportedly quick, easy to understand, straightforward, and suitable for use by nurses. However, the LAST subtests are different from the Frenchay Aphasia Screening Test (FAST), which has been used in the past to evaluate aphasia. This study aims to determine the relationship between LAST and FAST in ischemic stroke aphasic patients for aphasia screening. The study used an analytic observational method with a cross-sectional approach. The population in this study were stroke patients at Tugurejo Semarang Hospital who had their CT Scan checked. The sample was taken using a purposive sampling technique. Bivariate data analysis with Spearman Rank Correlation test. We obtained 50 samples 19 male patients and 31 female patients with the highest age range of 51-60 years as many as 26 patients. The highest Education and profession is primary school and housewife. Patients who experienced aphasia with LAST and FAST scored as many as 27 and 29 people, while the rest not have aphasia. The Spearman's Rank test obtained p-values of 0,000 and < 0,05 respectively. Meanwhile, the correlation coefficients of LAST and FAST on aphasia were 0,678 and 0,678 respectively. There is a positive correlation between the LAST and FAST in ischemic stroke aphasic patients for aphasia screening. Nurses can use the LAST instrument to collect data and establish nursing diagnoses of verbal communication disorders.

Abstract

Oral hygiene is an indicator of oral and dental health which can be assessed based on the presence or absence of organic deposits, such as pellicle, materia alba (dental deposit), food residue, calculus, and dental plaque. Plaque is the cause of tooth decay among the world's population. Plaque on the tooth surface can be used as an indicator of oral hygiene. Poor cleaning can lead to stickier plaque and tartar after calcification. The thin plaque is almost the same as the color of the teeth, so that plaque cannot be seen with the naked eye. The presence of plaque that is formed from contact with oral fluids can be detected by using certain dye. This study aims to determine the Effectiveness of Purple Sweet Potato Extract and Disclosing substance for Plaque Identification among the children in Aisyah Orphanage, Tilatang Kamang Sub-District, Agam District. This was a quasi-experimental study with a post-test-only design. The samples of this study were 30 children at Aisyah Orphanage, Tilatang Kamang Sub-District, Agam District, with the inclusion criteria of children with permanent teeth and index teeth. The samples were selected through total sampling technique. The results showed that after applying purple sweet potato extract to the children at Aisyah Orphanage in Bukittinggi City, most of them had the Patient Hygiene Performance index (PHP) in the moderate criteria (50%). Furthermore, after applying disclosing substances to the children at Aisyah Orphanage in Bukittinggi City, most of them had the PHP index in the poor criteria (56.7%). The t-test independent statistical test results obtained a p-value of 0.000 (p <0.005), indicating a significant difference between sweet potato extract and disclosing substance. Disclosing substance was more effective than purple sweet potato extract in identifying plaque on the tooth surface. Further study is recommended to apply purple sweet potato extract with different concentrations as an alternative ingredient to identify plaque on the tooth surface.

Abstract

Neutrophil Lymphocyte Ratio (NLR) is a laboratory available as a marker used for the evaluation of systemic inflammation, NLR is a significant predictor and is a critical prognosis for COVID-19 infection and can serve as a useful factor to reflect the intensity of the imbalance of inflammation and immune response in COVID-19 patients. This study aims to determine the difference in NLR values in negative and confirmed COVID-19 patients and description for comorbid for both. This study is an analytic observational study with a cross-sectional design. The study sample was 423 suspected COVID-19 patients at hospitals in Cilacap district for the period in March – October 2020. The data obtained were analyzed descriptively and using the fisher-exact test. In these results from suspected patients with negative COVID-19, lung illness were present 31.8%, viral infections 22.9%, other respiratory disorders 6.1%, diabetes mellitus 4.7%, and anemia 4.7%. Whereas suspected patients with confirmed COVID-19 were, without comorbid diseases (40.2%), lung disease (12.4%), diabetes mellitus (7.7%), hypertension (6.2%), and other respiratory illnesses (5.2%). The mean of NLR in confirmed patients is 3.57 but not any difference between negative and confirmed patients COVID-19, but there’s any a relationship between NLR and ARDS conditions.
