Abstract

Most studies tended to focus on the association of health conditions or tantrum behavior in toddlers with maternal mental health. This study aimed to determine the differences in WHO SRQ-20 scores and the association of WHO SRQ-20 outcomes with fecal disposal factors and drinking water sources. This study used secondary data from RISKESDAS 2018, involving 670 mothers with children aged 6-23 months. The WHO SRQ-20 questionnaire was used to measure general mental health disorders. Mann-Whitney analysis was conducted to determine the difference in scores, while Chi-Square was used to examine the association of WHO SRQ-20 outcomes with toddler feces disposal factors and drinking water sources. All analyses were performed using SPSS 22.0, with a significance value of <0.05. The analysis showed that inappropriate fecal disposal and poor drinking water sources increased the total WHO SRQ-20 score (p=0.003, & p=0.041). Both factors were positively correlated with seven common mental health symptoms in mothers with children aged 6-23 months, including no appetite, difficulty thinking, difficulty making decisions, feelings of fear, trembling hands, and indigestion, with significant p-values (p<0.05) for each symptom. Environmental quality, especially inappropriate fecal disposal and poor drinking water sources, was significantly correlated with increased mental health symptoms among mothers with children aged 6-23 months.

Abstract

Background: The WHO (2019) reported that approximately 10% of pregnant women and 13% of postpartum women globally experience maternal mental health disorders. Several studies have identified unintended pregnancy as a contributing factor to these disorders. This systematic review and meta-analysis aimed to analyze and estimate the association between unintended pregnancy and maternal mental health disorders (MHD), using studies that applied the WHO Self-Reporting Questionnaire-20 (SRQ-20).Subjects and Method: A systematic review and meta-analysis followed PRISMA guidelines and was registered in PROSPERO (CRD42025634410). Article searches were conducted up to January 15, 2025, in PubMed, ScienceDirect, Google Scholar, and Scopus using the keywords: “SRQ” AND (“mental health” OR “psychological well-being” OR “mental disorder”) AND (“unwanted pregnancy” OR “unintended pregnancy” OR “unplanned pregnancy”) AND (“mother” OR “maternal”). Inclusion criteria focused on observational studies using the WHO SRQ-20 to assess maternal mental health among women with unintended and intended pregnancies. Meta-analysis was performed using R Studio, with effect sizes reported as risk ratios (RR) or adjusted odds ratios (aOR) and 95% confidence intervals. Heterogeneity was assessed using the I² statistic, and funnel plots were generated to examine publication bias. Results: A total of 12 studies from Ethiopia, Indonesia, India, Vietnam, Brazil, and Malawi, comprising 85,862 participants, were included in this meta-analysis. A total of 12 studies with a total of 85,862 participants were analyzed in this meta-analysis. The results showed that unintended pregnancy significantly increased the risk of maternal MHD. This finding was consistent in both cohort and cross-sectional studies, with effect sizes of (RR = 1.49; CI95%= 1.03 to 2.15; p= 0.030) and (aOR= 1.83; CI95%= 1.35 to 2.47; p < 0.001), respectively. Conclusion: These findings highlight the strong association between unintended pregnancy and MHD. Limitations include high heterogeneity among studies, potential publication bias, and the predominance of observational designs, which preclude causal inferences. Despite these limitations, the results underscore the need for interventions that prioritize the overall health of pregnant women.

Abstract

Loss to follow (LTFU) -up among HIV patients, defined as failure to continue therapy as schduled, increases the risk of drug resistance, deteriorating health conditions, and death due to HIV-relatedcomplications. This study aimed to analyze the survival of HIV patients againts LTFU events amongthose undergoing antiretroviral therapy (ARV) in Samarinda City. Secondary data from the SIHAdatabase (2023-2024) were utilized. Kaplan-Meier analysis was applied to evaluate the factors ofgender, residence, population group, and timing of ARV initiation associated with the risk of LTFU.The results showed that gender and residance were not significantly associated with the risk of LTFU.However, specific population groups, such as children of PLHIV and pregnant women, exhibitedhigher vulnerability and lower survival rates (log rank = 0.015). ART initiation within ≤ 1 monthsignificantly improved survival probability compared to initiation after > 1 month. These findinghighlight that while gender and residance do not significantly influence LTFU, the vulnerability ofspesific populations and the critical importance of early ART initiation are key factors. It isrecommende to develop targeted interventions focusing on vulnerable groups, such as children ofPLHIV and pregnant women, to enhance their survival rate. Additionally, strategies promoting ARTinitiation within ≤ 1 month are esential to improve treatment sustainability.

Abstract

Background : Non-communicable diseases (NCDs) have become a major public health concern, with risk factors increasingly observed among younger populations. Examining how these risks differ between age groups may provide insight into early prevention priorities. Objective : To describe and compare behavioral and biological NCD risk factors among older adolescents (15–19 years), young adults (20–24 years), and adults (25–59 years) in Samarinda, Indonesia. Research Methods/ Implementation Methods : A descriptive cross-sectional study was conducted among 242 respondents selected through household interviews in Samarinda. Data were collected using the WHO STEPwise approach to NCD risk factor surveillance (STEPS) questionnaire, covering behavioral factors (tobacco, alcohol, physical activity, diet) and biological factors (BMI, blood pressure, blood glucose, cholesterol). Data were analyzed descriptively using STATA 17. Results : Variations were observed across age groups. Smoking was more frequent among young adults (22.2%) compared to adolescents (9.3%) and adults (17.7%). Alcohol use was generally low but relatively higher among adolescents (12.4%). Physical activity levels and sedentary time showed comparable patterns, with young adults reporting slightly more sedentary behavior (40%). Most respondents consumed fruits less than four days per week. The proportion of overweight and obesity was greater among adults (54.4%) than in young adults (24.4%) and adolescents (19.4%). Differences were also seen in clinical indicators, with adults showing a higher proportion of hypertension (44.1%) and high cholesterol (35.3%) compared to younger groups. Conclusion/Lesson Learned : The study highlights varying patterns of NCD risk factors among age groups, with several behavioral risks already present during adolescence. These findings emphasize the importance of early preventive actions focused on healthy lifestyles, especially among youth, supported by community-based education and health monitoring initiatives.

Abstract

Pasundan Public Health Center ranks among the top three areas with the highest prevalence of Non- Communicable Diseases (NCDs) in Samarinda City, with 23.2% hypertension and 2.95% diabetes mellitus cases. The main challenge lies in the suboptimal role of health cadres in implementing the Integrated Primary Care (ILP) program and their limited digital health literacy. This issue highlights a practical and scientific gap in the effective utilization of health cadres for NCD prevention and control through technology-based health education. To optimize the role of health cadres in the implementation of ILP by improving their knowledge, communication, and digital literacy skills for NCD prevention and control in tropical regions. The program was implemented through community service activities, including socialization, health education, and practical demonstrations involving 37 health cadres from 35 integrated health posts. Evaluation of outcomes was conducted using interactive pre–post tests and participant feedback to measure improvements in knowledge and skills. The evaluation showed a significant increase in cadres’ knowledge from 68% to 89%, along with an improvement in communication and digital literacy skills by 35–40%. The training enhanced cadres’ confidence and ability to utilize digital platforms in disseminating health information related to NCD prevention and control. The program successfully strengthened the capacity of health cadres in technology-based health promotion, supporting the sustainability and effectiveness of Integrated Primary Care at the primary healthcare level. Enhancing digital literacy among cadres is crucial for improving NCD prevention and control efforts in tropical regions.
