Abstract

Background: The WHO (2019) reported that approximately 10% of pregnant women and 13% of postpartum women globally experience maternal mental health disorders. Several studies have identified unintended pregnancy as a contributing factor to these disorders. This systematic review and meta-analysis aimed to analyze and estimate the association between unintended pregnancy and maternal mental health disorders (MHD), using studies that applied the WHO Self-Reporting Questionnaire-20 (SRQ-20).Subjects and Method: A systematic review and meta-analysis followed PRISMA guidelines and was registered in PROSPERO (CRD42025634410). Article searches were conducted up to January 15, 2025, in PubMed, ScienceDirect, Google Scholar, and Scopus using the keywords: “SRQ” AND (“mental health” OR “psychological well-being” OR “mental disorder”) AND (“unwanted pregnancy” OR “unintended pregnancy” OR “unplanned pregnancy”) AND (“mother” OR “maternal”). Inclusion criteria focused on observational studies using the WHO SRQ-20 to assess maternal mental health among women with unintended and intended pregnancies. Meta-analysis was performed using R Studio, with effect sizes reported as risk ratios (RR) or adjusted odds ratios (aOR) and 95% confidence intervals. Heterogeneity was assessed using the I² statistic, and funnel plots were generated to examine publication bias.Results: A total of 12 studies from Ethiopia, Indonesia, India, Vietnam, Brazil, and Malawi, comprising 85,862 participants, were included in this meta-analysis. A total of 12 studies with a total of 85,862 participants were analyzed in this meta-analysis. The results showed that unintended pregnancy significantly increased the risk of maternal MHD. This finding was consistent in both cohort and cross-sectional studies, with effect sizes of (RR = 1.49; CI95%= 1.03 to 2.15; p= 0.030) and (aOR= 1.83; CI95%= 1.35 to 2.47; p < 0.001), respectively.Conclusion: These findings highlight the strong association between unintended pregnancy and MHD. Limitations include high heterogeneity among studies, potential publication bias, and the predominance of observational designs, which preclude causal inferences. Despite these limitations, the results underscore the need for interventions that prioritize the overall health of pregnant women.

Abstract

Loss to follow (LTFU) -up among HIV patients, defined as failure to continue therapy as schduled,increases the risk of drug resistance, deteriorating health conditions, and death due to HIV-relatedcomplications. This study aimed to analyze the survival of HIV patients againts LTFU events amongthose undergoing antiretroviral therapy (ARV) in Samarinda City. Secondary data from the SIHAdatabase (2023-2024) were utilized. Kaplan-Meier analysis was applied to evaluate the factors ofgender, residence, population group, and timing of ARV initiation associated with the risk of LTFU.The results showed that gender and residance were not significantly associated with the risk of LTFU.However, specific population groups, such as children of PLHIV and pregnant women, exhibitedhigher vulnerability and lower survival rates (log rank = 0.015). ART initiation within ≤ 1 monthsignificantly improved survival probability compared to initiation after &gt; 1 month. These findinghighlight that while gender and residance do not significantly influence LTFU, the vulnerability ofspesific populations and the critical importance of early ART initiation are key factors. It isrecommende to develop targeted interventions focusing on vulnerable groups, such as children ofPLHIV and pregnant women, to enhance their survival rate. Additionally, strategies promoting ARTinitiation within ≤ 1 month are esential to improve treatment sustainability.

Abstract

Non-communicable diseases (NCDs) are a major public health concern, with risk factors increasingly observed among younger populations. To describe and compare behavioral and biological NCD risk factors among adolescents (15–19 years), young adults (20–24 years), and adults (25–59 years) in Samarinda, Indonesia. A cross-sectional study was conducted among 242 respondents using the WHO STEPS questionnaire, covering behavioral factors (tobacco, alcohol, physical activity, diet) and biological factors (BMI, blood pressure, blood glucose, cholesterol). Smoking was highest among young adults (22.2%) versus adolescents (9.3%) and adults (17.7%). Adolescents reported relatively higher alcohol use (12.4%). Young adults showed more sedentary behavior (40%). Fruit consumption was inadequate across all groups. Overweight/obesity prevalence increased with age: adolescents (19.4%), young adults (24.4%), adults (54.4%). Adults showed higher hypertension (44.1%) and high cholesterol (35.3%). Behavioral risks emerge during adolescence, while biological risks accumulate with age. Early prevention targeting youth through community-based education and health monitoring is essential.

Abstract

Toxoplasmosis remains a persistent public health concern among women of reproductive age due to the risk of congenital transmission and severe fetal outcomes. This narrative review aimed to synthesize recent evidence on infection determinants and evaluate evidence-based strategies to mitigate risks, addressing persistent gaps in awareness and intervention efficacy. A comprehensive literature search of PubMed and Google Scholar was conducted for studies published between 2016 and 2025. The findings reveal substantial geographic variation in seroprevalence, influenced by environmental conditions, cultural practices, and socioeconomic disparities. Higher infection rates were consistently reported among older, multiparous, rural, and less-educated women. Key risk factors included consumption of contaminated water, undercooked meat, and poor hygiene practices, while associations with cat ownership were inconsistent. Overall, knowledge and awareness of toxoplasmosis were low among women and healthcare providers. Although educational interventions and antenatal screening demonstrated potential benefits, their effectiveness remains insufficiently evaluated. Integrated, context-specific prevention strategies are urgently needed to reduce infection risk and prevent congenital toxoplasmosis.

Abstract

The total number of dengue cases reported nationally has reached 88,593, resulting in 621 deaths in 456 districts/cities in 34 provinces. Temindung Health Center is the health center with the highest cases in Samarinda City in 2023. This study aims to analyze the distribution of DHF cases in the working area of Temindung Health Center in Samarinda City. This study uses the Nearest Neighbour Analysis method to analyze the spatial pattern of the distribution of DHF cases and shows that the distribution of cases does not occur randomly which is complemented by the buffer analysis method to identify risk areas based on geographical proximity to the Puskesmas of DHF cases in January-September 2024. The results showed that there were 71 cases of DHF in the working area of Temindung Health Center, where the peak of cases occurred in January with 19 cases. The results of Nearest Neighbor Analysis showed a nearest neighbor index value of 0.648 and a Z-score of -5.67, which indicated that the distribution pattern of DHF cases was very clustered, the results of the buffer analysis showed that the distribution of cases occurred mostly around the 500 meter zone from the Puskesmas. DHF prevention and control efforts should focus on areas with high case cluster patterns, especially around densely populated zones and far from access to health facilities, by strengthening active surveillance and risk area-based interventions.

Abstract

Schizophrenia contributes significantly to global disease burden, with hospitalization management being crucial for treatment outcomes. This study aimed to determine factors influencing length of stay and recovery outcomes among schizophrenia patients in Indonesian referral health facilities. Secondary data analysis was conducted using BPJS Kesehatan sample data from 2023. Of 54,820 mental health records, 2,562 schizophrenia patients treated at referral facilities were analyzed. Cox proportional hazards regression was used to identify factors associated with discharge status, while Kaplan-Meier analysis examined length of stay distribution. Mean length of stay was 19.9 days. Significant factors associated with better recovery outcomes included: treatment at general hospitals (HR=2.31, p<0.001), public hospitals (HR=0.57, p<0.001), undifferentiated (HR=1.45, p<0.001) and catatonic schizophrenia subtypes (HR=1.70, p=0.029), single medical procedure (HR=0.73, p<0.001), and local treatment facilities (HR=1.42, p=0.028). Healthcare service factors and clinical characteristics significantly outweigh demographic factors in determining recovery outcomes. These findings emphasize the importance of healthcare service quality and accessibility in schizophrenia treatment, supporting integrated care models and universal healthcare access policies.

Abstract

Abstrak: Puskesmas Pasundan merupakan salah satu puskesmas dengan prevalensi Penyakit Tidak Menular (PTM) tertinggi di Kota Samarinda, dengan 23,2% penderita hipertensi dan 2,95% penderita diabetes melitus. Permasalahan yang dihadapi mitra adalah belum optimalnya peran kader dalam pelaksanaan Integrasi Layanan Primer (ILP) serta rendahnya literasi kesehatan digital. Kegiatan pengabdian ini bertujuan meningkatkan pemahaman dan keterampilan kader kesehatan melalui pelatihan literasi digital berbasis ILP. Metode kegiatan meliputi sosialisasi, penyuluhan, dan demonstrasi yang diikuti oleh 37 kader dari 35 posyandu wilayah kerja Puskesmas Pasundan. Evaluasi dilakukan melalui pre–post test interaktif dan umpan balik peserta. Hasil kegiatan menunjukkan peningkatan pengetahuan kader dari 68% menjadi 89% dan peningkatan keterampilan komunikasi serta literasi digital sebesar 35–40%. Kegiatan ini memperkuat kapasitas kader dalam promosi kesehatan berbasis teknologi dan mendukung keberlanjutan ILP di tingkat pelayanan primer.Abstract: Pasundan Public Health Center is among the top three with the highest prevalence of Non-Communicable Diseases (NCDs) in Samarinda City, with 23.2% hypertension and 2.95% diabetes mellitus cases. The main issue faced by the partner is the suboptimal role of health cadres in implementing the Integrated Primary Care (ILP) and limited digital health literacy. This community service aimed to enhance cadres’ understanding and skills through digital literacy training based on the ILP approach. Activities included socialization, health education, and demonstrations involving 37 cadres from 35 integrated health posts. Evaluation was conducted using interactive pre–post tests and participant feedback. Results showed an increase in cadre knowledge from 68% to 89% and improvement in communication and digital literacy skills by 35–40%. The program strengthened cadres’ capacity in technology-based health promotion and supported the sustainability of ILP at the primary healthcare level.

Abstract

Abstrak Wilayah kerja Puskesmas Palaran di Samarinda dihadapkan dengan berbagai tantangan kesehatan dalam 3 tahun terakhir, termasuk tingginya angka penyakit menular seperti tuberkulosis, serta masalah stunting dan malnutrisi pada balita yang diperparah oleh komplikasi kehamilan pada ibu dengan obesitas, hipertensi, dan diabetes. Program pengabdian masyarakat ini dirancang untuk memperkuat kapasitas kader kesehatan melalui ceramah, demonstrasi serta praktik langsung, sehingga mereka dapat berkontribusi secara efektif dalam pencegahan dan deteksi dini masalah kesehatan masyarakat. Kegiatan dilaksanakan pada tanggal 16 November 2024 bertempat di Puskesmas Palaran, Kecamatan Palaran, Kota Samarinda. Sebanyak 20 orang kader kesehatan di wilayah kerja Puskesmas Palaran, Kecamatan Palaran, Kota Samarinda menjadi sasaran dalam kegiatan ini. Pemberian edukasi dilakukan melalui presentasi materi, pembagian modul kesehatan serta praktik pemeriksaan kesehatan oleh kader, yang diawali dengan pemberian pre-test kemudian pemberian post-test pada sesi akhir. Hasil menunjukkan adanya peningkatan pengetahuan kader yang ditandai dengan peningkatan jumlah kader yang memiliki pengetahuan yang baik setelah mendapatkan edukasi dan pelatihan (TB: 5 menjadi 16 orang (220%), PTM: 13 menjadi 18 orang (38%), Antropometri: 16 menjadi 20 orang (25%)). Hasil tersebut menunjukkan adanya pengaruh positif dari pemberian edukasi terhadap tingkat pengetahuan kader kesehatan terkait TB, PTM (HT dan DM) serta Antropometri (Uji wilcoxon : p < 0.05). Kader posyandu kini lebih memahami pentingnya deteksi dini TBC, pemantauan pertumbuhan balita menggunakan antropometri, dan pencegahan serta pengelolaan penyakit tidak menular. Direkomendasikan untuk melaksanakan pelatihan kader secara rutin dengan materi terkini, didukung oleh monitoring dan evaluasi untuk memastikan penerapan di lapangan. Kata kunci: edukasi; hipertensi; kader kesehatan; pemberdayaan; tuberkulosis Abstract Palaran Public Health Center (Puskesmas Palaran) in Samarinda has faced various health challenges over the past three years, including a high incidence of infectious diseases such as tuberculosis (TB), as well as issues related to stunting and malnutrition among children under five. These conditions are further exacerbated by pregnancy complications in mothers with obesity, hypertension, and diabetes. This community service program was designed to strengthen the capacity of health cadres through lectures, demonstrations, and hands-on practice, enabling them to contribute effectively to the prevention and early detection of public health problems. The activity was conducted on November 16, 2024, at the Palaran Public Health Center, Palaran Sub-district, Samarinda City. A total of 20 health cadres in the Palaran working area participated in this initiative. The educational sessions were delivered through material presentations, distribution of health modules, and practical health check-ups conducted by the cadres. These activities were preceded by a pre-test and concluded with a post-test. The results showed an increase in cadre knowledge, as indicated by the rise in the number of cadres with good understanding following the education and training sessions (TB: from 5 to 16 cadres, a 220% increase; NCDs: from 13 to 18 cadres, a 38% increase; Anthropometry: from 16 to 20 cadres, a 25% increase). These findings indicate a positive impact of the educational intervention on cadre knowledge related to TB, non-communicable diseases (NCDs: hypertension and diabetes), and anthropometric measurement (Wilcoxon test: p < 0.05). As a result, Posyandu cadres now have a better understanding of the importance of early detection of TB, monitoring child growth using anthropometry, and the prevention and management of non-communicable diseases. It is recommended that cadre training be conducted regularly with updated materials, supported by monitoring and evaluation to ensure effective implementation in the field.. Keywords: education, empowerment, health cadres, hypertension, tuberculosis
