Abstract

An incision in the abdominal wall is made during laparatomy, a significant surgical surgery, and the healing duration for this incision can vary. Patients should engage in early mobilization, or physical activity to increase blood circulation and decrease the risk of problems, right after surgery. This can hasten the healing process of wounds. The purpose of this research is to find out how early mobilization affects wound healing, pain levels, and hospital stay duration for patients at Royal Prima Hospital Medan who have undergone a laparotomy. Using a cross-sectional design, this study employed quantitative analytical methods. Patients who underwent a laparotomy between 2020 and 2024 made up the study's population. Using inclusion and exclusion criteria, a convenience sample of 35 individuals was chosen. The data was examined using the ChiSquare test, which was derived from patient medical records. Early mobilization was performed by the majority of patients (91.4%), and 88.6% of those patients had satisfactory wound healing. "Significant relationships between early mobilization and wound healing (p = 0.000), pain severity (p = 0.003), and length of hospital stay (p = 0.018) were shown by statistical tests. It is evident that early mobilization is crucial for patients to experience faster wound healing, less pain, and a shorter hospital stay following laparotomy. In conclusion, patients' wound healing processes are significantly improved by early mobilization following laparotomy. The goal of healthcare providers is to educate patients and routinely practice early mobilization in order to hasten healing and avoid complications after surgery.

Abstract

Diabetes mellitus is a chronic disease that requires continuous monitoring of blood sugar levels to prevent serious complications. Wearable technology has emerged as an innovative solution for health monitoring, especially for people with diabetes. This study aims to evaluate the effectiveness of wearable technology in monitoring blood sugar levels in diabetes patients in Medan. The research employed a systematic literature review, analyzing scientific sources from PubMed, Google Scholar, ScienceDirect, and IEEE Xplore databases published between 2014 and 2024. Inclusion criteria encompassed studies on wearable technology for diabetes monitoring with clear methodologies, while exclusion criteria eliminated duplicates, irrelevant articles, or those with weak methodologies. From an initial 150 articles, 45 studies involving a total of 2,456 participants were ultimately analyzed. The results showed that wearable technologies—such as smartwatches and continuous glucose monitoring (CGM) devices—offer convenience in real-time monitoring, increasing patient compliance by 97%, lowering HbA1c by 0.8%, increasing time in range by 16%, and reducing hypoglycemia episodes by 55%. However, challenges persist, including measurement accuracy, FDA validation, high costs, and individual variability, all of which affect result consistency. Implementation in the Medan area remains limited due to economic and infrastructure factors. This study concludes that wearable technology holds great potential for improving the quality of life of people with diabetes, but further research is needed to validate its accuracy and reliability, along with policy support to enhance accessibility in Indonesia. The findings have significant implications for healthcare policy development, suggesting the need for government subsidies and insurance coverage to boost technology adoption rates among Indonesian diabetes patients, particularly in urban settings like Medan, where diabetes prevalence continues to rise

Abstract

Inpatient medical records have an important role as a source of data and health information for inpatients related to treatment and care planning. The main factor that causes problems in inpatient medical records is the undisciplined factor of the person in charge who handles patients in filling out medical records. This can cause inpatient medical record management, not in accordance with medical record service standards. This study aimed to analyze inpatient medical record management based on medical record service standards. This research is a quantitative analytic study with a cross-sectional approach. The study was conducted at the Government Hospital in Medan. The research population was 126 people and all of them were used as samples. Sampling by means of total sampling. Data analysis used univariate, bivariate with chi-square test, and multivariate with multiple logistic regression at 95% confidence level (0.05). The results showed that inpatient medical record management implementation based on medical record service standards at the Government Hospital in Medan was influenced by work motivation (p = 0.001) and supervision (p = 0.000). While the variables that have no effect are length of work (p = 1,000), education (p = 0.848), teamwork (p = 0.241), and completeness of infrastructure (p = 0.760). The supervision variable is the most dominant variable or has the greatest influence with the value of Exp(B)/OR = 19,542. Medical record employees who state that supervision is carried out well by superiors will improve the implementation of inpatient medical record management well, compared to those who state that supervision is still not good. It is recommended that the leadership carry out inherent supervision (waskat) to all medical record employees and provide motivation through motivational seminars to improve the implementation of inpatient medical record management based on medical record service standards.
