Abstract

Diabetes mellitus is a chronic disease that requires continuous monitoring of blood sugar levels to prevent serious complications. Wearable technology has emerged as an innovative solution for health monitoring, especially for people with diabetes. This study aims to evaluate the effectiveness of wearable technology in monitoring blood sugar levels in diabetes patients in Medan. The research employed a systematic literature review, analyzing scientific sources from PubMed, Google Scholar, ScienceDirect, and IEEE Xplore databases published between 2014 and 2024. Inclusion criteria encompassed studies on wearable technology for diabetes monitoring with clear methodologies, while exclusion criteria eliminated duplicates, irrelevant articles, or those with weak methodologies. From an initial 150 articles, 45 studies involving a total of 2,456 participants were ultimately analyzed. The results showed that wearable technologies—such as smartwatches and continuous glucose monitoring (CGM) devices—offer convenience in real-time monitoring, increasing patient compliance by 97%, lowering HbA1c by 0.8%, increasing time in range by 16%, and reducing hypoglycemia episodes by 55%. However, challenges persist, including measurement accuracy, FDA validation, high costs, and individual variability, all of which affect result consistency. Implementation in the Medan area remains limited due to economic and infrastructure factors. This study concludes that wearable technology holds great potential for improving the quality of life of people with diabetes, but further research is needed to validate its accuracy and reliability, along with policy support to enhance accessibility in Indonesia. The findings have significant implications for healthcare policy development, suggesting the need for government subsidies and insurance coverage to boost technology adoption rates among Indonesian diabetes patients, particularly in urban settings like Medan, where diabetes prevalence continues to rise

Abstract

Inpatient medical records have an important role as a source of data and health information for inpatients related to treatment and care planning. The main factor that causes problems in inpatient medical records is the undisciplined factor of the person in charge who handles patients in filling out medical records. This can cause inpatient medical record management, not in accordance with medical record service standards. This study aimed to analyze inpatient medical record management based on medical record service standards. This research is a quantitative analytic study with a cross-sectional approach. The study was conducted at the Government Hospital in Medan. The research population was 126 people and all of them were used as samples. Sampling by means of total sampling. Data analysis used univariate, bivariate with chi-square test, and multivariate with multiple logistic regression at 95% confidence level (0.05). The results showed that inpatient medical record management implementation based on medical record service standards at the Government Hospital in Medan was influenced by work motivation (p = 0.001) and supervision (p = 0.000). While the variables that have no effect are length of work (p = 1,000), education (p = 0.848), teamwork (p = 0.241), and completeness of infrastructure (p = 0.760). The supervision variable is the most dominant variable or has the greatest influence with the value of Exp(B)/OR = 19,542. Medical record employees who state that supervision is carried out well by superiors will improve the implementation of inpatient medical record management well, compared to those who state that supervision is still not good. It is recommended that the leadership carry out inherent supervision (waskat) to all medical record employees and provide motivation through motivational seminars to improve the implementation of inpatient medical record management based on medical record service standards.

Abstract

Nurse performance is a determining factor in the success of achieving hospital goals in nursing services. Various studies on the performance of nurses in Indonesia show that the performance of nurses is still relatively low between 47.6% - 53.4%. Nurse performance is thought to be influenced by 3 factors, namely individual factors, psychological factors and organizational factors. The purpose of this study was to analyze the effect of characteristic, psychological, and organizational factors on nurse performance. This research is a quantitative research with a cross sectional study approach. The research was conducted at the Royal Prima Hospital, Medan. The research population was 367 people, the sample obtained was 115 people. Sampling by purposive sampling. Data analysis was univariate, bivariate with chi-square test, and multivariate with multiple logistic regression at 95% confidence level ( a=0.05). The results showed that the factors that influence the performance of nurses at the Royal Prima General Hospital Medan are psychological factors (job satisfaction) (p=0.011), organizational factors (leadership and organizational culture) respectively p = 0.004 and p = 0.002. Individual characteristic factors had no effect, namely age (p=1,000), gender (p=0.661), education (p=0.454), and years of service (p=0.090). The most dominant variable influencing the nurse's performance is the leadership variable, Exp(B)/OR = 6.633, meaning that the nurse who states leadership is in a good category has a 6.6 times higher chance of having a good performance than the nurse who states that leadership is not good. It is recommended that the leadership of the Royal Prima General Hospital Medan to provide guaranteed incentives to all nurses so as to increase satisfaction. Head of the room to demonstrate leadership that is acceptable to all parties. Hospitals should maintain and improve the organizational culture that has been good so far.

Abstract

Insulin resistance is a condition with damage to target organs that generally respond to the activity of the hormone insulin. About 10% of the population in big cities in Indonesia suffer from insulin resistance. At the cellular level, insulin resistance can result from damage to the pre-receptor, receptor, or post-receptor. The most common cause of insulin resistance is post-receptor damage, namely disruption and damage to insulin signaling pathways. To find out that the methanol extract of Sunkist orange peel can reduce blood sugar levels in Wistar rats. This research uses the experimental laboratory method in vivo with the chosen research design form, namely True Experimental Pre Test and Post Test Control. The Kruskal Wallis test showed that the Sunkist Orange Peel Methanol Extract resulted in a significant decrease in blood sugar levels of Wistar rats with a p-value <0.05, and also the Anova test showed that the Sunkist Orange Peel Methanol Extract resulted in a decrease in blood sugar levels of Wistar rats, which is significant with p-value <0.05. All doses tested affect lowering blood sugar levels, namely, 300 mg/kg BW, 450 mg/kg BW, and 600 mg/kg BW.
