Abstract

Introduction: Malaria and dengue are endemic in Indonesia, and therefore, may result in the possibility of co-infection. Urban demographic expansion, deforestation, and agricultural settlements in peri-urban areas are known causes of the increase in the probability of concurrent infection of these two diseases. It is reasonable to envisage that the occurrence of concurrent infections would not be rare, mainly during outbreaks of dengue in the rainy season. However, due to non-systematic investigation of both diseases, only a few cases of malaria and dengue co-infection have been reported. These three patients demonstrated co-infection manifestations with good outcomes and prognoses. Case Illustrations: Two cases of co-infection of malaria vivax and dengue fever in male patients aged 35 and 43 years. The difference between these two cases is that one patient had experienced a previous episode of malaria with a fever lasting less than 5 days. Meanwhile, one patient who had never suffered from malaria experienced a relatively longer fever episode, up to 14 days. Thrombocytopenia in these two patients was still above 50,000/µL and rose quickly after anti-malarial administration. The third case was a 47-year-old woman with co-infection of malaria falciparum and dengue. This woman lived in Papua and had been infected with malaria before. This third case showed platelet numbers reaching levels as low as 38,000/µL and hypotension without signs of shock in the critical phase of dengue. The three patients did not show complications from other organs due to malaria or dengue and responded well to antimalarials with good outcomes. These patients were treated with a combination of DHP and primaquine, along with supportive care for fever and fluid management. Conclusion: Whenever co-infection is confirmed, we recommend careful monitoring for bleeding and hepatic complications, which may result in a higher chance of severity. Careful and intense monitoring of fluid requirements is necessary to avoid complications of fluid overload.

Abstract

Background Sepsis remains a leading cause of intensive-care mortality, primarily through multiple organ dysfunction syndrome (MODS) driven by overlapping pro-inflammatory and oxidative-stress pathways. Interleukin-6 (IL-6) and malondialdehyde (MDA) represent these two pathways, yet few studies have correlated both biomarkers with histopathological damage across five vital organs simultaneously. Therefore, this study aimed to analyze the association of serum IL-6 and MDA levels with multiorgan tissue-damage scores in an Escherichia coli-induced rat sepsis model. Methods This true-experimental, post-test-only control group study used nine male Wistar rats (Rattus norvegicus) allocated to three groups (n=3 each): an uninduced control and two groups given intraperitoneal Escherichia coli at 1 × 10⁶ and 1.5 × 10⁶ CFU/kgBW. Serum IL-6 (ELISA) and MDA (TBARS) levels, alongside semi-quantitative histopathology scores of five organs (liver, spleen, heart, kidney, and lung), were assessed 72 hours post-induction. Associations were analyzed using the Spearman correlation test. Results IL-6 correlated significantly with damage scores of the liver (r=0.828; p=0.006), spleen (r=0.887; p=0.001), heart (r=0.917; p<0.001), total lung score (r=0.874; p=0.002), and kidney (r=0.678; p=0.045), but not with alveolar dilatation (r=0.390; p=0.300). MDA correlated very strongly with the liver (r=0.931), spleen (r=0.965), heart (r=0.952), and total lung score (r=0.952; all p<0.001), but not significantly with the kidney (r=0.574; p=0.106). Furthermore, IL-6 and MDA were strongly inter-correlated (r=0.879; p=0.002). Conclusions In this Escherichia coli-induced rat sepsis model, serum IL-6 and MDA were both associated with the degree of multiorgan tissue damage and were strongly correlated with each other, a pattern compatible with coupling between inflammatory and oxidative pathways. Renal damage showed the weakest associations of the organs examined, suggesting organ-specific differences that warrant targeted investigation.

Abstract

Malaria adalah masalah kesehatan masyarakat yang serius yang terus menyebabkan penyakit dan kematian. Plasmodium knowlesi merupakan parasit malaria yang ditemukan di alam pada kera ekor panjang dan ekor babi. Plasmodium knowlesi, yang awalnya diketahui menyebabkan malaria simian, kini diakui sebagai parasit malaria kelima pada manusia.Infeksi pengetahuan pada manusia telah dilaporkan di hampir semua negara di Asia Tenggara dan pada wisatawan yang kembali dari negara-negara tersebut.Kasus kumulatif malaria pengetahuan di wilayah Asia Tenggara pada tahun 2004 hingga 2015 adalah 3413 kasus dengan 91,47% diantaranya terdapat di Kalimantan Malaysia. Spektrum penyakit klinis akibat infeksi Plasmodium knowlesi berkisar dari infeksi tanpa gejala, hingga malaria berat dan kematian.Kematian akibat malaria Knowlesi telah terdokumentasi dengan baik, dan P. Knowlesi kini menjadi penyebab kematian terbanyak akibat malaria di Malaysia. Tujuan penulisan agar semua orang mengetahui bahwa terdapat jenis malaria lain yang tersebar di wilayah tertentu yang sama membahayakan dengan jenis lain dan segera mendapat terapi.

Abstract

Tetanus merupakan suatu keadaan toksemia akut yang disebabkan oleh neurotoksin yang dihasilkan Clostridium tetani. Neurotoksin ini akan menghambat pelepasan neurotransmiter di sistem saraf pusat, menyebabkan spasme otot. Sehingga gejala tetanus ditandai dengan adanya spasme otot yang periodik dan progresif. Tetanus diklasifikasikan berdasarkan lokasi menjadi empat jenis meliputi tetanus lokal, sefalik, generalisata (umum) dan neonatorum. Tetanus generalisata merupakan jenis yang paling sering ditemui di lapangan. Prevalensi tetanus tinggi di negara dengan sumber daya rendah, dengan mortalitas mencapai 20-45%. Diagnosis tetanus dapat ditegakkan berdasarkan temuan klinis, riwayat luka terbuka dan riwayat imunisasi. Staging dan prognosis tetanus dilihat berdasarkan kriteria Pattel Joag atau Klasifikasi Albleets. Laporan kasus ini menyampaikan seorang pria berusia 60 tahun dan 78 tahun yang dirawat di RS PKU Solo Provinsi Jawa Tengah dengan keluhan kaku pada seluruh tubuh. Perbedaan antara kedua kasus di mana pada kasus 1 terdapat fokus infeksi tetanus, grade 2 tetanus generalisata. Kasus 2 tidak terdapat focus infeksi, Riwayat imunisasi tidak diketahui, kejang general, suara tambahan paru, dan grade 3 tetanus.

Abstract

Latar Belakang: Tetanus adalah penyakit infeksi akut yang disebabkan oleh Clostridium tetani, bakteri anaerob yang menghasilkan toksin tetanospasmin, yang menyerang sistem saraf pusat dan menyebabkan spasme otot yang menyakitkan serta gangguan fungsi otonom. Penyakit ini masih menjadi masalah kesehatan di negara berkembang, terutama pada individu yang tidak memiliki riwayat vaksinasi lengkap.Metode: Laporan kasus ini membahas seorang pria berusia 57 tahun yang datang ke IGD dengan keluhan perut kaku dan nyeri sejak empat hari sebelumnya.Hasil: Pasien disertai kesulitan membuka mulut (trismus), risus sardonikus, dan spasme otot generalisata. Pemeriksaan fisik menunjukkan hipertonus otot, opistotonus, dan tanda-tanda disfungsi otonom. Pasien didiagnosis dengan tetanus generalisata dan mendapat terapi suportif berupa pemberian imunoglobulin tetanus (TIG), antibiotik metronidazol, serta benzodiazepin untuk mengontrol spasme otot.Kesimpulan: Penatalaksanaan tetanus memerlukan pendekatan multidisiplin yang mencakup netralisasi toksin, pemberantasan sumber infeksi, serta dukungan pernapasan dan kardiovaskular. Studi kasus ini menyoroti pentingnya deteksi dini, vaksinasi lengkap, serta penanganan segera untuk mengurangi mortalitas dan morbiditas akibat tetanus.

Neutrophils and procalcitonin (PCT) are inflammatory biomarkers known to increase during sepsis and are suspected to be associated with the severity of organ injury. This study aimed to analyze the relationship between neutrophil and procalcitonin levels and multiple-organ tissue damage scores in a male rat sepsis model induced by intraperitoneal administration of Escherichia coli. This was a true experimental laboratory study using a post-test-only control group design involving nine male Rattus norvegicus rats divided into a control group, an E. coli-induced group receiving 1 × 10⁶ CFU/kg body weight, and an E. coli-induced group receiving 1.5 × 10⁶ CFU/kg body weight. Neutrophil levels were measured using a hematology analyzer, while PCT levels were determined using an Enzyme-Linked Immunosorbent Assay (ELISA). Multiple-organ damage was assessed through histopathological examination of the liver, kidney, spleen, heart, and lungs. Data were analyzed using Spearman’s correlation test. The results showed that neutrophil levels were significantly and positively correlated with damage scores of the liver (rs = 0.88; p = 0.002), spleen (rs = 0.90; p < 0.001), heart (rs = 0.92; p < 0.001), and lungs (rs = 0.91; p < 0.001). Procalcitonin levels were also significantly and positively correlated with damage scores of the liver (rs = 0.90; p < 0.001), spleen (rs = 0.87; p = 0.002), heart (rs = 0.85; p = 0.004), and lungs (rs = 0.88; p = 0.002). In conclusion, higher neutrophil and procalcitonin levels were associated with greater multiple-organ tissue damage in a male rat sepsis model induced by Escherichia coli.
