Abstract

Mental health disorders are still one of the most prominent health problems in the world, including in Indonesia. There is still a misguided stigma and discrimination on people with mental disorders, so more than 56,000 of them experience restraint. This study aims to determine the efforts and review the implementation of Law no. 18/2014 about Mental Health until 2017. This is a policy study using literature review. Descriptive exploration is done using a policy analysis framework in the implementation phase based on Edwards III model. The unit of analysis is mental health policy in Indonesia. The results show that only a few local governments initiate special mental health regulations as well as more operational programs. Activities are still focused on curative and rehabilitative efforts. There has been prevention of deprivation through the ‘Indonesia Free Restraint’ program since 1977 but this has not gone well. The communication process encountered problems of unclear and inconsistent information. Moreover, not all local governments use the authority to regulate the mental health policy to mobilize resources. Law no. 18/2014 has not been implemented optimally. Not all implementers and policy targets are dedicated to direction of the Law. Neither not all aspects of mental health efforts have programs, Standard Operating Procedure and coordination governance. Conversely, optimal implementation can be done by introducing it as an element of primary health in basic health service.

Abstract

Hypertension in pregnancy can cause death of mother and baby. According to the data for basic health research in 2018, there were 2,658 out of 80,544 people in hypertension during pregnancy among women aged 15-49. This study aims to reduce maternal and infant mortality by increasing antenatal care visits. Research methods: observational analytic with cross sectional test. Retrieval of data from the Medical Record from January to December 2019 at Sultan Imanuddin Hospital. The population used throughout the pregnant women with third trimester hypertension is taken out with a total sampling technique as many as 109 people, the excretion criteria of a pregnant women with normal blood pressure. Data were analyzed using chi-square test on version 23 of SPSS program. Statistical test results have the most sidnificant risk of a less suggestive antenatal care visit with hypertension being the pregnant women parity, having OR 0.004 p= 0.283 risky paritas having a tendency to lack antenatal care. The age og pregnant women OR 0.929 p= 0.145 means that she is at risk in life, with a tendency to develop a lack of antenatal care, of 1.929 or 1 times greater than that of a non-risk. Further gained a trust interval, bearing no ratio 1 value meaning there is a link between parity and the age of the pregnant women and poor antenatal care visit with a test of 5% significance. Conclucion management of pregnant women with hypertension where they have been diagnosed at least 4 times during pregnancy, if there are complication and high risk there is a policy of BPJS on the antenatal care every 2 weeks in trimester III as one of the prevention and control of maternal mortality and birth rates.

Abstract

RSUD Pasar Rebo sebagai BLUD, masih menerapkan remunerasi pada tahun 1992, ini mengindikasikan ketidaksesuaian antara status BLUD dengan remunerasi yang diterapkan. Salah satu profesi yang menjadi ujung tombak pelayanan kesehatan di Rumah Sakit dan berperan cukup besar dalam menjaga mutu dan reputasi Rumah Sakit adalah perawat. Maka manajemen Rumah Sakit perlu memperhatikan kesejahteraan perawat khususnya dalam bentuk insentif. Pemerintah mengeluarkan Permendagri No. 6 Tahun 2007 yang dapat dijadikan acuan RSUD Pasar rebo dalam menyempurnakan penetuan insentif untuk perawat, dimana salah satu indikator penentuan insentif adalah dengan memperhatikan Basic Index. Penelitian ini bertujuan untuk mengetahui penentuan insentif untuk perawat ynag berlaku di RSUD Pasar Rebo, serta menganalisis implementasi insentif perawat berdasarkan Permendagri No. 61 Tahun 2007 di RSUD Pasar Rebo. Penelitian operasional dengan pendekatan kualitatif ini menggunakan teknik Wawancara Mendalam. Informan penelitian adalah beberapa pejabat pengelola terkait serta Wakil Direktur Umum dan Keuangan RSUD Pasar Rebo, pejabat pengelola dan Direksi PT Rumah sakit Pasar Rebo sebagai Informan Triangulasi. Hasil penelitian ini menggambarkan insentif perawat yang berlaku di RSUD Pasar Rebo dinilai dari Variabel Tetap dan Tidak Tetap. Pola penentuan insentif yang berlaku di RSUD Pasar Rebo sudah ada unsur Basic Index. Saran penelitian ini adalah penetuan insentif di RSUD Pasar Rebo didasarkan pada prestasi kerja perawat, RSUD Pasar Rebo dapat mengacu pada Permendagri No. 6 Tahun 2007. Basic Index sudah ada dalam sistem remunerasi RSUDPasar Rebo, untuk selanjutnya dapat memasukkan indikator lainnya seperti Risiko Kerja, Tingkat Kegawatdaruratan dan pengoptimalan indikator Capaian Kinerja dalam menentukan insentif peraat di RSUD Pasar Rebo, serta peningkatan komitmen bersama dari unsur pimpinan dan Pejabat Pelaksana di RSUD Pasar Rebo.

Abstract

The high risk of transmission through eye examinations in the COVID-19 pandemic has resulted in teleophthalmology usage in diabetic retinopathy screening programs. This review assesses the effectiveness of teleophthalmology for diabetic retinopathy screening. This literature review uses Pubmed, Proquest, and GoogleScholar as sources. Screening using inclusion and exclusion criteria has been included in 7 articles for review. Results Teleophthalmology increases the number of diabetic retinopathy cases finding on diabetes mellitus, resulting in high patient satisfaction and low cost. Teleophthalmology showed its effectiveness as a screening program for diabetic retinopathy in the era of the COVID-19 pandemic.

Abstract

Background: Patient safety is one of the most important dimensions in quality of healthcare. Nurses are the key in care delivery, their compliance toward Patient Safety policy cannot be overlooked. Studies evaluating determinants of nurse’s compliance to Patient Safety policy is still lacking. Aims: To evaluate nurse’s attitude toward Patient Safety policy using Health Belief Model (HBM). Methods: An analytical cross-sectional study was carried on in Bali Royal Hospital, Indonesia, on December 2016. An anonymous, self-administered questionnaire was used to collect the data on socio demographics, knowledge, and attitude toward Patient Safety policy. Attitudes are specifically assessed using HBM. Results: A total of 124 nurses are included in this study. Mean age of the study population was 29.7±6.7 years. Mean score for knowledge about Patient Safety policy was 12.68±3.40 from 18.0- point scale. Nurses perceived a low risk of non-compliance (mean perceived risk 8.07±1.89) and a low barrier to comply with Patient Safety policy (mean perceived barrier 8.66±1.78). Knowledge and attitudes toward Patient Safety policy did not differ by sex and educational background but it did differ according to unit of workplace. Age and length of work correlated positively to knowledge (Spearman’s r=0.439, p-value 0.000; and r=0.400, p-value <0.001, respectively). A positive and moderate correlation was observed between knowledge vs. perceived severity (r= 0.394, p-value <0.001) and knowledge vs. perceived benefit (r=0.422, p-value < 0.001). There was a strong and positive correlation between perceived severity and perceived benefit (r= 0.725, p-value <0.001). Conclusion: Nurse’s attitudes toward Patient Safety policy were generally good and correlate positively with knowledge. The use of HBM provides an increased understanding of how individual perceptions can be influenced to improve nurse’s engagement in promoting safer health care.

Abstract

Background: Patient safety is a serious issue in a healthcare organization. Building a patient safety culture is the first step in the improvement of safety. As patient safety culture is an inherent component of organizational culture; therefore, a study of organizational culture is required to develop it. This study aims to analyze the patient safety culture and identify the profile of the organizational culture in the hospital to find strategies to improve the patient safety culture.Methods: This study was used the quantitative study using a cross-sectional research method, followed by a qualitative method to get descriptive information. This research conducted in Jambi Public Hospital, Indonesia with 128 respondents. Data were obtained from a survey by the HSOPSC (Hospital Survey on Patient Safety Culture) questionnaire and OCAI (Organizational Culture Assessment Instrument. Focus Group Discussion was conducted to identify obstacles and expectations in the implementation of patient safety culture. Results: The highest positive perceptions are organizational learning and continuous improvement (90.6%) and teamwork within units (78.91%). While "staffing" (39.25) and "non-punitive responses to errors" (38.54 %) are the weakest dimension of patient safety. The most dominant type of organizational culture, followed by a hierarchy with a minimal score difference. This cultural assessment serves as a guide for changes with a quality strategy of clan culture and hierarchy culture were decided in the development of patients safety culture in this hospital.Conclusion: The patient safety culture belongs to the moderate culture category, the reporting culture dimension is the weakest. A management’s commitment and staff empowerment, as well as system development, are all needed on the development of a patient safety culture. Further research is required to investigate various factors to develop the reporting culture.

Abstract

Background: Recently medical tourism is a new form of niche tourism market, which has grown rapidly. The term of Medical Tourism describes tourists who travel to foreign countries to obtain health services and facilities such as medical, dental, surgical care, and so forth. It is necessary to hold a marketing strategy that is able to increase tourist interest in carrying out medical tourism activities in efforts to develop medical tourism. Therefore, the aim of this study was to analyze marketing strategies for medical tourism programs in hospitals. Methods: A systematic review through marketing strategy journals of medical tourism was conducted. Research articles accessed from the internet through databases namely: EBSCO, Emerald, Science direct, Neliti, and Research gate with the keywords medical tourism and medical tourism marketing strategy. The literature analysis from 2 journals with mix methods design, a journal with descriptive qualitative design, a journal with descriptive study design, a journal with SWOT analysis design, a journal with desk research methods design, and 2 journals with data analysis design. Results: The 11 articles that are considered to be in accordance with the objectives of the study. From 11 articles, then screening again based on feasibility according to inclusion and exclusion criteria obtained 8 articles for further review. This systematic review shows that the marketing concepts of segmentation, targeting, and positioning could compete competitively with other hospitals. The marketing strategy of a health service was able to increase the interest of tourists in conducting medical tourism activities in a country. Marketing efforts carried out by hospitals focus on domestic and foreign tourists. Conclusion: The existence of a marketing strategy can increase competitiveness between hospitals and increase the interest of domestic and foreign tourists to carry out medical tourism activities competitively.

Abstract

Background: Throughout the world, emergency departments (ED) are characterized by overcrowding and excessive waiting times. Furthermore, the related delays significantly increase patient mortality and make inefficient use of resources to the detriment of the satisfaction of employees and patients. Lean thinking is a philosophy that focuses on eliminating waste or non-value added elements from the processes so that customers are given greater value. Therefore, many Emergency Departments have begun to apply lean as a way to fight against the problems of crowding, delays and medical incidents.Methods: Eligible studies for this systematic review constitute result summarized that lean method implementation was capable of reducing overcrowding in Emergency Departement. The journal employed in this systematic review sourced from ProQuest, Pubmed, and Google Scholar, whereas the analyzed journals were seven journals.Results: From the total of seven journals reviewed, the studies analyzed the result after implementing the lean method in the Emergency Department. In addition, all of the studies were reported successful in implementing lean intervention. Several studies explained that lean method implementation was capable of reducing waiting time, staying length, and the proportion of patients leaving without being seen in the emergency department. One study suggested that reducing radiography transportation time can reduce turnaround time and can help improve the Emergency Department flow.Conclusion: Lean method implementation can contribute to a decrease in waiting time, stay length, and the proportion of patients leaving without being seen. According to the results of the study, the lean implementation identifies that the lean is capable of decreasing waiting times, stay length, and the proportion of patients leaving without being seen. There must be a policy that governs the lean process. Lean can inspired changes to employee roles, staffing and scheduling, communication and coordination, expertise, workspace layout, and problem-solving.

Abstract

COVID-19 telah menjadi permasalahan global di seluruh dunia dan telah menelan banyak korban. Tenaga kesehatan merupakan kelompok orang yang sangat berisiko tertular COVID-19. Tujuan dari tinjauan sistematis ini adalah untuk menganalisis kesiapan tenaga kesehatan di Rumah sakit dalam menghadapi Pandemi COVID-19. Tinjauan sistematis ini berdasarkan beberapa artikel yang telah dipilih mengikuti PRISMA (Preferred Reporting Items for Systematic). Tenaga kesehatan harus memahami pedoman WHO untuk pencegahan COVID-19, terutama untuk mengidentifikasi gejala dan skrining pasien COVID-19. Jumlah SDM, standar operasional, dan fasilitas pelayanan kesehatan mempengaruhi kesiapsiagaan tenaga kesehatan dalam menangani pandemi COVID-19. WHO telah mengeluarkan pedoman bagi tenaga kesehatan dalam menghadapi pandemi COVID-19 yang berguna untuk menjaga kesinambungan layanan, melaksanakan tindakan prioritas yang terkoordinasi dengan baik, komunikasi internal dan eksternal yang jelas dan akurat, adaptasi cepat terhadap lonjakan kasus, penggunaan sumber daya secara lebih efektif dan melindungi lingkungan yang aman bagi petugas kesehatan. Sebagian besar tenaga kesehatan memiliki pengetahuan yang baik, sikap positif dan menggunakan APD dengan baik. Dari hasil tersebut dapat disimpulkan bahwa tenaga kesehatan di rumah sakit cukup siap dalam menghadapi Pandemi COVID-19.

Abstract

Pusat Kesehatan Masyarakat (Puskesmas) merupakan pelayanan kesehatan yang paling dekat dengan masyarakat. Bagi Penyandang Disabilitas, fasilitas ini belum menjadi solusi untuk mendapatkan pelayanan kesehatan yang optimal. Hal ini disebabkan karena kurangnya aspek-aspek yang memudahkan penyandang disabilitas dalam mendapatkan pelayanan kesehatan. Artikel ini memperlihatkan gambaran puskesmas ramah disabilitas dibeberapa wilayah di Indonesia. Saat ini belum ada kebijakan mewajibkan suatu daerah memilki puskesmas ramah disabilitas dan belum ada ketentuan pemerintah terkait kriteria puskesmas ramah disabilitas. Adapun puskesmas yang diperlihatkan telah mendapat pengakuan dan kelayakan sebagai puskesmas ramah disabilitas, diantaranya Puskesmas Lingsar Kabupaten Lombok Barat, Puskesmas Lendah 1 Kabupaten Kulon Progo, Puskesmas Salam Kecamatan Bandung Wetan Kota Bandung dan beberapa puskesmas lainnya. Aspek yang diperhatikan pada artikel ini dalam perwujudan Puskesmas Ramah Disabilitas adalah aspek pelaksana dan pembuat kebijakan, aspek aksesibilitas fisik dan aksesibilitas non-fisik. Aspek pelaksana atau pihak yang dapat dilibatkan meliputi pemerintah, tenaga kesehatan, organisasi atau lembaga penyandang disabilitas, keluarga penyandang disabilitas, tokoh masyakat, tokoh agama, masyarakat pada umumnya. Aspek aksesibilitas fisik dapat meliputi kebutuhan-kebutuhan yang sesuai dengan fungsi bagi penyandang disabilitas seperti guiding block, handrail, ramp, running text, Huruf braile, Tenaga Kesehatan, Pendamping Disabilitas (GAPENTAS), loket pendaftaran yang disesuaikan dan sarana lainnya. Aspek aksesibilitas non-fisik dapat meliputi pelatihan pendamping disabilitas, sosialisasi kepada masyarakat, program jemput bola, pelatihan komunikasi bagi tenaga kesehatan dan lainnya.
