Abstract

Patient safety is the most important indicator in the healthcare system that is expected to be a reference for producing ideal healthcare services and reducing patient incidents. The nurse factors are the main factor related to patient safety because nurses interact more often with patients. This study aims to identify the factors influencing nurses implementing patient safety in hospitals. This article was a literature review with a descriptive approach by collecting several journals and articles from the Indonesia One Search and Google Scholar databases with specified inclusion criteria. Based on the results of a review of six articles shows that several factors influence nurses in implementing patient safety, including age, attitude, knowledge, work motivation, workload, length of work, supervision, and organizational culture. The implementation of patient safety goals by nurses in hospitals can be categorized as either good or bad because several factors influence it. For this reason, the coordination of the hospital must carry out activities to strengthen the factors that affect nurses so that patient safety can be adequately implemented.

Abstract

Pendahuluan: Patient Safety merupakan parameter utama dalam sistem pelayanan kesehatan yang diharapkan dapat menjadi acuan untuk menghasilkan pelayanan kesehatan yang ideal dan menurunkan insiden bagi pasien. Faktor perawat menjadi faktor utama yang berkaitan dengan keselamatan pasien, karena perawat lebih sering berinteraksi dengan pasien. Penelitian ini bertujuan untuk mengetahui faktor - faktor yang mempengaruhi perawat dalam penerapan keselamatan pasien di rumah sakit. Metode: Penulisan artikel ini menggunakan metode literature review dengan mengumpulkan beberapa jurnal dan artikel dari database Indonesia One Search dan Google Scholar dengan kriteria inklusi yang ditetapkan. Hasil: Dari hasil telaah enam literatur, menunjukkan bahwa ada beberapa faktor yang mempengaruhi perawat dalam penerapan keselamatan pasien, meliputi usia, sikap, pengetahuan, motivasi kerja, beban kerja, lama kerja, supervisi, dan budaya organisasi. Kesimpulan: Penerapan sasaran keselamatan pasien oleh perawat di rumah sakit dapat dikategorikan baik dan kurang baik karena dipengaruhi oleh beberapa faktor. Untuk itu diperlukan adanya koordinasi dari pihak rumah sakit agar melaksanakan kegiatan untuk memperkuat faktor – faktor yang berpengaruh pada perawat sehingga keselamatan pasien dapat diimplementasikan dengan baik.

Abstract

Pendahuluan: Isu keselamatan pasien di puskesmas harus menjadi perhatian khusus mengingat keselamatan pasien menjadi salah satu komponen penting dalam penilaian akreditasi puskesmas. Tujuh Langkah Menuju Keselamatan Pasien merupakan pedoman bagi puskesmas untuk melaksanakan program keselamatan pasien. Tujuan: Menganalisis pelaksanaan program keselamatan pasien di puskesmas dilihat dari penerapan terhadap tujuh langkah menuju keselamatan pasien. Metodologi: Penelusuran artikel dilakukan melalui database Google Scholar dan GARUDA menggunakan kata kunci “Tujuh Langkah Menuju Keselamatan Pasien” DAN “Pelaksanaan Keselamatan Pasien di Puskesmas”. Total temuan artikel sebanyak 51, tetapi hanya 5 artikel yang sesuai dengan kriteria inklusi, yaitu penelitian kualitatif, primary source, topik penerapan tujuh langkah menuju keselamatan pasien, dan lokasi penelitian adalah Puskesmas. Hasil: Terdapat total studi 5 puskesmas dari 5 artikel yang digunakan dalam penelitian ini dengan lokasi di Provinsi Kalimantan Timur dan Jawa Tengah yang telah melaksanakan penerapan tujuh langkah menuju keselamatan pasien. Apabila ditinjau dari setiap langkahnya, terdapat contoh penerapan pada masing - masing langkah menuju keselamatan pasien. Namun, penerapan tersebut belum dilaksanakan sepenuhnya oleh kelima puskesmas yang menjadi sampel dalam penelitian ini. Kesimpulan: Penerapan tujuh langkah menuju keselamatan pasien belum dilaksanakan dengan baik dan optimal oleh puskesmas. Langkah yang paling banyak diterapkan oleh puskesmas adalah langkah enam dan langkah tujuh, yaitu melakukan investigasi insiden dengan Root Cause Analysis (RCA) dan melaksanakan evaluasi dan pemberian solusi dalam pencegahan insiden serta melaksanakan metode FMEA (Failure Mode and Effect Analysis). Abstract

The emergency room is the main gateway for the spread of viruses and diseases, both between patients and between patients and medical personnel. WHO said that in the era of the COVID-19 pandemic, the highest risk of spreading COVID-19 was in the emergency room due to the high mobility of the emergency room, but the patient's disease had not been fully detected, so that the prevention of this disease was carried out through efforts to change the management of the hospital's emergency room. This research was conducted in order to find out the efforts made by the government through a comparison was made between the policy settings for hospital emergency rooms before during the COVID-19 pandemic which included entry flows, service procedures, zone mapping, and the distribution of health human resources in hospital emergency rooms. The new regulations for hospital emergency rooms during the COVID-19 pandemic regulate facilities and infrastructure in the emergency room as well as management of health human resources that adapt to the conditions of the COVID-19 pandemic in Indonesia. The new regulation also requires health screening and the application of health protocols, supporting examinations (swab tests), and the flow of patient examinations in the emergency room which refers to the guidelines of the Ministry of Health of the Republic of Indonesia and guidelines from the CDC. The implementation of the new regulations is a form of responsive and adaptive attitude of hospitals in Indonesia to the conditions of the COVID-19 pandemic.

Abstract

Petugas rekam medis berperan penting dalam menunjang pelayanan kesehatan yang diberikan di rumah sakit. Kepuasan kerja dari petugas rekam medis akan mempengaruhi kinerja dari petugas sehingga berpengaruh secara tidak langsung terhadap kualitas pelayanan rumah sakit. Rendahnya kinerja unit rekam medis menyebabkan pelayanan kesehatan menjadi terhambat sehingga kepuasan pasien terhadap pelayanan rumah sakit dapat menurun. Penelitian ini bertujuan untuk mengetahui faktor yang mempengaruhi kepuasan kerja petugas rekam medis di rumah sakit. Metode yang digunakan dalam penelitian ini yaitu literature review. Strategi penelusuran artikel sesuai topik penelitian dilakukan melalui database PubMed, ScienceDirect, dan Google Scholar menggunakan kata kunci “factor” AND “job satisfaction” AND “medical record officer” OR “medical record unit” AND “hospital” dan kriteria inklusi pada tahun 2019-2023. Total temuan artikel sebanyak 257 artikel, tetapi hanya 10 artikel yang sesuai dengan kriteria inklusi. Terdapat total studi pada dua negara, yaitu Indonesia dan Polandia. Faktor paling banyak ditemukan yaitu faktor lingkungan kerja terkait kondisi kerja dan fasilitas kerja yang tidak memadai menyebabkan ketidakpuasan pada petugas rekam medis. Faktor kedua yang dapat mempengaruhi kepuasan kerja adalah kompensasi atau penghargaan yang diberikan. Kemudian, dilanjutkan karena faktor beban kerja yang berlebihan dan menyebabkan stress kerja sehingga menurunkan kepuasan dari petugas rekam medis. Berdasarkan studi literatur, dapat disimpulkan bahwa faktor yang mempengaruhi kepuasan kerja petugas rekam medis di rumah sakit dikategorikan menjadi 2 faktor, yaitu faktor intrinsik dan faktor ekstrinsik.

Abstract

Background: Patient safety research has primarily focused on hospitals, despite the fact that patient safety incidents in primary care are quite common. Patient safety incident in primary care caused by the implementation of patient safety culture has not been widely applied. Measuring patient safety culture can improve primary care patient safety and decrease patient safety incidents. Objective:  To identify patient safety culture dimensions with low scores that require improvement in primary care. Methods: Article searches were conducted through the PubMed and ScienceDirect databases using the keywords “patient safety” AND “safety culture” AND “primary care”. A total of 199 articles were found, and only 10 articles fit the inclusion criteria. Results:  The SAQ-AV, MOSPSC, and HSOPSC patient safety culture measurement instruments were used in 242 primary healthcare settings across eight countries. According to research, each primary care setting has aspects of patient safety culture that could be improved. The dimensions of working conditions, work pressure and pace, and non-punitive response to error were the most frequently observed. Conclusion: The dimensions of management perception, working conditions, job satisfaction, work pressure, and speed, leadership support, staff training, incident reporting frequency, staffing, non-blame response, and hands-off and transition have low scores and require improvement in measuring patient safety culture.

Abstract

The implementation of safe and quality care with attention to patient safety, requires organization’s effort to create and cultivating patient safety culture. The purpose of this article was to map the instruments used in measuring patient safety culture in healthcare organizations. The method used integrated literature review from various sources of research articles published from 2015 to 2020. The article included if it was available in full text and open access as well as articles described the instruments of patient safety culture or measurement of patient safety culture using one of the instruments of measurement of patient safety culture. The results of the literature review unravel the findings of three instruments such as HSOPSC (Hospital Survey on Patient Safety Culture), MaPSaF (Manchester Patient Safety Assessment Framework) and SAQ (Safety Attitudes Questionnaire). We concluded all three instruments contained four dimensions of patient safety culture, namely open culture, just culture, reporting culture and learning culture and were widely used to measure patient safety culture in hospitals, primary health facilities and other health facilities.

Abstract

Insiden keselamatan pasien di Instalasi Bedah Sentral masih sering ditemukan hingga saat ini. Salah satu faktor yang menyebabkan terjadinya insiden tersebut adalah ketidakpatuhan staf bedah dalam mengimplementasikan surgical safety checklist. Oleh karena itu, penelitian ini bertujuan untuk mengetahui faktor-faktor determinan kepatuhan terhadap implementasi surgical safety checklist di rumah sakit. Metode penelitian yang digunakan adalah studi literatur dengan analisis Preferred Reporting Item for Systematic Review and Meta-Analytic. Penelusuran artikel dilakukan melalui basis data PubMed, ScienceDirect, dan Google Scholar dengan menggunakan kata kunci “patient safety incident” OR “adverse event” AND “surgical safety checklist” AND “compliance” AND “hospital”. Jumlah artikel yang ditemukan adalah sebanyak 732 artikel, tetapi hanya 10 artikel yang memenuhi kriteria inklusi. Penelitian dilakukan di 9 negara dan ditemukan fakta bahwa terdapat beberapa faktor determinan terkait kepatuhan terhadap implementasi surgical safety checklist. Faktor-faktor determinan tersebut dapat diklasifikasikan menjadi 4 faktor, yaitu faktor surgical safety checklist itu sendiri, faktor individu, faktor tim, dan faktor organisasi. Faktor determinan kepatuhan terhadap implementasi surgical safety checklist paling banyak berasal dari dalam diri individu staf bedah. Oleh karena itu, standar prosedur operasional, pelatihan, pendidikan, komunikasi, sistem manajemen rumah sakit, pemantauan, serta evaluasi perlu diperbaiki dan dikembangkan dengan melibatkan kerja sama di seluruh tingkatan manajemen.

Abstract

Introduction: Medication errors do not only occur in adult patients, but can also occur in pediatric patients. Systematic preventive measures are needed to prevent the occurrence of Medication errors in children. This is because pediatric patients have a weaker body than adult patients so that the negative impact of Medication errors will also have a greater effect on pediatric patients than on adult patients. Therefore, it is necessary to identify what factors can lead to the occurrence of Medication errors so that organizational leaders or related parties can determine what intervention actions can be taken to prevent the adverse effects of Medication errors for pediatric patients in hospitals. Objective: The aim of this literature review is to analyze and determine what factors influence the occurrence of medication errors in hospitals, especially in the pediatrics unit. Method: Article searches were conducted through several databases including PubMed, ScienceDirect, and Google Scholar using the keywords "Factor" AND "Medication errors" AND "Pediatrics" AND "Hospital". The total number of articles found was 259 articles, but only 14 articles were relevant to the topic raised. Result: The study was conducted in 24 hospitals in thirteen countries where each of these hospital has several factors that can cause medication errors in pediatric services with the most common factor being the lack of adherence of health workers to procedures or guidelines for drug administration to patients. Conclusion: Medication errors that occur in hospital pediatric services are influenced by 3 factors, namely health human resource factors, patient family factors, and environmental factors.

Abstract

Improving the safety of vulnerable patient groups in health care is a priority. Patients with disabilities are among the most vulnerable groups. Persons with disabilities have more complex and extensive healthcare needs than non-disabled patients. This literature review aims to identify the types of patient safety incidents that can be used in patients with disabilities. The keywords "patient safety" and "disability" were used to search the Pubmed and Google Scholar databases for articles. The final results revealed four articles that met the inclusion criteria. The findings revealed that patients with disabilities are still frequently subjected to patient safety incidents such as falls, medication errors, infections, burns, and fluid leaks beneath the skin. According to the findings of the analysis using the Donabedian framework, there are still many problems that occur in structural components and processes, which can have an impact on the results obtained. Because of their vulnerability and uniqueness, patients with disabilities are more vulnerable to patient safety incidents. Furthermore, the issue of disability and patient safety remains the primary concern, so studies on disability and patient safety are still limited.
