Abstract

Background: Hospitals are often scary places for children. Especially when undergoing invasive medical procedures such as blood draws and injections. Children feel greater pain and anxiety than adults, causing fussiness and psychological trauma. The role of nurses is very important in creating a safe and comfortable environment. They must apply pain management techniques that include pharmacological approaches such as the use of analgesics, and non-pharmacological methods such as distraction and relaxation techniques. One non-pharmacological method is the use of ice packs. This technique is thought to reduce pain with a cold sensation. Cold applications using ice packs can be considered because they are more affordable and easy to access. Purpose: To analyze the results of research related to the effect of cold application on pain in children undergoing invasive procedures. Method: The systematic review study used the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) scheme guidelines. The search for articles was conducted by searching the literature through online databases, namely PubMed, ScienceDirect, ProQuest, Scopus, and ClinicalKey for Nursing. Results: The application of the ice pack application to reduce the pain experienced by children when undergoing invasive procedures has proven to be effective. However, further research with a broader scope and stronger research design is needed to generalize these findings to a wider population with various types of invasive procedures. Conclusion: Ice packs are effective in reducing pain in children undergoing invasive procedures. The mechanism of action is by slowing the transmission of pain signals to the brain. Ice packs are easy to use, inexpensive, readily available, and environmentally friendly.   Keywords: Children; Ice Pack; Invasive Procedures; Pain.   Pendahuluan: Rumah sakit sering menjadi tempat yang menakutkan bagi anak-anak, terutama saat menjalani tindakan medis invasif seperti pengambilan darah dan suntikan. Anak-anak merasakan rasa sakit dan kecemasan yang lebih besar dibandingkan orang dewasa, sehingga menyebabkan rewel dan trauma psikologis. Peran perawat sangat penting dalam menciptakan lingkungan yang aman dan nyaman. Mereka harus menerapkan teknik manajemen nyeri yang mencakup pendekatan farmakologis penggunaan analgesik dan nonfarmakologis, seperti teknik distraksi dan relaksasi. Salah satu metode nonfarmakologis adalah penggunaan ice pack yang dapat mengurangi rasa sakit dengan sensasi dingin. Tujuan: Untuk menganalisis hasil-hasil penelitian terkait pengaruh cold application terhadap nyeri pada anak yang dilakukan tindakan invasif. Metode: Penelitian systematic review menggunakan panduan skema Preferred Reporting Items for Systematic Review and Meta-Analyses (PRISMA). Pencarian artikel dilakukan dengan penelusuran literatur melalui database online yaitu PubMed, ScienceDirect, ProQuest, Scopus, dan ClinicalKey for Nursing. Hasil: Penerapan aplikasi ice pack dalam mengurangi nyeri yang dialami anak-anak saat menjalani tindakan invasif terbukti efektif. Namun, perlu dilakukan penelitian lebih lanjut dengan cakupan yang lebih luas dan desain penelitian yang lebih kuat untuk menggeneralisasi temuan ini pada populasi yang lebih luas dengan berbagai jenis prosedur invasif. Simpulan: Aplikasi ice pack efektif dalam mengurangi nyeri pada anak saat menjalani tindakan invasif. Mekanisme kerjanya dengan memperlambat transmisi sinyal nyeri ke otak. Aplikasi ice pack mudah dalam penggunaannya, murah, mudah didapat, dan ramah lingkungan.   Kata Kunci: Anak-anak; Ice Pack; Nyeri; Tindakan Invasif.

Abstract

Pain is a health problem to consider especially for children and adolescents. Studies showed 15-35% of children in the nonclinical population suffered persistent and chronic pain. This study aims to provide an overview of the fulfillment of comfort needs in children with pain problems through the provision of Virtual Reality interventions. The application of Kolcaba's theory in providing nursing care for children with pain problems can provide holistic benefits and guide nurses in conducting systematic assessments. The application of virtual reality as an intervention in nursing care has also been proven to provide a sense of comfort and reduce pain and anxiety. The method used is a case study with a nursing care approach to 5 patients with pain problems aged > 10 years. The comfort level will be measured using the Comfort Behavior Checklist Scale. Cases were taken in the pediatric infection room of National Referral Hospital DR. Cipto Mangunkusumo.

Abstract

The challenge of malnutrition in hospitalized children is a pressing concern, and the development of screening tools such as STRONGKIDS and STAMP is a step forward to address this issue. This study aimed to highlight important aspects of their sensitivity and specificity in different clinical settings. A systematic review was conducted through a literature search in Google Scholar, Scopus, Wiley, and ScienceDirect with keywords related to hospitalized child malnutrition. Articles that met the inclusion criteria were analyzed using the QUADAS-2 tool to assess methodological quality. The review results showed that the STRONGKIDS and STAMP nutritional screening tools are highly sensitive in detecting the risk of malnutrition in children. Although both have high sensitivity, STRONGKIDS tends to have lower specificity than STAMP. The study showed that STRONGKIDS effectively identifies nutritional risk in hospitalized children, with a sensitivity of 77.3% to 84.8%. On the other hand, STAMP had also been shown to be reliable in assessing the risk of malnutrition in pediatric patients with spinal cord injury, with a sensitivity of up to 83.3% and higher specificity in some cases. It is concluded that STRONGKIDS and STAMP are sensitive nutritional screening tools in detecting the risk of malnutrition in childrenKeywords: malnutrition; risk detection; children ABSTRAK Tantangan kekurangan gizi pada anak-anak yang dirawat di rumah sakit menjadi perhatian yang mendesak, dan pengembangan alat skrining seperti STRONGKIDS dan STAMP merupakan langkah maju untuk mengatasi masalah ini. Studi ini bertujuan untuk menyoroti aspek penting dari sensitivitas dan spesifisikasinya dalam pengaturan klinis yang berbeda. Tinjauan sistematis dilakukan melalui pencarian literatur di Google Scholar, Scopus, Wiley, dan ScienceDirect dengan kata kunci yang berkaitan dengan malnutrisi anak di rumah sakit. Artikel yang memenuhi kriteria inklusi dianalisis menggunakan alat QUADAS-2 untuk menilai kualitas metodologis. Hasil tinjauan menunjukkan alat skrining gizi STRONGKIDS dan STAMP sangat sensitif dalam mendeteksi risiko malnutrisi pada anak. Meskipun keduanya memiliki sensitivitas yang tinggi, STRONGKIDS cenderung memiliki spesifisitas yang lebih rendah daripada STAMP. Studi menunjukkan bahwa STRONGKIDS secara efektif mengidentifikasi risiko gizi pada anak-anak yang dirawat di rumah sakit, dengan sensitivitas 77,3% hingga 84,8%. Di sisi lain, STAMP juga terbukti dapat diandalkan dalam menilai risiko malnutrisi pada pasien anak dengan cedera tulang belakang, dengan sensitivitas hingga 83,3% dan spesifisitas yang lebih tinggi dalam beberapa kasus. Abstract

Aims: Providing medical equipment intervention increases the risk of MARSI in newborns with laceration and traumatic wounds in the subcutaneous tissue of the skin. The application of an adhesive or a skin barrier is one technique to lower the risk of MARSI. Nonetheless, past research has yielded mixed results in terms of its usefulness in reducing the risk of MARSI. Purpose: This study is to conduct a literature evaluation on the effectiveness of using a skin barrier to reduce the occurrence of MARSI. Methods: A systematic review of four databases was conducted, including Scopus, EBSCOhost, SAGE, and Wiley. A total of 8 publications were reviewed utilizing inclusion and exclusion criteria with a total end of article review. Results: The application of adhesive efficiently and considerably reduces the risk of MARSI, according to 5 of 8 articles. Conclusion: It is recommended that adhesive be used in an integrated manner on a regular basis to prevent the incidence of MARSI in babies.

Abstract

Acute and critical illness threats can cause death. Having an acute and critically ill child who is admissionprovides unique experiences for parents. The aim to describe the experiences during which children undergotreatment for acute and critical illnesses. Methods with online data-based searches ScienceDirect, PubMed,and Wiley published in 2016-2020. The keywords used were mother OR parent AND child AND "acuteillness" AND hospitalization OR inpatient care. The results of the analysis of the themes: world and foreignenvironment; problems controlling the parent; negative emotions;characteristic of parent contribute; needfor health service facilities; and care needs for parents. Conclusion: parents experience a psychologicalresponse that requires attention and intervention from health workers that they are able to adapt to thesituation they feel. It is important to apply the FCC concept with appropriate strategies in nursing care forchildren during child in acute and critical care setting.

Abstract

Background: The interaction between mothers and premature babies in the early period of oral feeding is beneficial for forming social, cognitive, and emotional development. It is essential to assess maternal interactions with premature babies using valid and reliable instruments. However, culturally adapted tools in the Indonesian context remain limited, making localized validation efforts crucial. This research was to analyze the translation and adaptation process of the Indonesian version of MIRI.Methods: The translation and adaptation process followed standard procedures, including forward translation, expert panel review, back translation, pilot testing, and finalization of the instrument. The process involved collaboration with the International Language Institute and experts in pediatric nursing, neonatology, and nutrition. A pilot test was conducted with 30 mothers of premature infants who were receiving care in a neonatal intensive care unit. The evaluation focuses on content validity and reliability of the translation instruments.Results: The translation and adaptation results showed I-CVI between 0.67 to 1.00. The MIRI S-CVI results for all items were 0.91 in the excellent category. The reliability test on mothers showed a Cronbach's Alpha value of 0.74 in the valid and reliable category. Items with lower I-CVI scores were reviewed and refined based on expert feedback to enhance clarity and cultural relevance. Furthermore, participants reported that the questionnaire items were clear and relevant to their experiences during oral feeding interactions with their premature infants.Conclusions: The Indonesian version of the MIRI demonstrates strong validity and reliability for assessing maternal interaction with premature infants during oral feeding. The instrument can be effectively utilized in both research settings and clinical practice, particularly in neonatal intensive care environments where understanding maternal-infant interaction is crucial for supporting early developmental outcomes.

Abstract

Background: Anthropometric measurements during early childhood are critical as they represent the initial stages at which nutritional status is determined. Practitioners who measure children’s growth must have similar and adequate skills in anthropometric measurement to ensure the validity of data regarding children’s nutritional status, particularly concerning stunting prevalence. Objective: This study aimed to explore the measurement practices conducted by community health workers (CHWs) in Indonesia, who are responsible for providing data related to children’s growth and, consequently, their nutritional status. Methods: A qualitative descriptive design was employed using online focus group discussions ranging from 1 to 1.5 hours. Data were collected from ten health professionals and eight CHWs, with analysis performed using thematic analysis. Results: Four themes emerged: barriers to measurement accuracy, varied skills in measurement, mothers’ behavior influenced by children’s reactions, and strategies for dealing with traumatized children. Conclusion: Practical issues that arose during routine anthropometric measurements included the reliability of measurements, the skill levels of examiners, the interplay between mothers’ and children’s reactions, and approaches to managing traumatized children during measurements. To effectively enhance integrated health posts (Posyandu) services, it is crucial to maintain calibrated, validated equipment and implement comprehensive training with periodic refreshers for CHWs. Additionally, improving facility infrastructure, fostering CHWs’ communication skills, and simplifying child-friendly measurement procedures will collectively optimize service quality, promote cooperation, and strengthen community trust.

Abstract

This study aimed to analyze complementary food fulfillment during disaster situations in Indonesia since it frequently experiences natural disasters and young children face unique challenges related to their feeding needs in disaster situations. This qualitative case study based on the Mount Merapi eruption and Sumedang landslide in 2021 collected data using observations, forum group discussions, and in-depth interviews; then, a thematic analysis was conducted. This study involved 17 participants, including Non-governmental Organizations (NGO) activists, health workers, residents involved in making complementary foods, and primary caregivers of children aged 6–24 months. This study found that there were five themes generated from the data analysis; 1) the donation of home-based complementary foods based on local wisdom, 2) inadequate complementary feeding, 3) limited resources to manage the complementary feeding, 4) clean versus dirty conditions, and 5) mothers’ endeavour in complementary feeding practices. This study concluded that the complementary feeding practices during a disaster in Indonesia were supported by NGOs, residents, and nearby healthcare workers and thus represent a form of mutual cooperation among the Indonesian people.

Abstract

Ruam popok merupakan reaksi inflamasi akut pada daerah kulit perianal yang umum terjadi pada anak-anak. Tujuan dari studi kasus ini adalah melakukan pemenuhan kebutuhan kenyamanan pada anak yang mengalami gangguan integritas kulit dengan implementasi virgin coconut oil dengan pendekatan Teori Comfort Kolcaba. Hasil implementasi pada lima kasus terpilih menunjukkan bahwa terjadi perbaikan pada masalah keperawatan kerusakan integritas kulit dan nyeri akut yang dialami anak dengan ruam popok. Implementasi virgin coconut oil dapat memperbaiki kerusakan jaringan yang terjadi. Penerapan virgin coconut oil melalui pendekatan Teori Comfort Kolkaba dapat diterapkan pada anak dengan gangguan kenyamanan.

Abstract

An increase in HIV cases in housewives is at risk of increasing the population of children exposed to HIV. Caregivers of children exposed to HIV are at risk of having problems caring for children exposed to HIV. The difficulties experienced by caregivers can be affected by the condition of the child, who may or may not be infected with HIV. This study aims to get an n overview of the emotional experience of caregivers in caring for children underfive who are exposed to HIV. Methods: The research design uses a qualitative method with a phenomenology approach. Data collection was carried out using snowball sampling techniques and obtained from ten participants domiciled in Jakarta and its surroundings. The participants recruited in this study are caregiver of HIV-exposed children. Data collection was carried out using in-depth interview techniques. Data analysis uses thematic analysis using the Colaizzi method. This study obtained ten participants consisting of seven biological mothers, two children’s relatives and one foster parent. There were seven children with HIV-negative and five children with HIV-positive. Two caregivers in this study cared for more than one child exposed to HIV, namely one child with HIV-positive and one child with HIV-negative. The study found three themes: easy versus complex; grace and entrustment; and love and pity. This study concluded the that there are differences in emotional experiences among caregivers in caring for children underfive exposed to HIV influenced by the child’s HIV status. Positive affirmations and emotional support from health professionals adjusted to the values and socio-cultural condition of caregivers will benefit caregivers in providing adequate care for children.
