Abstract

Aims: Critically ill patients' severe symptoms and higher mortality risk can impact sleep quality. Non-pharmacological therapy, including lavender aromatherapy and earplugs, can improve sleep quality in these patients. To determine the effect of the combination of lavender aromatherapy and the use of earplugs on sleep quality in critically ill patients. Methods: This study uses a Quasy Experiment design with a one-group pretest-posttest design, involving 33 participants aged 18-65, with The Richards-Campbell Sleep Questionnaire, and uses Paired Sample t-Test for data processing. Results: In this study, an overview of the sleep quality of critical patients showed an improvement. An analysis of the sleep quality of critically ill individuals revealed a notable enhancement. The study found a significant improvement in sleep quality after the lavender aromatherapy intervention and the use of earplugs in the intervention group, as indicated by a p-value of 0.001 (p<0.05). Conclusion: The findings of this study indicate that the use of lavender aromatherapy intervention with earplugs can positively impact the sleep quality of critically ill patients. Furthermore, nurses can independently use lavender aromatherapy and earplug use as crucial nursing treatments.

Abstract

Aims: Critical patients in ICU and HCU often experience poor sleep due to excessive stress and various symptoms like environmental factors, pain, noise, anxiety, and physical discomfort. Foot massage and natural music interventions can help meet their sleep needs, providing a relaxing effect and improving physical and psychological conditions. The purpose of this study is to determine the effect of providing combination therapy of foot massage and natural music therapy in Dr. Hospital's peacock room Salamun 2023. Methods:  This type of research is pre-experimental with a pre-post test of one group and uses the Richard Champbell Sleep Questionnaire (RCSQ). Then the data was processed using a paired sample T-test. The sample in this study is 16 respondents. Results: The results of the statistical test obtained a p-value of 0.01, meaning that there was an effect between the pretest and posttest of the combination of foot massage and natural music therapy on the quality of sleep in critical patients in the peacock room at Dr. Salamun Hospital, Bandung City, 2023. Conclusions : the conclusion of this study is that there is an effect of foot massage therapy and natural music on the sleep quality of critical patients as shown by the results of measuring the quality of critical patients before and after consecutive interventions with massage of both feet combined with natural music. For 10 minutes it has been proven to be able to improve sleep quality in critically ill patients so that this intervention can be applied as a therapy to improve sleep quality.

Abstract

ABSTRACT This study aims to determine the application of therapeutic positions to the vital signs parameters of stroke patients at the Prof. Dr. National Brain Center Hospital. Dr. Mahar Mardjono Jakarta. This research is an application of Evidence-Based Nursing which was applied to 12 respondents. Each patient in the study group was positioned in various therapeutic positions every 2 hours following a semi-fowler's rotating schedule, lying on their side to the healthy side (Unaffected Side Position), lying on their back (Supine Position), and lying on their side to the side with hemiparesis or hemiplegia (Affected Side Position). Based on the results, the average systolic blood pressure decreased on the second post-intervention day (day 2) with a mean of 124.08 and a standard deviation of 3,147, while the diastolic mean was not much different from the first day, namely 83.17 and a standard deviation of 2,657. The average heart rate on the second-day post-intervention decreased with a mean of 84.92 and a standard deviation of 0.900. In the respiration rate variable, the decrease in the mean value post-intervention on the second day was 15.33 (standard deviation 0.888). The mean value for oxygen saturation levels (SPO2) with the mean post-test value on the second day was 99.75 (standard deviation 0.452). So it can be concluded that providing therapeutic positions intervention can reduce blood pressure, heart rate, and respiration rate and increase oxygen saturation. Keywords: Therapeutic Positions, Vital Signs  ABSTRAK Penelitian ini bertujuan untuk mengetahui Penerapan Therapeutic Positions Terhadap Parameter Vital Signs Pasien Stroke Di Rumah Sakit Pusat Otak Nasional Prof Dr. dr. Mahar Mardjono Jakarta. Penelitian ini merupakan Penerapan Evidence-Based Nursing yang diterapkan pada 12 responden. Setiap pasien dalam kelompok studi diposisikan pada berbagai posisi terapeutik setiap 2 jam dengan mengikuti jadwal berputar seperti semi fowler's, berbaring miring ke sisi yang sehat (Unaffected Side Position), berbaring terlentang (Supine Position) dan berbaring miring ke sisi yang hemiparesis atau hemiplegia (Affected Side Position). Berdasarkan hasil bahwa rata-rata tekanan darah sistole terjadi penurunan pada post intervensi hari kedua (day 2) dengan mean 124.08 dan standar deviasi 3.147, sedangkan diastole diperoleh mean yang tidak berbeda jauh dengan hari pertama yaitu 83.17 dan standar deviasi 2.657. Rata-rata heart rate post intervensi hari kedua terjadi penurunan dengan mean 84.92 dan standar deviasi 0.900. Pada variabel respiration rate diperoleh penurunan untuk nilai mean post intervensi hari kedua adalah 15.33 (standar deviasi 0.888). Nilai mean pada kadar saturasi oksigen (SPO2) dengan nilai mean post-test hari kedua 99.75 (standar deviasi 0.452). Pemberian intervensi therapeutic positions dapat menurunkan tekanan darah, heart rate, respiration rate dan meningkatkan saturasi oksigen. Kata Kunci: Posisi Terapeutik, Tanda-Tanda Vital

Abstract

ABSTRACT This study aims to determine the implementation of the Nurse-LED Early Mobilization Guide in patients undergoing elective spinal surgery. This research is an implementation of an Innovation Project which was applied to 10 respondents. Each patient in the study group was mobilized in a sequence starting from right lateral/supine/left, sitting 45o, sitting upright 90o, and dangling sitting mobilization according to the specified time. Post intervention results were evaluated where pain started from the first 24 hours (POD-1) to the third day (POD-3) while functional status after 72 hours post surgery (POD-3). Based on the results, the average Barthel Index in the post-intervention group evaluated on the third day (POD-3) was 15.30 with a standard deviation of 3.40. Meanwhile, the average pain score in the first day post-intervention group (POD-1) was 4.50 with a standard deviation of 0.97. The mean pain score on the second day (POD-2) was 3.60 with a standard deviation of 0.69. Meanwhile, the average pain score on the third day was 2.50 with a standard deviation of 0.70. Conclusion: Nurse-LED early mobilization intervention can be carried out in degenerative disc disease patients undergoing elective spine surgery to improve the patient's clinical outcomes, especially functional status and decreased pain. Keywords: Decreased Pain, Functional Status, Nurse-LED Early Mobilization  ABSTRAK Penelitian ini bertujuan untuk mengetahui Penerapan Panduan Nurse-LED Early Mobilization pada pasien yang Menjalani pembedahan elektif tulang belakang. Penelitian ini merupakan Penerapan Proyek Inovasi yang diterapkan pada 10 responden. Setiap pasien dalam kelompok studi dimobilisasikan dengan urutan mulai dari lateral kanan/supine/kiri, duduk 45o, duduk tegak 90o, dan mobiliasi duduk berjuntai sesuai dengan waktu yang telah ditentukan. Hasil post intervensi dievaluasi dimana nyeri mulai dari 24 jam pertama (POD-1) sampai dengan hari ketiga (POD-3) sedangkan status fungsional setelah 72 jam post operasi (POD-3).  Berdasarkan hasil bahwa rata-rata Barthel Index pada kelompok post intervensi yang dievaluasi pada hari ketiga (POD-3) adalah 15,30 dengan standar deviasi 3,40. Sedangkan rata-rata skor nyeri pada kelompok post intervensi hari pertama (POD-1) adalah 4.50 dengan standar deviasi 0.97. Mean skor nyeri pada hari kedua (POD-2) adalah 3,60 dengan standar deviasi 0,69. Sedangkan rerata skor nyeri pada hari ketiga adalah 2,50 dengan standar deviasi 0,70. Kesimpulan : intervensi Nurse-LED Early mobilization dapat dilakukan pada pasien-pasien degenerative disc disease yang menjalani pembadahan elektif tulang belakang untuk meningkatkan outcome klinis pasien khususnya adalah status fungsional dan penurunan nyeri. Kata Kunci: Penurunan Nyeri, Status Fungsional, Nurse-LED Early Mobilization

Abstract

Cancer remains one of the leading causes of mortality worldwide, with over 20 million newly diagnosed cases and nearly 10 million deaths globally, as reported by the International Agency for Research on Cancer (IARC) in 2020. In Indonesia, the incidence reached 408,661 new cases with 242,988 cancer-related deaths recorded in 2022. This growing burden has resulted in numerous clinical implications, with pain identified as the most frequently reported symptom among cancer patients, significantly impairing their quality of life. Among emerging approaches in pain management, non-pharmacological interventions such as hypnotherapy have gained attention due to their potential to reduce pain intensity without inducing serious adverse effects. This systematic review aims to evaluate the effectiveness of hypnosis therapy in managing cancer-related pain by analyzing six selected studies sourced from five international databases: ScienceDirect, EBSCO, PubMed, ProQuest, and Sage Journals. All included studies met the eligibility criteria, comprising randomized controlled trials (RCTs) and one meta-analysis, published within the last ten years. Five out of the six studies demonstrated that hypnosis interventions were effective in reducing pain intensity, while one study showed no statistically significant effect. Additionally, several studies explored integrative approaches combining hypnosis with complementary modalities such as music therapy and cognitive behavioral therapy. The findings of this review suggest that hypnotherapy serves as a promising complementary intervention to support effective pain management in oncology care. Further research employing more rigorous methodologies and larger sample sizes is recommended to strengthen the evidence base regarding its clinical utility.

Abstract

Declining self-care ability in stroke survivors impairs recovery and lowers long-termquality of life. This systematic review evaluates the effectiveness of telehealth interventionsin improving self-care capacity. Based on the PRISMA framework, five databases:PubMed, Science Direct, Scopus, Sage Journals, and ProQuest were searched for literaturepublished between 2020-2024. This search returned 387 results, of which 10 randomizedcontrolled trials were selected and evaluated according to a critical checklist. The resultsof this review suggest that self-care telehealth interventions significantly improve physical(mobility and muscle strength, performance of daily activities) and psychologicalcomponents (self-efficacy, depression and anxiety) that constitute a better quality of life.Telehealth technologies promote autonomy in stroke survivors by remote healthmonitoring, and lessen dependence on direct care. Abstract

Background: Stroke often causes physical impairments and risk of disability, making patients highly dependent on caregivers for physical and emotional support. The complex caregiving role frequently leads to physical, psychological, and social pressures, known as caregiver burden, which negatively impacts caregiver well-being and patient care. Purpose: To identify interventions provided to stroke caregivers to reduce caregiver burden. Method: A systematic review was conducted following PRISMA guidelines on randomized controlled trials (RCTs) published in English between 2014 and 2024 with full-text availability. Pilot and feasibility studies were excluded. Literature searches were performed through Science Direct, Medline (via EBSCOhost), Sage Journals, Taylor & Francis, and Scopus. Results: Eleven studies with a total of 1,229 stroke caregivers were included. Identified interventions comprised psychoeducation, counseling, coaching, education, and training, all designed to alleviate caregiver burden. Conclusion: Family-based interventions are effective in reducing caregiver burden. Their implementation can be initiated during the acute phase in the hospital and continued through follow-up approaches such as telenursing after discharge. This review emphasizes the potential of family-based interventions as a comprehensive and sustainable strategy to support stroke caregivers.   Keywords: Caregiver Burden; Family Intervention; Stroke Survivors.   Pendahuluan: Stroke sering menimbulkan gangguan fisik dan risiko kecacatan sehingga pasien mengalami sangat bergantung pada caregiver dalam pemenuhan kebutuhan fisik dan emosional. Peran yang kompleks kerap menimbulkan tekanan fisik, psikologis, dan sosial, yang dikenal sebagai caregiver burden. Hal ini akan berdampak pada kesejahteraan caregiver dan kualitas perawatan pasien. Tujuan: Untuk mengidentifikasi intervensi keluarga untuk menurunkan caregiver burden pada keluarga yang merawat penyintas stroke. Metode: Tinjauan literatur sistematis dilakukan sesuai panduan PRISMA terhadap artikel Randomized Controlled Trial (RCT) berbahasa Inggris, terbit antara tahun 2014-2024, dan tersedia teks lengkap. Artikel pilot atau feasibility study dikecualikan. Penelusuran menggunakan database Science Direct, Medline (via EBSCOhost), Sage Journals, Taylor & Francis, dan Scopus. Hasil: Sebanyak 11 penelitian dengan 1,229 partisipan (caregiver pasien stroke) diikutkan. Berbagai intervensi keluarga teridentifikasi, antara lain psikoedukasi, konseling, coaching, edukasi, dan pelatihan, yang bertujuan menurunkan beban caregiver. Simpulan: Intervensi keluarga terbukti efektif menurunkan beban caregiver. Pelaksanaannya dapat dimulai sejak fase akut di rumah sakit dan dilanjutkan dengan follow-up seperti telenursing setelah pasien pulang. Tinjauan ini menegaskan bahwa pendekatan intervensi keluarga berpotensi menjadi strategi komprehensif dalam mendukung caregiver penyintas stroke secara berkelanjutan.   Kata Kunci: Caregiver Burden; Intervensi Keluarga; Penyintas Stroke.

Abstract

Background: Stroke is one of the leading causes of long-term disability and morbidity worldwide. Post-stroke depression (PSD) is a common psychological complication that negatively affects recovery, functional outcomes, and quality of life. Psychological resilience is considered a key factor in helping patients adapt to post-stroke challenges; however, evidence regarding effective interventions to strengthen resilience in patients with PSD remains limited. Purpose: To evaluate the effectiveness of interventions that potentially enhance resilience-related components in patients with post-stroke depression and to provide evidence-based recommendations for nursing and clinical practice. Method: A systematic review was conducted in accordance with the PRISMA 2020 guidelines. Literature searches were performed across five databases such as PubMed, Scopus, ScienceDirect, ProQuest, and Wiley using the keywords post-stroke depression, resilience, and intervention. Randomized controlled trials published in English that involved PSD patients and reported psychological outcomes related to resilience were included. Of 9,241 identified records, nine studies involving 779 participants met the inclusion criteria and were analyzed using narrative synthesis. Results: Psychological interventions, including Acceptance and Commitment Therapy (ACT) and Mindfulness-Based Interventions (MBI), showed significant improvements in psychological flexibility (25–35%), mindfulness (20–32%), and coping ability (15–25%), alongside a significant reduction in depressive symptoms (p < 0.001). Physical-based interventions such as Tai Chi and Yijinjing Qigong were associated with improved emotional regulation and reductions in depressive symptoms of up to 45%. Technology-assisted approaches, particularly Virtual Reality interventions, reduced depressive symptoms by up to 22%. Pharmacological treatments demonstrated faster mood improvement but showed limited effects on strengthening psychological resilience. Conclusion: ACT, MBI, and structured physical exercise appear to be the most effective interventions for enhancing resilience-related components and alleviating depressive symptoms in patients with post-stroke depression. Future studies are recommended to directly measure resilience outcomes and examine the long-term sustainability of these interventions.

Abstract

Background: Sleep quality is a critical determinant of recovery outcomes among patients admitted to intensive care units (ICUs); however, accurate sleep assessment remains challenging due to environmental disturbances and intensive medical interventions. The Richards–Campbell Sleep Questionnaire (RCSQ) is widely used to assess subjective sleep quality, yet evidence regarding its psychometric performance in Indonesian ICU settings remains limited. Objective: The purpose of this study was to evaluate the validity and reliability of RCSQ in the ICU and assess the extent to which this instrument can accurately reflect the patient's sleep experience in the ICU. Methods: A methodological cross-sectional study was conducted involving 11 adult ICU patients recruited using a total sampling technique. Data were collected between April and May 2025 using the Indonesian version of the RCSQ. Construct validity was assessed using item–total correlation analysis, while internal consistency was evaluated using Cronbach’s alpha coefficient. Descriptive statistics were used to summarize participants’ demographic characteristics. Results: All RCSQ items demonstrated satisfactory construct validity, with item–total correlation coefficients exceeding the critical r-value (r > 0.521) and statistical significance at p < 0.05. The instrument showed good internal consistency, with a Cronbach’s alpha value of 0.810, indicating reliable measurement of subjective sleep quality among ICU patients. Conclusion: The Indonesian version of the Richards–Campbell Sleep Questionnaire demonstrates acceptable validity and reliability for assessing subjective sleep quality in ICU patients. Nevertheless, further studies with larger and more diverse samples are recommended to confirm its robustness and to explore integration with objective sleep assessment methods in intensive care settings.

Abstract

Latar belakang : Stroke pada pasien dengan SLE menjadi tantangan dalam praktik keperawatan karena melibatkan kerumitan kondisi yang disebabkan oleh kombinasi dua penyakit utama: SLE sebagai penyakit autoimun sistemik dan stroke sebagai gangguan pada sistem neurologis. Pengkajian dengan pendekatan Model Adaptasi Roy memberikan kerangka kerja yang komprehensif untuk memahami bagaimana pasien merespons perubahan fisik, psikologis, dan sosial akibat kombinasi kondisi tersebut. Tujuan : Studi ini untuk menggambarkan penerapan model adaptasi roy dalam pengkajian keperawatan pada pasien stroke iskemik dengan komorbiditas SLE. Presentasi Kasus: Seorang pasien perempuan berusia 28 tahun dirawat di unit stroke dengan keluhan spastik hemiparesis kanan dan infark maligna pada wilayah middle cerebral crtery (MCA) kiri, serta telah menjalani kraniektomi pada hari ke-0. Hasil: Masalah keperawatan yang muncul dari hasil pengkajian dengan pendekatan model adaptasi roy yaitu  risiko penurunan curah jantung, ketidakefektifan perfusi jaringans perifer, risiko nutrisi kurang dari kebutuhan, gangguan mobilitas fisik, nyeri akut, risiko infeksi, ketidakefektifan perfusi jaringan serebral,  dan  gangguan komunikasi verbal. Kesimpulan: Pengkajian keperawatan dengan pendekatan model adaptasi roy  memberikan kerangka untuk pengkajian keperawatan yang holistik dan terfokus, yang memungkinkan perawat menilai proses adaptasi pasien dalam menghadapi kondisi yang kompeks.
