Abstract

Background: Steroid-induced glaucoma (SIG) is a secondary glaucoma characterized by elevated intraocular pressure (IOP) due to steroid use. This study aimed to analyze the clinical characteristics and management of SIG patients at Dr. M. Djamil General Hospital Padang, Indonesia, from January 2019 to August 2023. Methods: A retrospective descriptive study was conducted using medical records of patients diagnosed with SIG. Data collected included age, gender, steroid type, route of administration, duration of use, IOP at diagnosis, underlying diseases, glaucoma stage, and treatment. Results: Seventeen patients were diagnosed with SIG. The majority were female (70.58%) and aged 4-39 years (58.83%). The most common underlying diseases were systemic lupus erythematosus (SLE) and allergic conjunctivitis (47%). Oral steroid administration was most frequent (76.4%), with a usage duration of 2-12 months in most cases (70.6%). IOP at diagnosis ranged from 22 to 31 mmHg in most patients (82.3% right eye, 64.7% left eye). Most patients presented with mild glaucoma (70.5%). Topical anti-glaucoma medications were the primary treatment (76.4%), with trabeculectomy performed in some cases (23.6%). Conclusion: SIG is a preventable condition. Early detection and appropriate management are crucial to prevent vision loss. The clinical characteristics identified in this study contribute to a better understanding of SIG in our population. Further research on the interplay of risk factors, genetics, and histopathology is needed to enhance our comprehension of SIG.

Abstract

Eye diseases, particularly cataracts, are the leading cause of blindness worldwide. This community service program by the Department of Ophthalmology, Faculty of Medicine, Andalas University, was conducted at the Pangkalan Public Health Center, Lima Puluh Kota Regency, by empowering PELITA (Posyandu) cadres in early detection of cataracts. Methods included counseling, training, and eye examination simulations for 40 cadres, followed by community screening using visual acuity and anterior segment examinations. Evaluation was carried out with pre- and post-tests, satisfaction surveys, and follow-up on patients referred for surgery. The average cadre score improved from 68 (pre-test) to 74.5 (post-test), indicating increased knowledge. A total of 71 patients were screened, including 40 cataract and 8 pterygium cases; 26 cataract and 4 pterygium patients subsequently underwent surgery. Cadres reported high satisfaction with the training and expressed readiness to continue participation. This program demonstrates that cadre training is effective in improving early detection of cataracts and other visual impairments, and provides a foundation for establishing a Vision Center in Lima Puluh Kota.

Abstract

Background: Complex open globe injuries (OGIs) involving the simultaneous destruction of multiple anterior segment structures represent a formidable challenge in ophthalmic traumatology. These injuries necessitate a staged surgical approach to restore ocular integrity and optimize the potential for visual rehabilitation. Case presentation: A 44-year-old male presented with a severe Zone I OGI in the left eye from a high-velocity projectile stone. His initial best-corrected visual acuity was limited to light perception. Examination revealed an 8-mm corneal rupture with uveal prolapse, complete anterior extrusion of the crystalline lens, and a Grade IV "blackball" hyphema. The Ocular Trauma Score (OTS) was calculated as 1, predicting a very poor visual outcome. Immediate primary repair involved anterior vitrectomy, lensectomy, hyphema evacuation, and corneal suturing. After a six-month stabilization period, a secondary scleral-fixated intraocular lens (IOL) was implanted using a double-needle Yamane technique. Conclusion: Despite the devastating nature of the initial injury and a grim prognosis as per the OTS, a principled, staged surgical strategy resulted in a final best-corrected visual acuity of 20/50. This case highlights that adherence to damage-control principles—prioritizing anatomical restoration in the primary surgery and deferring refractive correction—can lead to unexpectedly favorable functional outcomes in the most severe categories of ocular trauma.

Abstract

Primary open-angle glaucoma (POAG) is a progressive chronic optic neuropathy that often affects adults with characteristics of optic nerve damage in the eye due to increased intraocular pressure (IOP). POAG is characterized by an open anterior chamber angle. POAG with increased IOP is understood to mean decreased outflow of aqueous humor due to increased resistance to such outflow. This study aims to determine the association between intraocular pressure and stage of primary open-angle glaucoma. This study is an analytic study with a cross sectional approach using retrospective data. The sampling technique used was total sampling, in which all subjects who met the inclusion and exclusion criteria were selected as a sample of 84 eye samples. Analysis of the data used in the form of univariate analysis and bivariate analysis using Spearman correlation test. POAG is more common in women (55,6%). The majority of POAG experienced by the age group over 40 years (95,6%). There is a significant relationship between IOP and stage of POAG (p=0.001) with medium correlation strength and positive correlation direction (r=0,371).  There is a relationship between IOP and POAG with a positive correlation direction, where the higher the IOP value, the POAG stage will increase.
