Abstract

Background: Breast cancer is the second leading cause of death in women in developing countries. The activity of Warburg and Reverse-Warburg effects on breast cancer is reflected by the expression patterns of two molecules, namely caveolin-1 and Monocarboxylate Transporter-4 (MCT-4). MCT-4 is a transmembrane transport protein that transports lactate from the cytoplasm to the intercellular fluid.Method: This is a cross-sectional analytical study to determine the relationship between MCT-4 expression and breast cancer clinicopathology and subtypes. The study was conducted between April and May of 2020 with 62  breast cancer patients as samples in Sanglah General Hospital, Denpasar. Analysis was done with SPSS 25.Results: A logistic regression analysis was performed to analyze the relationship between the dependent variable (MCT-4) and the covariates (stage, grade, and subtype). Of the three variables significantly associated with MCT-4 expression, only clinical-stage and subtype (luminal and non-luminal) remained independently associated with MCT-4 expression. Analysis on the clinical stage and subtype variables showed an adjusted OR of 4.727 (p = 0.047; 95% CI: 1.109 - 21.922) and 17.850 (p = 0.009; 95% CI: 2.069 - 154.003) , respectively. This suggests that MCT-4 has a significant association with subtype and clinical-stage, increasing the risk of cancer stage progression and developing a more malignant (non-luminal) subtype.Conclusion: High MCT-4 expression was significantly associated with malignant subtypes, high histological-grade cancer and advanced breast cancer.

Abstract

Hiperkolesterolemia adalah kelebihan kolesterol karena sering makan makanan yang mengandung kadar kolesterol tinggi. Kelebihan atau kekurangan status nutrisi dapat diukur dengan menghitung Indeks Massa Tubuh (IMT) dan Lingkar Pinggang (LP). Penelitian ini bertujuan untuk mengetahui ada tidaknya korelasi antara IMT dan LP terhadap kolesterol total pada mahasiswa kedokteran angkatan 2015 Universitas Udayana. Jenis penelitian ini menggunakan metode cross-sectional analisis dengan subjek 37 orang yang dilakukan pengukuran LP, kadar kolesterol total, dan berat serta tinggi badan untuk mendapatkan nilai IMT. Subjek dianalisis menggunakan Pearson Chi-Square Test atau Fisher Exact Test jika nilai yang diharapkan kurang dari 5. Dari 37 subjek yang telah diukur, hasil penelitian menunjukkan adanya korelasi signifikan dengan korelasi negatif dari IMT dan LP dengan kolesterol total mahasiswa kedokteran angkatan 2015 Universitas Udayana. Korelasi IMT dengan kolesterol total adalah p = 0,007, dengan r = -0,434. Selanjutnya, korelasi LP dengan kolesterol total adalah p = 0,044. Berdasarkan hasil penelitian, didapatkan korelasi signifikan dengan korelasi negatif dari IMT dan LP dengan kolesterol total pada mahasiswa kedokteran angkatan 2015 Universitas Udayana. Oleh karena itu, sangatlah penting untuk melakukan upaya preventif dalam menjaga kadar kolesterol total harus ditargetkan pada orang normal dan orang kurus. Abstract

ABSTRAK Pendahuluan: Age-Related Macular Degeneration (AMD) merupakan penyakit yang banyak diderita para lansia ditandai dengan adanya degenerasi makula pada retina. Pencegahan perlu dilakukan, salah satunya menggunakan bahan alam yaitu bunga marigold dengan kandungan lutein. Lutein memiliki bioavailabilitas yang rendah sehingga diperlukan formulasi enkapsulasi nanopartikel PolyLactic-co-Glycolic Acid (PLGA). Pembahasan: Age-Related Macular Degeneration (AMD) dapat diakibatkan oleh akumulasi Reactive Oxygen Species (ROS) di bagian makula pada retina yang kemudian menyebabkan terjadinya stres oksidatif. Lutein pada bunga marigold bekerja dengan cara bereaksi dengan zat ROS sehingga mencegah terjadinya akumulasi dan stes oksidatif pada retina. Lutein aman dikonsumsi sebagai suplemen namun memiliki bioavailabilitas yang rendah sehingga diperlukan formulasi enkapsulasi nanopartikel PolyLactic-co-Glycolic Acid (PLGA). Lutein dengan formulasi ini memiliki efek klinis pada AMD, yaitu meningkatkan konsentrasi plasma dari lutein, kemudian meningkatkan skor Macular Pigment Density, dan meningkatan Contrast Sensitivity serta Glare Sensitivity. Simpulan: Lutein sebagai antioksidan yang kemudian terenkapsulasi nanopartikel PolyLactic-co-Glycolic Acid (PLGA) memiliki efek potensi untuk dijadikan agen pencegahan AMD. Diperlukan penelitian terkait dosis efektif, tingkat efikasi dalam pencegahan, serta efek samping dari lutein yang terenkapsulasi PLGA.

Abstract

K-RAS is one of the most frequently mutated genes found in colorectal cancer cases. The K-RAS gene has about 30%-40% contribution to the formation of colorectal cancer. This research is an analytic observational study with a cross-sectional design to explain the characteristics of patients with colorectal cancer at Prof. dr. IGNG. Ngoerah 2017-2019 and assessed the relationship between K-RAS gene mutations and response to therapy. The results of the chi-square test showing a significant relationship between the type of KRAS gene mutation and the response to therapy where p=0.035 (p<0.05). This shows that there is a significant relationship between mutations in the K-RAS gene in colorectal cancer and the response to therapy in colorectal cancer patients at Prof. Dr. IGNG. Ngoerah General Hospital in 2017-2019.

Abstract

Background: Colorectal cancer is the third most diagnosed cancer worldwide and ranks second as the cause of cancer-related mortality. Matrix Metalloproteinase (MMP) enzymes are known to be closely associated with the carcinogenesis of colorectal cancer. The MMP-7-181A>G polymorphism has been previously linked to the risk of various types of cancer. However, the association of the MMP-7-181A>G polymorphism with the risk of colorectal cancer has not been investigated in Indonesia. Objective: This study aims to determine the relationship between the MMP-7-181A>G polymorphism and the risk of colorectal cancer. Methods: Polymerase chain reaction (PCR) followed by Sanger sequencing was employed to obtain genotype data from 26 cases and 26 controls. Univariate and bivariate statistical analyses were conducted to determine the association of MMP-7-181A>G polymorphism to the risk of colorectal cancer by evaluating obtained Odds Ratio (OR) and p-values. Results: The distribution of the AG and GG variants in the case group was 23.1% and 3.8%, respectively. Meanwhile, the distribution of the AG and GG variants in the control group was 15.4% and 0%. Data analysis shows an insignificant increase in colorectal cancer risk in genotypes with the G allele (OR=2.02, 95% CI: 0.51-8.00, p=0.308). Conclusion: The study suggests that the MMP-7-181A>G polymorphism cannot be directly associated with colorectal cancer risk, emphasizing the need to consider multiple interactions with other MMP gene polymorphisms. Further investigations into direct associations of this polymorphism with MMP-7 expression levels in colorectal cancer lesions could help give a clearer understanding of genetic risk factors in colorectal cancer.

Abstract

Hypertension treatment can be helped through complementary therapies, one of which is fruit juices. This study aimed to systematically review the evidence from various study regarding the effect of fruit juices on blood pressure in hypertensive patients and the types of fruit juices that are proven to be effective in reducing hypertension.This study is systematic review. Literature searching was conducted in four electronic databases, namely CINAHL, EMBASE, Pubmed, and Sciencedirect with the phrase fruit juices AND (hypertension OR blood pressure). Studies obtained from searching will be selected based on a screening title and abstract, eligibility criteria, and assesment risk of bias. The results of the study obtained 21 final studies with RCT research designs that passed the screening with complete eligibility criteria. However, the studies with valid results only amounted to 8 studies. These data show that as many as 5 studies prove fruit juices can reduce blood pressure in hypertensive patients and 3 other studies have stated the opposite. Thus, it can be concluded that there is strong evidence that fruit juices has a significant effect on reducing blood pressure in hypertensive patients. Fruit juices that have been shown to be effective are orange juice and queen garnet plum juice

Abstract

Pendahuluan: Diet tinggi lemak dapat menyebabkan berbagai macam kondisi salah satunya adalah obesitas. Ketidakseimbangan profil lipid secara langsung bekontrobusi pada stress oksidatif kronis sehingga dapat mempengaruhi struktur dan fungsi otak. Manifestasi klinis dari gangguan fungsi otak yaitu penurunan memori. Clitoria ternatea L. mengandung flavonoid dan polifenol yang berperan sebagai antioksidan sehingga dapat mencegah kerusakan organ khususnya otak maupun gangguan memori. Tujuan: mengetahui efek pemberian ekstrak air Clitoria ternatea L. terhadap gambaran histologis otak dan gangguan memori tikus dengan induksi diet tinggi lemak. Metode: Jenis penelitian ini adalah true experimental post-test only menggunakan 25 ekor tikus yang dibagi menjadi lima kelompok yaitu diet normal (ND), diet tinggi lemak (HFD), dan kelompok perlakuan (HFD+CTE250; HFD+CTE500; HFD+CTE750). Ekstrak Clitoria ternatea L. diperoleh dengan metode ekstrak air. Berat badan pasca perawatan, profil lipid (kolesterol, trigliserida, HDL-C, dan LDL), fungsi memori (uji labirin Y), dan gambaran histologis otak diukur dan dianalisis. Hasil: Hasil penelitian ini menunjukkan penurunan sel hipokampus yang signifikan pada kelompok dengan HFD dibandingkan dengan kelompok HFD+CT750 (MD: -82.33; 95% CI (-128.87 hingga -35.79); p=0,03). Kondisi ini juga diperparah dengan kejadian oklusi pembuluh darah yang dapat menghambat peredaran darah pada otak tikus kelompok HFD. Memori spasial tikus dan jumlah entri pada uji Y-maze menunjukkan terdapat penurunan jumlah entri dan memori spasial yang signifikan pada kelompok HFD+CT250 (MD: 37,70; 95% CI (18,38-57,02); p=0,001). Simpulan: ekstrak air Clitoria ternatea L. secara signifikan mempengaruhi gambaran histologis otak dan gangguan memori tikus dengan induksi diet tinggi lemak. Abstract

Background: Problems to COVID-19 are closely related to the level of knowledge and community prevention. Therefore, to overcome COVID-19, increased knowledge and prevention are needed. This study aimed to evaluate the correlation between prevention and knowledge level about COVID-19.Methods: A cross-sectional study using a convenience sampling approach was conducted in Banjar Gambang, Karangasem, Indonesia, in April 2022. The knowledge level and preventive behavior towards COVID-19 were measured using the COVID-19 Preventive Behaviors Index (CPBI) and the knowledge, attitudes, and practice toward COVID-19 (KAPCOV-19) questionnaire. The data were analyzed using SPSS software version 26.0.Results: A total of 52 respondents were included, who had excellent level of knowledge (44.2%) and moderate prevention behaviour (48.1%). A strong and significant correlation was found between the preventive index and the knowledge levels of COVID-19 (r = 0.548; p<0.001). The level of knowledge was significantly related to the level of preventive behavior (p= 0.003), as well as the education level (r = 0.323; p = 0.02) and age (r= -0.346; p=0.012).Conclusion: The level of knowledge and the individual prevention behavior toward COVID-19 are directly proportional to each other. Those who have a low level of knowledge, might affect their prevention behavior toward COVID-19, therefore, personalized socialization of COVID-19 prevention is still required.

Abstract

Penatalaksanaan hepatitis C yang umum diberikan di Indonesia adalah terapi kombinasi Peg-IFNα dan RBV, namun terapi tersebut mempunyai efek samping seperti depresi, hipotiroidisme, memperlambat motorik, risiko kardiovaskular tinggi, dan lain sebagainya. Berdasarkan hal tersebut, maka dibutuhkan alternatif yang dapat meminimalisir efek samping yang ditimbulkan dalam penatalaksanaan hepatitis C, salah satunya menggunakan bahan herbal naringenin. Naringenin adalah flavanon alami yang diketahui memiliki efek hepatoprotektif sebagai anti-viral dan anti-fibrotik pada penatalaksanaan hepatitis C.  Metode penulisan yang digunakan dalam karya tulis ilmiah ini adalah metode kajian pustaka. Data yang digunakan berasal dari beberapa sumber literatur yang relevan dan sesuai dengan topik masalah yang dibahas. Naringenin berpotensi sebagai modalitas anti-viral dan anti-fibrotik pada penatalaksanaan hepatitis C. Sebagai anti-viral, naringenin mampu menghambat sekresi virus hepatitis C sebanyak 80% melalui penghambatan aktivitas MTP, ACAT2, dan HMGR. Sebagai anti-fibrotik, naringenin mampu menghambat jalur NF-kB dan TGF-β Smad3, tetapi naringenin memiliki bioavailabilitas dan solubilitas yang rendah, sehingga penyerapannya hanya sebesar 15%. Enkapsulasi naringenin dengan liposom dapat meningkatkan jumlah serapannya pada tubuh, meningkatkan waktu paruh, meningkatkan konsentrasi obat di plasma, serta mudah terkonsentrasi di hati.Kata kunci: hepatitis C, naringenin, liposom, anti-viral, anti-fibrotik

Abstract

Ovarian cancer is one of the major health problems in gynecology worldwide. Globocan 2012 data shows that ovarian cancer occurs in 239.000 cases, and is placed fourth as the most malignancy occurred in Indonesia. So far, cisplatin is the most effective therapeutic agent, but the high dosage of administration often caused side effects. A new alternative therapy to overcome the problems by using withaferin-A and nanoparticle PEGylated liposome. WFA has the potential effect to reduce cisplatin resistance but has low bioavailability thus, it needs to be encapsulated. This review’s goal is to depict the potency of withaferin-A, cisplatin, and PEGylated liposome combination as a therapeutic agent to treat ovarian cancer. The combination of Cisplatin-Withaferin PEGylated Liposome is constructed by the modified thin lipid film hydration method then administered orally. This combination suppresses 70-80% of the ovarian cancer cell growth in vivo and inhibits NGFR from binding to TRKA, prevents cell migration as much as 50%. Nano-Cisferin-Liposome, inhibits migration of ovarian cancer cell significantly through ?-catenin signaling through ALDH1 that disturb the spheroid formation, and increase apoptosis event as much 70-80% by increasing ROS production up to 2,5 times. Thus, Nano-Cisferin-Liposome (NCL) has potential as a therapeutic agent for treating ovarian cancer. Keywords: Cisplatin; Ovarian Cancer; PEGylated Liposome; Withaferin A
