Abstract

Basic Nursing Clinical Practice is the first clinical learning experience for vocational nursing students. The achievement of complete competence will provide a positive experience, ability, willingness to learn actively and independently, and direct continuous learning so that competent vocational nurse graduates are achieved. Self-Directed Learning Readiness (SDLR) is a condition where students have full control in the process of making decisions related to their learning and accept full responsibility for it, even though later they need help and advice from lecturers and Clinical Instructors. Therefore, this research was conducted to determine the effectiveness of SDLR on the competency achievement of basic nursing clinical practice students. This study used a quasi-experimental design with one group pretest and posttest design. The sampling technique used total sampling, with as many as 91 respondents. Respondents were 2nd semester D3 Nursing Study Program students. The analysis used an independent sample t-test, with a 95% degree of confidence. The results of statistical tests using the Independent T-Test showed that there was a positive effect of SDLR on the Competency Achievements of Basic Nursing Clinical Students with a p-value <0.05. Of course, a high level of Self Directed Learning Readiness, can have a positive impact on student competency outcomes in carrying out Nursing Clinical Practice, where competency achievement targets will be met. Achievement of plenary competence will provide a positive experience, ability, willingness to learn actively and independently, and direct continuous learning so that competent vocational nurse graduates are achieved.

Abstract

Self-directed learning (SDL) solves learning challenges using various learning tools. Nursing requires lifelong learning, and SDL is an important skill for practitioners to stay current and effective. It is required of nursing students to embrace SDL and hone these abilities. Nevertheless, there is no indication of this novel procedure in Lumajang. This study aims to assess nursing students' preparation for SDL and its impact on learning outcomes. This quasi-experimental study purposefully included 91 students who carry out basic nursing clinical practice as participants. The instrument used in this study was the Self-Directed Learning Readiness Scale (SDLRS) questionnaire developed by Fisher. The results of this study showed that there was an increase in the SDLR level from 9 students (9.9%) having a high SDLR level to 78 students (85.8%) and 82 students (90.1%) having a moderate SDLR level to 13 students (14.2%). The nursing students' average SDL preparedness level has a substantial impact on learning outcomes. Nursing training institutes should give the resources required to adopt SDL as a primary teaching technique to ensure competent lifelong professionals.

Abstract

Clinical preceptors are essential to nursing education because they help students learn clinical skills and develop as professionals. However, assessing preceptor competency can be challenging due to the lack of standardized and reliable tools. While various tools are available to measure multiple aspects of competency, there is no clear agreement on how these tools should be selected or used across clinical settings. The purpose of this systematic review was to identify and evaluate tools measuring competence of nursing clinical preceptor. The review focused on the instruments and their psychometric qualities, how they are used, and how effective they are in improving clinical education. A systematic literature review was conducted to identify existing instruments used to measure the level of competency of nursing clinical preceptors. Science Direct, Google Scholar, Sage Journal, Pubmed, and Springer were the databases selected for this study. Of the 13 eligible articles in this study, 12 different instruments were found. The instruments found included the Clinical Educators' Competence Questionnaire, Clinical Teaching Competence Inventory, Maastricht Clinical Teaching Questionnaire (MCTQ), Preceptors Orientation Competence Instrument (POCI), Preceptors' Competency Domain Guide Tool, Clinical Core Competency of Preceptors (CCCP), Clinical Preceptor Experience Evaluation Tool (CPEET), Competence of Clinical Nurse Educators Instrument, Nursing Clinical Teacher Effectiveness Inventory (NCTEI), Mentors' Competence Instrument (MCI), Mentorship Effectiveness Scale, IMSOC Questionnaire. Although there are several tools available to evaluate nursing clinical preceptor competency, each has its advantages and disadvantages, according to the review's findings. For a thorough evaluation, the suitable instruments must be chosen depending on particular educational circumstances. The results show that these instruments need to be further developed and standardized to increase their applicability and reliability in a variety of healthcare settings. This will ultimately assist successful preceptorships and raise the caliber of nursing education.

Abstract

Abstract

Low adherence of patients with Non-Communicable Diseases (NCDs) and the elderly to scheduled follow-up visits remains a major challenge in primary healthcare services. This issue contributes to delayed detection of complications and reduced effectiveness in managing chronic conditions. This community service program aimed to evaluate the effectiveness of the TERKONTROL application—an automated WhatsApp-based reminder system designed to improve the regularity of patient follow-up visits. The program was implemented at Dasan Agung Public Health Center using a Participatory Action Research (PAR) approach, actively involving program officers, academic facilitators, and patients. The desktop-based application features patient data input, visit scheduling, a monitoring dashboard, and an automated reminder system. Implementation results showed that out of 16 patients who received reminder messages, 13 attended their scheduled visits, achieving a compliance rate of 81.3%. Evaluation revealed positive perceptions from both patients and healthcare staff regarding the application's usability and usefulness. These findings suggest that simple digital innovations such as TERKONTROL (Tertib Kontrol dan Respon Lanjut Pasien PTM & Lansia) can serve as effective tools for real-time patient monitoring, program reporting enhancement, and improved communication in primary healthcare settings.

Abstract

Background: Nurses often face high job demands and emotional stress that may reduce work happiness and caring behavior. Previous studies have examined workplace well-being interventions; however, culturally grounded spiritual programs integrating local wisdom remain insufficiently tested using rigorous experimental designs, particularly in low- and middle-income countries (LMICs). This study aimed to evaluate the effectiveness of a local wisdom–based spiritual program in improving work happiness and caring behavior among inpatient nurses in Indonesia. Methods: A mixed-methods randomized controlled trial following CONSORT guidelines was conducted with 120 inpatient nurses at two hospitals in Jember, Indonesia. Eligible nurses (>1-year inpatient experience) were randomly assigned to either an intervention group (n = 60) or a control group (n = 60). The intervention group participated in a 12-week spiritual program (Pengajian and Tari Molong Kopi) that included routine activities. The independent variable was program participation, and the dependent variables were work happiness and caring behavior measured using the Oxford Happiness Questionnaire (OHQ) and Caring Behavior Inventory (CBI) at baseline, week 6, and week 12. Data were analyzed using repeated-measures ANOVA, effect sizes (Cohen’s d), correlation, and 95% confidence intervals. Qualitative responses were analyzed thematically. Results: Participants were mostly female (87%) with a mean age of 32 years. The intervention group showed significantly greater improvements in OHQ and CBI scores at weeks 6 and 12 compared with controls (p < 0.001), with moderate-to-large effects (d = 0.65–0.82). Work happiness correlated with caring behavior (r = 0.52). Themes included enhanced mindfulness, emotional release, renewed professional meaning, and stronger collegial relationships. Conclusion: Local wisdom–based spiritual programs significantly enhance nurses’ work happiness and caring behavior. Integrating culturally grounded spiritual practices into workplace well-being initiatives may strengthen the quality of nursing care in LMIC hospital settings.
