Abstract

ABSTRACT Patient safety is considered an important part of healthcare and also plays an important role in preventing and reducing harm, errors, and injuries that can occur during the care cycle. One way to address this challenge is to develop systematic clinical risk management. This study aims to determine the ability of nurses to identify clinical risks through risk management. The research used was a systematic review using the PRISMA protocol in selecting articles. The results showed 10 articles were identified and published in 2014-2024. All articles studied were indexed by Scopus. The study found two main factors that can affect nurses' ability to identify clinical risks, namely intrinsic and extrinsic factors. The largest factor was found in extrinsic factors which found that effective communication is the main factor that can affect nurses' ability to identify clinical risks in patients in hospitals. Keywords: Nurses, Nursing abilities, Identification Skills, Clinical Risk, Patient Safety

Abstract

Kepatuhan terhadap standar dokumentasi dalam Rekam Medis Elektronik (EHR) sangat penting untuk keselamatan pasien dan kualitas layanan. Namun, tantangan seperti beban administratif, keterbatasan teknologi, dan kebijakan institusi yang bervariasi memengaruhi tingkat kepatuhan tenaga kesehatan. Dokumentasi yang akurat mendukung pengambilan keputusan klinis dan menurunkan risiko kesalahan medis. Studi ini bertujuan mengidentifikasi faktor-faktor utama yang memengaruhi kepatuhan terhadap standar EHR, mengevaluasi tantangan teknologi dan sumber daya manusia, serta mengidentifikasi kesenjangan penelitian. Metode: Mengikuti pedoman JBI dan PRISMA-ScR, tinjauan ini menelaah literatur dari ProQuest, ScienceDirect, BMC, dan sumber lain. Dari 961 artikel, 22 dipilih untuk telaah penuh, dan 10 dianalisis kritis (1 kualitatif, 1 cross-sectional, dan 8 quasi-eksperimental). Hasil: Kepatuhan dipengaruhi oleh kebijakan institusi, beban administratif, integrasi sistem, dan pelatihan. Sistem EHR yang tidak terintegrasi memperbesar kesalahan. Tantangan lain mencakup privasi data (HIPAA, GDPR) dan keterbatasan pelatihan. Ditemukan kesenjangan terkait efektivitas jangka panjang kepatuhan terhadap outcome pasien. Kesimpulan: Diperlukan pendekatan multidimensional, termasuk penguatan kebijakan, optimalisasi teknologi, dan pelatihan berkelanjutan. Automasi dan AI berpotensi mengurangi beban dokumentasi dan meningkatkan kepatuhan.

Abstract

Effective leadership behavior in hospitals can influence nurses’ performance, including job expectations. Positive job expectations can contribute to improving the quality of health services. In addition, organizational commitment is essential as a mediating factor that can connect leadership behavior with work expectations. However, this relationship has not been studied in depth in the context of hospitals. Therefore, this study aims to analyze the influence of leadership behavior on nurses’ work expectations through organizational commitment at Jember Lung Hospital. This study used a quantitative approach with a cross-sectional design. The total sample size was 146 nurses working at the Hospital of Paru in Jember. Data were collected using a questionnaire consisting of three questions: leadership behavior, organizational commitment, and work expectations. The data were analyzed using a logistic regression analysis to explore the influence between the variables studied. This study indicated that leadership behavior positively influences organizational commitment (p < 0.00). In addition, leadership behavior also has a positive effect on organizational commitment (p < 0.001). Organizational commitment mediates the relationship between leadership behavior and work expectations with a value (p < 0.00), which indicates that organizational commitment is a significant mediator in the process. The influence of mediation was 0.000, indicating a substantial contribution of mediation to work expectations. Positive leadership behavior can increase the commitment of nursing organizations and job expectations. This study suggests that hospital management needs to pay attention to leadership behavior to improve nurses’ motivation and work expectations, ultimately impacting the quality of health services.

Abstract

During nursing education, professional values are developed in theoretical and clinical education. Nursing students are expected to cultivate professional values when they enter university and continue their nursing practice. This study aims to analyze the professionalism values of Generation Z nursing students. The research employed a descriptive design, including 182 respondents selected through total sampling. Data were collected using a questionnaire. The research instrument used the Nurse Professional Values Scale-Revised (NPVS-R) questionnaire. Data analysis used descriptive analysis with data not normally distributed (median). The average age of the students was 21, with a majority being female. The indicators of professional values nursing students’ altruism, autonomy, human dignity, integrity, and social justice have median values of 31, 30, 50, 46, and 27. The results showed that nursing students who had good and less professional values were in the same amount. This shows that Generation Z nursing students have a different understanding of the current values of professionalism. The results of this study are expected to be a reference in improving the ability of professionalism values in Generation Z nursing students.

Abstract

The agriculture sector plays an important role in a country's economy, and one of the important sectors is food crops such as rice. Agricultural activities can put farm workers at risk of work-related accidents, and one of the factors that influences this is work fatigue. Work fatigue is the body's defense mechanism to reduce more severe damage when someone does excessive activity. This research analyzed work fatigue among farmers in Antirogo village, Sumbersari District, Jember Regency. This research was quantitative descriptive research using a sampling method on 290 respondents. This research was conducted using univariate analysis with frequency distribution. This research collected data using the Swedish Occupational Fatigue Inventory (SOFI) questionnaire. Data collection results showed that the highest result of assessment indicators in the medium category for lack of energy was 68.3%. Exertion of physical exertion was 62.4%, and physical discomfort was 46.6%; the lowest results were found in a mild category: motivation at 89.7% and sleepiness at 60%. The overall results of work fatigue among rice farmers in Antirogo Village are in the medium category of 60%. The bivariate analysis showed a meaningful relationship between the characteristics of the respondents, gender, and working period to the occurrence of work fatigue among rice farmers in Antirogo Village. This research showed that rice farmers experience fatigue influenced by the work environment and having good motivation can reduce fatigue.

Abstract

Hospitals can spread nosocomial infections, including phlebitis, within 72 hours of admission. Phlebitis, an inflammation of veins often caused by IV lines, can lead to severe systemic infections. This study examines the incidence and contributing factors of phlebitis in Class 2 inpatient wards at RSD dr. Soebandi Jember, using observational analytic methods and a quota sampling of 88 adult patients from a 756-patient population, selected via Slovin's formula. Observations from September 10-18, 2024, involved the Visual Infusion Phlebitis Scale (VIP) and medical records. Results showed 77 patients (87.5%) without phlebitis, and 11 patients (12.5%) with phlebitis: 8 (9.1%) with mild degree 1 and 3 (3.4%) with degree 2 symptoms. Contributing factors include age, gender, chronic diseases, and reactions to certain medications. These findings suggest the importance of adherence to standard operating procedures for IV care to prevent infections. Practical implications include improving nurse training on IV care protocols to minimize phlebitis and related complications, thereby enhancing patient outcomes.

Abstract

Introduction: Nursing care is the most essential element in health services. Professional nurses must provide safe, effective services and participate in maintaining patient safety. Safety for patients is essential because it can increase the value of professionalism. The research aims to analyze internal and external factors regarding nurses' compliance in implementing Standard Operating Procedures to prevent the risk of falls.Methods: This research used an analytical observational design with a cross-sectional approach using a sample of 107 respondents from two hospitals in Jember.Results: The results showed that internal factors influenced nurse compliance in implementing SOP for patients at risk of falling (p-value 0.045). Regarding external factors, it was found that compliance in implementing SOP for patients with fall risk did not have a significant effect (p-value 0.948).Conclusion: Continuous training, effective supervision, and work environment support are important to increase nurses' compliance in implementing SOPs to prevent falls. This research recommends strengthening hospital training and supervision strategies to increase nurse compliance, ultimately improving overall patient safety.

Abstract

Kompetensi perawat merupakan kemampuan perawat dalam melaksanakan tugas atau tindakan asuhan keperawatan, maka dari itu kompetensi perawat terhadap keselamatan pasien harus dimiliki dan diterapkan dengan tujuan menjamin keselamatan pasien. Tujuan penelitian ini adalah untuk menganalisis perbedaan kompetensi keselamatan pasien pada perawat instalasi rawat inap dan instalasi rawat jalan di rumah sakit perkebunan jember klinik. Penelitian ini menggunakan desain survei komparatif dengan melakukan pendekatan deskriptif kuantitatif. Penelitian ini dilakukan pada perawat instalasi rawat inap dewasa non intensive care sebanyak 76 responden dan 16 responden pada instalasi rawat jalan dengan total 92 responden, penelitian ini menggunakan tehnik total sampling. Instrumen yang digunakan yaitu kuesioner karakteristik responden dan kuesioner H-PEPPS (Health Professional Education in Patient Safety) menggunakan analisis uji Mann-Whitney. Hasil pengukuran kompetensi keselamatan pasien pada perawat instalasi rawat inap menunjukkan bahwa nilai median 113,50 mendekati nilai maksimal (135), pada instalasi rawat jalan menunjukkan bahwa nilai median 117 mendekati nilai maksimal (135), sehingga dapat diinterpretasikan bahwa kompetensi keselamatan pada kedua instalasi tersebut sangat baik, hasil uji komparasi menunjukkan nilai Asymp.sig p 0,300. Kesimpulan dari penelitian ini tidak terdapat perbedaan yang signifikan kompetensi keselamatan pasien pada perawat instalasi rawat inap dan instalasi rawat jalan. Pada kedua instalasi tersebut sama-sama memiliki kompetensi keselamatan pasien dengan kategori sangat baik.

Abstract

Dokter Muter (Near by Doctors and other health workers through integrated services) is an innovation of the Lumajang District Health, Population Control and Family Planning Office (Dinkes P2KB). This innovation aims to bring access to health services closer to remote or geographically challenging areas. The purpose of this study was to determine the effect of health service quality and the Doctor Muter service innovation on patient satisfaction in Lumajang District. The population of this study were patients who had received or were receiving Dokter Muter services in 25 health centers. The data collection method was a questionnaire using Likert scale. This study used a cross-sectional design with proportional stratified random sampling technique. The number of samples in this study was 384 respondents. Data analysis was done using path analysis. Based on the results of the study, it is known that the path coefficient value of service quality on patient satisfaction β = 0.234 is positive with a p-value of 0.000, so that service quality has an influence on patient satisfaction. The path coefficient value of Dokter Muter’s service innovation on patient satisfaction β = 0.639 is positive with a p-value of 0.000, so Dokter Muter's service innovation has an influence on patient satisfaction. The R-squared value for the patient satisfaction variable is 0.707, which means that 70.7 percent of the patient satisfaction variable is influenced by Dokter Muter's service quality and service innovation, and the remaining 29.3 percent is influenced by constructs outside the model.

Abstract

The measurement of spiritual care competence is a crucial aspect of healthcare, particularly in nursing and mental health settings, where holistic care is emphasized to achieve well-being for patients, especially those with terminal conditions. However, determining the appropriate instrument to measure spiritual care competence in nurses is not an easy task. This study aims to identify the most suitable instruments for measuring spiritual care competence in nurses in Indonesia. The method used in this study is a literature review. The keywords used to search for relevant journals are "Nurse" AND "Competence" AND "Spiritual Care." The researcher utilized the Scopus, ScienceDirect, PubMed, and ProQuest databases. The journals included in the review are limited to those published in the last five years over the period from 2020 to 2024 and written in English. Initially, 2,301 journal articles were identified; following a screening and elimination process, 12 articles were selected as suitable for inclusion in this research. This study identified 2,301 journals, which were subsequently filtered for duplicates, irrelevant content, and interventions, resulting in 12 relevant journals. The review found that the instruments used to measure nurses' competence in spiritual care included the Spiritual Care Competence Scale (SCCS), the Spirituality and Spiritual Care Assessment Scale (SSCAS), the Spiritual Care Rating Scale (SSCRS), and the Spiritual Care-Giving Scale (SCGS). This review indicates that the variation in the use of instruments to measure nurses' competence in providing spiritual care is due to differences in respondent characteristics, as well as variations in beliefs and cultures.
