Abstract

Introduction: GISTs usually occur in older adults with a median age of about 60–65 years. The incidence rate between men and women is equal (1:1). The most common location of GISTs is the stomach which accounts for 60%, followed by the jejunum and ileum around 30%, duodenum 5%, and colorectal 5% and a small percentage of cases occur in the esophagus, appendix, gallbladder, and extraintestinal areas such as the mesentery, omentum, and retroperitoneum. Case Presentation: A 59-year-old male, suffered from a lump under the umbilicus 3 months before admission to the hospital. The results of the ultrasound examination showed a mass in the superior area of the bladder, and the mass did not appear to infiltrate the intestines, bladder, umbilicus, and surrounding organs. The biopsy tissues showed spindles and round-to-oval morphology with eosinophilic cytoplasm. Mitoses 65/50 High Power Field In addition, immunohistochemistry examination showed positive stains in DOG 1, CD117, SMA, CD 34, and negative for S100.Conclusion: Extragastrointestinal stromal tumor is a rare tumor. Based on, clinical, imaging, histopathological, and immunohistochemical examination, this case was diagnosed as an extragastrointestinal stromal tumor, spindle cell type.

Abstract

Background: Colorectal adenocarcinoma (CA) is one of the most common malignancies. Tumor budding (TB) status is associated with poor prognosis in patients. Prognosis is influenced by the clinicopathological profile. This study aims to determine the relationship between the clinicopathological profile with TB status in CA at Dr. Cipto Mangunkusumo General National Hospital.Method: A cross-sectional retrospective analytic study using secondary data in the form of cases in large bowel malignancy resection preparations at the Department of Anatomical Pathology in 2019-2021. A total of 213 samples were taken from all cases according to the inclusion and exclusion criteria. Chi square statistical analysis was performed to see the clinicopathological relationship with TB status.Results: Most common TB status were low grade with 92 cases. Most cases were ≥ 50 years old (64.3%), male (50.7%), located in the left colon (77.5%), histopathological degree low grade (85.9%), depth of invasion on pT3 (61.5%), lymphovascular invasion (LVI) (50.2%), lymph node metastasis (52.6%), stage 3 American Joint Committee on Cancer (AJCC (42.3%), without perineural invasion (PNI) (79.3%) and without distant metastases (82.6%). Statistical analysis test showed that there was a significant relationship between the degree of histopathology, depth of invasion, LVI, lymph node metastasis, and AJCC stage (p 0.001) and tumor location (p = 0.036).Conclusion: TB status was significantly related histopathological degree, LVI, lymph node metastasis, depth of invasion, AJCC stage, and tumor location. TB status was not associated with PNI and distant organ metastases.

Abstract

In this era of COVID-19, suspected breast cancer patients experience delay in diagnosis due to the fear of contracting the virus and reduction of non-COVID-19 health services. Furthermore, it may lead to potential increase in the incidence of advanced cancers in the future. Ultrasound-guided (US-guided) percutaneous core needle biopsy (CNB) is a great option for the diagnosis of cancer but it is poorly utilized. This study aimed to prove that the US-guided CNBis accurate when performed in a local setting and a potential solution for diagnosing breast cancer patients in this pandemic. In addition, it was a single health center cross-sectional study, and the participants were all breast cancer patients that had US-guided CNB from 2013-2019. The pathology results from US-guided CNB were compared to specimens from post-CNB surgeries. The data were collected from medical records and the immunohistochemistry (IHC) examinations were carried out for malignancy. There were 163 patients who were included in this study, 86 had malignancies and 77 had benign tumor reported in their CNB results. The US-guided CNB had 100% sensitivity and specificity compared to surgery. With its lower cost, time usage, and patient exposure to the hospital environment, US-guided CNB should replace open surgery biopsy for diagnosing suspicious breast cancers during the pandemic in Indonesia.

Abstract

Background Hirschsprung’s disease (HD) is a developmental disorder of the enteric nervous system, which is characterized by the absence of ganglion cells in the submucosal and myenteric plexuses. The success of surgery is influenced by many factors, one of which is the presence of histopathological abnormalities in the proximal part of the HD rectosigmoid resection. The aim of this study is to determine whether there are any histological characteristics associated with Hirschsprung's disease that would necessitate reoperation at the Department of Anatomical Pathology Faculty of Medicine Universitas Indonesia/Dr. Cipto Mangunkusumo Hospital (FMUI/CMH). Methods This was an analytical study using a cross-sectional design and secondary data from the Department of Anatomical Pathology Faculty of Medicine Universitas Indonesia/Dr. Cipto Mangunkusumo Hospital (FMUI/CMH) archives between 2015 and 2019. At the proximal end, histopathological examinations were performed to determine mature ganglion, nerve thickening, normal ganglion number, perineural inflammation, and serositis. The patient's age at the time of surgery, gender, the type of surgery, and the patient's age at the time of reoperation were all recorded. Results From the 64 resection cases, 15 reoperation resection cases and 49 non-reoperation resection cases were obtained. Of the 15 reoperation cases, the histopathological investigation revealed fifteen cases of mature ganglion, ten cases of nerve thickening, and fourteen cases of normal ganglion. There was no perineural inflammation in fifteen cases, and no serositis in fourteen cases. A p value of 0.026 was obtained for the patient's age at the time of the operation. Conclusion There are several factors that determine pasients with Hirschsprung disease who undergo reoperation after resection. The age of the patient at the time of resection is one of the factors that can affect the occurence of reoperation in patients with Hirschsprung disease.
