Abstract

This study examines quality control issues in a carton printing company experiencing defect rates exceeding the established standard. General Background: Quality control plays a critical role in maintaining product consistency and competitiveness in manufacturing industries. Specific Background: The observed defect rates in the company surpassed the acceptable threshold, reaching up to 1.6% in recent months. Knowledge Gap: Previous approaches have not fully integrated structured statistical analysis with strategic planning tools to address recurring defects. Aims: This research aims to identify defect types, analyze root causes, and propose improvement strategies using Six Sigma with the DMAIC approach combined with SWOT analysis. Results: The findings indicate an average DPMO value of 3874.569 and a sigma level of 4.16, revealing that the process performance is still below the Six Sigma target. Root causes were identified across five factors: human, machine, method, material, and measurement. Novelty: The integration of Six Sigma and SWOT provides a structured analytical and strategic framework for quality improvement. Implications: The proposed strategies, including SOP standardization, operator training, real-time monitoring, and quality audits, are expected to reduce defects and support sustainable operational performance. Abstract

UD. Tiga Putra is an industry engaged in cracker processing. There are product defects in the UD cracker production process. Tiga Putra so that it has an impact on the order rate from 50% to 30%. The purpose of the study is to determine the level of product defects with the highest risk level of failure that occurs and an improvement is made. The method used is the six sigma method and FMEA. The results of this study indicate an average DPMO value of 10848.08, a sigma value of 3.80 with the highest defect value of 41.1% in the type of crumbling defect. To get the best quality results, improvements can be made by performing maintenance and supervision on every aspect or factor that allows failure. Improvements can minimize defects and can increase company productivity so that it can be said that the company has achieved the desired target. Highlights : Utilizing Six Sigma Method and FMEA: Implementing these methodologies ensures a systematic approach to identifying and addressing product defects. Focus on High-Risk Areas: Prioritize improvement efforts based on the identified highest-risk failure types, such as crumbling defects, to maximize quality enhancement. Continuous Improvement Culture: By integrating maintenance and supervision across all factors contributing to failures, the company fosters a culture of ongoing improvement, ultimately enhancing productivity and achieving set targets. Abstract

PT. Bumi Lestari Karya Perkasa experiences 25 workplace accidents in a year. The purpose of this study is to estimate the amount of potential work accident risks in the field of roll forming machines and to control risks with appropriate Personal Protective Equipment (PPE) to reduce occupational risks. The Hazard Identification Risk Assessment and Risk Control (HIRARC) method was used in this study. This research is a qualitative research. Field observations, literature study of company documents, and in-depth interviews with the SMK3 division at PT. Bumi Lestari Karya Perkasa is used to collect data. The risk score is calculated first in the analysis. This research resulted in identification of companies in an effort to improve the safety of all work in the roll forming machine area to avoid unsafe actions and unsafe conditions, as well as the use of Personal Protective Equipment (PPE) by adjusting the type of work being carried out and the lack of awareness about the importance of tools. Highlights : Quantifying Risk: Using HIRARC method to assess and quantify potential work accident risks provides a structured approach to understanding and addressing safety concerns. This study investigates supply chain performance prioritization and improvement strategies for PT. XYZ, a starter card distributor facing operational challenges. Utilizing SCOR and F-AHP methodologies, the research identifies delivery as the highest priority criterion for enhancement, with a weight of 0.49. Recommendations include refining delivery systems, engaging customer feedback, and implementing software solutions to minimize inconsistencies. Abstract

General Background: Halal tourism represents a growing segment in the global tourism industry, integrating Islamic principles with sustainable practices aligned to social, economic, and environmental dimensions. Specific Background: In Indonesia, particularly in Sidoarjo Regency, the halal tourism sector is emerging as a strategic driver of local competitiveness and community empowerment; however, its operational sustainability framework remains underdeveloped. Knowledge Gap: Previous studies rarely integrate sustainability principles with supply chain performance models to evaluate halal tourism competitiveness. Aims: This study aims to integrate the concept of sustainability with the Supply Chain Operations Reference (SCOR) model to assess and strengthen sustainable halal tourism competitiveness in Sidoarjo. Results: Using a descriptive–quantitative approach with 26 respondents, findings reveal high sustainability scores across all pillars—social (4.15), economic (4.20), and environmental (4.13)—with a total sustainability index of 4.16. The economic pillar is most dominant due to strong regulatory support and inter-sectoral collaboration. Novelty: The study introduces a hybrid SCOR–Sustainability framework that operationalizes the halal value chain through the five SCOR processes (Plan–Source–Make–Deliver–Return). Implications: The integration model provides an effective tool for policymakers and tourism stakeholders to harmonize operational efficiency, social responsibility, and environmental stewardship in sustainable halal tourism development. Abstract

PT. XYZ, produsen beton pracetak seperti Corrugated Concrete Sheet Piles (CCSP), menghadapi pemborosan produksi yang menghambat pencapaian target. Produksi hanya memenuhi target pada beberapa minggu, sementara di minggu lain hasilnya lebih rendah, seperti minggu ke-2 (58/60 unit) dan minggu ke-9 (67/70 unit). Penelitian ini menggunakan Lean Manufacturing dengan VSM dan VALSAT untuk mengidentifikasi pemborosan dan meningkatkan efisiensi. Pemborosan utama adalah inappropriate processing akibat ketiadaan SOP terstruktur dan alur kerja tidak efisien. Solusi perbaikan seperti desain ulang proses produksi dan SOP baru berhasil mengurangi aktivitas dari 31 menjadi 21, menurunkan waktu siklus dari 1038,92 menjadi 889,54 menit, dan meningkatkan aktivitas bernilai tambah dari 79,42% menjadi 95,03%. Lean Manufacturing terbukti efektif dalam mengurangi pemborosan dan meningkatkan produktivitas.

Abstract

General Background: Product quality control is a critical requirement in manufacturing industries to maintain production consistency, reduce defects, and support customer satisfaction. Specific Background: PT. XYZ, a wafer manufacturing company, experienced a packaging defect rate of 1.24% over a 12-month period, exceeding the company’s tolerance limit of 1%, with 7,962 defective packages recorded during production. Knowledge Gap: Previous studies have applied Six Sigma to packaging quality problems; however, limited studies have integrated Six Sigma (DMAIC), Root Cause Analysis (RCA), and a 5W+1H approach using a long-term observation period to identify the root causes of packaging defects in halal food products. Aims: This study aimed to identify the dominant packaging defect, determine its root causes, and formulate improvement recommendations to reduce defects within the company’s tolerance limits. Results: The findings revealed 12 types of packaging defects, with endseal corner tear defects representing the largest proportion at 44% of total defects. The average Defect Per Million Opportunities (DPMO) value was 1,131.46, and the average sigma level was 4.56. Root cause analysis identified five contributing factors: method, machine, man, environment, and material. Novelty: The study integrates Six Sigma DMAIC, RCA, fishbone analysis, and 5W+1H within a 12-month evaluation framework to provide a comprehensive diagnosis of packaging quality issues. Implications: The proposed corrective actions, including machine maintenance, standardized operating procedures, process monitoring, operator training, and material control, can serve as a reference for reducing packaging defects and improving production quality management in halal food manufacturing.Highlights: Endseal corner tear accounted for 44% of total packaging nonconformities. Abstract

Rantai pasok padi di Kabupaten Sidoarjo menghadapi risiko kegagalan panen yang meningkat akibat perubahan iklim dan ketidakpastian cuaca. Penelitian ini bertujuan untuk mengidentifikasi, memetakan, dan memprioritaskan risiko kegagalan panen pada setiap tahapan rantai pasok padi menggunakan model Supply Chain Operations Reference (SCOR) dan metode Failure Mode and Effect Analysis (FMEA). Hasil identifikasi menunjukkan terdapat 24 risiko yang tersebar pada tahap plan, source, make, deliver, dan return. Hasil analisis FMEA menunjukkan risiko tertinggi adalah ketidakakuratan informasi iklim dan cuaca pada tahap perencanaan serta serangan Organisme Pengganggu Tanaman (OPT) pada tahap produksi. Berdasarkan hasil analisis, usulan perbaikan mencakup penguatan sistem data iklim, peningkatan kapasitas petani, penerapan Pengendalian Hama Terpadu, dan koordinasi rantai pasok.

Abstract

Management is an important process in achieving goals through the coordination and supervision of others. In the context of Early Childhood Education (ECE), effective management is essential to ensure optimal education quality. Teachers play a central role in the success of education, yet a lack of discipline among teachers is often an obstacle. In the midst of increasing competition between schools, improving the quality of Human Resources (HR) is crucial. This article explores the application of Kaizen culture as a strategy to improve the quality of human resources at KB Permata Sunnah Sidoarjo. Kaizen, which focuses on continuous improvement through individual contributions, is applied to establish better work discipline and improve the quality of education. The results of the application of Kaizen are expected to create a more productive work environment and support the success of education in PAUD.

Abstract

 This study aims to analyze the priority of integrating the halal supply chain in halal tourism, the opportunities and challenges faced by business actors, and the impact level of halal tourism on entrepreneurs, surrounding communities, and the government. Halal tourism is a tourism concept that provides services, facilities, and primarily to meet the needs of Muslim travelers. The research findings indicate that the current priority in halal tourism lies in enhancing competitiveness and achieving substantial financial benefits. This study seeks to identify priority factors in the integration of the halal supply chain within the tourism sector, focusing on key elements that support the creation of a sustainable halal tourism ecosystem. The research methodology employs a mixed methods approach, combining quantitative analysis through questionnaires distributed to supply chain actors (business owners, academics, tourists) and qualitative analysis through interviews with stakeholders.
