Abstract

General Background: Supplier evaluation is a critical component of supply chain management to ensure operational continuity and procurement quality. Specific Background: Organizations require structured decision-making tools to assess multiple criteria in selecting and evaluating suppliers objectively. Knowledge Gap: Although the Analytical Hierarchy Process (AHP) has been widely applied, its structured implementation for supplier performance prioritization in the observed company context has not been systematically documented. Aims: This study aims to determine priority criteria and identify the best supplier alternative using the AHP framework. Results: The analysis produced weighted criteria rankings, verified consistency ratios below the acceptable threshold, and generated a final supplier priority order based on global weights. Novelty: The study presents a comprehensive hierarchical structuring of criteria and sub-criteria aligned with company-specific procurement conditions. Implications: The findings provide a structured decision support basis for managerial policy in supplier selection and contribute to applied research in supplier performance measurement using multi-criteria decision-making methods. Abstract

General Background: Technology capability plays a critical role in transforming raw materials into value-added products within small and medium manufacturing enterprises. Specific Background: Krian Custom Tools, a knife manufacturing IKM established in 2013, relies on technoware, humanware, infoware, and orgaware components to maintain product quality and production consistency. Knowledge Gap: The technological components had not previously been quantitatively measured to determine their contribution level and corresponding strategic direction. Aims: This study aims to calculate the Technology Contribution Coefficient (TCC) using the technometric method and formulate strategic recommendations through SWOT analysis. Results: The overall TCC value reached 0.805, categorized as very good and modern. Component sophistication values were 0.689, 0.880, 0.858, and 0.847, and all components were positioned in Quadrant I of the SWOT diagram. Novelty: The study integrates technometric evaluation and SWOT positioning within a knife manufacturing IKM context. Implications: The findings provide a structured basis for strategic technology development and decision-making in small-scale manufacturing enterprises. Abstract

The provincial board of Muhammadiyah East Java categorizes three kinds of superior schools: The Outstanding School, The Excellent School, and The Inspiring School of Muhammadiyah. In 2021 there were seven schools categorized as The Outstanding School of Muhammadiyah, one of which is SMA Muhammadiyah 2 Sidoarjo. As the Outstanding School of Muhammadiyah East Java, SMA Muhammadiyah 2 Sidoarjo manages its finances by applying management principles to meet all funding needs for academic and non-academic processes. This study aims to analyze the implementation of financial management at SMA Muhammadiyah 2 Sidoarjo as a school with the category of The Outstanding School of Muhammadiyah. This study uses a qualitative method with a case study design that collects and analyzes financial management data at SMAMDA Sidoarjo. Collecting data using the method of observation, interviews, and documentation. Data analysis was carried out in three stages: data reduction, data exposure, and conclusion examination and verification. The study revealed that the financial management of SMAMDA Sidoarjo runs in accordance with the principles and procedures of financial management of Muhammadiyah schools, including planning, organizing, implementing, controlling, and accountability. In addition to the principles of transparency and accountability, this school also strengthens the principle of budget surplus so that every year the school is able to generate a significant plus balance. In the process of recording, bookkeeping, and reporting using the Sango Professional application system in accordance with financial accounting standards for non-profit institutions (ISAK 35). Financial management in this school shows high transparency and accountability. This can be seen from the achievement of the results of the school financial audit conducted by the Public Accounting Firm in 2021 with the results of an Unqualified Opinion.

Abstract

UD. Tiga Putra is an industry engaged in cracker processing. There are product defects in the UD cracker production process. Tiga Putra so that it has an impact on the order rate from 50% to 30%. The purpose of the study is to determine the level of product defects with the highest risk level of failure that occurs and an improvement is made. The method used is the six sigma method and FMEA. The results of this study indicate an average DPMO value of 10848.08, a sigma value of 3.80 with the highest defect value of 41.1% in the type of crumbling defect. To get the best quality results, improvements can be made by performing maintenance and supervision on every aspect or factor that allows failure. Improvements can minimize defects and can increase company productivity so that it can be said that the company has achieved the desired target. Highlights : Utilizing Six Sigma Method and FMEA: Implementing these methodologies ensures a systematic approach to identifying and addressing product defects. Focus on High-Risk Areas: Prioritize improvement efforts based on the identified highest-risk failure types, such as crumbling defects, to maximize quality enhancement. Continuous Improvement Culture: By integrating maintenance and supervision across all factors contributing to failures, the company fosters a culture of ongoing improvement, ultimately enhancing productivity and achieving set targets. Abstract

PT. Bumi Lestari Karya Perkasa experiences 25 workplace accidents in a year. The purpose of this study is to estimate the amount of potential work accident risks in the field of roll forming machines and to control risks with appropriate Personal Protective Equipment (PPE) to reduce occupational risks. The Hazard Identification Risk Assessment and Risk Control (HIRARC) method was used in this study. This research is a qualitative research. Field observations, literature study of company documents, and in-depth interviews with the SMK3 division at PT. Bumi Lestari Karya Perkasa is used to collect data. The risk score is calculated first in the analysis. This research resulted in identification of companies in an effort to improve the safety of all work in the roll forming machine area to avoid unsafe actions and unsafe conditions, as well as the use of Personal Protective Equipment (PPE) by adjusting the type of work being carried out and the lack of awareness about the importance of tools. This study investigates supply chain performance prioritization and improvement strategies for PT. XYZ, a starter card distributor facing operational challenges. Utilizing SCOR and F-AHP methodologies, the research identifies delivery as the highest priority criterion for enhancement, with a weight of 0.49. Recommendations include refining delivery systems, engaging customer feedback, and implementing software solutions to minimize inconsistencies. These findings offer valuable insights into supply chain management for similar distributors, aiming to enhance efficiency, customer satisfaction, and market competitiveness. Abstract

General Background: Logistics services play a critical role in supply chain performance, particularly in ensuring timely and accurate goods delivery. Specific Background: PT. APL faces recurring operational issues in delivery activities, including delays, damages, and discrepancies, which lead to financial losses due to customer claims. Knowledge Gap: Existing studies often apply risk analysis methods separately, leaving limited integration between structured risk identification and mitigation prioritization approaches. Aims: This study aims to identify, prioritize, and develop risk mitigation strategies in logistics service operations using an integrated Risk Breakdown Structure (RBS) and House of Risk (HOR) approach. Results: The analysis identified 18 risk events across six delivery stages caused by 29 risk agents, with the highest Aggregate Risk Potential value reaching 2,752. The study proposed four main mitigation priorities based on Effectiveness to Difficulty values, including double checking systems, checklist implementation, employee training, and operational standard setting. Novelty: This research demonstrates a structured integration of RBS and HOR to systematically connect risk identification, prioritization, and mitigation strategy development in logistics operations. Implications: The findings provide practical guidance for logistics companies in reducing operational risks and improving service quality through targeted and prioritized mitigation actions. Specific Background: Many production processes experience failures that lead to decreased performance due to unstructured identification and prioritization of risks. Knowledge Gap: Previous studies often apply Failure Mode and Effect Analysis (FMEA) and Analytical Hierarchy Process (AHP) separately, limiting the ability to systematically determine risk priorities. Aims: This study aims to identify potential failures, assess risk levels, and determine priority mitigation strategies using an integrated FMEA and AHP approach. Results: The analysis identified multiple failure modes with varying Risk Priority Number values, where dominant risks were determined through severity, occurrence, and detection criteria. The AHP method produced weighted priorities that guided the selection of key mitigation actions. Novelty: This study presents an integrated framework combining FMEA and AHP to support structured and hierarchical risk prioritization. Implications: The findings provide a practical basis for decision making in managing operational risks and improving process reliability through prioritized mitigation actions. Abstract

General Background Industrial manufacturing systems require stable production processes to maintain productivity and product quality. Specific Background PT. XYZ experiences significant production delays in liquid glucose processing due to machine performance degradation, raw material issues, employee factors, and consumer-related constraints, resulting in a production decrease of up to 20% from the daily target. Knowledge Gap Previous studies tend to analyze production delays separately, lacking an integrated approach combining root cause identification, risk prioritization, and decision-making. Aims This study aims to identify the causes of production delays, evaluate failure risks, and determine optimal corrective actions using Fishbone Diagram, Failure Mode and Effects Analysis (FMEA), and Analytical Hierarchy Process (AHP). Results The findings indicate that machine maintenance issues and external factors are dominant causes, with the highest Risk Priority Number (RPN) identified in the filtration and ion exchanger process (192). AHP analysis shows that dismantling and repairing the heating pipeline is the top-priority solution with a weight of 67.4%. Novelty This study integrates Fishbone, FMEA, and AHP methods to provide a structured and multi-criteria decision-making framework in glucose production systems. Implications The results support improved maintenance planning and production management strategies to reduce delays and maintain operational stability in manufacturing environments. Abstract

PT. XYZ, produsen beton pracetak seperti Corrugated Concrete Sheet Piles (CCSP), menghadapi pemborosan produksi yang menghambat pencapaian target. Produksi hanya memenuhi target pada beberapa minggu, sementara di minggu lain hasilnya lebih rendah, seperti minggu ke-2 (58/60 unit) dan minggu ke-9 (67/70 unit). Penelitian ini menggunakan Lean Manufacturing dengan VSM dan VALSAT untuk mengidentifikasi pemborosan dan meningkatkan efisiensi. Pemborosan utama adalah inappropriate processing akibat ketiadaan SOP terstruktur dan alur kerja tidak efisien. Solusi perbaikan seperti desain ulang proses produksi dan SOP baru berhasil mengurangi aktivitas dari 31 menjadi 21, menurunkan waktu siklus dari 1038,92 menjadi 889,54 menit, dan meningkatkan aktivitas bernilai tambah dari 79,42% menjadi 95,03%. Lean Manufacturing terbukti efektif dalam mengurangi pemborosan dan meningkatkan produktivitas.
