Abstract

General Background: Logistics services play a critical role in supply chain performance, particularly in ensuring timely and accurate goods delivery. Specific Background: PT. APL faces recurring operational issues in delivery activities, including delays, damages, and discrepancies, which lead to financial losses due to customer claims. Knowledge Gap: Existing studies often apply risk analysis methods separately, leaving limited integration between structured risk identification and mitigation prioritization approaches. Aims: This study aims to identify, prioritize, and develop risk mitigation strategies in logistics service operations using an integrated Risk Breakdown Structure (RBS) and House of Risk (HOR) approach. Results: The analysis identified 18 risk events across six delivery stages caused by 29 risk agents, with the highest Aggregate Risk Potential value reaching 2,752. The study proposed four main mitigation priorities based on Effectiveness to Difficulty values, including double checking systems, checklist implementation, employee training, and operational standard setting. Novelty: This research demonstrates a structured integration of RBS and HOR to systematically connect risk identification, prioritization, and mitigation strategy development in logistics operations. Implications: The findings provide practical guidance for logistics companies in reducing operational risks and improving service quality through targeted and prioritized mitigation actions. Abstract

General Background: Risk management plays a crucial role in maintaining productivity and operational stability in industrial systems. Specific Background: Many production processes experience failures that lead to decreased performance due to unstructured identification and prioritization of risks. Knowledge Gap: Previous studies often apply Failure Mode and Effect Analysis (FMEA) and Analytical Hierarchy Process (AHP) separately, limiting the ability to systematically determine risk priorities. Aims: This study aims to identify potential failures, assess risk levels, and determine priority mitigation strategies using an integrated FMEA and AHP approach. Results: The analysis identified multiple failure modes with varying Risk Priority Number values, where dominant risks were determined through severity, occurrence, and detection criteria. The AHP method produced weighted priorities that guided the selection of key mitigation actions. Novelty: This study presents an integrated framework combining FMEA and AHP to support structured and hierarchical risk prioritization. Implications: The findings provide a practical basis for decision making in managing operational risks and improving process reliability through prioritized mitigation actions. Abstract

General Background Industrial manufacturing systems require stable production processes to maintain productivity and product quality. Specific Background PT. XYZ experiences significant production delays in liquid glucose processing due to machine performance degradation, raw material issues, employee factors, and consumer-related constraints, resulting in a production decrease of up to 20% from the daily target. Knowledge Gap Previous studies tend to analyze production delays separately, lacking an integrated approach combining root cause identification, risk prioritization, and decision-making. Aims This study aims to identify the causes of production delays, evaluate failure risks, and determine optimal corrective actions using Fishbone Diagram, Failure Mode and Effects Analysis (FMEA), and Analytical Hierarchy Process (AHP). Results The findings indicate that machine maintenance issues and external factors are dominant causes, with the highest Risk Priority Number (RPN) identified in the filtration and ion exchanger process (192). AHP analysis shows that dismantling and repairing the heating pipeline is the top-priority solution with a weight of 67.4%. Novelty This study integrates Fishbone, FMEA, and AHP methods to provide a structured and multi-criteria decision-making framework in glucose production systems. Implications The results support improved maintenance planning and production management strategies to reduce delays and maintain operational stability in manufacturing environments. Abstract

Objective: This research was conducted due to the condition of the company which was only founded in June, so that in July the company received few orders and in the following month, namely August, the company received quite a lot of orders and so this research uses a base period in September because the company received 500 orders. pcs and in October the company received a decrease in orders so it experienced a decrease in profits. Therefore this research aims to measure the company's productivity and profitability based on changes in labor, material, energy, capital and total input at UD. Lontong Irfan. Method: The method used involves calculating productivity and profitability indices as well as evaluating price improvement indices to evaluate the impact of changes in input costs. Results: The analysis results show that labor productivity remains stable despite an increase in wages, while material profitability has increased but productivity has decreased. Energy and capital productivity remained stable with no decline in profitability. Although total input productivity decreased, profitability increased, highlighting the complexity of the influence of input variables on a company's overall performance. Novelty: This study provides an in-depth view of how variations in different input factors such as labor, material, energy, and capital simultaneously affect productivity and profitability within a newly established company context, emphasizing the nuanced relationship between cost efficiency and performance outcomes.

Abstract

PT. XYZ is a food manufacturing company withshrimp crackers as its main product. Thisresearch aims to analyze and make improvementsto the food safety traceability system that hasbeen applied previously and to develop productsof PT. XYZ using Quality Function DeploymentMethod (QFD). This research uses paper-basedtraceability and Quality Function DeploymentMethod (QFD).After analyzing the food safetytraceability system and analyzing consumerneeds using the QFD method, the results of theanalysis show that the food safety traceabilitysystem is improved by integrating coding alongthe supply chain and improving documentation.Improvements using QFD method is based onthree main priorities, those are maintainingconsistency in the aspect of packaging accordingto prevailing standards, packaging product that isinformative, as well as maintaining andimproving the aspects of color, taste, and smell ofthe product.

Abstract

Abstract — CV. 777 is a spice import company that has problems in mitigating events that are quite detrimental to the company. Starting from late arrival until over stock in the warehouse. The method used is the House Of Risk and SWOT methods, where with these methods the highest risk can be known and mitigation actions will be taken. This research resulted in 14 Risk Events and 27 Risk Agents where the Risk Agent with the highest ARP value will be followed up using the SWOT method. In the study, one Risk Agent with the highest ARP value or the one with the strongest influence was found, namely Risk Agent A12 with an ARP value of 255 (fluctuations in demand for goods by customers), while the Risk Agent with the weakest influence was A4 with an ARP value of 3 (imported goods). excessive) and A26 with an ARP value of 3 (miscommunication between employees). The risk agent with the highest ARP then continued using the SWOT method and resulted in four strategies, namely SO strategy, ST strategy, WO strategy, and WT strategy. With the mitigation strategy produced by the company, it will be able to minimize the risks that can occur to the company.

Abstract

CV Yunanda is one of the companies whose manufacturing process is in the form of shoes and sandals, CV Yunanda has one of PDL's main products in the field of leather footwear. The problem with this company is a production defect. Based on these problems, the purpose of this study is to determine the extent of the defects, determine the underlying causes of defective products, and make recommendations for improvement to reduce the number of defective products. This study uses seven tools and six sigma methods to identify the key factors that cause defective products and identify corrective actions to be taken based on the underlying factors. The results showed that there were 4 types of product defects, namely 13% overlapping skin defects and 11% flexed skin, 10% wrinkled skin, 7% scratched skin. The proposed solution is to control, inspect and perform maintenance on machines or equipment used in the production process, establish SOPs for each machine area, improve human resources through training and improvement of the work environment.
