Abstract

Background: Diabetes mellitus is a chronic disease that causes neuronal plasticity and increased hypothalamic pituitary adrenal (HPA) axis of stress disorders. The change in metabolism is reportedly associated with inadequate response to antianxiety and antidepressant agents. Objective: This study aimed to determine the effect of milnacipran antidepressants on anxiety-like behavior in mice with diabetes mellitus. Methods: Male ICR mice were divided into naive, stress, diabetes mellitus (DM), DM + stress groups to measure anxiety-like behavior. Diabetes mellitus was induced using alloxan, and electric footshock stress was used as a stressor for 14 consecutive days. Anxiety-like behavior was measured using the light-dark box (LDB) and elevated plus maze (EPM) test at days 0, 7 and 14. The antidepressant milnacipran (MIL) was given for 7 days, on days 8 to 14. On day 14, evaluation of anxiety-like behavior after administration of MIL was carried out in all groups using LDB and EPM tests. Results: The results showed that administration of milnacipran effectively ameliorated anxiety-like behavior in the non-DM, but not in the DM group, using the LDB test. A similar result was demonstrated in the EPM test showing the non-DM group's attenuation after milnacipran administration. Conclusion: The present results indicate that there is an inadequate attenuation of the anxiety-like behavior after treatment with milnacipran in diabetes conditions.

Abstract

Drug information off label is not widely known and understood by health workers, both doctors and pharmacists. Prescribing drugs off label will also have an impact on ethical rules and drug costs, whether legal and can be covered by insurance. Therefore, there needs to be a continuing education activity for Pharmacists and health professionals to provide information and understanding of participants with the development of drugs off the label. This activity aims to improve the competencies of the participants, as well as understand the legal prescribing rules and as input for insurance and pharmaceutical regulations. The form of activities that will be carried out are workshops and training. The event was held for 2 days, 14-15 October 2018 at Graha Iptekdok, FKUA. There are 10 resource persons with 11 materials divided into 6 workshops, and 1 Micromedex training activity. This activity was attended by 383 participants and 5 invitations. Among the 383 participants, 55% were pharmacists working in hospitals, 30% were pharmacists working in pharmacies, and the rest were pharmacists working in industries, health centers, health services, and pharmacy students. Domicile participants come from various regions, with the largest percentage from Java. Based on the results of the questionnaire evaluation, the average material suitability assessment with participants’ needs was 8 out of 10. On the other hand, the material mastery by the speakers was also very good (8 out of 10). While in terms of the event, the average participant rated very well (7-9 of a scale of 10). A value of 7 is given to the parameters of room facilities. Seeing the enthusiasm of the participants of the Off Label Workshop andTraining in Community and Clinical Settings, the committee plans to follow up by holding a similar event to accommodate potential participants who cannot be accommodated in this event.AbstrakInformasi obat off label belum banyak diketahui dan dipahami oleh para tenaga kesehatan, baik dokter maupun apoteker. Peresepan obat off label juga akan berdampak pada aturan etik dan biaya obat, apakah legal dan bisakah ditanggung oleh asuransi. Oleh sebab itu, perlu adanya suatu kegiatan pendidikan berkelanjutan kepada para Apoteker dan profesi kesehatan untuk memberikan informasi dan pemahaman peserta dengan perkembangan obat off label. Kegiatan ini bertujuan untuk meningkatkan kompetensi para peserta, juga memahami aturan peresepan yang legal dan sebagai masukan untuk asuransi serta peraturan farmasi. Bentuk kegiatan yang akan dilakukan adalah workshop dan training. Acara dilakukan selama 2 hari, 14–15 Oktober 2018 di Graha Iptekdok, FKUA. Terdapat 10 narasumber dengan 11 materi yang dibagi dalam 6 workshop, dan 1 kegiatan pelatihan Micromedex. Kegiatan ini dihadiri oleh 383 peserta dan 5 undangan. Diantara 383 peserta, 55% merupakan apoteker yang bekerja di rumah sakit, 30% adalah apoteker yang bekerja di apotek, dan sisanya adalah apoteker yang bekerja di industri, puskesmas, dinas kesehatan, dan mahasiswa farmasi. Domisili peserta berasal dari berbagai daerah, dengan prosentase terbesar dari Jawa. Berdasarkan hasil evaluasi kuesioner, rata-rata penilaian kesesuaian materi dengan kebutuhan peserta adalah 8 dari skala 10. Selain itu, penguasaan materi oleh narasumber juga sangat baik (8 dari skala 10). Sedangkan dari segi acara, rata-rata peserta menilai sangat baik (7–9 dari skala 10). Nilai 7 diberikan pada parameter fasilitas ruangan. Melihat antusiasme dari peserta off Label Drugs Workshop and Training in Community and Clinical Settings, maka panitia berencana akan melakukan tindak lanjut dengan mengadakan acara yang serupa untuk mengakomodir calon peserta yang tidak dapat tertampung pada acara ini.

Abstract

The prevalence of diabetes mellitus (DM) is higher among other degenerative diseases in Surabaya, Indonesia. Providing information is not sufficient to improve diabetic patients’ compliance and achieve goals of its therapy. Patients’ good awareness as well as emotional and social supports from community plays an important role to improve their compliance and clinical outcomes. Therefore, diabetes training program was delivered to geriatric worker community to improve their knowledge on DM and assessment skill to check the blood glucose level of geriatric diabetic patients. A total of 20 female workers were enrolled in the program. They received a series of lectures on diabetes and its management and gained practical skill on using the self-check blood glucose test. A validated questionnaire was administered to all workers at baseline and at final follow-up to assess knowledge and practice. The overall workers’ awareness increased over than 100%. After finishing the program, all workers joined a diabetic counseling program for geriatric patients in their community. Glucose levels were evaluated from their patients to assess the outcome of the counseling program. The random and fasting blood glucose levels decreased around 31% and 14%, consecutively. This study reveals that diabetes training program for geriatric community workers is beneficial to control blood glucose levels. AbstrakDiabetes melitus (DM) merupakan penyakit degeneratif yang memiliki prevalensi lebih tinggi di antara penyakit degeneratif lainnya di Surabaya, Indonesia. Pemberian informasi saja masih dirasakan belum cukup untuk memperbaiki tingkat kepatuhan pasien diabetes dan mencapai tujuan terapi. Kesadaran pasien serta dukungan emosional dan sosial dari masyarakat berperan penting untuk meningkatkan kepatuhan dan hasil terapi. Oleh karena itu, program pelatihan diabetes dilakukan kepada komunitas pekerja lanjut usia (lansia) untuk meningkatkan pengetahuan mereka tentang penyakit DM dan ketrampilan dalam pemeriksaan kadar glukosa darah. Sebanyak 20 orang pekerja perempuan ikut serta sebagai peserta dalam program ini. Mereka menerima serangkaian materi tentang penyakit DM dan pengelolaannya, serta mendapatkan praktek keterampilan untuk menggunakan tes glukosa darah secara mandiri. Sebagai evaluasi kegiatan, kuesioner diberikan kepada peserta pada awal dan akhir kegiatan. Kesadaran pekerja secara keseluruhan meningkat lebih dari 100%. Setelah akhir kegiatan, semua peserta bergabung dengan program konseling diabetes untuk pasien lansia di komunitas mereka. Tingkat glukosa dievaluasi dari pasien mereka untuk menilai hasil program konseling. Tingkat glukosa darah acak dan puasa menurun sekitar 31% dan 14%, berturut-turut. Studi ini mengungkapkan bahwa program pelatihan diabetes untuk pekerja komunitas lansia ini sangat bermanfaat untuk mengendalikan kadar glukosa darah di komunitas.

Abstract

Background: In developing a pharmaceutical product, it is necessary to conduct pre-clinical and clinical trials to ensure its safety and effectiveness. The toxicity test is conducted to assess the safety of a substance to determine its toxic effect of the substance. Objective: This study aims to determine the acute and subchronic toxicity of administering IHDM allergenic extract using experimental animal models. Methods: Female BALB/c mice and female and male Wistar rats were used as experimental animal models. While the IHDM allergenic extract was used with the level of Der p1 is 11.3-26.6 ng/mL and was administered by intravenous route. The acute toxicity test was carried out for 14 days on four different dose groups of experimental animals. The subchronic toxicity test was carried out for 28 days using three other dose groups of experimental animals. Results: The administration of a single dose of IHDM allergenic extract at various doses did not cause mice behaviour changes, and no death was shown in each group. Likewise, there was no change in the principal organs by macroscopic observations. Meanwhile, administering IHDM allergenic extract at repeated doses for 28 days could show signs of toxicity. The symptoms were shown in the histopathological structure of the liver, kidney, and heart organs. Conclusion: It can be concluded that the IHDM allergenic extract is safe for single-dose administration but shows toxic signs when given in repeated doses. Further tests are needed for 90 days of subchronic toxicity and satellite testing.

Abstract

Background: CIPN (Chemotherapy-induced Peripheral Neuropathy) primarily affects the sensory system and is accompanied by pain, autonomic dysfunction, and motor impairments. Alterations of intracellular second messengers at the supraspinal level in CIPN needed to be explored more. In addition, there is a lack of evidence regarding implications for the supraspinal area through the propagation of pain via the ascending pathway. Objective: In this study, we evaluated the effect of fenofibrate as a PPARα agonist in suppressing the development of CIPN. Methods: Twenty-four mice were distributed to the normal control group, neuropathy group, and neuropathy with the treatment of fenofibrate 75 and 150 mg/kg groups, resulting in 6 animals per group. Oxaliplatin was injected on days 0, 2, 4, and 6. The hot plate test was performed before the oxaliplatin administration and then continued on the 7th, 14th, and 21st days. Thalamus tissues were collected to measure the TRPA1 mRNA expression using qPCR. Results: Fenofibrate 75 mg/kg co-treatment with oxaliplatin tended to prevent the enhancement of oxaliplatin-induced thermal hyperalgesia in hind-paw withdrawal and rubbing responses. Furthermore, fenofibrate 75 and 150 mg/kg co-treatment with oxaliplatin significantly reduced the relative TRPA1 mRNA expression but did not modulate the relative BDNF mRNA expression in the thalamus. Conclusion: PPARα agonist has a potential effect in suppressing the development of CIPN. However, given the various perspectives on the role of neurotrophins in CIPN, additional non-clinical investigations, are needed to provide more insight into other mechanisms of CIPN and the role of PPAR agonists.

Abstract

This study aims to determine the effect of surfactant concentration on Quercetin SLM as a potential carrier of respiratory diseases, especially ones resulting from oxidative stress injury. Quercetin is a natural antioxidant with high activity. SLM was formed with 5% compritol 888 ATO as a lipid and different concentrations of poloxamer 188 as a surfactant. SLM was produced by a combination of emulsification and sonication involving freeze drying. SLM is characterized by organolepsis, morphology, yield, particle size, drug loading, and entrapment efficiency. The antioxidant activity of quercetin SLMs was tested using the ABTS method. SLMs are characterized as having round and smooth morphology, high yield (F1 88.53%; F2 91.44%; F3 92.87%); particle size (F1 1.81 um; F2 1.90 um; F3 1.94 um); high drug loading (F1 15.96%; F2 13.74%; F3 13.19%); and high entrapment efficiency (F1 96.53%; F2 87.94%; F3 87.48%). Increasing surfactant concentration did not produce a significant difference between formulas. Quercetin SLM showed high antioxidant activity (Quercetin 94.43%; F1 94.35%; F2 94.36%; F3 94.37%). SLM was stable at storage temperatures between 25°C and 40°C. The effect of surfactant can be seen on particle size, drug loading, and entrapment efficiency at 40°C. Results of in vivo deposition study indicated that all SLM formulas were able to deliver quercetin to the lungs. Increasing the concentration of surfactant in Quercetin SLMs made no difference to the lung deposition as confirmed by observations conducted at 1 hour and 4 hours. Quercetin SLM has the potential for lung delivery by inhalation.

Abstract

Background: Stress hurts problem-solving abilities, while anxiety disorders and depression are associated with unpleasant feelings and impaired daily functioning. Open field test (OFT) is used to assess anxiety-like behavior in animals by observing their exploratory behavior. Pregnant women are vulnerable to stress, which increases the risk of premature birth and low birth weight. Stress during pregnancy can also lead to preeclampsia and behavioural problems in newborns. Selective serotonin reuptake inhibitors (SSRIs), commonly used antidepressants during pregnancy, help manage mental disorders by increasing serotonin levels and regulating the endocrine system. They reduce glucocorticoid levels, thereby alleviating anxiety and stress in pregnant women. Objectives: This research investigates the correlation between the administration of SSRIs to stress-induced expectant mothers, the manifestation of depression-like behaviour, and the expression of glucocorticoid receptors in the offspring's hippocampus. Material and Methods: This study included four mouse groups: control without stress, control with offspring stress, stress model with footshock on dams and o ffspring, and stress with fluvoxamine treatment. Depressive and stress-related behaviors were measured using OFT. Results: OFT was used to assess the behaviour of offspring mice. Significant differences were observed in the number of crossings in the centre area and behaviour. The SSRI treatment showed potential anxiolytic effects, while stress led to reduced behaviour. These findings contribute to understanding anxiety-related responses and the effects of stress and SSRI treatment in animal models. Conclusions: OFT revealed significant differences in anxiety behaviour, specifically in the number of crossing centres, the centre area, and behaviour. SSRI treatment exhibited anxiolytic effects, while stress-decreased behaviour findings enhanced our understanding of anxiety responses and the impact of stress and SSRI treatment in animal models.

Abstract

Ciprofloxacin HCl-Alginate-Carrageenan Pulmosphere has emerged as a therapy for lung infections. Ciprofloxacin HCl-Alginate-Carrageenan Pulmosphere consists of Ciprofloxacin HCl, Sodium Alginate, Kappa Carrageenan, and CaCl2 made using the ionotropic gelation method with the aim of delivering the drug via inhalation. Ciprofloxacin HCl-Alginate-Carrageenan Pulmosphere exhibits good physical characteristics that can be further studied in vitro using alveolar macrophage cells. Alveolar macrophages demonstrate a high level of effectiveness in removing pathogenic microorganisms from the lungs. More than 80% of alveolar macrophages can be easily obtained through lung lavage for study. This research aims to determine the uptake of Ciprofloxacin HCl-Alginate-Carrageenan Pulmosphere by alveolar macrophages. The extraction of alveolar macrophage cells was carried out through the utilization of the BAL (Bronchoalveolar Lavage) method. Pulmosphere was labeled with a fluorescent dye, Rhodamine-B, for visualization. Uptake evaluation was performed using the Nikon Eclipse TS2R Inverted Microscope. The findings suggest that alveolar macrophages have the ability to uptake Ciprofloxacin HCl-Alginate-Carrageenan Pulmosphere. This research significantly enhances current knowledge by revealing previously unidentified variables. The results not only question established theories but also provide practical insights for the field of pharmaceutics. As a result, this study deepens our comprehension of certain aspects and sets the stage for future research in related domains.

Abstract

Tuberculosis (TB) is one of the leading causes of death from infectious diseases worldwide. Indonesia ranks as the country with the second-highest number of TB cases after India. TB awareness training and medication adherence support for village health cadres are strategic steps to enhance their capacity in supporting TB treatment. This study aims to analyze the impact of TB awareness training and medication adherence support on improving the knowledge of village health cadres. A pre-experimental study with a pretest-posttest design was conducted involving 50 village health cadres in Bluto Subdistrict, Sumenep Regency. Data were collected using questionnaires administered before and after the training. Analysis was performed using the Mann-Whitney test. There was a significant increase in the knowledge scores of village health cadres regarding TB (pretest: 3.50 ± 1.65; posttest: 5.64 ± 2.70; p<0.05) and anti-TB drug side effects (pretest: 7.66 ± 4.07; posttest: 15.04 ± 3.11; p<0.05). The training also successfully enhanced the cadres' understanding of TB symptoms, prevention, and treatment, including drug side effects. TB awareness training and medication adherence support significantly improved the knowledge of village health cadres, making them more competent in supporting TB control efforts at the community level.

Abstract

Pulmonary tuberculosis (TB) is an infectious disease caused by Mycobacterium tuberculosis, which mostly attacks the lungs, but can also affect other organs. Tuberculosis is one of the biggest health problems worldwide. Conventional oral rifampicin preparations have several limitations, such as poor bioavailability, low solubility, and drug instability in the gastrointestinal fluid. Only a small portion of the tuberculosis drug can reach the alveoli, the main target of the tuberculosis drug. Drug delivery Systems are one of the solutions to this problem. They are a formulation or system that can mediate the delivery of therapeutic substances in the body to increase therapeutic effects, reduce drug side effects, increase bioavailability, and improve patient compliance. Pulmonary drug delivery requires a small dose and particle size, so microspheres are selected for lung delivery. This research aims to study the effect of sodium alginate concentration and carrageenan (0.75%, 1%, 1.25%) with a ratio of 1:1 on physical characteristics, in vitro release, and anti-tuberculosis activity. Preparation of Rifampicin Sodium Alginate-Carageenan Pulmospheres with Ionotropic Gelation-Aerosolization. Pulmospheres were evaluated for entrapment efficiency, morphology, yield, particle size, drug loading, in vitro release, and Mycobacterium smegmatis activity. Increasing concentrations of sodium alginate and carrageenan produce rifampicin pulmospheres with good physical characteristics, increase rifampicin release, and result in inhibitory activity against Mycobacterium smegmatis
