Abstract

Managing anemia in Chronic Kidney Disease (CKD) patients with hemodialysis (HD) is a challenge to physicians. The present consensus does not address the proper time of blood sampling in HD patients, but higher ultrafiltration (UF) volume (a process of removing fluid excess during HD) may alter hematologic parameters. The objective of this study was to compare some parameters of the Complete Blood Count (CBC); hemoglobin (Hb), hematocrit (Hct), leukocyte (WBC), and platelet counts (Plt) before and after HD. This method was a cross-sectional study performed in the HD Unit, Dr.Soetomo Hospital, including 51 CKD patients selected consecutively, divided into two groups based on the UF volume (2 L and >2 L). Complete blood count pre- and post-HD were measured using Sysmex XN 1000. The results were 25 males and 26 females in this study, age ranged from 20 to 74-year-old, and 36 patients with UF volume >2 L. Only HD with UF >2 L showed significant increases for Hb (9.35g/dL to 10.00 g/dL), Hct (29.80% to 31.15 %), and Plt (209.00x103/µL to 213.00x103/µL) but WBC did not change significantly. These changes were believed to be caused by ultrafiltration. The conclusion was Hb, Hct, and Plt increased significantly with UF ≥2 L in HD CKD patients.

Abstract

Leukemia Limfoblastik Akut (LLA) adalah penyakit neoplasma yang dihasilkan dari perpindahan somatik multistep progenitorlimfoid di sumsum tulang, ditandai maturation arrest, proliferasi tidak terkendali seri limfoid serta penumpukan limfoblas di sumsumtulang dan darah tepi. Kelainan terkait aktivitas proliferasi sel berkaitan dengan kendali apoptosis. Penelitian ini bertujuan mengetahuiperbandingan persentase proliferasi dan apoptosis limfoblas di darah tepi pasien LLA anak sebelum dan sesudah kemoterapi tahapinduksi. Subjek penelitian sebesar 12 pasien LLA anak kasus baru yang diperiksa sebelum dan sesudah kemoterapi tahap induksi. Jenispenelitian ini cohort prospektif tanpa pembanding. Pemeriksaan proliferasi limfoblas dilakukan menggunakan spesimen darah tepisedangkan pengecatannya menggunakan reagen PI/RNase. Pemeriksaan apoptosis limfoblas dilakukan menggunakan spesimen darahtepi sedangkan pengecatannya menggunakan reagen FITC Annexin V. Pembacaan proliferasi dan apoptosis limfoblas menggunakan alatBD FACSCallibur dengan metode flow cytometry. Rerata persentase proliferasi dan apoptosis limfoblas sebelum kemoterapi tahap induksi7,84%±7,50 dan 11,50%±8,60 sesudah kemoterapi tahap induksi 3,2%±1,89 dan 13,42%±8,10. Persentase proliferasi limfoblas didarah tepi sesudah pemberian kemoterapi tahap induksi terdapat penurunan bermakna, sedangkan pemeriksaan apoptosis limfoblasdidapatkan peningkatan yang tidak bermakna. Persentase proliferasi limfoblas di darah tepi sesudah kemoterapi tahap induksi terdapatpenurunan bermakna, sehingga dapat dipergunakan sebagai peramal keberhasilan pengobatan pasien LLA anak. Pemeriksaan apoptosislimfoblas tidak terdapat perbedaan bermakna sebelum dan sesudah kemoterapi tahap induksi. Perlu penelitian lebih lanjut untukmenganalisis hasil yang didapat.

Abstract

The 2019 coronavirus disease (COVID-19)-associated coagulopathy one of the significant complications often found in hospitalized COVID-19 patients. A hypercoagulable and prothrombotic state characterize coagulopathy with an increased risk of thrombotic events. Abnormal coagulation tests could predict bleeding risk, thrombosis, and disease severity. In addition to increasing the D-dimer, prolonged prothrombin time has decreased survival and increased treatment requirements. Based on laboratory findings, it was reported that 70% of patients with COVID-19 had disseminated intravascular coagulation (DIC) and 30% had thrombosis. Approximately 8% of patients with COVID-19 have hemorrhagic complications, the most common one is gastrointestinal bleeding. Variations in hypercoagulability and bleeding occur in COVID-19 patients. Therefore, the anticoagulant drug should be considered to minimize bleeding risk. An anti-bleeding agent for bleeding complications should be considered for the potential increase of coagulopathy. Thromboelastography (TEG) is a tool that is used to analyze the characteristics of viscoelastic clots, platelet function, and fibrinolysis in whole blood, providing a complete picture of the patient's coagulation status. From various therapeutic guidelines for COVID-19 patients, heparin is used as an anticoagulant drug to prevent thrombosis in COVID-19 patients. Starting from prophylactic doses to therapeutic doses, heparin is used to prevent the severity of COVID-19 patient course. The effect of coagulation on COVID-19 patients varies from no impact to hypercoagulation in TEG results.

Abstract

Acute Coronary Syndrome (ACS) is one of the highest causes of death globally, with the number of deaths reaching more than 9 million people in 2016. Therefore, a fast and accurate ACS diagnosis is needed. This study aimed to determine the relationship between the neutrophil-lymphocyte ratio, the number of coronary artery lesions evaluated by angiography, and troponin I in ACS patients Material and Methods: This research is an analytic observational with a retrospective cross-sectional design. Sampling was carried out using a total sampling technique and obtained 87 samples that met the inclusion and exclusion criteria for the 2019-2020 period at Dr. Soetomo Regional General Hospital Surabaya. The correlation between the number of coronary artery lesions and the neutrophil-lymphocyte ratio to troponin I levels were respectively analyzed using chi-square and spearman-rho with SPSS ver. 25. Results: The results showed a moderately significant correlation between the neutrophil-lymphocyte ratio and troponin I levels (p =0.003, rs = 0.319). While the correlation analysis between the number of coronary artery lesions and the vessel score on troponin I showed insignificant results (p = 0.525), which means that the number of coronary artery lesions was not correlated with troponin I. Conclusion: This study concludes a significant correlation with moderate correlation between the neutrophil-lymphocyte ratio and troponin I. However, there is no significant correlation between the number of coronary artery lesions assessed by the Sullivan and troponin I levels.

Abstract

Desa wisata merupakan kawasan yang mempunyai kearifan lokal yang berpotensi mempunyai daya tarik wisata. Meningkatnya peminat wisata desa dan melimpahnya desa wisata di Indonesia harus diimbangi dengan sumber daya manusia (SDM) yang memadai dalam mengelola dan mengembangkannya, terutama dalam hal kesehatan. Tujuan pengabdian masyarakat ini untuk memberikan edukasi dan pemahaman pada SDM dalam bentuk pelatihan dan penyuluhan kesehatan khususnya di desa wisata, desa kampung adat Segunung Wonosalam Jombang Jawa Timur. Metode yang diaplikasikan dalam pelaksanaan pengabdian masyarakat di desa wisata kampung adat Segunung ini ada dua langkah, yaitu penyuluhan dan pelatihan, dilaksanakan selama 3 hari. Hari pertama pelatihan dengan tema first aid, peserta diberikan pre-test dan post-test sebelum dan sesudah pelatihan. Hari kedua Penyuluhan kesehatan (tema: pengetahuan tentang tanaman obat, pengolahan bahan pangan, dan pola hidup sehat), peserta pada penyuluhan kesehatan diberikan pre-test dan post-test sebelum dan sesudah penyuluhan. Hari ketiga penyuluhan tentang NAPZA. Hasil pre-test dan post-test pada pelatihan first aid didapatkan peningkatan persentase jumlah peserta yang mendapatkan nilai ≥ 60 sebesar 56% dan pada penyuluhan kesehatan didapatkan peningkatan 47%. Penyuluhan dan pelatihan ini diharapkan dapat dipraktikkan dikemudian hari dan bermanfaat untuk kelangsungan objek wisata kampung adat Segunung.

Abstract

ABSTRAK Wabah COVID-19 (coronavirus disease 2019) yang pertama kali berasal dari Wuhan, Cina telah menyebar ke 219 negara dan telah menginfeksi sekitar 104.911.243 penduduk di seluruh dunia. Sejak kasus ini pertama kali dilaporkan pada tanggal 2 Maret 2020 di Indonesia, jumlah kasus terkonfirmasi terus meningkat dan penularan penyakit menyebar dengan cepat di seluruh wilayah tanah air. Hal ini turut berdampak pada keselamatan dan kesehatan tenaga kesehatan yang bertugas dalam penanganan COVID-19. Faktor paparan virus, tekanan kerja yang terlalu berat, penggunaan APD yang tidak adekuat, hingga kelelahan fisik dan mental menjadi pemicu utama krisis kesehatan di lingkungan tenaga kesehatan. Kegiatan pengabdian masyarakat ini diharapkan dapat mengendalikan jumlah tenaga kesehatan yang tertular COVID-19 saat melakukan pelayanan kesehatan kepada pasien. Kegiatan pengabdian masyarakat yang dilaksanakan di ruang pertemuan Hotel Ningrat, Kabupaten Bangkalan berupa edukasi dan penyuluhan tentang pencegahan dan pengendalian infeksi (PPI) di dalam pemeriksaan spesimen COVID-19. Sebanyak 100 peserta tenaga kesehatan puskesmas Dinas Kesehatan Kabupaten Bangkalan mempraktekkan Gerakan Cuci Tangan (hand hygiene) sesuai standar WHO, serta pemasangan dan pelepasan alat pelindung diri (APD) yang benar. Nilai post-test para partisipan petugas kesehatan puskesmas baik dokter umum, perawat, bidan, maupun ATLM yang telah mendapatkan pengetahuan, informasi, dan ketrampilan pencegahan dan pengendalian infeksi (PPI) sebesar 78 peserta (86,7%) dari 100 partisipan mengalami peningkatan dibandingkan nilai pre-test sebelum pemberian materi edukasi. Kegiatan ini dapat meningkatkan pemahaman mengenai penyakit COVID-19 dan cara penularannya, yang dapat diamati pada peningkatan nilai postes peserta penyuluhan. Kegiatan edukasi dan penyuluhan tentang PPI perlu dilakukan berkesinambungan agar dapat menurunkan jumlah tenaga Kesehatan yang tertular COVID-19. Kata Kunci: COVID-19, Pencegahan dan Pengendalian Infeksi.  ABSTRACT The COVID-19 (coronavirus disease 2019) outbreak, which first originated in Wuhan, China, has spread to 219 countries and has infected around 104,911,243 people worldwide. Since this case was first reported on March 2, 2020, in Indonesia, the number of confirmed cases has continued to increase, and disease transmission has spread rapidly throughout the country. This also has an impact on the safety and health of health workers on duty in handling COVID-19. Factors such as exposure to viruses, excessive work pressure, inadequate use of PPE, and physical and mental exhaustion are the main triggers for the health crisis in the health workforce. This activity is expected to control the number of health workers who are infected with COVID-19 when providing health services to patients. Community service activities carried out in the Ningrat Hotel meeting room, Bangkalan Regency in the form of education and counseling about infection prevention and control in examining COVID-19 specimens. A total of 100 health workers at the Bangkalan District Health Office practiced hand hygiene according to WHO standards and the correct installation and removal of personal protective equipment (PPE) The post-test scores of participants in health center health workers, including general practitioners, nurses, midwives, and ATLM who had received knowledge, information, and infection prevention and control (PPI) skills, were 78 participants (86.7%) out of 100 participants, an increase compared to pre-test values before giving educational materials. This activity can increase understanding of the COVID-19 disease and its mode of transmission, which can be observed in the increase in the posttest value of the counseling participants. Education and outreach activities about PPI must be carried out continuously to reduce the number of health workers infected with COVID-19. Keywords: COVID-19, Infection Prevention and Control.

Abstract

Chronic Kidney Disease (CKD) is a condition characterized by kidney damage and a decrease of Glomerular Filtration Rate of less than 60 mL/ min/1.73 m2 in more than three months. Anemia is the most common complication in patients with CKD who regularly undergo hemodialysis. Reticulocyte Hemoglobin Equivalent (Ret-He) is a new parameter that can reflect the storage of iron for erythropoiesis. This study compared the Ret-He level pre and post-hemodialysis and evaluated the effect of ultrafiltration (UF) hemodialysis to Ret-He level in CKD patients. This research was an observational analytical study. Samples were 50 patients with CKD who underwent hemodialysis regularly in Dr. Soetomo Hospital Surabaya by consecutive sampling from August–September 2017. The measurement of the Ret-He level pre ultrafiltration hemodialysis was divided into UF < 2 L and UF ≥ 2 L. Both groups showed homogenous results. The group with UF < 2 L increased significantly from pre to post ultrafiltration (p=0.010). The group with UF ≥ 2 L was not increased considerably from 30.57±3.62 to 32.69±3.45 (p=0.413). Ret-He level in the group with UF < 2 L was 0.81±1.10, significantly higher than the group with UF  ≥ 2 L  0.12±0.83 (p=0.017). The difference of Ret-He level pre and post ultrafiltration was significant in UF < 2 L. There was a significant increase of the Ret-He level in hemodialysis with  UF < 2 L compared to UF ≥ 2 L. The measurement of Ret-He should be performed before hemodialysis due to an increase in Ret-He after ultrafiltration hemodialysis.

Abstract

Acute Coronary Syndrome (ACS) includes ST-Elevation Myocardial Infarction (STEMI), non-ST Elevation Myocardial Infarction (NSTEMI), and Unstable Angina (UA). Platelet plays an essential role in ACS pathogenesis. Immature Platelet Fraction (IPF) and platelet indices can predict platelet activations. Platelet indices consist of platelet count, Mean Platelet Volume (MPV), Platelet Distribution Width (PDW), plateletcrit (Pct). This study aimed to analyze the differences of IPF and platelet indices among ACS patients. This study was an observational analytical cross-sectional study conducted in Dr. Soetomo Hospital during May-September 2019. The subjects consisted of 30-STEMI, 25-NSTEMI, and 24-UA patients. The EDTA-samples were measured for platelet indices and IPF using Sysmex XN-1000. The differences between IPF and platelet indices among STEMI, NSTEMI, and UA patients were analyzed using Kruskal-Wallis and Mann-Whitney test. The IPF values were significantly higher in STEMI patients than NSTEMI and UA patients. The IPF values of NSTEMI patients were higher than UA patients. The MPV, PDW, and P-LCR were significantly higher in STEMI and NSTEMI compared to UA. The MPV, PDW, and P-LCR values of NSTEMI patients were significantly higher than UA patients. The significant differences between STEMI and NSTEMI toward UA might be caused by the more severe thrombotic conditions in myocardial infarction patients than UA. The IPF values were significantly different among each type of ACS patients gave an opportunity using this parameter to differentiate the ACS types. The MPV, PDW, and P-LCR were significantly higher in myocardial infarction patients than UA patients, which also allowed them to use those parameters to differentiate both conditions.

Abstract

Sepsis is a state of life-threatening organ dysfunction caused by dysregulation of the body's response to infection. Organ dysfunction is marked by an increase in SOFA score ≥ 2 or qSOFA score ≥ 2. Septic shock is a subset of sepsis with fairly severe circulatory disorders that can significantly increase mortality. Although the current gold standard diagnosis method for sepsis is bacterial culture, some researchers believe PCT can help identify sepsis severity because bacterial culture requires a relatively long time. This study aims to determine the cut-off point of procalcitonin in patients with sepsis and septic shock. The data taken were secondary data from the medical records of sepsis and septic shock patients in Dr. Soetomo General Hospital from 2017 to 2019. Determination of cut-off PCT for sepsis and septic shock using Receiver Operating Characteristic (ROC) analysis curve. Most sepsis patients were young (18 - 65 years) (69%) (p = 0.331) and male (60%) (p = 0.156). The majority of the clinical sepsis patients have focal infections of the respiratory system (55.17%). Patients with respiratory tract infections who develop sepsis have an OR of 6.182, which means it is six times more likely to develop septic shock. There was a significant difference between septic and non-septic PCT levels (p = 0.000), and there was a positive correlation between PCT and sepsis. The cut-off of procalcitonin in sepsis was 0.6 ng/mL, and the cut-off of procalcitonin in septic shock was 10 ng/mL.
