Abstract

Diabetic foot ulcers are caused by neuropathy and/or peripheral arterial disease in patients with type 2 diabetes mellitus. These infections are commonly treated with empirical antibiotics. However, excessive or inappropriate antibiotic use may lead to resistance and poor treatment outcomes. This study aimed to evaluate antibiotic prescribing patterns, rationality, and potential drug interactions in diabetic ulcer patients at a government hospital in Bandung from January to December 2023. A retrospective observational design was used to analyze medical records of 63 patients. The most frequently used single antibiotics were ceftriaxone (51%), cefixime (20%), and ceftizoxime (17%), while the most common combination therapy was ceftizoxime + metronidazole (24%). Evaluation of antibiotic rationality showed 100% appropriate indication, 90.48% appropriate drug selection, and 98.41% appropriate dosage. Despite this, 33.33% of prescriptions had potential drug interactions with commonly co-prescribed medications. These findings indicate generally rational antibiotic use with areas for improvement in drug selection and interaction management. The study highlights the importance of continuous monitoring and rational prescribing to prevent antimicrobial resistance and ensure patient safety.

Abstract

The IDF estimates that the prevalence of diabetes mellitus based on age will continue to increase from 2021 to 2045 (24.0% vs 24.7%). Patients with diabetes mellitus have comorbidities that result from elevated blood glucose levels, leading to polypharmacy and potential drug interactions. This study aims to determine the percentage of drug interactions and the relationship between polypharmacy and the potential for drug interactions in patients with type 2 diabetes mellitus at RSU Rama Hadi Purwakarta. The study was conducted using a cross-sectional, retrospective method, analyzing prescriptions of patients with type 2 diabetes mellitus at RSU Rama Hadi from September 1 to 30, 2022. Prescriptions were identified for interaction types and severity using drugs.com, Medscape, and Stockley's Drug Interaction Handbook, 9th edition, with results analyzed using Fisher's exact test. The study found that patients aged 56-65 years (40.1%) were more likely to suffer from type 2 diabetes mellitus, with females having a higher incidence compared to males (70.7% vs 29.3%). Among 147 prescriptions, 97% had drug interactions, with moderate severity predominating (97.8%), and pharmacodynamic interactions being the most common (76%). Fisher's exact test revealed a p-value of 0.001 (p < 0.005). Thus, it can be concluded that there is a significant relationship between polypharmacy and the potential for drug interactions in the prescriptions of type 2 diabetes mellitus patients at the pharmacy department of RSU Rama Hadi Purwakarta.

Abstract

The ethanol extract of guava leaves (Psidium guajava L) in previous studies was known to have antiburn, antiinflammatory, antidiarrhea and antibacterial activity. The preparations made are gel preparations which are preferred by the public. The purpose of this study was to determine the activity of guava leaf ethanol extract gel and its effective concentration in healing burns in rabbits. The research method uses rabbits 4 tails made 5 wounds on the back, consisting of negative control, comparison (Bioplacenton®), gel preparations 3%, 5%, and 11%. Wound diameter measurements were carried out on days 3, 6, 9, 12, 15 and 18. The parameters observed were the size of the wound diameter, wound healing such as wound drying, scab formation, closure of the wound and the growth of new hair around the wound. An inflammatory process occurs for 3 days after injury and not processed wound healing. Fibroblasts appeared on day 6 and peaked on day 9, so that on day 9 all treatment and control groups experienced linear healing. On the 9th day the fibroblasts began to peel off on the skin indicating the group has reached the peak of the proliferation phase and wound contractions occur gradually urgent. The results of the guava leaf extract gel activity test (Psidium guajava L) showed a concentration of 11% gave the best effect.

Abstract

Pharmacist-led interventions, particularly through education and counselling, play an essential role in supporting treatment adherence and improving the quality of life of breast cancer patients. This article aims to review the influence of pharmacist interventions—such as medication counseling, side effect management, and adherence monitoring—on patient understanding, psychological aspects, and quality-of-life indicators. Results from various experimental studies and randomized controlled trials suggest that structured pharmacist interventions can improve patient understanding, reduce anxiety, and support adherence to long-term therapies such as tamoxifen and chemotherapy. Improvements were also observed in various domains of quality of life, including physical, emotional, and social functioning. However, some studies show inconsistent results, especially in terms of statistical differences in changes in quality of life, which may be due to factors such as sample size, duration of intervention, or other variables, including family support and disease prognosis. Overall, the involvement of pharmacists is an integral part of oncology care, serving as both an educator and a long-term therapy companion.
