Abstract

Background Spleen is an important organ in immunity to malariainfection which is always affected in malaria patients. Enlargementof the spleen is one of the main clinical symptoms of malaria. Levelof parasitemia is related to the risk of complications.Objective To describe the correlation between parasitemia andspleen size in patients with P.falciparum infection.Methods One hundred and four patients including children andadults with fever, pale, jaundice, diarrhea, and headache who vis-ited public health centers in 5 villages in Mandailing Natal, NorthSumatera were enrolled in this study. The diagnosis of malariawas based on blood smears stained by Giemsa method, while thespleen size was measured using Hackett method. Correlation be-tween parasitemia and spleen size was tested by Spearman rankcorrelation.Results Splenomegaly was found in 37 of 104 patients (35.6 %)with the size of H1 in 10.6 %, H2 in 15.4 %, H3 in 9.6 %. The meanparasite count between patients with and without splenomegalywas different, and the difference was significant between patientswith spleen size of H0 and H3. The higher the parasitemia, thebigger the spleen size was (r=0.291; p=0.03).Conclusion There was a correlation between parasitemia andspleen size in falciparum malarial infection

Abstract

Background Soil-transmitted helminthiasis (STH) is an importantpublic health problem in Indonesia.Objective To find out factors influencing the transmission of STHin two different communities in North Sumatera, Indonesia.Methods A cross sectional study was done on 96 primary schoolchildren in Suka village (located on a mountain area) and 96 pri-mary school children in Pantai Cermin village (a coastal area). Sub-jects were recruited by simple random sampling. Kato Katz andcentrifuge floatation methods were used for stool and soil exami-nations, respectively. Data were collected by interviewing parentsof children using a questionnaire and were analyzed using chi–square test by SPSS program version 11.0.Results There was no difference in the prevalence of STH in bothvillages (p>0.05). Parents’ knowledge and economic status hadsignificant relationships with the prevalence of STH in Suka vil-lage (p<0.05), but not in Pantai Cermin (p>0.05). In both villages,parents’ education did not have a significant relation with STH,while there were significant relations between STH and hygiene,environment, or soil contamination by worm eggs (p<0.05).Conclusion There were significant relationships between hygiene,environment, or soil contamination and the prevalence of STH inthe two villages.

Abstract

Objective To investigate the effect of vitamin A supplementationon malaria morbidity.Methods The study was a randomized double-blind placebo-con-trolled trial, conducted in Panyabungan, Mandailing Natal, NorthSumatera from April 2001 to April 2002. Children aged 6-60 monthssuffering from falciparum malaria were randomly assigned to highdose vitamin A or placebo every 4 months for a year. All childrenwere treated in accordance with health center policy. Malaria mor-bidity was assessed from health center visit due to fever, diarrhea,cough, or abdominal pain. Parents gave reports if their child re-ceived malaria treatment from other health centers.Results The number of febrile episodes (probable malaria illness)was lower in the treatment group than that of control, but not sig-nificant. The parasitemia was not different between both groups.There was a significant difference in spleen enlargement betweenthe treatment group and control (p=0.04). There was no differencein health center visit between the two groups.Conclusion The findings suggest that vitamin A supplementationhas only effect on spleen enlargement in malaria.

Abstract

Objectives To determine the association between intestinal hel-minthiasis and hemoglobin (Hb) concentration and to observe theeffect of single dose albendazole treatment on Hb concentration.Methods An experimental study was carried out from March toJuly 2002 on primary school children at Suka Village, Tiga PanahSubdistrict, Karo Regency, North Sumatera Province. From 366children who suffered from helminthiasis, 113 were selected assubjects by simple random sampling. Subjects were treated with asingle oral dose of 400 mg albendazole. Hb concentration wasexamined using the cyanide method twice i.e., prior to and threemonths after treatment with albendazole.Results It was found that among 113 subjects, the prevalences ofAscaris lumbricoides, Trichuris trichiura, and mixed infestation were18.3%, 40.4%, and 41.3%, respectively, while the prevalence ofanemia was 33.0%. There was no significant difference in age,gender, nutritional status, and mean Hb concentration betweenchildren suffering from the different types of worm infestation(P>0.05). For each type of infestation, there were significant differ-ences in mean Hb concentration and anemia prevalence beforeand after treatment (P<0.05).Conclusions There was no difference between the Hb concen-trations of children suffering from Ascaris lumbricoides, Trichuristrichiura, and mixed-type worm infestations. Single dose 400 mgalbendazole was beneficial in increasing Hb concentration andreducing the occurrence of anemia

Abstract

Objective This study was done to compare the dental and oralhygiene status of children with and without congenital heart dis-ease (CHD) by investigating oral microorganisms and dental andoral hygiene indices.Methods A cross-sectional study was done on children aged 2 to15 years from April 2002 to December 2003 at the Department ofChild Health, Haji Adam Malik Hospital, Medan. There were twosubject groups, the first consisting of children with CHD and thesecond of those without. The dental and oral hygiene status wasdetermined by determining caries index and oral hygiene index.Swabs from the subjects’ upper first molars were taken and cul-tured to determine the types of microorganisms present. Data wereanalyzed using the chi-square test, Fisher’s exact test, and t-test.Results There was a significant difference in mean oral hygieneindex between children with (mean 2.75; SD 1.85) and without(mean 1.45; SD 0.78) CHD (P<0.001). Positive bacterial cultureswere more frequent in children with CHD than in those without.The proportion of normal anaerobes was significantly higher in chil-dren with CHD (P=0.038). Such a significant difference was notfound for normal aerobes, pathogenic aerobes, and pathogenicanaerobes. In children with CHD, there was no significant asso-ciation between mean caries index and oral hygiene index withculture results. In those without CHD, a significant association wasfound between the caries index components of decayed tooth in-dex (P=0.003) and DMF-T (P=0.022) with culture results.Conclusions Oral-dental hygiene in children with CHD is poorrelative to that in children without CHD. Abstract

Introduction Little is known about the potential interaction of ironand zinc given to increase hemoglobin and serum ferritin in chil-dren with malaria.Objective To study the effect of iron compared with a combination ofiron and zinc supplementation on children with falciparum malaria.Method Children with positive Plasmodium falciparum (n=86) wererandomly assigned to a daily supplementation of 6 mg iron/kg perday plus placebo or plus 10 mg zinc per day for 30 days. All childrenwere treated with the same regimen for the treatment of P. falciparum.Venous blood samples were collected at the start and end of thestudy. After 30 days of supplementation, the baseline and follow-upblood samples were analyzed.Results The increase of hemoglobin concentration in the ironplus placebo group was 0.58 g/dl, while in the iron plus zinc groupwas 0.09 g/dl (P<0.05). Serum ferritin concentration was high inboth groups before trial, yet there was no significant differenceafter iron supplementation.Conclusions Iron supplementation showed significant increasein hemoglobin concentration in children with positive P. falciparumtreated with the same regimen of treatment. Supplementation ofiron alone as well as iron plus zinc had been proven ineffective toiincrease serum ferritin in children with malaria.

Abstract

Objective To compare the efficacy of quinine-doxycycline to quinine-clindamycin combination, as treatment for uncomplicated falciparum malaria in children.Methods This randomized open labelled controlled trial was conducted from July to August 2007 at Mandailing Natal, Sumatera Utara Province. The subjects were 8 – 18 year old children with positive Plasmodium falciparum from the peripheral blood smear. Simple randomization was performed to determine subject study into two groups of treatment, one group received quinine-clindamycin and the other received quinine-doxycycline treatment. The parasitemia was counted on day 0, 2, 7 and 28. We also observed the adverse effects of the antimalarial combination.Results Two hundred and forty six children who fulfilled the inclusion criteria were divided into two groups. All subjects completed the study. Cure rate achieved 100% from peripheral blood smear examination at the second day observation and showed no recrudescence at day 28th. (P=0.0001). During 28 days follow up, there were 21 (17.6%) patients suffered from headache, 18 (14.6%) vomit and 40 children (32.5%) suffered from tinnitus in quinine-doxycycline combination, compared to quinine-clindamycin combination group only 4 (3.3%) suffered from headache, 1 (0.8%) suffered from tinnitus and there was no vomiting experience in any patient (P < 0.0001).Conclusion Combination of quinine with either clindamycin or doxycycline are effective as an alternative antimalarial treatment. The combination of quinine-clindamycin is well tolerated than the combination of quinine-doxyciline, and this combination may be particular value for young children and pregnant women, as these two groups cannot receive doxycycline.

Abstract

Background Soil transmitted helminthiasis is still highly prevalent in Indonesia, especially in rural area and among poor socio-economic population. Helminthiasis is frequently found as a single or mixed infection. It is difficult to get a medication with better efficacy, low cost, and simple administration for all types of worms.Objective The aim of this sudy was to compare the effectiveness of albendazole and pyrantel pamoate-mebendazole combination in treating soil-transmitted helminthiasis.Methods A randomized clinical trial was perionned in September until November 1995 on students of a primary school in Tanjung Anom Village whose stool examinations showed mixed infection of helminthiasis. Subjects were randomly allocated into two groups. The A group was treated with 400 mg oral albendazole as a single dose, while the B group was treated with the combination of pyrantel pamoate 10 mg/kg body weight as a single dose and mebendazole 100 mg twice a day for three consecutive days. Cure was considered if in the stool examination, no wonn eggs were found. Statistical analysis was periormed by Chi-square test with confidence interval of 95% and p value of < 0.05 was considered significant.Results Out of 541 children, mixed infection of soil-transmitted helminthiasis was found in 374 children (69%). Three hundreds sixty-six children completed the study, consisted of 182 children in group A and 184 in group B. At 3 weeks after treatment, the cure rate in the A group was significantly better compared to that in B group.Conclusions Albendazole was more effective than the combination of pyrantel pamoate - mebendazole for treating mixed infection of soil-transmitted helminthiasis. Besides the administration was simpler and caused minimal side effect.

Abstract

Pirngadi Hospital revealing 41 cases of acute hepatitis, consisted of 61.0% boys and 39.0% girls. The majority of patients were below five years of age (68.3%). There were 37.1% cases with malnutrition. The common symptoms were fever, jaundice, vomiting and dark urine. On laboratory examination there were 10 cases with positive HBs Ag. Accompanying diseases were malnutrition, bronchopneumonia, anemia, bronchitis and pulmonary tuberculosis. A total of 63.4% oj patients were healed, 29.3% discharged against medical advice and 7.3 % died because of bronchopneumonia and hepatic coma.

Abstract

During the period of 3 months (February - April 1987), a prospective study on oxyuariasis among children had been conducted at Child Health Deparment of Dr. Pirngadi Hospital Medan. All children over 8 months of age were included in this study.The diagnosis was based on the modified Scoth 's technique. Oxyuriasis ova were found in 21 out 119 children (17,65%). The peak incidence was found in the school age.
