Abstract

Introduction: Indonesia faces a critical situation with 8% of its COVID-19 deaths being healthcare workers (HCWs). These workers are highly susceptible to the virus due to contact with infected patients. Approximately 35.5% of HCWs treating COVID-19 patients develop symptoms within 14 days, and 7% test positive via PCR. Alarmingly, 36% of HCWs are asymptomatic, posing a significant risk of unknowingly spreading the virus. The pandemic has induced fear of death, infection, and transmitting the virus to family members among HCWs, necessitating psychological support to prevent burnout and maintain a functional healthcare system. Objective: This study aims to analyze the effectiveness of online psychological assistance for HCWs at the forefront of the COVID-19 battle. Methods: One of the psychological assistance methods offered is through social media applications. This type of psychological assistance can be considered the most practical and safe method because it prevents transmission between medical personnel. Results: The increasing number of COVID-19 patients has increased fatigue and infection rates among HCWs. They face uncertainty and feelings of loneliness during the pandemic. However, social support is crucial for psychological first aid and recovery. Misconceptions about COVID-19 being a conspiracy hinder HCWs' efforts. Promoting healthy behaviors such as hygiene practices and mask usage can mitigate the severity of the virus and improve survival rates. Conclusions: In 2020, the surge in COVID-19 cases has forced HCWs to adapt. Nonetheless, conspiracy theories about COVID-19 complicate their duties. Implementing an online psychological assistance application is essential to provide HCWs with the psychological support needed to adapt more effectively to the pandemic's challenges.

Abstract

Schizophrenia is a chronic psychiatric disorder that imposes a substantial burden on patients and caregivers. Antipsychotic polypharmacy remains a common clinical practice to manage persistent positive and negative symptoms, although growing evidence indicates it does not significantly improve treatment outcomes and may instead increase adverse effects, treatment costs, and non-adherence. This case study aimed to describe the clinical benefits and rational considerations of switching from antipsychotic polypharmacy to monotherapy in a patient with hebephrenic schizophrenia. A 37-year-old woman with a 20-year history of hebephrenic schizophrenia was observed. She had previously received a combination of chlorpromazine and trifluoperazine with trihexyphenidyl for extrapyramidal symptom prevention. Clinical evaluations were performed to assess therapeutic effects, side effects, and medication adherence before and after the switch to chlorpromazine monotherapy. During the polypharmacy phase, the patient experienced several side effects, including insomnia, restlessness, irritability, and mild extrapyramidal symptoms. After transitioning to monotherapy with an adjusted chlorpromazine dose, she showed notable improvement in psychotic symptoms, sleep quality, and social functioning without severe adverse reactions. Medication adherence increased as the patient reported greater comfort and tolerance with a single-drug regimen. This case suggests that switching from antipsychotic polypharmacy to monotherapy can reduce side effects, improve adherence, and maintain clinical stability. Rational and individualized use of antipsychotics should be prioritized to achieve better long-term outcomes and quality of life in schizophrenia management.

Abstract

Micro and small-scale digital startups face intense psychological and operational pressures that often result in burnout, chronic stress, and adjustment problems. Yet, mental health issues within startup ecosystems remain insufficiently addressed and lack structured intervention. This literature review synthesizes cross-disciplinary studies published between 2013 and 2025 to examine how workplace spirituality can be integrated into startup work culture and its potential application within community psychiatry. The review indicates that workplace spirituality enhances mental well-being, reduces stress symptoms, and strengthens resilience, functioning as a psychological buffer for entrepreneurs facing high uncertainty. Spirituality also provides a conceptual bridge between managerial and clinical perspectives, enabling more holistic and human-centered approaches to mental health in startup environments. Integrating spiritual values into entrepreneurial settings emerges as an effective non-clinical strategy to sustain founders' mental health and expand opportunities for contextual community psychiatry interventions. The novelty of this study lies in its cross-disciplinary mapping that links startup management, spiritual psychology, and community psychiatry into a practical and preventive intervention framework.

Abstract

Children with Special Needs (CSN) in Surabaya still face limitations in conventional therapy services that tend to focus on symptom management, necessitating a more inclusive and enjoyable alternative approach. The Special Children Festival program presents an innovation through implementing Japanese culture-based behavioral therapy to improve children's emotional regulation, focus, and fine motor skills. The novelty of this activity lies in integrating cultural elements into behavioral therapy and strengthening collaboration between hospitals, universities, and the community. The activity was held on June 28, 2025, at Campus B UNUSA, involving 14 CSN and their parents, three therapists from RSI Jemursari Surabaya, one Japan Corner facilitator, and six UNUSA Medical Faculty students. The implementation method included participatory observation and feedback analysis. The intervention consisted of onigiri-making and Japanese calligraphy (shodo) sessions, preceded by education for parents about culture-based behavioral therapy. Evaluation was conducted by assessing children’s engagement, responses during the activities, and parents’ perceptions of the program’s usefulness. This activity increased CSN participation and had a positive impact on focus, emotional regulation, and fine motor skills. Promotion strategies through parent communities and digital media proved effective in participant recruitment. Service standardization through training and certification of support staff enhanced service quality and sustainability, making this program a replicable model for culture-based behavioral therapy for CSN.

Abstract

Treatment for people with mental disorders (orang dengan gangguan jiwa, ODGJ) in Gresik Regency is still limited to curative (medication) because Gresik Regency does not yet have adequate psychosocial rehabilitation facilities (RPS). RPS aims to restore the social functions of ODGJ so that they are more productive according to their abilities. Activity goals. To initiate the hospital-based RPS activities. Problem-solving framework. The framework is carried out through several activities, such as conducting assessments and focused group discussions with hospitals managers as potential partners, creating RPS service guidelines, conducting training and upgrading skills for health workers, involving ODGJ, families, and health cadres in RPS services, and planning a follow-up with the community health center (community health center, puskesmas), social institutions, and local officials. Results. Grha Husada Gresik Hospital (RSGH) is willing to initiate RPS services in Gresik Regency. This RPS service will be carried out for a minimum of two years, with ODGJ clinical evaluations every three months. The RSGH’s advantage is that it has 4 (four) certified health workers as behavior therapists for children with special needs (ABK). The RPS module was made by modifying the ABK behavior therapy module to suit the needs of adult ODGJ. RPS training is carried out based on the module. A total of 30 participants attended the initial RPS service, consisting of 10 ODGJ patients, 10 companions, and 10 local health cadres. The consistency of the arrival of ODGJ and companions is still low, due to the lack of awareness of the benefits of RPS services. RSGH and the health cadres took the initiative together to pick them up, but the arrival rate was still around 5-6 people per meeting. Conclusion. Good cooperation between hospitals, health cadres, and families is the first step for sustainable RPS services. The main challenge is to raise awareness of the need for RPS services for patients and their families.

Abstract

Islamic boarding schools, known as pesantren, offer a distinctive educational experience where learning is intertwined with religious growth and community life. This research aims to evaluate the impact of school well-being and social support on the tendency for bullying in Islamic boarding schools. Specifically, the study investigates how personal motives and the perceived religious importance inherent in the pedagogical practices of these campuses can predict bullying behavior within the unique social environment and disciplinary system of pesantren institutions. We conducted interviews with 300 students across various pesantren. The cross-sectional survey utilized structural equation modeling (SEM). The findings indicate that personal qualities significantly predict an individual's spiritual meaning (β = 0.21, T = 3.18, p < 0.01) but do not directly relate to motivation or interest experience. In the context of pesantren, spiritual meaning is positively associated with these factors (β = 0.19, T = 2.87, p < 0.01). Spiritual factors significantly enhance motivation (β = 0.34, T = 5.62, p < 0.001) and spiritual meaning (β = 0.41, T = 6.48, p < 0.001). Social support emerges as the most reliable predictor of motivation (β = 0.38, T = 6.11, p < 0.001), interest experience (β = 0.29, T = 4.57, p < 0.001), and spiritual meaning (β = 0.26, T = 4.02, p < 0.001). Notably, interest experience negatively predicts bullying behavior (β = −0.31, T = 4.89, p < 0.001). These results underscore the crucial role of community and spiritual support in enhancing students' well-being and reducing harmful activities in pesantren.

Abstract

The COVID-19 pandemic has profoundly affected mental health and health-related behaviors, including tobacco use. University students represent a vulnerable group due to academic disruptions, social isolation, and lifestyle changes. This cross-sectional study examined the psychosocial impact of the COVID-19 pandemic on smoking behavior among Indonesian university students. An online survey of 62 respondents assessed demographics, smoking status, changes in smoking patterns, and psychosocial factors such as anxiety, stress, mood, and sleep problems. Among participants, 29% were smokers, mostly male (94%) and aged 20–23 years (61%). Conventional tobacco use predominated (83%), while e-cigarettes accounted for 17%. During the pandemic, 61% reported no change in smoking frequency, 33% a moderate increase, and 6% a heavy increase. Interestingly, students with unchanged smoking habits reported higher psychosocial distress, while those who increased smoking reported fewer complaints, suggesting diverse coping strategies. These findings highlight the complex relationship between stress and tobacco use among Indonesian students and underscore the need for integrated mental health and smoking prevention programs in universities.
