Abstract

Background: Children with HIV undergoing treatment cannot be separated from the provision of Antiretroviral Drugs (ARVs). ARV adherence is a very important treatment for HIV sufferers, especially for children because it can improve their quality of life. There are several interventions that can be done to improve ARV treatment adherence, one of which is by providing Short Message Service (SMS). The SMS method has been proven to help improve children's adherence in undergoing ARV treatment. Purpose: To provide an overview of the effect of short message service (SMS) interventions on improving ARV treatment adherence in children with HIV. Method: This study used a systematic review to obtain the latest scientific evidence. The research method was carried out by following the guidelines according to the recommendations of the PRISMA statement. Results: Five articles that have been reviewed reported that SMS interventions can improve treatment adherence in children with HIV. Conclusion: An SMS-based approach can be an effective and feasible method to improve treatment adherence in children with HIV.   Keywords: Adherence; Antiretroviral (ARV); Children with HIV; Short Message Service (SMS).   Pendahuluan: Anak dengan HIV yang menjalani perawatan tak terlepas dari pemberian Antiretroviral (ARV). Kepatuhan ARV merupakan pengobatan yang sangat penting dilakukan pada penderita HIV, khususnya bagi anak karena dapat meningkatkan kualitas hidupnya. Ada beberapa intervensi yang dapat dilakukan untuk meningkatkan kepatuhan pengobatan ARV, salah satunya adalah dengan pemberian Short Message Service (SMS). Metode SMS terbukti dapat membantu meningkatkan kepatuhan anak dalam menjalani pengobatan ARV. Tujuan: Untuk memberikan gambaran pengaruh intervensi pesan singkat (SMS) terhadap peningkatan kepatuhan pengobatan ARV pada anak dengan HIV. Metode: Penelitian ini menggunakan tinjauan sistematis (systematic review) untuk mendapatkan bukti-bukti ilmiah terkini. Metode penelitian dilakukan dengan mengikuti panduan sesuai rekomendasi dari pernyataan  dalam  PRISMA. Hasil: Lima artikel yang telah di review melaporkan bahwa intervensi SMS dapat meningkatkan kepatuhan pengobatan pada anak dengan HIV. Simpulan: Pendekatan berbasis SMS dapat menjadi metode yang efektif dan layak untuk meningkatkan kepatuhan terhadap pengobatan di kalangan anak yang hidup dengan HIV.   Kata Kunci: Anak dengan HIV; Antiretroviral (ARV); Kepatuhan; Pesan Singkat (SMS).

Abstract

The activeness of the elderly coming to the posyandu has an important role for the health of the elderly. In this case, the factor that plays an important role in increasing the activeness of the elderly to the posyandu is information support from the family. This study aims to determine the role of information support from the family in increasing the activeness of the elderly to come to the posyandu. Through the use of correlative quantitative design through a cross-sectional approach with a sample of 370 elderly by random sampling, using a family support questionnaire and a list of elderly attendance at posyandu. This study shows that the frequency distribution of good information support (52.2%) and analysis of the significant relationship between information support and the activeness of the elderly in attending posyandu (p0.05). The important role of information support from the family in supporting the elderly to continue to actively participate in coming to posyandu. Recommendation: Family involvement is needed in providing health information to the elderly to support the elderly to actively attend posyandu as part of health interventions for the elderly

Abstract

Falls in the elderly are a serious problem because they can cause physical injuries, such as fractures, head trauma, and even death. Preventing falls in the elderly requires a multidimensional approach that is not only focused on the individual, but also involves the family as the main support unit. This review aims to identify family nursing-based fall prevention interventions. This systematic review method follows the PRISMA framework with article searches through Cinahl, ProQuest, Pubmed, ScienceDirect, Google scholar, and the Cochrane database of systematic reviews. The findings of this review revealed that of the 541 articles identified in the initial stage, nine articles met the eligibility criteria and were retained for further in-depth analysis. The results of the analysis provide insights into fall prevention interventions including; health education, mindfulness and psychoeducation, Otago exercise program, family involvement, fall risk assessment, and home modification. These findings have important implications for community health nurses in providing family nursing care, through empowering the family as the main caregiver in managing fall risk at home.

Abstract

Elders need friendly, competent, and responsive staff and an organized, clean and welcoming environment. However, preliminary interviews with multidisciplinary teams in nursing homes showed they sometimes felt bored. This study aimed to understand the multidisciplinary teams’ experiences of caring for elders in nursing homes. This is descriptive qualitative research used semi-structured interviews, forum group discussions (FGDs) with 64 participants in Jakarta provincial government-owned nursing homes, document analysis, and observations. The data were collected from September 2022 to July 2023 and analyzed using Colaizzi's measures. The results showed the elders at the nursing homes experienced physical, psychological, behavioral and social changes, where some of them came from poverty or neglect background. Their needs included biological, psychological, social, and spiritual aspects. The multidisciplinary teams felt a variety of emotions when serving the elders, such as happiness, sadness, and sometimes annoyance. Some also felt “reframing,” a feeling of being forced due to no other choice. Therefore, cultivating characters such as empathy, compassion, patience, and sincerity in providing the care is important. Changes in elders with different backgrounds require integrated support. Nursing home management should provide periodic training to the multidisciplinary teams; thus, the service quality can be improved.

Abstract

Background: Type 2 diabetes mellitus (T2DM) is a global health problem with an increasing prevalence. Most related studies focus on Diabetes Self-Management (DSM) in urban populations or general contexts, leaving a gap in understanding the specific factors affecting DSM among rural T2DM patients. Therefore, there is limited understanding of the factors that influence DSM in rural T2DM patients.Purpose: This study aimed to investigate DSM in rural T2DM patients and its associated factorsMethods: This study employed a cross-sectional design. The samples involved 146 patients with T2DM from the community health center in Ogan Komering Ulu Regency, Indonesia, selected using a simple random sampling technique. Diabetes Knowledge Questionnaire (DKQ), Diabetes Management Self-Efficacy Scale (DMSES), Hensarling Diabetes Family Support Scale (HDFSS), and Diabetes Self-Management Instrument (DSMI) were used for data collection. The t-test, ANOVA, Pearson correlation, and multiple linear regression were used to analyze the data.Results: The study found a significant relationship between gender (p=0.001), knowledge (p=0.001), self-efficacy (p=0.001), and family support (p=0.032) with DSM in rural T2DM patients. However, the multiple regression analysis revealed that self-efficacy (β=0.392, p=0.001), gender (β=0.283, p=0.001), and duration of DM (β=-0.189, p=0.012) significantly affected  DSM in patients with T2DM in rural areas, explaining  28 percent of the variance  (R2=0.280).Conclusion: The results indicated a significant relationship between gender, knowledge, self-efficacy, and family support with DSM in T2DM patients in rural areas, with self-efficacy being the most dominant factor affecting DSM. This study recommends that community nurses can play a pivotal role in designing and implementing interventions enhancing self-efficacy. Additionally, the involvement of PROLANIS, collaborating with key community figures, is crucial to increasing community participation and support for effective diabetes self-management.

Abstract

Hypertension have been a trend of urban health problems. One of prevention hypertension’scomplications is to assist in families by community health workers. Community health nurseshave a role to provide community health workers assistance and supervision. The partnership doimplementation between nurse and community health worker to manage the Health City’sprogram in Indonesia. However, the phenomenon occurring in Indonesia shows a lack of supportgiven for community health workers to be involved in implementing health promotion. Thisliterature review describes the involvement of community health workers to prevention ofcomplications of hypertension in the community. This study uses the literature review method byusing search on online database in Cambridge Core e-resources, Wiley Online, andScienceDirect with the publication period of articles from 2008-2018. The result of literature search show 15 articles that focused on the role of community health workers to prevention ofhypertensive complications. The studies show that the community health worker have beeneffective to prevent hypertension’s complications. The involvement of community health workerscan be done by providing self-management assistance to improve healthy lifestyle, such asmotivating physical activities and limiting salt consumption. In general, the chosen communityhealth workers are women, willing to be involved in health promotion efforts, and be able tocommunicate and make a report. The ability of community health nurses is needed to provide andmaximize community empowerment.

Abstract

Seksualitas merupakan salah satu kebutuhan dasar manusia sepanjang rentang kehidupannya termasuk lansia dimana kesehatan fungsi seksual dapat berpengaruh terhadap aspek kesehatan yang lain. Fungsi seksual yang terpelihara dengan baik dapat menjadi salah satu indikator pencapaian kesejahteraan kualitas hidup. Seiring bertambahnya usia, perubahan pada fungsi seksual terjadi secara progresif hingga berdampak terjadinya disfungsi seksual dan berpengaruh terhadap pencapaian kualitas hidup yang optimal. Tujuan penelitian kuantitatif ini adalah menggambarkan hubungan antara fungsi seksual denga kualitas hidup lansia. Sampel penelitian adalah lansia dengan usia ≥60 tahun di Puskesmas Kecamatan Kebon Jeruk, Jakarta Barat. Desain penelitian ini menggunakan cross sectional dengan metode purposive sampling yang melibatkan 69 responden lansia. Hasil penelitian dianalisa dengan Chi-square menunjukkan adanya hubungan yang bermakna antara fungsi seksual dan kualitas hidup pada lansia (p value = 0,026; OR 3,551; 95%CI 1,271 – 9,925) dimana lansia yang memiliki disfungsi seksual memiliki kecenderungan 3,5 kali lebih besar mengalami kualitas hidup yang rendah. Penelitian ini diharapkan dapat berguna untuk pengembangan ilmu keperawatan ke depannya mengenai pemeliharaan fungsi seksual untuk meningkatkan kualitas hidup pada lansia. Selain itu, penelitian selanjutnya diharapkan dapat melakukan studi mengenai faktor – faktor lain yang mempengaruhi kualitas hidup lansia dan studi mengenai pengalihan aktivitas seksual pada lansia.

Abstract

Urinary incontinence (UI) is a prevalent condition that disproportionately affects institutionalized older adults and manifests differently across genders. This study aimed to examine gender-based risk factors associated with UI among older adults living in nursing homes. A cross-sectional study was conducted involving 317 older adults from four government-run nursing homes. Participants were selected using purposive sampling based on inclusion criteria. Participants completed the Questionnaire for Urinary Incontinence Diagnosis (QUID), the Geriatric Depression Scale (GDS), and the Pelvic Floor Distress Inventory-20 (PFDI-20). Data were analyzed using descriptive statistics and logistic regression across three models (overall, female, and male). The prevalence of UI was 28.07%, with higher rates among females (15.77%) compared to males. Pelvic floor muscle weakness was significantly associated with UI in all models. Among women, reproductive factors such as a menopause duration exceeding 10 years and a history of childbirth were also significantly associated with an increased risk of UI. Education level emerged as a significant confounding variable in the general model. Pelvic floor muscle weakness is a key gender-linked risk factor for UI, particularly among women with specific reproductive histories. These findings underscore the importance of incorporating gender-responsive assessments and pelvic floor interventions into institutional geriatric care practices.

Abstract

Functional status means the ability to perform self-care, self-maintenance, and daily activity. Older adults are at risk of experiencing a decline in functional status regarding their condition and other factors. The study literature found no clear evidence of the factors related functional status of older adults, especially those who have comorbid and regularly come to the geriatric clinic in the Primary Health Care Center. This study aimed to identify the prevalence and factors related to the functional status of older people who visit geriatric out-patient in the primary health care center. A cross-sectional design was utilized to assess 100 samples of older adult patients in one Primary Health Care Center, in Indonesia. The functional status of the respondent was good where the mean score was 7.45 (SD = 1.07) with a minimum score was 2, and a maximum score was 8. The mean age of the sample was 66 years old, the gender was primarily women (59%), the education level of the respondents was more likely only reach elementary school (45%), and primarily they consume drugs (71%) and also more than a half of respondent have multimorbidity (60%). The model fit of multivariate analysis resulted in 44% of all the variables giving effect to the functional status where the most influential factor was balance and health status of the older adults. Discussion: More than half of the older people who regularly visit the Primary Health Care Center have an excellent functional status. This condition is in accordance with the experience of older people itself who most of them could come to the clinic independently by public or private transportation. Older people who have good health or balance status and less morbidity also show a good performance in functional status, though, their age was older. All the variables give 44% effects of older people’s functional status where the most influential factor for functional status of the older adults was balance status. Therefore, the accessibility of health services for the older adult in the community can be improved by providing more elderly-friendly health facilities.

Abstract

Background: Adolescents with HIV who have low self-efficacy are less likely to adhere to their treatment plans, experience psychological issues, become socially isolated, and make poor treatment management decisions. Peer support therapy is therefore required to raise these teenagers' self-efficacy. Evaluating self-efficacy is crucial for determining how peer support interventions affect adolescents living with HIV. Purpose: Assessing the impact of peer support interventions on HIV-positive adolescents' self-efficacy is essential. Method: A systematic review with article selection guided by the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. The systematic review process begins with the formulation of clinical questions relevant to the topic. Before this, the authors establish PICOS criteria, which stand for: P (problem, patient, or population), I (intervention, prognostic factor, or exposure), C (comparison or control), O (outcome), and S (study design). For this article, P: Adolescents with HIV, I: Peer support intervention, C: Individual counseling, standard education, or regular therapy, O: Improving self-efficacy, S: Randomized controlled trials. The article search was conducted systematically using the keywords: “Peer Support,” AND “Self-efficacy,” AND “Adolescents” AND “HIV.” Results: Among adolescents with HIV, the peer support intervention demonstrated an increase in self-efficacy. Participants reported feeling more confident in managing their treatment and navigating daily challenges related to their condition. The supportive peer environment helped reduce feelings of isolation and encouraged adherence to antiretroviral therapy (ART). Conclusion: Peer support interventions effectively enhance self-efficacy in adolescents living with HIV by improving confidence in treatment management, stigma coping, and ART adherence. Their empathetic, equal, and communicative approach makes them more acceptable to adolescents who may be reluctant to share with professionals.
