Abstract

Background: : Factors related to the severity and the outcome of coronavirus disease 2019 (COVID19) are emerging subjects of interest to be studied. Smoking has long been known to have a negative impact on the response to infection. The purpose of this study was to determine the association between smoking habits with the severity, the length of hospitalization and the outcome of COVID19 patients.Methods: This study was a prospective cohort study of COVID-19 patients admitted at Persahabatan Hospital, Jakarta, Indonesia. Subjects were included by consecutive sampling from August to October 2021. Subjects were assigned into smokers and non-smokers group. All subjects were assessed for their severity, length of hospitalization and outcome; in smoking patients, the Brinkman Index (IB) and the Fagerström Test for Nicotine Dependent (FTND) score were also recorded. Statistical tests were then carried out to assess the significant association between smoking history and the degree of severity, length of hospitalization and outcome of COVID-19 in all patients and the FTND and IB scores with the degree of severity, length of hospitalization and outcome of COVID-19 in smokers.Results: Authors found that 39/100 (39%) of participants were smokers. Smoking history had a correlation with the outcome (p=0.001) but not with the degree of severity (p=0.410) and length of hospitalization (p=0.780). Among subjects with smoking history, there were correlations found between FTND and the degree of severity and the outcome (p=0.022 and p=0.012), but not with the length of hospitalization (p=0.716). The Brinkman Index was correlated with only the degree of severity (p=0.020).Conclusion: Our study found that smoking habits were correlated with the degree of severity and outcomes in COVID-19 patients.

Abstract

Since the beginning of 2020 there has been an increase in the number COVID-19 virus infection, previously known as 2019-nCov. It is known that up to February 2020, more than 70,000 positive cases of COVID-19 infection have been confirmed. COVID-19 is known to be a virus from the coronaviridae family, which is also a family of the SARS-CoV and MERS-CoV viruses that caused an outbreak in 2002 and 2012. The exact pathogenesis of COVID-19 is not clearly known, but is thought to be similar to the pathogenesis of SARS-CoV and MERS-CoV. The average COVID-19 mortality was 2.3%, lower than the SARS-CoV and MERS-CoV mortality. The COVID-19 outbreak was also thought to have started from a local seafood sales market in winter, an environment that was almost the same as the SARS virus outbreak. This scientific article will discuss in depth the operational definitions that can be used in the management of patients with suspected COVID-19 infection. It will also discuss clinical manifestations and management in patients with suspected and confirmed COVID-19 infection.

Abstract

Background: Chronic obstructive pulmonary disease (COPD) is an important disease worldwide in both high-income and low-income countries. Dust has been known to increase COPD risk. During sweeping activity, sweepers are exposed to dust. The street sweepers are exposed to dust particles, bioaerosols, and various harmful gases. In this study we evaluates the prevalence of COPD among street sweepers in Jakarta Method: This is a cross-sectional study among 153 street sweepers in Jakarta, Indonesia with age more than 40 years old with working period more than 2 years. Subjects were collected by cluster sampling method based on working location correlated with Jakarta regional district area. COPD was diagnosed by using questionnaires of COPD Assessment Test (CAT), The Modified British Medical Research Council (mMRC), spirometry examination based on Pneumobile Project Indonesia, and bronchodilator test if there was obstructive results. Results: A total of 153 subjects was selected for spirometry examination. The prevalence of COPD among street sweepers in Jakarta, Indonesia was 10 of 153 subject (6.5%). Six of them were males (60%), do not use face mask (80%), working years greater than 10 years (70%), smokers (60%), and BMI ≤25 kg/m2 (80%). There was a statistically significant relationship between age and COPD (p less than 0,05). Conclusion: Prevalence of COPD among street sweepers in Jakarta is 6.5%. Factor related to the occurrence of COPD is age.

Abstract

Seiring masuknya covid-19 ke tanah air yang kemudian menjadi bencana wabah sehingga infeksi meluas secara cepat dengan fatalitas yang terbatas, cukup membuat kewalahan pelayanan kesehatan. Terbitnya Surat Keputusan Konsil Kedokteran Indonesia Nomor 18/KKI/KEP/III/2020 tentang Kewenangan Dokter Penanggung Jawab Penanganan Pasien di Fasilitas Pelayanan Kesehatan Pada Masa Darurat Bencana Wabah Penyakit Akibat Corona Virus Disease 2019 (COVID-19) di Indonesia mengijinkan mobilisasi seluruh tenaga medis dan spesialis baik paru dan non paru di saat pandemi covid guna mencukupi layanan covid, menjadikan dasar bagi RSUP. Persahabatan yang ditunjuk sebagai rumah sakit khusus covid untuk menerapkan hal tersebut.

Abstract

This study looks at how self-efficacy and learning motivation affect the learning results of cadets at the Surabaya Aviation Polytechnic. There is a change in how vocational education is seen, with a focus on practical skills and being ready for work. This means that cadets need strong learning skills and confidence in their academic and technical abilities. However, in practice, there are differences in how well cadets perform, which aren't always linked to their intelligence. So, it's important to understand psychological factors like self-efficacy and learning motivation more deeply. The research used a quantitative method with an explanatory correlational design. The sample included 120 cadets, and data was collected using total sampling. The tools used were questionnaires for self-efficacy and learning motivation as the main factors, and the GPA as a measure of learning outcomes. The data was analyzed using multiple linear regression through SPSS version 26. The results showed that both self-efficacy and learning motivation had a significant impact on learning outcomes. The self-efficacy had a β coefficient of 0.000 (p 0.05), and motivation had a β of 0.000 (p 0.05). The coefficient of determination (R²) was 0.667, which means that together, these factors explain 66.7% of the variation in learning outcomes. These results show that improving self-efficacy and learning motivation is very important for helping cadets achieve better learning results at aviation vocational schools. The study suggests that the Surabaya Aviation Polytechnic should create programs to boost self-efficacy and support both internal and external motivation to help cadets reach their best learning outcomes.

Abstract

Background: Pneumonia is an acute inflammation of the lung parenchyma with a prevalence of 2% in Indonesia. Risk factors for community-acquired pneumonia (CAP) include host factors, pathogens, and environmental factors such as air pollution. According to various international studies, several air pollution parameters have been linked to CAP, including particulate matter (PM), particularly PM10 and PM2.5, sulphur dioxide (SO2), carbon monoxide (CO), ozone (O3), and nitrogen dioxide (NO2). This study aimed to assess the relationship between air pollutant levels and CAP hospitalization rates at Persahabatan Hospital in 2023. Methods: This retrospective cohort study was conducted at Persahabatan Hospital, involving all adult patients diagnosed with CAP who were hospitalized during 2023. Subjects were selected based on inclusion and exclusion criteria. Air pollutant data were obtained from the official website of the Department of Environment, DKI Jakarta. Statistical analysis included Pearson or Spearman correlation tests, followed by multiple linear regression. Results: A total of 729 subjects met the inclusion criteria. Most patients were over 56 years old (61.3%), had more than one comorbidity (73%), had a high pneumonia severity index (PSI) score between 91 and 130 (46.1%), and were unemployed (55.3%). The highest number of CAP hospitalizations occurred in October (88 patients, 12%), coinciding with the highest recorded PM2.5 level (API=125, unhealthy category) and O3 level (API=28.4, good category) a month before in September. Statistical analysis showed a significant relationship between PM2.5 levels 7 days prior to admission (b=0.732; P<0.001), alongside a negative relation from the mean of O3 level during the admission month (b= -1.601; P=0.009) with the number of CAP hospitalizations. Conclusion: Average PM2.5 levels measured 7 days prior to hospitalization have a significant positive association with the number of CAP admissions, alongside a negative relation of average O₃ levels during the month of hospitalization, at Persahabatan Hospital in 2023.
