Abstract

Coronavirus Disease (COVID-19) muncul sebagai sebuah pandemi yang menjadi masalah kesehatan global. Terapi untuk mengobati COVID-19 hingga saat ini masih berkembang dinamis. Penelitian ini bertujuan untuk memberikan gambaran karakteristik, regimen terapi, dan luaran pasien COVID-19 yang dirawat pada periode Maret – Agustus 2020 di Rumah Sakit Akademik Universitas Gadjah Mada Yogyakarta. Penelitian ini merupakan penelitian deskriptif dengan rancangan serial kasus yang dilakukan secara retrospektif. Jumlah pasien yang memenuhi kriteria inklusi adalah 24 pasien dengan 18 pasien laki-laki dan 6 pasien perempuan. Derajat keparahan pasien COVID-19 dalam penelitian ini terdiri dari tanpa gejala (25%), ringan (37,5%), sedang (29,2%), dan berat (8,3%). Batuk merupakan gejala yang paling banyak dialami oleh pasien (62,5%) diikuti demam (37,5%). Gejala lain yang juga dialami oleh pasien antara lain sesak (16,7%), pilek (12,5%), pusing (8,3%), sakit tenggorokan (8,3%), mual (8,3%), dan myalgia (11,1%). Data laboratorium menunjukkan nilai hitung leukosit dan limfosit cenderung normal namun nilai NLR cenderung meningkat. Regimen terapi yang diperoleh pasien adalah adalah antibiotik (91,7%), antivirus (4,2%), hidroksiklorokuin/klorokuin (75%), kortikosteroid (12,5%), antikoagulan (16,7%), suplemen (100%). Lama rawat inap semua pasien dalam penelitian ini adalah 9-15 hari dengan status luaran klinis pasien sembuh sebesar 20,8% dan membaik sebesar 79,2%.

Abstract

Tuberculosis is currently a public health problem both in Indonesia and internationally, so it is one of the Sustainable Development Goals (SDGs) for sustainable health development. Therefore, good drug management is needed. The purpose of this study is to determine the level of suitability of the tuberculosis program drug management process at the planning, distribution, and control stages in 2020 and 2021 compared to drug management quality indicators and determine the factors that affect it. This study was conducted in October–November 2022. Descriptive observational research methods with quantitative data were obtained retrospectively, and qualitative data were obtained through observation and interviews with selected resource persons by purposive sampling. The results showed that the planning, distribution, and control of tuberculosis drugs in the Bengkulu Provincial Health Office had not met the indicator standards, which were shown by seven indicators not in accordance with drug management standards, namely the accuracy of planning, ITOR, empty stock, less stock, safe stock, excess stock, and expired. There are six indicators that meet the standards: accuracy of distribution; level of availability of drug; organization; funding; information systems; and the amount of human resources. Influencing factors are a lack of coordination in  planning, record keeping, and reporting, and a lack of operational funds and transportation tools for anti TB drugs distribution

Abstract

Koagulopati merupakan kondisi yang kerap dialami oleh pasien positif COVID-19. Kondisi ini berhubungan dengan resiko terjadinya trombosis serta kematian pada pasien COVID-19. Data penggunaan kedua antikoagulan profilaksis yakni heparin dan enoxaparin didapatkan di RSUD dr. Loemnono Hadi Kudus, dengan melihat nilai laboratorium yakni kadar D-dimer pasien sebagai evaluasi terkait efektivitas penggunaan obat, dan episode kejadian perdarahan sebagai evaluasi terkait profil keamanan kedua obat tersebut. Penelusuran data dilakukan secara retrospektif yakni pasien COVID-19 yang mendapatkan perawatan di rumah sakit mulai tahun 2020 hingga 2022. Metode penelitian ini adalah kohort observasional menggunakan 127 sampel yang memenuhi kriteria eksklusi dan inklusi, dengan 55 data pada kelompok heparin dan 72 pada enoxaparin. Analisis efektivitas menggunakan uji Mann-Whitney menunjukkan terdapat penurunan kadar D-dimer yang signifikan pada pasien yang menggunakan antikoagulan enoxaparin  (p-value < 0.05) sedangkan pada pasien yang mendapatkan heparin tidak menunjukkan penurunan yang signifikan (p-value > 0.05). Berdasarkan evaluasi keamanan penggunaan kedua antikoagulan, enoxaparin juga memiliki insidensi kejadian perdarahan yang lebih kecil (33.3%) dibandingkan heparin (40.0%). Kondisi perdarahan yang paling sering dialami pasien COVID-19 yang menerima antikoagulan profilaksis adalah batuk darah atau hemoptisis (6.3%). Namun secara statistik tidak terdapat perbedaan yang signifikan pada kejadian perdarahan antar kedua kelompok obat (p-value > 0.05).

Abstract

Penelitian ini dilatarbelakangi oleh kesulitan yang dialami oleh PAK Pusat di DIY dalam mendapatkan sertifikat CDAKB dari Kemenkes RI. Tujuan penelitian ini adalah untuk mengetahui tingkat kesesuaian pelaksanaan kegiatan distribusi PAK Pusat di DIY terhadap standar CDAKB dan memahami faktor yang memengaruhi tercapainya kesesuaian kegiatan distribusi PAK Pusat dengan standar CDAKB. Desain penelitian adalah deskriptif evaluatif pada 20 PAK Pusat secara purposive sampling di DIY selama 3 bulan penelitian. Pendekatan kuantitatif menggunakan instrumen kuesioner dan pendekatan kualitatif menggunakan pedoman wawancara kepada PJT Alkes. Analisa data kuantitatif dengan perhitungan persentase tingkat kesesuaian pelaksanaan 13 aspek standar CDAKB pada PAK Pusat, kemudian dikategorikan dalam katagori sesuai dan belum sesuai, sedangkan data kualitatif disajikan dengan menarasikan faktor yang memengaruhi tercapainya kesesuaian dengan standar CDAKB. Hasil penelitian menunjukkan tingkat kesesuaian kegiatan distribusi PAK Pusat di DIY terhadap standar CDAKB adalah 75%, sedangkan faktor yang memengaruhi kesesuaian kegiatan distribusi PAK Pusat dengan standar CDAKB adalah karakteristik PAK Pusat, faktor manusia, faktor sistem dan faktor lingkungan. Kesimpulan penelitian, masih terdapat ketidaksesuaian kegiatan distribusi PAK Pusat di DIY dengan standar CDAKB dan faktor yang memengaruhi kesesuaian perlu ditindaklanjuti dengan usaha melaksanakan kegiatan distribusi sesuai standar CDAKB baik dari sisi ketersediaan sarana-prasarana serta dokumen pendukung.

Abstract

Intervensi hemodialisa memunculkan berbagai komplikasi, paling banyak yaitu hipertensi yang dapat meningkatkan resiko rawat inap dan kematian lebih cepat. Polifarmasi, ketidakpatuhan minum obat, dan kurangnya pemahaman pasien menjadi faktor penyebab target tekanan darah sulit tercapai. Penelitian ini bertujuan untuk mengetahui pengaruh brief counseling apoteker terhadap kepatuhan pengobatan dan perbaikan tekanan darah pasien hemodialisa. Rancangan penelitian quasi-experimental dengan desain pretest-posttest with control group. Intervensi brief counseling dilakukan 1x seminggu selama 1 bulan menggunakan teknik 5A dan alat bantu leaflet pada pasien hemodialisa dengan kriteria inklusi eksklusi selama bulan September-Oktober 2023. Data dianalisis statistik menggunakan uji Paired sample t-test dan Wilcoxon test untuk melihat perbedaan masing-masing kelompok, uji Independent sample t-test dan Mann Whitney test untuk melihat perbedaan antar kelompok setelah intervensi. Sebanyak 57 responden terdiri dari 29 kelompok kontrol dan 28 kelompok intervensi, menunjukkan bahwa kelompok intervensi terdapat peningkatan kepatuhan dengan selisih skor pretest posttest Δ2,71±1,822 dibandingkan kelompok kontrol Δ-0,44±1,152 (p<0,001), perbaikan tekanan darah yaitu penurunan sistolik dengan selisih skor pretest posttest Δ-12,07±8,969 dibandingkan kelompok kontrol Δ-2,55±4,695 (p<0,001) dan diastolik dengan selisih skor pretest posttest Δ-5,03±5,439 dibandingkan kelompok kontrol Δ-0,86±4,420 (p=0,004). Intervensi brief counseling apoteker disimpulkan dapat memperbaiki kepatuhan dan tekanan darah pasien hemodialisa.

Abstract

Background: Telepharmacy has emerged as the latest approach for remote pharmaceutical services. However, some factors influence the success of this technology, such as pharmacists knowledge, readiness to adopt, and perspectives. Objective: This study aims to evaluate the level of knowledge, perception, and readiness to use telepharmacy systems among pharmacists. Methods: Articles from PubMed, Google Scholar, and Scopus (November 2023) were reviewed following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines in 2022. Inclusion criteria encompassed cross-sectional studies examining knowledge, perception, and readiness to use telepharmacy among pharmacists within the last ten years. The quality of studies was assessed using the Joanna Briggs Institute Critical Appraisal Checklist. Results: Eight articles were included in the study, with the largest respondent pool from a study in Vietnam (414) and the smallest from Indonesia (78). All included articles demonstrated good quality with no significant bias. Five studies discussed knowledge levels, with four showing good knowledge (89.74%, 82.67%, 62.3%, 96.83%). Regarding telepharmacy perspectives, two studies had less favorable perspectives. Three studies indicated adequate readiness for telepharmacy implementation (87%, 72%, 63.23%), while one was unprepared (27.74%). Conclusion: Most studies show good knowledge, perception, and readiness to implement telepharmacy. There is a need for telepharmacy training programs in pharmacy schools or hospitals. Abstract

Drug management is part of pharmaceutical service activities, which includes interrelated stages: selection, planning, requesting, receiving, storage, distribution, controlling, recording, reporting, monitoring and evaluation, which play an essential role in ensuring the availability of drugs at PHCs. The systematic review aims to identify indicators of drug management at PHCs in Indonesia and analyze factors that influenced it. This systematic review used the PRISMA methods. Google Scholar is an electronic database to identify related studies. This study is limited publications from 2013 to 2023. The quality of the articles was assessed using JBI Critical Appraisal. The electronics database search earned 570 articles, while only nine articles met the criteria for analysis. Several indicators are different from the standards used due to several factors. Based on the result, factors influencing drug management at PHCs include human resources, financing, information management, facilities, and technology. For most of them, human resources are a determining factor that significantly influences the quality of pharmaceutical services at PHC’s in Indonesia. The implementation of pharmaceutical services at the PHCs must be carried out by at least one pharmacist, the person in charge, who can be assisted by pharmaceutical technical staff as needed. Resources humans influence indicators of planning accuracy; suitability of requests; storage; distribution; control; recording, reporting, and archiving. This study's results are to make improvement strategies, including interventions to improve the quality of pharmaceutical services that impact community satisfaction and stakeholders.

Abstract

The distribution stage is an important part of drug management in hospitals. However, there are problems with the indicators of the drug distribution stage in hospitals. Thus, the purpose of this study is to determine the problems in distribution indicators in hospital pharmaceutical installations based on indicators the Ministry of Health the Republic of Indonesia (2008), Pudjaningsih (1996) and WHO (1993). A literatur search performed in October 2023 using Google Schloar, researchgate and Garuda Kemendikbud. Methodological quality was assessed using tools from the Joanna Briggs Institute (JBI). We retrieved 3.237 articles and included eight studies. Regarding the design, they were observation documen retrospective (n=8). It was found that there were problems in the distribution stage indicators, such as turn over ratio, expired or damaged drugs, dead drug stocks and drug availability that was not optimal in hospitals. Most articles report discrepancies in drug distribution indicators based on WHO (1993), Ministry of Health (2008) and Pudjaningsih (1996). This shows that drug management in hospitals is not optimal. The findings of this study may be of interest to pharmacists, doctors and other health workers to pay attention to the smooth cooperation of drug management in hospitals.

Abstract

Tahap pengelolaan obat di rumah sakit merupakan komponen vital dalam sistem perawatan kesehatan, hal ini mencerminkan peran krusial obat-obatan dalam pengobatan pasien, dampaknya terhadap biaya operasional rumah sakit, serta keselamatan dan kualitas perawatan pasien. Tujuan dari literature review ini adalah mengetahui faktor-faktor yang mempengaruhi terhadap indikator dengan nilai standar efisiensi pada tahap selection, procurement, distribution, dan use melalui penelitian terdahulu. Metode yang digunakan adalah menerapkan metode Prisma serta meneliti artikel yang tersedia dalam dua basis data ilmiah, yaitu Google Scholar dan Garuda, yang diterbitkan antara tahun 2012 hingga 2022. Dari proses ini, berhasil diidentifikasi tujuh artikel yang memenuhi kriteria inklusi yang telah ditetapkan untuk dilakukan analisis sistematis. Hasil dari penelitian ini menunjukkan faktor pengaruh penyebab tercapainya indikator dalam sistem pengelolaan obat diinstalasi farmasi rumah sakit antara lain temuan terkait  sistem pengadaan yang belum efektif, anggaran terbatas, perencanaan kebutuhan obat yang tidak sesuai, serta sistem distribusi dan penggunaan obat yang belum optimal sehingga perlunya perbaikan dalam pengelolaan obat di rumah sakit guna meningkatkan efisiensi dan kualitas pelayanan kesehatan.

Abstract

Diabetes mellitus (DM) is considered as ”the mother of all diseases" because it causes many complications. Knowing and measuring medication adherence may have a greater effect on DM patients. Several studies in Indonesia used a questionnaire scale to measure adherence, however they do not validate the study population, so it could still be found anomalous correlation analysis between adherence and clinical data even though it measured in the same country and scale. This study measure the adherence level of type 2 diabetes patients, evaluates the validity of the medication adherence scale, and analyze the correlation with the clinical outcome of type 2 diabetes patients in four health centers in Banyumas district. The study uses a cross-sectional design in Prolanis type 2 DM patients of January -April 2020. The adherence is measured by MARS-10, backward-forward translation method followed by content and internal validation. Clinical outcome is evaluated based on fasting blood glucose measurement. The results of the MARS-10 Gregory index analysis showed content validity in the high category (IG ≥ 0.8). The content validity showed the results of 9 questions with the value of r count> r table (n = 30, r table = 0.361). Reliability analysis showed Cronbach's Alpha 0.747> 0.6. The measurement showed 80.3% was adherent patients and 19.3% was non-adherent patients. Correlation analysis showed that there was no significant relationship (p> 0.05) between patient adherence and clinical outcome. Those results showed that type 2 diabetes mellitus patients in 4 health centers were categorized as adherent but not correlated with the clinical outcome. This was enable due to the clinical outcome was simultaneously influenced by several factors: general factors, individual factors, and unpredictable factors.
