Abstract

Background: Postpartum haemorrhage (PPH) is a leading cause of maternal mortality in low- and middle-income countries, including Timor-Leste. Socioeconomic factors may significantly influence the risk of PPH, yet limited local evidence is available. This study aimed to analyse the influence of education, occupation, and household income on the incidence of PPH in three Community Health Centres (CHCs) in Dili City, Timor-Leste. Methods: A cross-sectional study was conducted from January to December 2024, involving 130 postpartum mothers who delivered at CHC Comoro, CHC Becora, and CHC Vera-Cruz. Inclusion criteria were postpartum mothers aged ≥19 years, vaginal delivery at ≥37 weeks gestation, and complete medical records, including maternal and child health books or Liziu, and family socioeconomic data. Exclusion criteria included mothers with incomplete records or those who underwent medical interventions such as oxytocin induction or vacuum extraction. Data were collected retrospectively from medical records, KIA books, and antenatal care cohorts. Socioeconomic variables (education, occupation, household income) were categorised using standardised criteria. Data were analysed using chi-square tests and simple logistic regression to estimate odds ratios (OR) and 95% confidence intervals (CI). This report follows the STROBE guidelines for observational studies. Results: Most mothers (84.6%) were aged 19–34 years, and 61.5% had high-risk obstetric factors. Among respondents, 54.6% had high education, 62.3% were employed in the non-formal sector, and 64.6% had household income at or below the national minimum wage (USD 115). Only household income was significantly associated with PPH (OR = 3.309; 95% CI: 1.429–6.465; p = 0.006), indicating that mothers from low-income families had over three times higher risk of PPH. Education and occupation were not significantly associated with PPH. Conclusion: Household income is a significant socioeconomic determinant of postpartum haemorrhage in Dili, Timor-Leste. Strengthening community-based interventions targeting low-income families, including improved access to maternal health services and socioeconomic support is recommended to reduce PPH incidence

Abstract

Breastfeeding is the best strategy to ensure optimal growth and development for infants. Social support, fatigue, and stress are the main factors that hinder breastfeeding for primiparous mothers. This cross-sectional study analyzes husband support and coping stress of exclusive breastfeeding for primiparous mothers with a quantitative approach. The samples were 70 respondents taken with a total sampling technique. The research instruments were the husband support questionnaire and coping strategies inventory. The researchers analyzed the data with chi-square and multiple logistic regression. The results found 48 (68.5%) mothers with excellent husband support and exclusive breastfeeding. Meanwhile, mothers with excellent husband support but without exclusive breastfeeding were 18 respondents (25.71%). The results of statistical tests with a significance level of 5% obtained a sig value (p-value) of husband support (p=0.002) and coping stress p=0.000. The result indicates a correlation between husband support and coping stress on exclusive breastfeeding with OR value = (3.667). On the other hand, mothers with excellent husband support could provide exclusive breastfeeding 4 times higher than those with poor husband support. Husband support and coping stress affect exclusive breastfeeding of primiparous mothers in the employed area of Puskesmas Dinoyo Malang City. Coping stress is the dominant factor influencing exclusive breastfeeding in primiparous mothers in the employing area of Puskesmas Dinoyo Malang City. The research recommends public health centers improve programs that support maternal care in helping to improve skills in managing stress, on exclusive breastfeeding both at the public health center, the integrated public health service, and the auxiliary health center.

Abstract

Good breastfeeding competence can help the baby get enough calories to increase the baby's weight. The Family-Center Approach (FCA) model is a method that emphasizes family support to assist mothers to increase breastfeeding competence in primiparous mothers who have no previous breastfeeding experience. This research aimed to analyze the effect of FCA on increasing the competence of primiparous mothers in increasing baby weight. This study used a quasy experiment with a pretest-posttest with a control group design on 34 respondents who were divided into 2 groups (17 respondents in the mother group with high breastfeeding competence and 17 respondents in the mother group with low breastfeeding competence). The sample in this study was primiparous mothers who had babies aged 0-6 months in the working area of the Dinoyo Health Center, Malang City. The results from this research using the Wilcoxon Test followed by the Mann-Whitney Test showed p= 0.000 (0.000 < 0.05), this shows that the highest increase in baby weight was in the group of mothers with high breastfeeding competence (mean rank 25.3) compared to the group of mothers with low breastfeeding competence (mean rank 9.7). FCA can increase the competence of primiparous mothers in breastfeeding. This was accompanied by a higher increase in baby weight compared to the comparison group.

Abstract

Due to elevated iron requirements during growth and menstruation, anemia in adolescent girls remains a major global health concern. Hemoglobin (Hb) testing alone has low sensitivity in detecting early iron deficiency anemia (IDA). This study aims to evaluate the diagnostic accuracy of hematological indicators (MCV, MCH, MCHC) and iron biomarkers compared to Hb alone. A systematic literature review was conducted in PubMed, Cochrane Library, SAGE Journals, ScienceDirect, Wiley Online Library, Taylor and Francis, and DOAJ, covering publications from 2015 to 2025, following PRISMA guidelines. The Joanna Briggs Institute (JBI) approach was used to assess methodological quality and risk of bias. Analysis of five relevant publications showed that Hb alone often misses subclinical iron deficiency. Sensitivity and specificity improve when combined with ferritin, soluble transferrin receptor (sTfR), and adjustments for inflammatory markers such as alpha-1-acid glycoprotein (AGP) and C-reactive protein (CRP). These findings support a multifaceted approach that integrates hematological indicators, iron biomarkers, and inflammation markers for early and accurate anemia detection in adolescent girls, in line with national and international recommendations to reduce prevalence and long-term complications.

Abstract

Child malnutrition remains a pressing challenge in Indonesia, including in the catchment area of Sentolo II Health Center, Kulon Progo. In 2025, despite high coverage of child weighing (D/S 88.8%), only 50.88% of children gained weight (N/D), falling below the national target of 60%. Suboptimal complementary feeding (CF) practices, particularly in terms of dietary quality and limited family involvement, were identified as contributing factors. This community service introduced the SEKAR MPASI program (Srawung Edukasi Keluarga lan Anak Remen MPASI), a multi-media nutrition education strategy combining short recipe videos, an illustrated booklet with QR codes, interactive “Ngobrol Pawon” discussions, and local complementary feeding competitions. Evaluation showed a marked improvement in maternal knowledge (post-test scores increased from 82 to 97), stronger family engagement, and competition participation exceeding targets. The program demonstrates that participatory, culturally rooted multimedia education can effectively improve complementary feeding practices and is feasible for integration into routine posyandu activities. Abstract

Postpartum hemorrhage (PPH) remains a leading cause of maternal mortality in low- and middle-income countries, including Timor-Leste. This analytical cross-sectional study, conducted from January to December 2024 in three community health centers (CHCs) in Dili, examined the association between maternal nutritional status and the incidence of PPH. Nutritional indicators included pre-pregnancy body mass index (BMI), gestational weight gain, anemia status, and mid-upper arm circumference (MUAC). Data were collected from medical records of 130 postpartum mothers (65 with PPH, 65 without). The prevalence of PPH was 50%. Among participants, 22.1% had anemia (hemoglobin <11 g/dL), 50.8% had MUAC <23.5 cm, and 41.5% had a pre-pregnancy BMI outside the WHO normal range (18.5-24.9 kg/m²). Only 26.4% achieved gestational weight gain within Institute of Medicine guidelines. Logistic regression analysis revealed that anemia significantly increased the risk of PPH (OR = 3.4; 95% CI: 1.38-8.40; p = 0.011), while other nutritional indicators showed no significant association. These findings highlight the critical role of antenatal anemia screening and targeted nutritional interventions in reducing PPH risk. This study provides locally relevant evidence to inform maternal health strategies in Timor-Leste.
