Abstract

Background: Scabies is a contagious disease caused by Sarcoptes scabiei characterized by itching and lesions that affect quality of human life. Permethrin 5% as standard therapy for scabies has disadvantages in terms of price and side effects. Neem is a traditional plant that has a scabicide effect and could be an alternative treatment for scabies. Objective: to compare the quality of life of patients who were treated with 5% permethrin lotion and 10% neem leaf extract lotion using DLQI and Skindex-29 instruments. Methods: This study is an experimental design with a purposive sampling method. The sample was 42 male students in one of the Islamic boarding schools in Malang who were clinically diagnosed with scabies and met the research criteria. The research sample consisted of two groups: a group with 5% permethrin lotion therapy (21 students) and a group with 10% neem leaf extract lotion therapy (21 students). Data was collected with DLQI and Skindex-29 on day 0 before and day 14 after therapy. Data analysis used the Wilcoxon test and dependent t-test for comparison before and after therapy and independent t-test for comparison of quality of life between two therapies by using SPSS 25. Results: Statistic test to differentiate the two therapies showed p=0.670 (DLQI) and p=0.391 (Skindex-29). So, the two therapies did not differ significantly in reducing DLQI and Skindex-29 scores. Conclusion: 5% permethrin lotion and 10% neem leaf extract lotion are equally good in improving the quality of life of scabies patients. Abstrak Latar Belakang: Skabies adalah penyakit kulit menular yang disebabkan Sarcoptes scabiei dengan gejala gatal dan lesi yang berdampak pada kualitas hidup manusia. Permetrin 5% sebagai standar pengobatan skabies memiliki kekurangan dalam segi harga dan efek samping. Tanaman mimba adalah tanaman tradisional yang diduga bisa menjadi alternatif pengobatan untuk skabies karena memiliki efek skabisida. Tujuan: membandingkan kualitas hidup pasien skabies yang diterapi losion permetrin 5% dan losion ekstrak daun mimba 10% menggunakan instrumen Dermatology Life Quality Index (DLQI) dan Skindex-29. Metode: Penelitian ini merupakan design eksperimental dengan metode purposive sampling. Sampel penelitian adalah 42 santri laki-laki di salah satu pondok pesantren di Malang yang secara klinis terdiagnosis skabies. Sampel penelitian terdiri dari dua kelompok yaitu: satu kelompok dengan terapi losion permetrin 5% (21 santri) dan satu kelompok dengan terapi losion ekstrak daun mimba 10% (21 santri). Pengumpulan data menggunakan DLQI dan Skindex-29 pada hari 0 sebelum terapi dan hari 14 setelah terapi. Analisis data menggunakan uji Wilcoxon dan uji t-dependen untuk perbandingan sebelum dan sesudah terapi dan uji t-independen untuk perbandingan kualitas hidup antara dua terapi mengunakan SPSS 25. Hasil: Hasil uji statistik perbedaan 2 obat menunjukkan p=0,670 dengan parameter DLQI dan p=0,391 dengan parameter Skindex-29. Sehingga terapi dua losion tersebut tidak berbeda secara signifikan dalam menurunkan skor DLQI dan Skindex-29). Kesimpulan: losion permetrin 5% dan losion ekstrak daun mimba 10% sama baiknya dalam meningkatkan kualitas hidup pasien skabies. Abstract

Osteoarthritis is a chronic global and musculoskeletal disorder. This disease is associated with reduced articular cartilage integrity. The most commonly affected joint is the knee, followed by the hand and the hip. Knee osteoarthritis is associated with aging, obesity, articular cartilage, fatigue, trauma, and congenital joint deformities. Excess body weight is a factor that will exacerbate osteoarthritis. There are still meta-analysis studies involving cohort and case-control studies on osteoarthritis and body mass index (BMI) available, but the relationship between the two variables is still not assessed quantitatively. In addition, research on knee osteoarthritis and BMI is still limited, particularly at Baptis Hospital, where it has never been done. As a result, this study was carried out to determine the relationship between BMI and the degree of knee osteoarthritis. This retrospective cross-sectional study involved 33 patients who suffered from knee osteoarthritis. Data were collected from medical records, and the Kellgren-Lawrence system to assess the degree of knee osteoarthritis based on radiology findings. A total of 7 patients (21.2%) suffered from osteoarthritis grade 1, 10 patients (30.3%) had degree 2, 15 patients (45.5%) had degree 3, and one patient had degree 4. The Spearman correlation test showed that there was no significant relationship (p>0.05). So, it can be concluded from this study that there was no correlation between BMI and the degree of knee osteoarthritis patients at Baptis Hospital, Batu City.

Abstract

Introduction: Herpes zoster (HZ) is caused by reactivation of latent varicella-zoster virus (VZV) infection due to disturbance in T-cell mediated immunity. Risk factors include malignancy and therapy related to malignancy. Bladder cancer has a moderate association with HZ, with a risk increment of 10-50%. The rate of HZ in bladder cancer patients is 8/1000. Case description: A 57-years-old man with bladder cancer T4bN3M0 complained of a painful erythematous vesicular rash on the left front abdomen extended to the back 1 week prior, accompanied by fever and burning sensation. He has had regular chemotherapy using Gemcitabine and Cisplatin, and radiotherapy with 2 Gray doses each session. Dermatological examination showed multiple well-defined erythematous-based vesicles, clustered on the left abdominal, flank, and posterior thorax region at T7-T9 dermatome. Tzanck-smear examination showed multinucleated giant cells. The patient was given acyclovir, paracetamol, and wet dressing on vesicles. His chemoradiotherapy was also halted. On the 7th day of evaluation, there were no new lesions and his complaints improved. In cancer patients, CD4+ and CD8+ levels decreased, accompanied by lymphocyte proliferation impairment. Cytotoxic chemotherapy agents work by attacking proliferation cells, affecting immune-related cells. Ion radiation in radiotherapy interferes with a regional cellular immune response that inhibits viral reactivation. Conclusion: Principal HZ management in cancer patients includes antiviral and analgesics. In cancer patients, the occurrence of HZ can disturb and postpone management related to malignancy. The postponement of chemoradiotherapy is based on the patient's conditions and the severity of the disease.

Abstract

Background: Sensory nerve damage is the earliest leprosy sign which can lead to disability. Previous studies showed that autologous platelet-rich plasma (PRP) perineural blind injection can stimulate leprosy sensory nerve regeneration. Our study provided a safer and more accurate PRP agent delivery method through ultrasound-guided injection and was the first to compare PRP versus standard neuropathy treatment, the neurotropic vitamin. This study aimed to determine the effectiveness of combination therapy of ultrasound-guided PRP perineural injection and oral vitamin B complex compared to single oral vitamin B complex in sensory peripheral neuropathy of posterior tibial nerve in leprosy patients.

Abstract

Pendahuluan: Aspergilosis kutis primer (AKP) adalah infeksi jamur invasif pada kulit yang jarang terjadi namun dapat mengancam jiwa. Penyakit ini disebabkan oleh Aspergillus spp dan termasuk ke dalam mikosis profunda (subkutan). Laporan kasus: Seorang laki-laki berusia 63 tahun datang dengan keluhan luka pada kulit di kaki kanan sejak 5 tahun yang gatal, nyeri dan bernanah. Pemeriksaan dermatologis menunjukkan papul dan plak eritematosa multipel, permukaan verukosa sebagian tertutup krusta kuning kecoklatan, xerosis dan beberapa ulkus dengan pus. Diagnosis banding yang diajukan adalah Tuberkulosis verukosa kutis, Kromoblastomikosis dan Misetoma. Hasil kultur jaringan menunjukkan Aspergillus niger. Hasil biopsi menunjang diagnosis mikosis profunda. Pasien kemudian didiagnosis sebagai Aspergilosis kutis primer, diberikan terapi dosis denyut Itrakonazol dan mengalami perbaikan klinis dalam 2 minggu. Kesimpulan: Gambaran klinis atipikal pada pasien ini memberikan tantangan diagnostik bagi dermatologis. Pentingnya penegakan diagnosis pada pasien dengan mikosis profunda adalah untuk mengidentifikasi penyebab, menentukan tatalaksana tepat dan mencegah komplikasi.

Abstract

Onikomikosis merupakan suatu infeksi jamur pada kuku yang dapat disebabkan oleh dermatofita, non-dermatofita, dan yeast, sedangkan tinea unguium merupakan suatu infeksi oleh golongan dermatofita pada kuku. Onikomikosis diketahui terjadi pada 2-18,5% populasi di seluruh dunia dimana 4-20 kali lebih sering terdapat pada kuku jari kaki dibandingkan kuku jari tangan. Pada pasien sistemik lupus eritematosus dilaporkan 25% mengalami adanya perubahan kuku dimana abnormalitas yang paling sering ditemukan adalah onikolisis. Berikut dilaporkan pasien wanita berusia 27 tahun dengan keluhan kuku jari kaki kiri yang rusak dan rapuh disertai gatal. Dari pemeriksaan dermatologis digiti I-V pedis sinistra didapatkan discoloration dan onychodystrophy. Pemeriksaan KOH 20% pada potongan kuku menunjukkan gambaran hifa panjang bercabang. Hasil kultur jamur menunjukkan pertumbuhan Microsporum sp. Laporan kasus ini membahas tentang kasus jarang tinea unguium yang disebabkan Microsporum sp.

Abstract

Human immunodeficiency virus (HIV) infection and sexually transmitted infections (STIs) remain a public health problem and extend to social, economic, and cultural issues. HIV cases in the Southeast Asia region account for 10% of the total global HIV burden. In Indonesia, there are five provinces with the highest number of HIV cases as of December 2021, including DKI Jakarta (73,442), East Java (68,112), West Java (49,435), Central Java (42,012), and Papua (40,277). In Indonesia in 2021, the number of people living with HIV (ODHIV) reported was 36,902 and in 2022 up to the third quarter (July - September 2022) there were 34,213 people. New HIV infections in Indonesia continue to decline, in line with the global decline in new HIV infections. However, this decline has not been as large as expected.

Abstract

Introduction: Bullous Cellulitis is initially characterized by erythema and rapid development of bullae. This disease is most often caused by Beta-hemolytic streptococci and rarely by other bacteria such as Coagulase-Negative Staphylococcus (CoNS) type Staphylococcus haemolyticus. Immunocompromised state and history of malignancy are at high risk of infection with this bacterium. Local complications such as bullae can occur in obesity. Case Presentation: A 48-year-old woman with complaints of tensed and loose-walled blisters with a reddish base on the right thigh. The patient had a medical history of Thyroid Cancer, Acute Renal Failure, and Bulging Mass pelvic femur. On physical examination, the patient was unconscious with a ventilator. Dermatological examination of the dextra inferior extremity showed loose bullae with a positive Nikolsky sign and tensed bullae over the erythematous patches, ill-defined, irregular edges, and warm palpable. Laboratory results of leukocytosis, hypoalbumin, and CoNS bacteria from blood culture. An ALT-70 score obtained a total score of 5. Conclusion: Cases of bullous cellulitis are quite rare; cases like this usually occur in patients with an immunocompromised predisposition and obesity. CoNS is a commensal bacteria but, in some cases, could be a pathogen causing cellulitis. Poor prognosis that ends in death could occur in patients who have various complications.

Abstract

Onychomycosis is the most common nail disease and contributes to 50% of all cause of onychodystropy. Onychomycosis cases due to Cryptoccus laurentii and Trychophyton spp. are quite rare. The most common cause of onychomycosis reported in Indonesia is Candida sp. Risk factors for onychomycosis include moisture, occlusion, nail trauma, decreased immunity, slow nail growth, wide nail surface, and genetic factors. Old age, cancer, psoriasis, dermatophyte infection at other sites is also a risk factor. A 54 year-old woman, with brittle nails and cracked palms since 6 months before came to outpatient clinic. She is a farmer and work in the rice fields without gloves daily; she also rarely wash her hands after work. Dermatologic examination of all right and left finger nail is revealed yellow, white, and some blackish discoloration, subungual hyperkeratotic, onycholysis, and onychodystrophy. In the right and left palm, revealed multiple erythematous and yellow-white hyperkeratotic plaques, with fissures and rough scales. From nail clippings, a 20% KOH examination revealed long branching hyphae, periodic Acid-Schiff (PAS) staining revealed spores, and fungal culture revealed growth of Criptococcus laurentii and Trichophyton spp. colonies. We gave itraconazole pulse dose: 2 x 200 mg tablet orally per day for a week every month. Other treatment was using 20% urea cream. Follow up at 8 weeks after therapy is showed clinical and mycologic improvement. The incidence of onychomycosis due to Cryptoccus laurentii and Trychophyton spp. is still rare. Thepredisposing factor of infection in these case is suspected due to old age and her habitual contact with the paddy fields without gloves and rarely wash her hand after work.

Abstract

Background: According to WHO data, the number of new cases of leprosy has decreased in 2019. However, Indonesia continues to provide a significant number of cases. According to statistics, India, Brazil, and Indonesia account for 79 percent of all instances. Purpose: This study aims to describe the profile of leprosy patients, and involves all Dermatology and Venereology Academic Hospitals in Indonesia. Methods: This study was a retrospective study of 2461 patients from Dermatology and Venereology Outpatient Clinic at 13 Academic Hospitals in Indonesia between January 2018 and December 2020. Result: Subjects in this study were dominated by males (66.8%) and aged > 14 years (95.3%). The most common type of leprosy was multibacillary (MB) (86.2%), and erythema nodosum leprosum (ENL) was the most leprosy reaction (20.3%). Majority of the subjects experienced disability in the hands (26.6%), in grade 1. Conclusion: Leprosy cases in Indonesia are mostly experienced by adult males. The most common type of leprosy is MB, with ENL being the most common leprosy reaction. Grade 1 disability is the most prevalent, therefore proper education is necessary to keep patients from progressing to grade 2 disability.
