Abstract

Background: Brøset Violence Checklist (BVC) and Dynamic Appraisal of Situational Aggression-Inpatient Version (DASA-IV) are risk assessment instruments of violence that nurses can use in psychiatric services, but their accuracy of the instruments in predicting violent behavior in 24 hours need to be tested.Objective: This study aims to examine the sensitivity and specificity of BVC and DASA-IV instruments in predicting violent behavior within 24 hours and which is more accurate.Method: This is a diagnostic study with a cross-sectional approach. The sample of this study was 112 patients. The respondents' behaviors were observed, based on parameters assessed by BVC and DASA-IV as well as recording incidents of violent behavior using AOS (Aggression Observation Short) as the gold standard. Sensitivity, specificity, and Receiver Operating Characteristic (ROC) tests were conducted to assess the accuracy of the two instruments in predicting violent behavior within 24 hours.Results: 23.2% of the respondents in this study experienced violent behavior in the first 24 hours. BVC has a sensitivity value of 65.4% and specificity of 94.5%. DASA-IV has a sensitivity value of 69.2% and a specificity of 95.5%. The level of accuracy of BVC is 87.9%, and of DASA-IV is 92.2%.Conclusion: DASA-IV is more sensitive, higher in its specificity, and is more accurate in predicting violent behavior within 24 hours in schizophrenic patients compared to BVC

Abstract

Instagram, which ranks fourth globally in usage in Indonesia, has become an effective and cost-efficient marketing tool, especially in the healthcare sector. Hospitals, such as RSUD Dr. Saiful Anwar Malang, utilize Instagram to promote services and engage patients. However, despite a growing following and content, fluctuating patient visits suggest that the hospital's Instagram marketing strategy may need further optimization. The purpose of this study was to identify social media marketing activities (SMMA) and customer experience on Instagram regarding the relationship quality of outpatient patients at RSUD Dr. Saiful Anwar Malang. This study used a quantitative approach with a cross-sectional design and accidental sampling of outpatients during February-March 2024. Data were collected through surveys and analyzed statistically to examine the impact of SMMA and customer experience on Instagram on the relationship quality of outpatients.  The results of the multiple linear regression analysis show that SMMA has a significant effect on relationship quality (regression coefficient = 0.34, p=0.039), while customer experience has a significant positive effect (regression coefficient = 1.003, p=0.000), indicating that improving customer experience has a stronger impact on relationship quality compared to SMMA. The study concludes that SMMA has a partial effect on relationship quality, meaning that better SMMA increases relationship quality. This suggests that future media services should prioritize building connections, satisfaction, and patient retention.

Abstract

Background: Primary cutaneous aspergillosis (PCA) is a rare invasive fungal infection of the skin that can be life-threatening. This disease is caused by Aspergillus spp and belongs to the deep mycosis (subcutaneous) group. Case report: A 63-year-old man came with complaint of wound that feels itchy, painful and exudative on right lower leg since 5 years ago. Dermatological examination showed multiple erythematous papules and plaques with verrucous surface partially covered in brownish yellow crust accompanied by xerosis and some ulcers with pus. The proposed differential diagnoses are Tuberculosis verrucosa cutis, Chromoblastomycosis and Mycetoma. The result of tissue culture showed Aspergillus niger. Biopsy result supports the diagnose of deep mycosis. The patient then diagnosed as primary cutaneous aspergillosis, treated with pulse dose Itraconazole and improved clinically within 2 weeks. Conclusion: Atypical manifestation in this patient gives diagnostic challenge for dermatologist. The importance of establishing diagnosis in patient with deep mycoses are to identify the cause, determine the appropriate treatment and prevent cimplication.

Abstract

Onychomycosis is a fungal infection of the nails that can be caused by dermatophytes, non-dermatophytes, and yeast, while tinea unguium is an infection of the nails caused by the dermatophyte group. Onychomycosis is known to occur in 2-18.5% of the population worldwide where it is 4-20 times more common in toenails than fingernails. In patients with systemic lupus erythematosus, it was reported that 25% experienced nail changes where the most common abnormality was onycholysis. The following is a report of a 27-year-old female patient with complaints of damaged and brittle left toe nails accompanied by itching. From the dermatological examination of the left I-V digiti pedis, discoloration and onychodystrophy were found. Examination of 20% KOH on nail clippings shows long-branched hyphae. The results of the fungal culture showed the growth of Microsporum sp. This case report discusses a rare case of tinea unguium caused by Microsporum sp.

Abstract

Perilaku Hidup Bersih dan Sehat (PHBS) merupakan upaya preventif yang dilaksanakan secara sadar untuk meningkatkan derajat kesehatan individu, keluarga, maupun masyarakat. Implementasi PHBS di fasilitas pelayanan kesehatan memiliki peran strategis dalam meminimalisasi risiko penularan penyakit sekaligus mendukung pencapaian standar mutu layanan serta akreditasi rumah sakit. Penelitian ini bertujuan mengidentifikasi penerapan PHBS di RSUD Dr. Saiful Anwar Malang sebagai strategi peningkatan mutu pelayanan kesehatan. Penelitian menggunakan metode kualitatif dengan teknik pengumpulan data melalui observasi, wawancara yang melibatkan 41 informan kunci, dan studi literatur. Pengumpulan data dilaksanakan pada periode Januari-Maret 2024. Hasil penelitian menunjukkan bahwa implementasi PHBS di RSUD Dr. Saiful Anwar Malang telah mengacu pada kriteria akreditasi rumah sakit dan pedoman pelayanan kesehatan. Namun, ditemukan sejumlah tantangan, antara lain keterbatasan sumber daya, kepatuhan tenaga kesehatan, serta konsistensi pengawasan terhadap pengunjung dan pasien. Faktor-faktor tersebut berpotensi menurunkan efektivitas PHBS dalam mendukung mutu pelayanan. Oleh karena itu, diperlukan optimalisasi melalui peningkatan kapasitas sumber daya manusia, penguatan sistem monitoring dan evaluasi, serta internalisasi budaya PHBS di seluruh lapisan rumah sakit. Kesimpulannya, penerapan PHBS yang konsisten dan terstandar berkontribusi signifikan terhadap peningkatan mutu layanan kesehatan rumah sakit. Implikasi penelitian ini menegaskan pentingnya PHBS sebagai manajemen strategi yang berkelanjutan dalam mendukung akreditasi dan pencapaian pelayanan kesehatan yang unggul.

Abstract

Scabies is a contagious parasitic skin disease caused by Sarcoptes scabiei var. hominis. A 14-year-old male student residing in an Islamic boarding school presented to a public health centre with a long-standing history of scab-like skin lesions. The patient had sought treatment for more than one month without clinical improvement. He complained of intense nocturnal pruritus affecting the interdigital spaces and palms, accompanied by erythematous lesions, excoriations, vesicles, purulent discharge, swelling, pain, and fever. He also reported pain in the axillary and inguinal regions. Physical examination revealed multiple discrete erythematous papules and vesicles on the interdigital spaces and palms, accompanied by crusts, excoriations, and purulent discharge. Enlarged and tender axillary lymph nodes were also noted. Based on clinical findings, the patient was diagnosed with scabies complicated by secondary bacterial infection. Non-pharmacological management included patient and family education regarding the disease, personal hygiene, environmental sanitation, and a visit to the boarding school to prevent further transmission. Pharmacological therapy consisted of oral antibiotics, analgesics, antihistamines, and anti-inflammatory agents. Topical treatment included anti-scabietic therapy and a combination of topical antibiotic and anti-inflammatory agents. Scabies may be complicated by secondary bacterial infection due to delayed diagnosis and treatment, persistent scratching, and poor personal and environmental hygiene. Overcrowded living conditions, such as those in boarding schools, facilitate transmission and increase the risk of complications. A family-centred and community-based approach, particularly involving boarding school environments, plays a crucial role in controlling transmission and improving outcomes in patients with scabies complicated by secondary infection.

Abstract

Leprosy is a chronic infectious disease caused by Mycobacterium leprae that affects the peripheral nerves, skin, and other body tissues. In addition to medical consequences, leprosy poses complex social, economic, cultural, and public health challenges. The Tirtayasa Public Health Center oversees 14 villages with a total population of 50,937 people. As part of leprosy elimination efforts, the center developed the BECAK Tirtayasa Innovation, aimed at supporting the achievement of Triple Zero leprosy. This innovation includes the development of a leprosy counseling module, the formation of BECAK Tirtayasa cadres, establishment of a telemedicine group connecting cadres, leprosy officers, and doctors, implementation of health education and educational games on leprosy, active Intensified Case Finding (ICF), and provision of chemoprophylaxis. To analyze the utilization of the BECAK Tirtayasa Innovation in reducing leprosy prevalence in the Tirtayasa Public Health Center area in 2024. This study employed an observational analytical cross-sectional design involving 50 respondents selected through total sampling from leprosy cases recorded between 2019 and 2024. Most leprosy patients were adults aged 20–60 years (44 individuals; 88.1%) and male (27 individuals; 54%). The highest proportion of cases originated from Lontar Village (12 individuals; 24%). Multibacillary leprosy predominated (44 individuals; 88%), and the majority of patients presented with grade 1 disability (23 individuals; 46%). The highest number of leprosy cases was recorded in 2019 (17 cases), while the lowest occurred in 2024, with only one new case identified. In 2024, the implementation of the BECAK Tirtayasa Innovation was associated with a decrease in leprosy prevalence to 0.1 per 10,000 population and achievement of the new case detection target at 1.9 per 100,000 population. No grade 2 disability cases or pediatric leprosy cases were reported (0%), and the treatment completion rate exceeded the national target (100%). Leprosy cases in the Tirtayasa Public Health Center area predominantly affected adult males, were mostly multibacillary in type, and commonly presented with grade 1 disability. The BECAK Tirtayasa Innovation contributed to a reduction in leprosy prevalence and supported progress toward Triple Zero leprosy in the Tirtayasa Public Health Center area.

Abstract

Ultraviolet-B (UVB)-induced photoaging accelerates collagen degradation, disrupts dermal structure, and compromises epidermal integrity. Natural extracts, such as Andrographis paniculata, Syzygium aromaticum, and Pogostemon cablin demonstrated antioxidant and anti-aging properties to mitigate these damages. This study investigated the protective effects of serum and moisturizer containing A. paniculata extract, S. aromaticum oil, and P. cablin oil on UVB-induced photoaging rats, by integrating network pharmacology and in vivo experiment. Compounds and photoaging-associated targets were retrieved through online databases. Enrichment analyses were performed using Metascape. Protein-protein interaction (PPI) construction and herbs-compounds-targets-pathways disease network were performed using STRING and Cytoscape 3.10.0, respectively, followed by molecular docking. An in vivo study was conducted to evaluate epidermal thickness, collagen density, and dermo-epidermal junction (DEJ) structure. Eleven key phytoconstituents and twelve core targets were identified in anti-photoaging molecular mechanisms using network pharmacology. Molecular docking analysis revealed that 7-O-methylwogonin demonstrated favorable binding against ten photoaging-associated targets. KEGG enrichment analysis showed potential inflammation processes inhibition, including photocarcinogenesis, AGE-RAGE signaling pathway, and TNF-α expression. Furthermore, treatment with serum, moisturizer, or combination of both significantly reduced epidermal thickening and enhanced collagen density compared to the UVB-exposed only group. Although no significant difference was observed in DEJ interdigitation index across groups, treated groups exhibited a trend towards better preservation of DEJ structure relative to the UVB group. In summary, serum and moisturizer containing A. paniculata, S. aromaticum, and P. cablin extracts demonstrated potential anti-photoaging effects by reducing epidermal thickness and preserving collagen density in UVB-exposed rats. These findings support these botanical-based formulations potential as effective anti-aging skincare.

Abstract

Background: Tinea incognito (TI) is a dermatophytic infection with atypical clinical manifestations resulting from inappropriate use of topical corticosteroids. The misuse of over-the-counter steroids and unregulated cosmetic products, such as whitening body lotions, can alter the classical presentation of dermatophytosis, leading to delayed diagnosis and treatment failure. This case report aims to highlight the clinical features of tinea incognito associated with topical steroid misuse and whitening body lotion application, emphasizing the importance of early recognition and appropriate management to prevent misdiagnosis and treatment delay. Case: A 29-year-old woman presented with persistent pruritic erythematous patches on her back that had progressively enlarged over two months. The lesions initially appeared as small erythematous patches and were treated with topical corticosteroids prescribed by non-physician health practitioners. The patient continued self-medication with freely purchased topical steroids without improvement. She subsequently applied a whitening body lotion in an attempt to resolve the lesions; however, the patches worsened and expanded. Physical examination of the thoracolumbosacral region revealed ill-defined erythematous plaques with multiple peripheral erythematous papules and fine scaling. Laboratory examinations were not performed. The diagnosis of tinea incognito was established based on clinical history and physical findings. Treatment consisted of oral ketoconazole 200 mg daily, topical ketoconazole 2% cream, ketoconazole 2% shampoo twice weekly, nonsteroidal moisturizer, and antihistamines for two weeks. The patient was advised to discontinue topical steroids and whitening body lotion and received education on clean and healthy living behaviors. Significant clinical improvement was observed during follow-up. Conclusion: Discontinuation of topical steroids and inappropriate cosmetic products, combined with appropriate antifungal therapy, resulted in favorable clinical outcomes. Improved regulation of steroid and cosmetic product distribution, along with increased awareness among healthcare providers, is essential to prevent misdiagnosis and the rising incidence of tinea incognito.

Abstract

Pendahuluan: Tinea kapitis adalah infeksi rambut dan kulit kepala akibat dermatofita. Kasus pada dewasa jarang ditemukan dan biasanya terkait kondisi imunodefisiensi, salah satunya Sistemik Lupus Eritematosus (SLE). Pemilihan terapi pada pasien dengan SLE perlu mempertimbangkan efektivitas terhadap patogen serta keamanan terhadap kondisi autoimun yang mendasari.Kasus:Seorang wanita 38 tahun dengan SLE yang mengeluhkan rambut patah membentuk area kebotakan disertai bintik hitam sejak satu bulan. Pemeriksaan KOH 10% dan KOH 20% pada rambut didapatkan hifa. Kultur pada media Sabouraud Dextrose Agar mengidentifikasi Trichophyton sp. Pasien diterapi itrakonazol 200 mg/hari selama empat minggu dan sampo ketokonazol 2% tiga kali seminggu. Diskusi: Pada kasus ini, itrakonazol dipilih karena efektif terhadap Trichophyton sp., dan pada pasien SLE, tidak memicu drug induced lupus. Setelah empat minggu, terjadi perbaikan klinis dan mikologis. Kesimpulan: Itrakonazol dapat menjadi pilihan terapi tinea kapitis pada pasien SLE yang disebabkan Trichophyton sp. karena memberikan perbaikan klinis dan mikologis tanpa memperburuk gejala lupus.
