Abstract

Treatment for pneumonia has always been a challenge due to the difficulties in diagnosis and the growing incidence of antibiotic resistance. Beta-lactam antibiotics are the first line treatment for pneumonia. The combination of beta-lactam with other antibiotics are preferred than single antibiotic therapy. However, there was limited information regarding the effectiveness of combination between beta-lactam with either macrolides or fluoroquinolone for the treatment of pneumonia.The purpose of this study was to determine the most effective combination of antibiotics for hospitalized adults pneumonia patients. This was a cross-sectional study with prospective data collection. The data source was the medical record of the subjects. We included adult pneumonia patients hospitalized at Dr. Hasan Sadikin General Hospital during June-August 2015. We found that mean reduction of body temperature in group who received combination of a beta-lactam antibiotic and azithromycin was 1.53 0C, while in levofloxacin group, the reduction was 1.22 0C (p=0.210). Reduction in leukocytes and respiratory rate were 7800 and 2.29 times/minute, respectively, in the former group, while in the latter group the reduction of leukocytes and respiratory rate were 2600 and 5 times/minute. The differences were not statistically significant in both parameters (p=0.036 and 0.149, respectively). In conclusion, better clinical outcomes were observed in patients treated with combination of beta-lactam and azithromycin compared to combination of beta-lactam and levofloxacin, although the difference was not statistically significant.Keywords: pneumonia, pneumonia therapy, combination therapy for pneumonia

Abstract

The prevalence of pneumonia is particularly high among pediatric patients. Appropriate antibioticsselection is required to reduce mortality and morbidity rates associated with thesediseases. However, information on cost-effectiveness of empirical antibiotics treatment forpneumonia was limited. This study was aimed to evaluate cost-effectiveness of cefotaximeand ceftazidime for pneumonia in pediatric patients. This study was a retrospective crosssectional study conducted at a hospital in Bandung during January-December 2012. Datawere derived from medical records of pediatric pneumonia inpatients during study period.Cost was calculated based on direct medical cost, i.e., inpatient care, medical support, andmedicines that were used from admission until hospital discharge. The results showed thatthere was no statistical difference in the average medical cost of the treatment using cefotaxime(1,197,017 IDR) and ceftazidime (2,245,748 IDR). Incremental cost effectivenessratio (ICER) showed that cefotaxime is more cost effective than ceftazidime with greaterreduction of leukocytes level (576 IDR/mm3 ). The use of cefotaxime is recommended forthe treatment of pnuemonia in pediatric patients.Keywords: cost minimization, cost effectiveness, cefotaxime, ceftazidime, pneumonia

Abstract

The community has used black garlic since ancient times for hypercholesterolemic. Until now, people still consume both raw and cooked black garlic. Black garlic is included in processed garlic products. People on the Asian continent have used black garlic for the past 10 years. This study aims to determine the sub-chronic toxic effect of black garlic ethanol extract on male and female white rats using hematological parameters consisting of hematocrit, hemoglobin, erythrocytes, leukocytes, platelets, MCV, MCH, and MCHC. This research method was a completely randomized design with the administration of ethanolic extract of black garlic to rats in 5 treatment groups, including a negative control group, a dose group of 1000 mg/kg BW, a dose group of 2000 mg/kg BW, a negative control satellite group and a satellite group with a dose of 2000 mg/kg BW. Data were analyzed statistically using one-way ANOVA with a 95% confidence level and SPSS version 20. The study results of black garlic ethanol extract showed no significant effect or were at normal levels on hematological parameters. Therefore, it can be concluded that the ethanolic extract of black garlic is safe for human use as a treatment for hypercholesterolemia.

Abstract

Plants in the Costus genus are known to have various activities, starting from an antibacterial agent, anti-inflammatory, antioxidant, anticancer, antidiabetic, and so on. From some previous studies that have been conducted, it is stated that some Costus plants have a high potential to be a prospective anticancer candidate in the future. This review will discuss botanical and chemical overview, traditional uses, and potential anticancer activity of several Costus plants, namely C.speciosus, C.igneus, C.spiralis, and C.pictus. In conclusion, anticancer activity assay was carried out by several methods using various cell cultures resulting in an IC50 value ranging from strong and very strong, with the strongest potency belonging to C.speciosus with an IC50 value of 13.87 µg/ml. In addition, the active phytoconstituent, mainly diosgenin, contributes to the anticancer activity, which also strengthens the potential of Costus plants.

Abstract

Penyakit Ginjal Kronis (PGK) adalah masalah kesehatan di seluruh dunia dengan peningkatan prevalensi penyakit, biaya pengobatan, morbiditas dan mortalitas yang tinggi. Pasien gagal ginjal kronis (GGK) dengan terapi hemodialisis membutuhkan terapi jangka panjang dan seumur hidup. Hal tersebut dapat mempengaruhi kualitas hidup pasien. Tujuan penelitian ini adalah untuk mengetahui pengaruh konseling terhadap kualitas hidup pasien GGK dengan terapi hemodialisis di RSUD Cibabat kota Cimahi yang dilaksanakan pada bulan Maret sampai April 2019. Penelitian ini menggunakan pre experimental study one group pre test and post test design. Jumlah sampel yang memenuhi kriteria inklusi sebanyak 62 sampel. Kualitas hidup akan diukur dengan menggunakan kuisioner EQ5D5L sebelum dan setelah konseling. Nilai utilitas sebelum konseling 0,75±0,18 dan setelah konseling 0,82±0,18. Sementara untuk perubahan skor EQ5D Visual Analog Scale (VAS) sebelum konseling adalah 3,74±10,21 dan setelah konseling 69,19±11,53. Hasil penelitian ini menunjukkan bahwa perubahan nilai utilitas dan skor EQ5D VAS pasien hemodialisis sebelum dan setelah konseling menunjukkan perbedaan yang bermakna secara signifikan (p<0,05).

Abstract

Antibiotik merupakan suatu senyawa yang bekerja mengatasi dan mencegah infeksi yang disebabkan oleh bakteri. Pada praktiknya banyak antibiotik yang masih digunakan secara tidak bijak maupun tidak rasional sehingga dapat menimbulkan resistensi bakteri. Untuk mengatasi hal tersebut upaya yang dapat dilakukan adalah melakukan evaluasi penggunaan antibiotik sehingga dapat dilihat pola penggunaan antibiotik. Evaluasi penggunaan antibiotik dapat dilakukan secara kuantitatif maupun kualitatif. Tujuan penelitian ini adalah untuk mengevaluasi penggunaan antibiotik pada pasien rawat inap non-bedah di Rumah Sakit X di Bandung pada periode Januari-Desember 2022. Penelitian ini dilakukan dengan menggunakan metode evaluasi penggunaan antibiotik dengan metode Anatomical Therapeutic Chemical/Defined Daily Dose (ATC/DDD). Selain itu dilakukan analisis terhadap Drug Utilization 90% (DU 90%). Hasil dari penelitian yang dilakukan didapatkan bahwa terdapat 28 antibiotik yang digunakan. Antibiotik yang memiliki nilai DDD/100 hari rawat tertinggi adalah cefixime yaitu sebesar 4,78 DDD/100 hari rawat. Sementara antibiotik yang masuk kedalam segmen DU 90% adalah levofloxacin, cefixime, ceftriaxone, azithromycin, cefadroxil, amoxicillin, cefotaxime, metronidazole, dan thiamphenicol.Kata kunci: Evaluasi Penggunaan Obat, Antibiotik, ATC/DDD

Abstract

AbstrakPerkembangan nanopartikel dalam bidang farmasetika semakin meluas dengan berbagai penelitian yang memformulasikan nanopartikel sebagai sistem penghantaran obat. Salah satunya yaitu sediaan insulin oral. Insulin merupakan hormon yang berperan penting dalam terapi diabetes melitus. Secara umum, insulin yang digunakan diberikan secara subkutan menimbulkan ketidaknyamanan bagi pasien dan berbagai efek samping. Berbagai penelitian kemudian mengembangkan sediaan insulin oral sebagai solusi dari permasalahan tersebut. Namun, pada praktiknya penghantaran insulin secara oral terhambat dengan karakter insulin yang mudah terdegradasi dan rusak di saluran gastrointestinal. Oleh karena itu, penelitian lainnya menemukan solusi lain dengan menggunakan nanocarrier sebagai pembawa insulin. Secara keseluruhan, studi menunjukkan formula nanocarrier insulin oral menghasilkan sediaan yang stabil, mampu melindungi insulin dari berbagai barrier saluran gastrointestinal, dan berpengaruh dalam menurunkan kadar glukosa darah tikus diabetes. Beberapa nanocarrier yang memenuhi kriteria karakterisasi nanopartikel yang baik untuk sistem penghantaran insulin oral yaitu pada nanocarrier berbasis PLGA-lipid-PEG, TMC-PLGA, Kitosan-PLGA, Kitosan-PSS-PGA, dan CMCD-g-Kitosan. ABSTRACT Insulin is a hormone that plays an important role in the treatment of diabetes mellitus. Generally, the used of insulin therapy is given subcutaneously, causing discomfort or inconveniences to the patient and various other side effects Various studies have developed oral insulin preparations as a solution to this problem. However practically, oral insulin administration is hampered by the character of insulin which is easily degraded in the gastrointestinal tract. Therefore, another study found another solution by applying a nano-carrier system as an insulin carrier. This review article aims to discuss the formulation of insulin preparations with nano-carrier systems. The data sources used as references in this article review consist of 25 international journals. From the review of this article, it was found that the nano-carrier system has good potential to be further developed as an oral insulin delivery system. Several oral insulin formulations with good nanoparticle characteristics are nano-carrier systems based on PLGA-lipid-PEG, TMC-PLGA, Ch-PLGA, Ch-PSS-PGA, CMCD-g-Ch, TMC-β-cyclodextrin, and Ch-TGA-based SNEDDS.

Abstract

Resistensi antibiotik menjadi salah satu topik yang mendapat perhatian lebih dari pemerintah Indonesia. Pada PMK No. 8 Tahun 2015 tentang Program Pengendalian Resistensi Antimikroba di Rumah Sakit mewajibkan Rumah sakit untuk membentuk tim pelaksana Program Pengendalian Resistensi Antimikroba (PPRA) di Rumah Sakit. Tugas dari tim pelaksana PPRA diantaranya melakukan evaluasi penggunaan antibiotik. Metode ATC/DDD (Anatomical Therapeutic Chemical/Daily Defined Dose) digunakan untuk evaluasi kuantitatif penggunaan antibiotik. Tujuan dari penelitian ini adalah untuk mengetahui gambaran penggunaan antibiotik dalam satuan jumlah ATC/DDD dan mengetahui antibiotik yang termasuk kedalam DU90% (Drug Utilization 90%) pada pasien rawat jalan di poli penyakit dalam (Internis) di RS Al-Islam Bandung. Pengumpulan data pada penelitian ini dilakukan secara retrospektif. Pengamatan dilakukan pada resepOktober-Desember 2019 sebanyak 990 resep. Hasil dari perhitungan menunjukkan bahwa 12% resep menggunakan antibiotik dan 88% resep tidakmenggunakan antibiotik. Isoniazid merupakan antibiotik dengan nilai ATC/DDD tertinggi yaitu 163750 dan amoxycillin merupakan antibiotik dengan nilai ATC/DDD terendah yaitu sebesar 750. Antibiotik yang termasuk kedalam DU90% adalah isoniazid, rifampisin, ethambutol dan pyrazinamide.
