Abstract

LL-37 is an antimicrobial protein expressed by the CAMP gene in humans. This protein has various antibacterial, antiviral and anticancer effects. Expressing LL-37 as a heterologous protein in E. coli cells has its challenges. LL-37 is a peptide that is so small that it must be engineered with a fusion protein to increase its size solubility and prevent proteolysis of the target protein in cells. Factor Xa is the protease chosen to cleave LL-37 from its fusion protein in this research due to leucine binding factor Xa quite strongly. The aim of this study is, therefore, to express LL-37 as a fusion protein with Histaq_SUMO at the N-terminus linked to LL-37 at the C-terminus through Factor Xa cleavage site. Then, the fusion protein was cleaved by Factor Xa to obtain pure LL-37. In this study, LL-37 was produced by recombinant DNA technology, starting with the construction, expression, purification and cleavage of the fusion protein. The constructed genes consisted of 6xHis, SUMO, the factor X cleavage site, and LL-37, a total of 450 bp inserted in the pD451-SR vector plasmid. The results of this study yielded a SUMO_LL-37 protein with a molecular weight of approximately 17.34 kDa, which could be purified by Ni-NTA under native purification conditions. Based on ImageJ SUMO_LL-37 quantification, it was 1.65 µg/µL. LL-37 can be cleaved by factor Xa from SUMO with an enzyme-to-substrate ratio of 1:12.5 at 37°C with a 24-hour incubation time and results as much as 0.144 µg/µL. This article has a related Erratum.

Abstract

The high incidence of infections and the increase in antibiotic resistance have become a global health concern. The emergence of strains of resistant bacteria has also encouraged the exploration of potential antibacterial agents, especially from natural sources. This review aims to provide information regarding the antibacterial activity of stems, leaves, fruits and seeds of mengkudu (M. citrifolia L,) in inhibiting various bacterial species. In this article review, online and offline literature were used. Online literature was obtained from local and international journals and scientific articles of the last 10 years, 2008-2018, which were obtained through direct search results online using online search engines namely Google and Google Scholar. Whereas the offline literature used is books and e-books. It is known that the root, stem, leaves, fruit, and seeds of mengkudu (M. citrifolia L,) with various solvents used can inhibit various species of bacteria with different diameter inhibition zones.

Abstract

Bakteri penyebab Infeksi saluran kemih (ISK) telah mengalami resistensi terhadap berbagai antibiotik. Pola resistensi bakteri tersebut perlu diperbaharui datanya untuk memastikan pemberian antibiotik yang tepat. Tujuan penelitian mengidentifikasi dan melihat pola resistensi bakteri penyebab ISK di Puskesmas Ibrahim Adjie Kota Bandung. Koloni bakteri yang berasal dari urin 9 pasien  suspect ISK ditumbuhkan dan hanya sampel yang berasal dari 3 pasien memenuhi persyaratan terindikasi  ISK karena adanya jumlah koloni di atas  105 cfu/ml. Selanjutnya dilakukan penentuan Gram bakteri, isolasi DNA total kromosom bakteri, amplifikasi gen pengkode 16S rRNA, sekuensing (penentuan urutan nukleotida) dan uji resistensi terhadap antibiotik golongan penisilin, penisilin+inhibitor β-lactamase, sefalosporin (generasi 1,2,3,4), fluorokuinolon, aminoglikosida dan karbapenem. Hasinya didapatkan tiga isolat (P1,P2,P3) yang teridentifikasi secara genetik Escherichia coli. Isolat P1 resisten terhadap penisilin, penisilin+inhibitor β-laktamse, sefalosporin generasi 1 dan fluorokuinolon. isolat P2 resisten terhadap penisilin. Isolat P3 resisten terhadap penisilin, penisilin+inhibitor β-laktamse, sefalosporin (generasi 1,2,3,4) dan fluorokuinolon. E. coli penyebab ISK telah mengalami resistensi terhadap antibiotik penisilin, penisilin+inhibitor β-lactamase, sefalosporin (generasi 1,2,3,4) dan fluorokuinolon. Aminoglikosida dan karbapenem masih memiliki aktivitas terhadap ketiga isolat E. coli resisten.Kata kunci: bakteri; ISK; isolasi; identifikasi; resistensi

Abstract

Candidiasis is a Candida sp. infection (Candida albicans in common) that happens in nails, hairs, skin, and oesophagus, with around 20-25% prevalence in Indonesia. Chitin is one of the main components of its cell wall and serves as a target for candidiasis therapy using natural degradant compounds, such as chitinase enzyme. Chitinase enzyme is collected from chitinolytic bacteria found massively in chitin-rich environments like seawater. The screening is needed to identify the varieties of bacteria with chitinolytic activity. The screening was done using green mussel waste (Perna viridis), which contains 14-35% chitin and reached 10.776,75 tons of trash in 2020. This research aims to get a chitinolytic bacteria using green mussels as a chitin substrate source and a chitinolytic bacteria with anti-candidiasis activity. This research was conducted by isolating chitin from green mussel shells, chitinolytic bacteria screening, and anti-candidiasis activity test. Results collected from 5.00, 5.08, and 5.18 km isolates were the Vibrio alginolyticus, Vibrio neocaledonicus, and Marinobacter persicus species, which had a chitinolytic index of 1.246, 1.560, 1.492 also 64.29; 61.30; 69.01 U/ml chitinase enzymatic activity. The highest anti-candidiasis activity was found in a sample 5,18 km from the coast of Kejawanan Beach based on the number of chitinase enzyme activity and the chitinolytic index.

Abstract

Public perceptions regarding the role and quality of pharmacists can show how patients view the services provided and are used to evaluate the quality of services from the patient's perspective. Pharmaceutical services are currently based on patient-oriented where the relationship between pharmacists and patients greatly influences patient satisfaction, compliance with treatment plans, and the quality of health services. This study aims to analyze patient perceptions and satisfaction with pharmaceutical services provided by pharmacists and the factors that influence them at X Hospital, Bandung. The method used is non-experimental research with observational methods. Data collection was carried out by distributing questionnaire sheets to 86 respondents. The Cronbach's Alpha value on the questionnaire that used was 0.736. The data obtained shows that 84% of respondents have a positive perception of pharmacists' role, 87% have good service experience, and 100% are satisfied with the pharmaceutical services provided by pharmacists at X Hospital, Bandung City. In addition, respondents also hope that the quality of pharmaceutical services can be maintained and improved by providing patients with more in-depth medicine information, increasing pharmacists' role in dealing with routine medication consumption problems, and shortening waiting times for medicine collection.

Abstract

Pada tahun 2017, WHO mengeluarkan daftar bakteri yang sangat membutuhkan antibiotik baru akibat tingkat resistensi yang tinggi, diantaranya yaitu Pseudomonas aeruginosa, Acinetobacter baumannnii, serta Klebsiella pneumoniae. Ketiga bakteri ini telah resisten terhadap sejumlah besar antibiotik, termasuk karbapenem yang merupakan lini terakhir dalam pengobatan (broad spectrum). Salah satu karbapenem yang seringkali digunakan adalah meropenem, yang digunakan untuk mengatasi infeksi pada kulit dan jaringan, infeksi saluran cerna, serta infeksi lainnya. Saat ini, penggunaan meropenem di rumah sakit semakin meningkat dan mengakibatkan resistensi pada bakteri gram negatif. Review ini bertujuan untuk menggambarkan perkembangan dari resistensi meropenem terhadap bakteri P. aeruginosa, A. baumannii, K. pneumoniae serta terapi yang telah digunakan untuk resistensi meropenem di Indonesia. Pengumpulan data dilakukan dengan peninjauan literatur yang terkait. Berdasarkan hasil penelusuran, ketiga bakteri tersebut telah mengalami resistensi terhadap meropenem dengan mekanisme resistensi pada mutasi gen pompa efflux pada P. aeruginosa, penurunan permeabilitas dan pembentukan biofilm pada A.baumanii, serta produksi karbapenemase oleh K. pneumoniae. Terapi yang telah digunakan saat ini terdiri dari kolistin, kombinasi seftolozan/tazobaktam, polimiksin serta fosfomisin.

Abstract

The limited scientific evidence on the analgesic and anti-inflammatory effects of Polypodium feei roots, despite their traditional use in rheumatism treatment, highlights the need for pharmacological validation. The study investigates the analgesic and anti-inflammatory properties of Polypodium feei roots, a traditional medicine used to treat rheumatism, in mice and rats with carrageenan-induced paw edema, using acetylsalicylic acid and tramadol as analgesics and diclofenac sodium as an anti-inflammatory. The writhing test results demonstrate that the extract of P. feei roots (EPFR) at dosages of 50 and 100 mg/kgBW lowered the writhing responses of rat significantly (p <0.01). The dose of 100 mg/kgBW provided more protection than acetylsalicylic acid at a level of 65 mg/kgBW. In the hot plate method, the extract increased the latency period significantly (p <0.05) as compared with the control. In the anti-inflammatory examination, the EPFR decreased edema of rat paws induced by carrageenan. The significant effect of the EPFR was shown by the dose of 100 mg/kgBW (p <0.05), but its inhibition was lower than that of a standard agent, diclofenac sodium. This study concluded that the EPFR may have peripherally and centrally analgesic activity and anti-inflammatory activity. This evidence supports the folklore claim of the efficacy of EPFR as traditional medicine for rheumatism.

Abstract

Kesalahan pemberian obat akibat kemiripan nama dan kemasan, atau dikenal sebagai Look-Alike Sound-Alike (LASA), merupakan salah satu tantangan serius dalam pelayanan farmasi rumah sakit. Penelitian ini bertujuan untuk mengevaluasi efektivitas sistem penataan dan penandaan obat LASA di unit rawat jalan salah satu rumah sakit swasta di Bandung. Metode penelitian yang digunakan adalah kualitatif deskriptif dengan teknik pengumpulan data melalui observasi langsung, wawancara semi-terstruktur dengan tenaga farmasi, serta dokumentasi laporan insiden medication error. Hasil menunjukkan bahwa rumah sakit telah menerapkan sistem FIFO dan FEFO, pemisahan obat LASA berdasarkan bentuk dan kekuatan sediaan, serta penggunaan stiker LASA dan Tall Man Lettering. Penandaan ini diterapkan pada berbagai bentuk sediaan seperti tablet, sirup, inhaler, salep, dan krim, dengan tujuan meningkatkan kewaspadaan petugas farmasi dan mengurangi risiko kesalahan pengambilan obat. Diskusi menunjukkan bahwa strategi visual seperti label mencolok dan huruf kapital sebagian pada nama obat terbukti efektif dalam membedakan produk serupa. Namun, pelatihan berkala dan audit internal tetap diperlukan untuk menjaga konsistensi dan efektivitas sistem. Kesimpulannya, penataan obat LASA di rumah sakit ini telah sesuai dengan standar keselamatan pasien dan dapat menjadi acuan penerapan sistem serupa di fasilitas pelayanan kesehatan lainnya.

Abstract

BACKGROUND: Systemic lupus erythematosus (SLE) is an autoimmune disease characterized by inflammation. The pathogenesis of SLE involves key proteins, including B cell-activating factor (BAFF) and the ubiquitin enzyme A20, both serving as negative regulators of inflammation and contributing to B cell homeostasis. In this review, focused on interventions directed at BAFF and the A20 enzyme, utilizing monoclonal antibodies either independently or in conjunction with conventional therapy for SLE patients.METHODS: A literature search was conducted on the PubMed platform by combining various terms, including "B-cells activating factor", "TNFAIP3 protein (human)", "therapeutics" or "drug therapy", and "lupus erythematosus, systemic" (limited to the last 10 years). From total of 104 articles discovered in thr search, the total number of articles collected after being filtered was 27 articles.RESULTS: Clinical development and evaluation have been conducted regarding the use of appropriate therapy for SLE patients. Selective BAFF inhibitor has been tested in clinical trials as a blocking agent in BAFF receptor (BAFF-R) and signaling nuclear factor-kappaB (NF-κB) by A20 bindings to inhibit the activation of autoreactive B cells. Just like other antimonoclonal therapies, BAFF and the A20 enzyme can be used as therapeutic targets with a single use or combined with the standard therapy in patients with SLE. In addition, the use of BAFF and A20 also shown to have safe side effects in patients with SLE. CONCLUSION: BAFF protein and A20 enzyme present promising therapeutic targets for managing autoimmune diseases like SLE. Therapeutic interventions can be administered individually or in conjunction with standard treatments.  KEYWORDS: systemic lupus erythematosus, therapeutic targets, BAFF, A20

Abstract

Drug interactions can occur when drugs are administered simultaneously with other drugs, both pharmacologically and clinically. This study aims to identify drug interactions that occur in prescriptions with patients experiencing endocrine system disorders for the period July 2024 at Hospital X, Bandung City. Endocrine disorders, involving hormonal dysfunction such as that of the thyroid, adrenals, or pancreas, can affect the pharmacokinetics and pharmacodynamics of drugs, thereby increasing the risk of interactions. This research method involved randomly collecting two prescriptions per day, with a total of 60 recipes analyzed. The results of the assessment showed that there were 15 potential drug interactions with a percentage of 25%, consisting of one minor interaction (6.67%), 15 moderate interactions (93.33%), and no major interactions were found.
