Abstract

Tuberculosis (TB) is an infectious disease that is still a global health problem, caused by the bacterium Mycobacterium tuberculosis, which is acid-resistant and mainly attacks the lungs, but can also attack other organs. This disease can affect people of all ages, including babies, children, adults, and the elderly. TB transmission occurs through the air from people who cough or sneeze, with the main factors affecting public health being environmental factors and behaviours, such as uncovered coughing and poor sanitation. TB cases in Southeast Sulawesi Province have shown a significant upward trend in recent years, with the incidence rate continuing to rise every year. The success of TB treatment is influenced by a variety of factors, including medication adherence, which is a major challenge to avoid the emergence of drug resistance, such as TB-MDR. The Government of Indonesia has issued policies and focuses on reducing TB incidence through education and socialisation to encourage active community involvement in TB control to achieve the national target by 2030.

Abstract

Background: Inflammation is a response process to tissue injury and infection. The inflammatory response causes the release of pro-inflammatory mediators such as IL-1, IL-6 and TNF-α which are the main cytokines in the acute inflammatory response. Plants that have the potential to be anti-inflammatory are notika leaves (Archboldiodendron calosericeum Kobuski). Purpose: This research was conducted to evaluate the anti-inflammatory activity of the ethanol extract of notika leaves against the levels of Tumor Necrosis Factor Alpha (TNF-α) in Wistar rats. Methods: The study used a post test only control group design. Rats were divided into five groups consisting of a positive control (diclofenac sodium), three extracts (100, 200 and 300 mg/kg BW) and Teeee carragenantrol gopre roups that are inlaporkan dari tanaman ketapang dalam spewa yang dilaporkan pada besar, dll --lar.negative control (NaCMC 0.5%). In vivo tests were carried out by induction of inflammatory using carragenan, administering preparations and measuring TNF-α levels using the ELISA method. Data were analyzed using the One Way ANOVA test. Results: The results showed that the mean levels of TNF-α in the positive control group (0.475 ng/L), negative control (0.816 ng/L), extract dose 100 mg/kgBW (0.67 ng/L), extract dose 200 mg/kgBW (0.7285 ng/L) and extract dosage of 300 mg/kgBW (0.7785 ng/L). Conclusion: The study concludes that the ethanol extract of notika leaves has anti-inflammatory activity against the levels of Tumor Necrosis Factor Alpha (TNF-α) in Wistar rats based on the reduction in levels with negative control and optimum dose was 100 mg/kgBW.

Abstract

Tumor necrosis factor-alpha (TNF-α), a cytokine involved in inflammation, plays a critical role in inflammatory processes, particularly in synovial cells. Therefore, TNF-α represents a promising target for anti-inflammatory interventions. This study investigates the anti-inflammatory potential of Tectona grandis leaf extract by evaluating its ability to inhibit TNF-α activity. To identify bioactive compounds from the extract that may interact with the TNF-α receptor, molecular docking simulations were employed. In vivo experiments were conducted using different doses of the extract (100, 200, and 300 mg/kg body weight), and TNF-α levels were quantified to assess the anti-inflammatory effect. At a dose of 300 mg/kg, TNF-α levels were significantly reduced to 0.358 μg/L, indicating superior anti-inflammatory activity compared to the lower doses (0.539 μg/L at 100 mg/kg and 0.433 μg/L at 200 mg/kg). Molecular docking simulations revealed two bioactive compounds, Phaeophorbid A and (132S, 17S, 18S)-132-Hydroxy-20-Chloro-Ethylpheophorbide A, which demonstrated strong binding affinities (-5.4 kcal/mol and -5.2 kcal/mol, respectively) and interacted with the TNF-α receptor through hydrogen bonds and hydrophobic interactions. These results underscore the potential of Tectona grandis as a source of novel anti-inflammatory agents, suggesting its value in the development of therapeutic strategies targeting TNF-α in inflammation-related disorders.
