Abstract

Formula alternatif diperlukan dalam proses pengembangan formula, formula ini dibuat untuk menurunkan biaya produksi tanpa menurunkan kualitas produk. Pada formula tablet yang akan dikembangkan dilakukan penggantian eksipien yaitu disintegran karena konsentrasi disintegran yang digunakan lebih tinggi jika dibandingkan eksipien lainnya dan memiliki harga yang cukup mahal. Disintegran pada formula sebelumnya adalah croscarmellose sodium dengan 5% yang merupakan jenis superdisintegran. Dalam mengganti disintegran diperlukan studi literatur untuk membandingkan kemampuan disintegran tersebut, selain itu juga harus mempertimbangkan harga dari bahan baku. Hasil studi literatur dan perbandingan harga, menunjukkan bahwa sodium starch glikolat dengan konsentrasi 4% merupakan disintegran yang paling tepat untuk menggantikan croscarmellose sodium.Kata kunci: Formula alternatif, disintegran, efisiensi biaya.

Abstract

Diabetes mellitus (DM) is a metabolic disease caused by a persistent increase in high blood glucose levels due to insufficient insulin production. Diabetes affects individuals of all ages and genders making it one of the most common global causes of death and disease. The most common types of DM are type 1 and type 2, which are generally caused by impaired insulin secretion and/or insulin action. Before an individual develops type 2 diabetes, the individual previously suffered from prediabetes, a condition where blood glucose levels are higher than normal but not yet high enough to be diagnosed as diabetes. The impact of diabetes can be reduced by taking preventative measures for type 2 diabetes and prediabetes through early detection, appropriate treatment and education regarding diabetes. Counseling activities were carried out at the Regional Health Center, Derwati, Rancasari, Bandung, West Java, Indonesia on June 8 2024, from 84 people who took part in the activity, 15% were in the diabetes category and 15% in the prediabetes category. Providing education and good lifestyle recommendations is carried out to increase public awareness and routine compliance with blood sugar control, which is an important action to maintain disease stability and prevent more serious complications.

Abstract

Hypertension poses significant global challenges and contributes to the rising morbidity and mortality rates associated with cardiovascular diseases, including heart disease and stroke. Commonly referred to as a "silent killer," hypertension frequently presents without significant symptoms, making early detection and appropriate management crucial. This community service program aims to conduct hypertension screening and provide education to enhance awareness and promote preventive measures. The activities were carried out at a integrated service post (Posyandu), involving 62 participants aged between 34 and 81 years. The program included health screenings, which assessed blood pressure and blood glucose levels, as well as educational sessions addressing hypertension risk factors, preventive strategies, and the importance of therapy adherence. Results indicated that 14.5% of participants had normal blood pressure, while 63% fell into the prehypertensive category, 17.7% were classified as stage 1 hypertension, and 4.8% were identified as having stage 2 hypertension. The educational sessions emphasized lifestyle changes and the importance of regular health monitoring to prevent further complications. This program underscores the necessity of continuous health education and promotion in the community to control hypertension and improve overall public health.

Abstract

Salah satu strategi dan intervensi dari WHO dalam menurunkan insiden tuberkulosis (TB) adalah memberikan terapi pencegahan tuberkulosis (TPT) kepada kelompok orang berisiko atau orang dengan infeksi laten tuberkulosis (ILTB). Beberapa penelitian dilapangan menunjukan masih terdapat hambatan sehingga program ini belum dikatakan berhasil. Kajian ini bertujuan untuk mengidentifikasi hambatan dan strategi potensial untuk mendukung program TPT. Pencarian artikel untuk narrative review ini yaitu abstrak artikel berbahasa inggris dengan rentang waktu terbit dari tahun 2008 sampai tahun 2024 yang diterbitkan dari database Scopus, Pubmed dan World of Sciences (WoS). Dari 106 artikel yang berpotensi memenuhi syarat, terdapat 29 artikel yang memenuhi kriteria inklusi. Hambatan yang teridentifikasi adalah kurangnya pengetahuan tenaga kesehatan mengenai TPT, kurangnya arahan dari pembuat kebijakan nasional, kurangnya pengetahuan pasien mengenai penyakit tuberkulosis, ketersediaan logistik belum memadai, kurangnya petugas diagnostik serta pasien kesulitan karena jarak jauh menuju layanan pemeriksaan. Dari hambatan yang teridentifikasi, adapun strategi potensial yang mendukung seperti pelatihan manajemen ILTB bagi tenaga kesehatan, sosialisasi dan edukasi kepada pasien, perencanaan obat dan layanan pemeriksaan yang memadai serta pedoman dan arahan kebijakan nasional yang terintegrasi. Meningkatkan kesadaran masyarakat akan resiko terjadinya infeksi TB, arahan kebijakan nasional yang serius serta kolaborasi antara fasilitas kesehatan pemerintah dan swasta untuk setiap kaskade pelayanan ILTB diharapkan dapat memberikan kontribusi yang mendukung program TPT sehingga dapat mengurangi insidensi TB aktif secara global.

Abstract

Hypertension is a significant health issue that significantly increases the risk of cardiovascular disease. Angiotensin II receptor blockers (ARBs) are commonly used in Indonesia to treat hypertension, yet comprehensive comparative efficacy data within this drug class remain limited. This review seeks to evaluate the blood pressure-lowering efficacy of three particular angiotensin receptor blockers (ARBs)—Valsartan, Candesartan, and Losartan—in individuals diagnosed with hypertension. This systematic review identified randomized controlled trials (RCT) obtained from PubMed and Google Scholar using several keyword combinations. Among 20 RCTs with 6.425 patients treated with three ARBs, all included studies demonstrated significant blood pressure reductions. The mean reductions in systolic blood pressure (SBP) and diastolic blood pressure (DBP) were the greatest for Candesartan (-18.69/-10.25 mmHg), followed by Losartan (-14.80/-7.8 mmHg), and Valsartan (-8.94/-7.58 mmHg). Candesartan achieved the largest reduction, decreasing baseline systolic blood pressure (SBP)/diastolic blood pressure (DBP) by 38.16/18.95 mmHg over 12 weeks. In contrast, Valsartan showed the smallest reduction, with SBP decreasing by 0.81/0.04 mmHg after 66 months. It can also be linked to the pharmacokinetic properties of the drugs, where Valsartan has a higher bioavailability, and Losartan has a lower bioavailability compared to the other ARBs. It can be concluded that ARBs, including Valsartan, Candesartan, and Losartan, are generally effective in reducing blood pressure in hypertensive patients, with Candesartan showing the greatest efficacy. Future research should involve direct comparative trials of Valsartan, Candesartan, and Losartan, along with studies on their molecular mechanisms, to increase effective hypertension treatments.

Abstract

Hypertension is a non-communicable disease that always increases every year. Hypertension is known as a silent killer because patients who suffer from hypertension rarely feel the symptoms. Common symptoms of hypertension can be headaches in the back of the neck, blurred vision, and others. The blood pressure of hypertension sufferers is above 140/90 mmHg. This study aims to see the pattern of prescribing antihypertensive drugs in BPJS Kesehatan patients including gender, age, patient diagnosis, and combination of antihypertensive drug classes. The method used is descriptive observational research. This study was conducted using 195 prescriptions in the period November 2023. The results obtained were that hypertension sufferers were more dominated by women with a total of 131 sufferers with a percentage of 67% compared to men with a total of 64 sufferers with a percentage of 33%. The age most affected by hypertension is people aged 51 years - 75 years with a total of 143 people and a percentage of 73.33%. 92 of 195 (47.18%) patients did not have other diseases such as diabetes mellitus, hyperlipidemia, heart, GI, and asthma. Single therapy of antihypertensive drugs was most commonly prescribed by doctors with a total of 127 patients with a presentation of 65.13% compared to patients who used combination therapy of two or more antihypertensive drugs. Calcium antagonist or CCB drugs were most commonly used by BPJS patients at several pharmacies in the Bandung area used in the prescription analysis with a total of 107 patients with a percentage of 54.87%.
