Abstract

The incidence of psychosomatic disorders is increasing in Indonesia, and therefore screening instruments that are culturally appropriate for the Indonesian population are needed. The aim of this study was to assess the validity and reliability of the Shatri Sinulingga psychosomatic test (SSPT) questionnaire as a screening instrument for psychosomatic disorders in Indonesia. An analytic descriptive cross-sectional study divided into two stages (questionnaire formulation and distribution through the Psikosom.id application) was conducted in 2023. The validity test was carried out using the product moment technique, and Cronbach's alpha assessment was carried out to evaluate the reliability of the questionnaire. A total of 372 participants were included, with a mean age of 39.24 years old. The SSPT questionnaire scores had strong correlations with the scores of hospital anxiety and depression scale (HADS) (r=0.668; p<0.001) and the Symptom Checklist-90 (SCL-90) (r=0.674; p<0.001); and moderately correlated with the score of fatigue severity scale (FSS) (r=0.505; p<0.001) and Pittsburgh Sleep Quality Index (PSQI) (r=0.492; p<0.001). The correlation coefficient (r) value of each SSPT question ranged from 0.389–0.726, and all r higher than r table with an overall Cronbach’s alpha of 0.744. The SSPT questionnaire's strong predictive performance: had a sensitivity of 75.6% and specificity of 71.4% with an area under the curve (AUC) of 81% (95%CI: 76.6–85.4%; p<0.001) to HADS; sensitivity of 80% and specificity of 58.6% with an AUC of 75.1% (95%CI: 70–80.2%; p<0.001) to FSS; sensitivity of 64% and specificity of 67.5% with an AUC of 71.2% (95%CI:  65.4–77%; p<0.001) to PSQI; and had sensitivity of 78% and specificity of 67.4% with an AUC of 80.9% (95%CI:  76.6–85.3%; p<0.001) to SCL-90. The study highlights that the SSPT questionnaire is valid and reliable to be used as a screening instrument for psychosomatic disorders in Indonesia.

Abstract

Chronic Kidney Disease (CKD) is a condition of kidney damage with a decrease in glomerulus filtration rate of 60 ml/min/1.73 m3 for a duration of more than three months. CKD is closely related to the patient’s quality of life because the patient must carry out various lifestyle restrictions to slow the progression of kidney damage. Lifestyle restrictions often cause problems in the psychosocial field that increase the severity of symptoms and decrease kidney function faster. This evidence-based case review was compiled to determine the effectiveness of hypnotherapy in improving the quality of life of CKD patients. Literature searches were conducted in four databases, namely PubMed, Cochrane, EMBASE, and Scopus. There are three literatures that fit the eligibility criteria. The critical review was conducted using guidelines created by the Centre for Evidence-Based Medicine, University of Oxford. Hypnotherapy consistently shows improved quality of life in the form of decreased symptoms of anxiety, depression, drowsiness, fatigue, and improved social functioning and relationships with family. Hypnotherapy can be considered a complementary therapy in the management of CKD patients with minimal side effects by considering the patient’s accessibility and preferences.

Abstract

Introduction. Dolutegravir (DTG) is one of the effective antiretroviral therapies (ART) for suppressing HIV replication and has a favorable safety profile. However, reports regarding neuropsychiatric side effects, especially in patients transitioning from a nevirapine (NVP) regimen to dolutegravir, have started to emerge. This study aimed to describe the changes in neuropsychiatric symptoms before and after the transition from a NVP-based ARV regimen to DTG in HIV patients at Cipto Mangunkusumo Hospital (RSCM), Jakarta. Methods. This study employed a prospective cohort design involving 292 HIV patients at Cipto Mangunkusumo Hospital who were transitioning from NVP to DTG regimen. Participants were monitored for six months, and neuropsychiatric symptoms were assessed using a modified HIV symptom index (mHSI). Data collection was conducted at the beginning of the transition (baseline) and during follow-up through interviews regarding HIV-related symptoms and ART. Results. At the start of the study, 70.2% of participants reported at least one neuropsychiatric symptom, which increased to 80.1% after 6 months of transitioning to TLD. The most commonly reported symptoms were insomnia, headaches, and anxiety, with women reporting more symptoms than men. Statistical analysis showed a significant increase in symptoms of sadness, headaches, and irritability (p < 0.01) after the transition to dtg. Conclusions. Although DTG shows good viral suppression capabilities, significant neuropsychiatric side effects still arose, especially in patients with a history of mental disorders. Close monitoring of neuropsychiatric symptoms and adjustments to the ART regimen need to be carried out to improve the quality of life for HIV patients undergoing DTG therapy.

Abstract

Background: The high incidence of gastrointestinal and pancreaticobiliary diseases among elderly has led to increased endoscopic procedures in this population. Previous studies indicate a higher risk of cardiopulmonary complications during and after endoscopy in elderly patients compared to younger individuals. This study investigated the incidence and contributing factors of endoscopy-related cardiopulmonary complications in elderly patients. Methods: A prospective cohort study was conducted on 194 patients aged ≥ 60 years undergoing endoscopy at the Gastrointestinal Endoscopy Center, Cipto Mangunkusumo National General Hospital, Jakarta, Indonesia, from August to October 2023. Consecutive sampling method was used. Multivariate analysis with logistic regression was conducted. Results: Among the 194 patients included, 49.52% experienced cardiopulmonary complications. The most common complications were tachycardia (23.20%), hypoxemia (15.03%), and hypotension (6.20%). Multivariate analysis identified the complexity of the procedure (ASGE level ≥ 3) as a significant risk factor (RR 1.505, 95% CI 1.039-2.179; p=0.03), while mild-moderate sedation was associated with a reduced risk of complications (RR 0.668, 95% CI 0.458-0.975; p=0,037). Conclusion: The incidence of cardiopulmonary complications during and after endoscopy is high among Indonesian elderly. Procedure complexity is an independent risk factor, whereas mild-moderate sedation mitigates the risk. In opposite, advanced age, multimorbidities, high ASA class, long duration of procedure, poor nutritional and functional status, and type of procedure did not affect outcomes

Abstract

Background: Inflammatory Bowel Disease (IBD), encompassing ulcerative colitis (UC) and Crohn's disease (CD), is an inflammatory digestive tract condition with unknown causes. Its unpredictable symptoms affect quality of life. In Indonesia, the quality of life of IBD patients remains unreported. Factors such as advanced age, long disease duration, active disease, corticosteroid use, comorbidities, unemployment, and poor sleep quality may reduce quality of life. This study aims to assess the quality of life profile of IBD patients and its associated factors. Methods: This cross-sectional study collected data from October to November 2024 through interviews at the outpatient unit of Dr. Cipto Mangunkusumo Hospital (RSCM). Quality of life was measured using the Inflammatory Bowel Disease Questionnaire 9 (IBDQ-9), and sleep quality was assessed with the Pittsburgh Sleep Quality Index (PSQI). Both tools were validated in Indonesian. Eligible participants met inclusion and exclusion criteria. Bivariate and multivariate logistic regression analyses identified factors associated with quality of life. Results: Among 201 participants, 95% reported a good quality of life. Multivariate analysis identified active disease (PR 4.072 [1.133–14.633], p = 0.031) and combination therapy (PR 12.803 [1.423–115.147], p = 0.023) as factors associated with poor quality of life. Age, disease duration, comorbidities, employment status, and sleep quality showed no significant associations. Conclusion: Most IBD patients (95%) in the RSCM outpatient unit reported a good quality of life. Active disease and combination therapy were linked to poorer quality of life.

Abstract

Depression, anxiety, and poor outcomes in cancer are possibly mediated by autonomic dysfunction. Autonomic disfunction found in cancer patients can cause increase in sympathetic tone and decrease in cardiac muscle vagal tone. Heart rate variability (HRV) is a non-invasive index for measuring vagal response and a potential stress response in autonomic dysfunction. In patients with cancer, high vagal tone measured by HRV is significantly correlated with increased overall survival. This evidence-based case report aims to investigate the role of HRV as a prognostic factor in cancer patients. Literature search was done by two authors on five databases: Pubmed, Cochrane Library, EMBASE, ProQuest dan MEDLINE (EbscoHost) in accordance with clinical question. Articles were selected using inclusion and exclusion criteria, then critical review was performed. Quality of the studies was evaluated using the Center of Evidence-Based Medicine, University of Oxford guideline. Two systematic review and four cohort study was included. Increased HRV index is consistently correlated with increased overall survival (OS) in cancer patients. One meta-analysis study found that OS is significantly longer in group with higher HRV (Hazard ratio, HR = 0.70, IK95% 0.60 – 0.82, p < 0.001, I2 = 27%). In conclusion, HRV can be used as a prognostic factor in advanced stage cancer patients. Increased HRV is associated with better OS.

Abstract

Background The pathophysiology of laryngopharyngeal reflux (LPR) is yet to be completely understood, but autonomic dysregulation may have a role in the opening of esophageal sphincters controlled by the parasympathetic nervous system, especially the vagal nerve. Autonomic dysregulation can be detected through heart rate variability (HRV). Currently, studies that identify autonomic nerve dysfunction through HRV in LPR patients are few, especially in the pediatric population. Objective To identify autonomic nerve dysfunction  in pediatric LPR patients with HRV components. Method This study involved 74 pediatric patients, 35 with LPR and 39 without LPR, from  Dr. Cipto Mangunkusumo Hospital, Jakarta, from September 2023 to April 2024. Diagnostics for LPR included endoscopy and Reflux Symptom and Sign Instrument (RSSI) scoring, while measurement of autonomic nerve dysfunction was determined by HRV. The HRV was evaluated by standard deviation normal to normal (SDNN) assessment of the heart’s interbeat interval (IBI) and low frequency/high frequency ratio (LF/HF). Results  Neither HRV measurement, SDNN or LF/HF, were significantly associated with autonomic nerve dysfunction in pediatric patients with LPR. Conclusion Autonomic nerve dysfunction measured by HRV was not associated with LPR in pediatric patients.

Abstract

Despite the numerous benefits of palliative care for cancer patients, there have been few studies on palliative care services for terminal cancer patients, particularly near the end of life. This study aimed to evaluate whether there were differences in length of stay and cost of care associated with how early or late a patient received palliative care intervention. Another objective was to compare the length of stay and cost of care of those who received palliative care intervention and those who did not. This study used a cohort retrospective design at Hospital A, Jakarta, Indonesia, from January to December 2019. The diagnosis of terminal cancer was based on medical records. Data on length of stay and costs of care were based on medical records and finance billing. The hospitalized terminal cancer patients (392) were recruited by consecutive sampling. The length of stay and costs of care for patients with advanced cancer who received palliative care consultations were longer and higher than for patients who did not receive them. However, if palliative care consultation is provided early, the increase in length of stay and costs are less.

Abstract

Diabetes mellitus is a group of metabolic diseases characterized by chronic hyperglycemia due to defects in insulin secretion, insulin action or both. The purpose of this study was to determine the relationship between intrinsic and extrinsic factors with the classification of diabetes mellitus. This research method is an analytic survey, with a cross sectional approach. The population is the entire number of outpatient diabetes mellitus patients as many as 4,365 people in 2019 and the sample in this study was 98 respondents, the sampling technique used a random sampling technique, analyzed univariate and bivariate using the chi-square test. The results showed that there was a significant relationship between social environmental factors (P-value: 0.000<0.05), cultural and religious environmental factors (P-value: 0.012<0.05), economic environmental factors (P-value: 0.037<0 0.05), emotional association factor (P-value: 0.000 < 0.05), physical and psychological state factors (P-value: 0.021<0.05), more assessment factor on food quality (P-value: 0.021<0.05) with the classification of diabetes mellitus. We are recommended for patients to take treatment as directed by health workers by using the 3J technique, namely paying attention to the type of food, amount of food and meal schedule to control blood sugar levels to avoid complications due to diabetes mellitus.
