Abstract

Background: The growth rate of pharmacy in the city of Yogyakarta is very rapid so that it can cause competition between pharmacy. Objective: Pharmacy service performance is very important to be reviewed because it can be used as a parameter to determine management strategies. One method that is recommended in assessing pharmacy performance is the balanced scorecard method. Methods: This research is non-experimental explorative descriptive design with the aim to find out the performance of the apotek XX Yogyakarta based on four perspectives of the balanced scorecard method, financial side, customer, internal business processes, learning and growth of pharmacy and provide recommendations for improvement based on the results of the performance evaluation. Data collection was conducted retrospectively and prospectively in the form of qualitative and quantitative data. Results: The results of the performance apotek XX Yogyakarta using the balanced scorecard method research from a financial perspective shows the value of ITOR has decreased every year, Internal business process perspective is still found the existence of DRP and pharmacies have not met 100% of drug needs and in the perspective of learning and employee productivity growth is valued less. Conclusion: However, over all the results are good because it meets the standard values.

Abstract

Hypertension is a clinical condition of when someone has high blood pressure above normal which can increasing the risk for morbidity and mortality. Antihypertensive drugs group which are ACE inhibitors, ARBs, CCBs, either alone or a combination was used for the therapy. Combination antihypertensive drug is needed if a single antihypertensive drug has not been able to control the desired of the patient's blood pressure. A non-experimental descriptive method was used for this study. The research was carried out without giving anything to the studied variable. A retrospective approach with past data were the sample for this study and 574 prescription sheets were obtained. Variables in this study were include patient characteristics data and drug characteristics data. The results of the research was most of the patients were female with a total of 314 prescriptions (55%). The type of drug therapy which often used were combination therapy with 504 prescriptions (88%). The type of drug group which most usually used on the prescrition was CCB with 482 prescriptions (49%) and the name of the drug that was often prescribed was amlodipine which the amount were 479 prescriptions (49%).

Abstract

Background: Formularies are an excellent tool to improve the quality and efficiency of hospital treatment costs and can demonstrate the level of effectiveness in achieving therapeutic, economic and/or goals, administration. This study aims to find out the description of the suitability of writing outpatient general prescriptions with the formulary of Umi Barokah Boyolali Hospital for the September-November 2020 period. Method:  This study used an observational research type. Data were collected retrospectively, namely by observing and evaluating prescription sheets taken from the general outpatient prescription sheet population for 3 months, namely September-November 2020. Sampling was carried out using systematic random sampling. The collection of data used is using secondary data. Secondary data is data obtained from all prescriptions that enter the Umi Barokah Boyolali Hospital Pharmacy Installation in the form of prescription archives, the number of prescriptions issued by each doctor and the drug list according to the formulary from September 2020 to November 2020. Result:  It shows that in the September-November 2020 period the percentage of conformity of prescriptions based on prescription sheets was 99.42%, based on polyclinics 98.87%, based on drug items 99.72%. Abstract

Leukemia is one type of cancer with a high mortality rate, caused by the proliferation of abnormal white blood cells that disrupt hematopoiesis function. Conventional therapies, such as chemotherapy and targeted therapy, often face challenges in the form of side effects and drug resistance. This study aims to evaluate the potential of Catharanthus roseus as a leukemia therapeutic agent through in silico. The docking process uses autodock, and ADMET prediction uses SwissADME and PKCMS. The study used the ABL1 protein (PDB ID: 4TWP) as a target with active compounds of Catharanthus roseus. The validation process of the docking method showed an RMSD value of 0.705 Å, indicating that the method used was valid. The results of the docking simulation showed that vindoline had the best affinity after native ligands with a binding energy of -8.64 kcal/mol, followed by catharanthine -6.16 kcal/mol and tryptophan -3.87 kcal/mol. ADMET prediction analysis showed that vindoline and catharanthine had promising pharmacokinetic and toxicity profiles, such as good blood-brain barrier (BBB) permeability and did not inhibit the CYP3A4 enzyme. These results indicate that vindoline and catharanthine are potential alternative leukemia treatments with high efficacy and low risk of side effects. This study provides a basis for further exploration of Catharanthus roseus in the development of effective and safe leukemia therapies.

Abstract

A child can be defined as a human being under the age of 18, including an unborn child. All aspects of a child's needs are heavily dependent on their parents, particularly their mother. This includes basic needs such as nutrition, affection during growth and development stages, and guidance on parenting, all of which are primarily provided by the mother. During the growth and development stages, children are vulnerable to various illnesses, including fever, cough, cold, diarrhea, itchiness, and rashes. As a result, mothers need to be able to provide self-medication or swamedikasi. This community service program aims to enhance the knowledge and understanding of mothers, specifically those of KB Aisyiah 1 Kudus, in providing self-medication for their children. The program consists of several stages, including obtaining permits, distributing pre-test questionnaires, presenting materials, and concluding with post-test questionnaires. The program's evaluation is based on the analysis of the pre-test and post-test questionnaires. The community service program lasted for approximately four days, with participants comprising mothers of KB Aisyiah 1 Kudus students. The prevalence of participants' ages ranged from 25-30 years, with most having more than one child and being housewives (seven participants). The program resulted in a significant increase in the average pre-test and post-test scores, with a 57.14% improvement. This indicates that the program has successfully enhanced the mothers' knowledge and understanding of self-medication.

Abstract

Tuberculosis (TB) remains a global health challenge with increasing drug resistance. This study aims to explore the potential of Andrographis paniculata as an alternative anti-TB therapy through an in silico approach. The study was conducted using the molecular docking method using Biovia Discovery Studio, AutoDock 1.5.7, and ChemDraw 3D software. The target proteins analyzed were 1TYP and 3R6C. which play a role in the biosynthesis of Mycobacterium tuberculosis cell walls. The docking results showed that dehydroandrographolide and neoandrographolide compounds have lower binding energies than ethambutol, with ΔG values of -9.54 kcal/mol and -9.04 kcal/mol at 3R6C, respectively. Stable hydrogen and non-hydrogen interactions indicate a stronger inhibitory potential against protein targets. Pharmacokinetic analysis through SwissADME and PKCMS confirmed that this compound meets Lipinski's Rule of 5 criteria, and has lower toxicity compared to conventional TB drugs. Thus, this study provides new insights into the development of natural compound-based TB therapy, which is potentially more effective and has minimal side effects. Further studies are needed to confirm the activity of this compound through in vitro and in vivo tests.

Abstract

Pharmacists are healthcare professionals with expertise in the field of pharmacy. Pharmacists play a role not only in drug delivery but also in clinical aspects related to therapy selection, side effect monitoring, and patient and healthcare provider education. Clinical pharmacy services are a crucial aspect in improving the quality of healthcare services in hospitals. This study aims to determine healthcare providers' perceptions of the role of pharmacists in clinical pharmacy services at Sarkies 'Aisyiyah Hospital, Kudus Regency. This study used a quantitative descriptive design with a purposive sampling technique on 90 healthcare provider respondents. The research instrument was tested for validity and reliability, with all items valid (r count > 0.361) and reliable with Cronbach's Alpha values of 0.780 and 0.789. The results showed that the majority of healthcare providers supported pharmacist involvement in medication selection (90%), side effect monitoring (93.3%), and drug interaction monitoring (91.1%). However, only 56.7% of respondents agreed with pharmacist involvement in bedside visits with patients. This indicates a limited understanding of the clinical role of pharmacists. Identified barriers included a lack of cross-professional coordination. Healthcare workers' positive perceptions of the pharmacist's role are crucial and strategic in supporting clinical pharmacy services, both for patients and the hospital environment.

Abstract

Background: According to the WHO, hypertension is a degenerative disease suffered by the majority of the world's population. Meanwhile, in Indonesia itself, the number of people suffering from hypertension increased by 25.8%. If left untreated, hypertension can cause various other diseases such as stroke, heart attack, and diabetes mellitus. Method: This study aims to determine the treatment patterns of hypertensive patients at Puskesmas Rejosari, Kudus Regency. This type of research is a non-experimental descriptive study using a retrospective approach with patient data from July to September 2024. Research Results: The results of the study show that the prevalence of anti-hypertensive medication use in the CCBs group with amlodipine was 50%, the ACE Inhibitor group with captopril was 20%, the use of Hydrochlorothiazide (HCT) was 10%, furosemide was 5%, and the use of combination therapy was 15%. Conclusion: The antihypertensive medication utilization pattern among hypertensive patients at the Puskesmas primarily involves the use of CCB (Amlodipine) as the first-line treatment. The effective and safe use of antihypertensive medications can contribute to blood pressure control and the prevention of hypertension-related complications.

Abstract

Irrational antibiotic use remains a serious challenge in efforts to control bacterial resistance globally. One form of this irrational use is the purchase of antibiotics without a prescription, which is still widespread in various regions, including rural areas such as Kebonagung Village, Demak Regency. Lack of supervision and community demographic characteristics also influence antibiotic acquisition methods. This study aims to determine the relationship between demographic characteristics and antibiotic use patterns on antibiotic acquisition methods in the community of Kebonagung Village. The research method used was an observational analytic study with a cross-sectional design. A total of 118 respondents were selected using a purposive sampling technique. Data were collected through questionnaires and analyzed using the Chi-square test with a significance level of 0.05. The analysis results showed a significant relationship between age (p = 0.011), education level (p = 0.015), and type of employment (p = 0.006) with antibiotic acquisition methods. Gender did not show a significant relationship (p = 0.138). The use pattern in this study was defined limitedly based on the type of antibiotic used according to the WHO AWaRe classification, as well as the reasons for antibiotic use based on perceived symptoms. The type of antibiotic based on the WHO AWaRe classification showed a significant relationship with the method of antibiotic acquisition (p = 0.001), with the finding that Access category antibiotics were more often obtained without a prescription, while the Reserve category was entirely obtained with a prescription. The analysis showed that there was no significant relationship between the reason for antibiotic use based on perceived symptoms and the method of antibiotic acquisition (p = 0.616). The results showed that age, education level, occupation, and type of antibiotic showed a significant relationship with the method of antibiotic acquisition, while gender and the reason for antibiotic use based on perceived symptoms did not show a significant relationship. These findings can be the basis for developing educational interventions to increase public awareness of how to obtain appropriate antibiotics, in order to prevent inappropriate access and support resistance prevention efforts at the community level.
