Abstract

The development of the hospital sector demands fast, efficient, and high-quality healthcare services, including pharmaceutical services. Waiting time for medication is an important indicator in assessing the quality of outpatient pharmacy services. This study aims to analyze waiting time for medication services, identify waste based on the Lean Hospital concept, and evaluate service flows to improve pharmacy service efficiency. This study used an observational method with a quantitative descriptive approach and was conducted in January 2026. The study sample consisted of 100 outpatient prescriptions determined using the Slovin formula and 23 pharmacists selected using a total sampling technique. Data collection was carried out through direct observation, questionnaires as primary data, and secondary data from the hospital information system (Hosmis). Data analysis was conducted using univariate and descriptive methods. The results showed that outpatient drug services were dominated by non-compounded prescriptions. Waiting times for both non-compounded and compounded prescriptions were still within the standard limits set by Regulation of the Minister of Health of the Republic of Indonesia Number 30 of 2022 concerning National Indicators of Health Service Quality. The implementation of Lean Hospital showed that most aspects, such as motion, inventory, and service flow, were running well. However, waste was still found in transportation, waiting, and overprocessing. The medication service flow was considered clear, structured, and supported service efficiency. The conclusion of this study is that the implementation of Lean Hospital in the outpatient pharmacy installation has been running quite well but not optimally. Improvements are needed, especially in reducing waste and improving coordination between staff to improve service efficiency and optimize the quality of pharmaceutical services.

Abstract

Hipertensi merupakan penyakit kronis yang ditandai dengan peningkatan tekanan darah pada dinding pembuluh darah arteri, di mana kepatuhan minum obat menjadi salah satu faktor penentu keberhasilan terapi yang memengaruhi kualitas hidup pasien. Penelitian ini bertujuan untuk mengetahui hubungan antara kepatuhan minum obat antihipertensi dengan kualitas hidup pasien di Puskesmas Gajah I Demak. Jenis penelitian yang digunakan adalah kuantitatif dengan pendekatan cross-sectional. Instrumen penelitian meliputi kuesioner data demografi, kuesioner Medication Adherence Report Scale (MARS-10), dan kuesioner World Health Organization Quality of Life (WHOQOL-BREF). Analisis data dilakukan secara univariat dan bivariat menggunakan uji Mann-Whitney karena data tidak homogen dan tidak terdistribusi normal. Hasil penelitian terhadap 150 responden menunjukkan bahwa karakteristik pasien didominasi oleh kelompok usia 56–60 tahun (32,7%), berjenis kelamin perempuan (64,7%), berpendidikan SMA/SMK (38,0%), berstatus menikah (99,3%), bekerja sebagai Ibu Rumah Tangga (38,0%), dan memiliki jam kerja 12 jam (36,0%). Mayoritas responden tidak memiliki riwayat penyakit keluarga (65,3%), tidak memiliki riwayat sosial buruk (100,0%), tidak memiliki penyakit penyerta lain (92,0%), dan tidak mengalami insiden kehilangan dalam 4 minggu terakhir (82,7%). Tingkat kepatuhan minum obat sebagian besar masuk dalam kategori patuh (60,7%) dan kualitas hidup responden terbanyak berada pada kategori sedang (55,3%). Hasil uji bivariat menunjukkan adanya hubungan yang signifikan antara kepatuhan minum obat antihipertensi dengan kualitas hidup pasien di Puskesmas Gajah I Demak ($p = 0,006$). Berdasarkan hasil tersebut, disimpulkan bahwa kepatuhan minum obat yang baik berkorelasi positif dengan kualitas hidup pasien. Disarankan bagi pasien hipertensi untuk meningkatkan kepatuhan minum obat secara konsisten guna memperbaiki dan mempertahankan kualitas hidup yang optimal.

Abstract

Pharmaceutical services in pharmacies aim to ensure the quality, safety, and effectiveness of medicines received and dispensed to patients. The processes of drug receiving and storage are essential components of drug management that must be carried out in accordance with the Technical Guidelines for Pharmaceutical Service Standards in Pharmacies. Non-compliance with these processes may affect drug quality and the overall quality of pharmaceutical services. This study aimed to evaluate the compliance of drug receiving and storage processes at Syafaka Pharmacy, Kudus, based on the Technical Guidelines for Pharmaceutical Service Standards in Pharmacies. This study employed a descriptive quantitative design with a cross-sectional approach. Data were collected through direct observation using an observation checklist developed based on pharmaceutical service standard indicators, supported by interviews and documentation. Data were analyzed descriptively by calculating the percentage of compliance and categorizing the results based on predetermined criteria. The results showed that the compliance level of the drug receiving process was 87.5%, categorized as highly compliant, while the compliance level of the drug storage process was 82.35%, also categorized as highly compliant. In conclusion, the drug receiving and storage processes at Syafaka Pharmacy, Kudus, were generally compliant with the Technical Guidelines for Pharmaceutical Service Standards in Pharmacies. However, improvements in administrative procedures and storage facilities are necessary to further enhance the quality of pharmaceutical services.
