Abstract

One of the domains of quality of life for people living with HIV (PLWH) is independence, which is how PLWH undergo antiretroviral therapy (ARV) adherently. PLWHA face various problems, including social problems such as stigma, ostracization, and discrimination. Social stigma can be a form of chronic stress for HIV/AIDS patients. One of the approaches used to improve intervention outcomes for PLWH is peer support. This study was conducted to explore in-depth the experiences of peer supporters in assisting PLHIV undergoing ARV therapy. This study was a qualitative research with a descriptive qualitative approach. Eight peer supporters who are members of peer support groups in Sorong City and Sorong Regency, and have been actively providing support to PLHIV for at least one year were purposively selected for this study. Data were obtained through in-depth interviews using a semi-structured interview guide. Data was collected over a period of one month, and analyzed thematically. Three main themes were generated, namely the benefits of peer support for ODHIV, barriers to the implementation of peer support, and efforts to overcome barriers. The conclusion of this study is that the support provided to ODHIV through peer supporters has a good impact on the achievement of care for ODHIV. The peer support program can be integrated with nursing programs and interventions for PLHIV. Although there are obstacles in its implementation, it can be resolved with the cooperation of various parties.

Abstract

Community-acquired pneumonia (CAP) is a leading cause of morbidity and mortality worldwide, particularly among the elderly and immunocompromised patients. Various factors, including age, gender, bacterial pattern, comorbidities, and Pneumonia Severity Index (PSI) score, influence CAP severity. Understanding these characteristics is crucial for optimizing treatment strategies and predicting clinical outcomes. This descriptive-analytical cross-sectional study was conducted at Dr. H. Abdul Moeloek Hospital, Lampung, from December 2022 to January 2023. A total of 102 CAP patients were analyzed based on their age, gender, bacterial pattern, comorbidities, and PSI score. Data were processed using IBM SPSS 21.0, with Pearson correlation applied to assess relationships between variables. A statistically significant correlation was found between age and PSI score (p = 0.018), with a low-strength positive correlation (R = 0.234). This suggests that as age increases, CAP severity also increases (p = 0.011). However, no significant correlations were found between PSI score and gender, bacterial pattern, or diabetes mellitus. This study confirms that age is a significant factor influencing CAP severity, emphasizing the need for early screening and intervention in elderly patients. Although gender, bacterial pattern, and diabetes mellitus were not significantly correlated with CAP severity.

Abstract

Improperly managed labor pain can disrupt uterine activity, leading to prolonged labor. Non-pharmacological methods, such as counterpressure and aromatherapy, are effective in reducing pain and anxiety during the first stage of labor. Aromatherapy, particularly with lavender, provides a calming effect on the body and mind, helping to alleviate stress and pain perception. This study aims to assess the combination of counterpressure and lavender aromatherapy in reducing labor pain during the active phase of stage I labor. A quasi-experimental design with a pretest-posttest control group approach was used. The study included 140 laboring women in the active phase of stage I labor from Bantul, Yogyakarta, and PMB Rejang Lebong, Bengkulu. Participants were selected through purposive sampling, and pain levels were measured using the Visual Analog Scale (VAS). The intervention group received both counterpressure and lavender aromatherapy, while the control group was given lavender aromatherapy and deep breathing techniques. Data were analyzed using a dependent t-test, revealing a significant effect of the combined intervention in reducing labor pain (p-value 0.005). Counterpressure and lavender aromatherapy are recommended as effective non-pharmacological strategies for managing labor pain during the active phase of stage I labor

Abstract

Sepsis-associated cholestasis itself has a mortality rate of 52.8% in neonates. This high mortality rate is accompanied by the absence of methods in predicting death due to sepsis-related cholestasis. This study aims to determine the role of transaminase and bilirubin levels in predicting the death of patients with sepsis-associated cholestasis in neonates. Analytic observational research study with a cross sectional approach at Dr. Moewardi Hospital. Samples were obtained from the medical records of patients with cholestasis related to neonatal sepsis from 2020-2022. Bilirubin and transaminase statistically analyzed using ROC curves and logistic regression multivariate analysis. Of the 85 samples, 48 (56.5%) male patients and 37 (43.5%) female patients were obtained. In the analysis with the ROC curve, it was obtained for total bilirubin cut-off 10.13 mg/dl, direct bilirubin obtained cut-off 6.7 mg/dl, and indirect bilirubin obtained cut-off 3.62. AST had a cut-off of 49.5 IU/L and ALT had a cut-off of 30.5. In multivariate analysis, the direct bilirubin level had a p value of 0.001 with an adjusted OR of 10.93 and the AST level had a p value of 0.000 with an adjusted OR of 19.73. Direct bilirubin and AST levels can predict mortality in patients with neonate sepsis-associated cholestasis.

Abstract

Chronic Obstructive Pulmonary Disease (COPD) and Lung Cancer are among the diseases with high mortality rates and a similar genetic disposition. COPD is known to be a significant risk factor for lung cancer. Matrix metalloproteinase-3 (MMP-3) is a protein that plays a role in the growth stage of cancer that has the potential to develop COPD into lung cancer. This descriptive observational study aimed to compare plasma MMP-3 levels among four groups: healthy individuals, lung cancer patients, COPD patients, and patients with both lung cancer and COPD. The findings revealed that plasma MMP-3 levels were lowest in the healthy group and higher in the other groups. The increase in plasma MMP-3 levels among pathological subjects suggests a potential association between serum MMP-3 levels and the development of lung cancer in COPD patients. However, further research is needed to clarify this relationship.

Abstract

This study explores the role of the trust economy in Islamic healthcare, focusing on how halal certification and service innovation contribute to building patient trust. Using a Systematic Literature Review (SLR) approach, this research analyzes 50 relevant studies published between 2015 and 2024. The findings reveal that halal certification plays a crucial role in assuring patients of compliance with Islamic principles, thereby enhancing their confidence in healthcare providers. Additionally, service innovations such as halal telemedicine and patient-centered care models significantly impact transparency, patient satisfaction, and loyalty. Studies indicate that hospitals implementing telemedicine solutions see a 40% improvement in service transparency, while those adopting patient-centered approaches report a 35% increase in patient loyalty. Despite these advancements, gaps remain in empirical research on the long-term economic impact of trust-based healthcare systems and the effectiveness of digital trust models. Future research should explore these aspects further and assess the role of policy frameworks in strengthening halal healthcare regulations. Islamic healthcare providers can enhance patient trust and ensure sustainable growth in the sector by integrating ethical, technological, and religious considerations.

Abstract

Introduction: Exacerbations of chronic obstructive pulmonary disease (COPD) contribute to 3 million annual deaths worldwide. Triple therapy (ICS + LABA + LAMA) helps reduce symptoms and exacerbation risk. Objective: To assess the effectiveness and clinical response of single versus multiple inhaler therapy in managing COPD.Methods: A pre-post study was conducted on two groups of COPD patients over 8 weeks. Group 1 (n=47) used a single inhaler, while Group 2 (n=45) used multiple inhalers. Outcomes were measured using the mMRC scale, blood eosinophil levels, and spirometry at weeks 2 and 8.Results: The single-inhaler group significantly reduced mMRC scores, with a median decrease from 4.00 to 2.00 (p0.05). Eosinophil levels also decreased significantly in both groups, with a larger reduction in the single-inhaler group (p0.05). Lung function improved in both groups, but no significant differences were found (p0.05) in terms of time or between-group comparisons. Conclusion: Single-inhaler therapy reduced dyspnea and eosinophil levels more effectively than multiple inhalers. The effectiveness of single inhalers may be attributed to higher adherence due to ease of use.

Abstract

ABSTRACT Chronic Kidney Disease (CKD) significantly affects the quality of life of patients, often due to limitations in their ability to care for themselves. Effective self-care is crucial in managing CKD, as it directly affects daily routines and overall well-being. The aim of this study is to assess the relationship between self-care and the quality of life of CKD patients undergoing hemodialysis treatment. A quantitative, cross-sectional approach was used, involving 91 participants selected through accidental sampling. Data were collected using the "Chronic Kidney Disease Self-Care Index" and the "WHOQOL-BREF" questionnaire to evaluate self-care and quality of life. Chi-Square analysis at p 0.05 revealed a significant correlation between self-care and quality of life (p = 0.001). The study results indicate that 1.1% of participants are fully dependent on self-care, 61.5% are partially dependent, and 37.4% are independent. Specifically, all independent participants reported a good quality of life, while no participants were fully dependent. These findings underscore the importance of promoting self-care practices among CKD patients to improve their quality of life. Healthcare professionals are encouraged to provide support, motivation, and targeted guidance to foster self-care behaviors in this population.

Abstract

Burns can cause disruption of skin integrity which makes patients susceptible to infection. It has been reported that morbidity and mortality in this population are almost 61% caused by infection. One of the best and optimal treatments in this problem is to use complementary therapy that accelerates the healing of burns, namely by administering petai cina leaves topically. The source of the article used was obtained from a search through PubMed, ScienceDirect, and Google Scholar from 2015 to 2018. After being obtained, an article assessment was carried out until the stage of making a literature review. Based on the literature review that has been carried out, it was found that the use of petai cina leaves topically can accelerate the healing time of burns. This is also supported by the results of statistical tests from several studies which show that petai cina leaves are effective in accelerating the healing of burns with p 0.05. Petai cina leaves have the potential as a complementary nursing intervention for healing burns that supports the performance of the main pharmacological therapy for burns, although clinical trials have not been carried out on humans.
