Abstract

Background: Bengkulu Province is one of the provinces in Indonesia that has the greatest incidence of early marriages. It is well knowledge that women in this province are more likely to be victims of crimes such as domestic violence, problems during pregnancy, and even death during childbirth. It is possible for moms who enter into early marriages to develop psychiatric illnesses, which can not only be a forerunner to a variety of health impacts in later life but also have the potential to have a detrimental impact on their children and families. Objective: This study seeks to examine the factors related to psychological issues in early marriage in Bengkulu Province. Method: For this investigation, a case-control study design is utilized, and the groups are separated according to geography. The group under consideration is a rural region with a greater rate of marriages occurring at a younger age. The control group, on the other hand, is urban. One hundred and thirty-seven mothers who had been married off at a young age made up the study's target sample. Each of the groups had 137 responses, ensuring that the ratio was one to one. The research instrument that was utilized in this study was a questionnaire, and the application Stata 14 was utilized for both univariate test analysis and logistic regression. Results: The husband's perspective (OR: 6.5; 95% CI: 2.1–13.2; P-value: less than 0.001) and societal stigma (OR: 6.4; 95% CI: 1.1–13.4; P-value: less than 0.001) were significant factors associated with elevated stress levels, correlating with the highest scores. The control group comprised quality of life (OR: 6.7; 95% CI: 2.1-11.9; P-value: 0.001) and family support (OR: 6.2; 95% CI: 2.1-11.4; P-value: less than 0.001). In the depressed symptoms component, the variables that exhibited a significant conclusive association with the highest score were the spouse's perception (OR 6.4; 95% CI: 2.1-11.7; P-value: less than 0.001) and quality of life (OR: 6.2; 95% CI: 1.6-13.1; P-value: less than 0.001). The control group exhibited poor health literacy (OR: 4.3; 95% CI: 1.3-7.7; P-value: 0.0033) and inadequate family support (OR: 4.1; 95% CI: 2.1-7.5; P-value: less than 0.001). Conclusion: As a conclusion, the findings of this study highlight the critical importance of implementing comprehensive strategies to address the issue of child marriage and the mental health repercussions that it can have. In order to effectively address the complex interaction of socioeconomic, cultural, and psychological variables that contribute to early marriage, these approaches need to prioritize prevention, intervention, and support services.

Abstract

In 2023, the global prevalence of tuberculosis (TB) reached 8.2 million cases. This represents the highest recorded figure associated with delayed diagnostics and a rising incidence of tuberculosis cases. Tuberculosis prevalence is elevated among individuals with compromised immune systems, including those with HIV and hypoalbuminemia, who exhibit increased vulnerability to infection. The challenge of sputum ejection impedes diagnosis, highlighting the need for a rapid and economical early detection method. The LF-LAM TB-Ag assay provides an alternative method for determining the presence of Lipoarabinomannan in urine, which is a component of the Mycobacterium tuberculosis cell wall. This study evaluated the efficacy of the LF-LAM TB-Ag assay compared to the Xpert MTB/RIF method for diagnosing tuberculosis. A comparative cross-sectional study was conducted at Abdoel Moeloek Hospital in Lampung Province, Indonesia, between January 2023 and June 2024. A total of 52 suspected pulmonary tuberculosis patients, who were HIV-negative and had hypoalbumin status, were evaluated using both the LF-LAM TB-Ag and Xpert MTB/RIF assays. The Wilcoxon and Chi-square tests have been utilised to assess the efficacy of LF-LAM TB-Ag in comparison to Xpert MTB/RIF. A p-value below 0.002 was considered statistically significant. Xpert MTB/RIF demonstrated improved diagnostic accuracy for tuberculosis in individuals with hypoalbuminemia. This study emphasises the necessity for early detection of pulmonary tuberculosis in patients with hypoalbuminemia. The combining of LF-LAM TB-Ag with Xpert MTB/RIF improves detection, especially among high-risk populations, thereby enabling prompt treatment and enhanced disease management.

Abstract

Background: Chemical larvicides that have been used for a long time carry some risks by causing environmental risk. Besides, temephos has been documented to have the growth of resistance in various regions throughout the world among mosquito populations, consequently decreasing its effectiveness in eliminating the larval stage.  Therefore, developing alternative plant-based larvicides was considered a worthy step to reduce the risk of resistance to chemical larvicides. Illicium verum (IV), a plant-based spice, contains secondary metabolites such as flavonoids, saponins, tannins, and alkaloids acting as larvicidal agents. This study was conducted to attain Illicium verum ethanolic extract as an alternate plant-based mosquito larvicide against the dengue vector Aedes aegypti. Methods: The research design was an experimental method with a post-test-only control group design. The larvicide test in this study was divided into five groups with four repetitions and analyzed by Kruskal-Wallis test, post-hoc Mann-Whitney test, and probit analysis conducted with 95% confidence interval. Results: No significant difference was shown between concentrations of 0.25%, 0.125%, and 1% temephos, which showed a value of p0.05 on the Mann-Whitney test. The probit test showed the LC50 value was 0.476%, and the LC90 value was 2.42% at the maximum observation time. At the highest concentration, the LT50 value was 0.037 hours, and the LT90 value was 0.269 hours. Conclusion: This study reveals that natural substance of Illicium verum ethanolic extract surpassed 1% temephos as a botanical larvicide against Aedes aegypti larvae. This should be an environment-friendly solution to synthetic larvicide resistance.

Abstract

This cross-sectional study aimed to evaluate the sensitivity and specificity of the Tuberculosis Sign and Symptom (TBSS) score in comparison to chest radiography and the Rapid Molecular Test (RMT) among brick kiln workers. The study employed a total sampling method, involving 92 participants. Data collection was conducted from February to March 2024 in a brick kiln located in Saribumi Village, Gading Rejo District, Pringsewu Regency, Lampung Province. The majority of participants were female (77.3%), while males accounted for 22.8%. The most common age range was 41–60 years, representing 48.9% of the total sample. Regarding job categories, most workers were employed in the molding section. Screening with the TBSS score and chest radiography showed a sensitivity of 0.25, a specificity of 0.93, a positive predictive value of 0.14, and a negative predictive value of 0.96. The sensitivity and specificity of the RMT compared to the TBSS score could not be assessed as all RMT results were negative for TB. This limitation may be attributed to suboptimal sputum sample quality. The study concludes that while the TBSS score demonstrates high specificity and a strong negative predictive value, the diagnostic utility of the RMT could not be evaluated due to uniformly negative results.

Abstract

Neonatal jaundice is a common condition that can lead to severe complications if not detected and treated early. This study focuses on the development of a non-invasive jaundice detection tool using a TCS3200 color sensor, which aims to provide early diagnosis by measuring skin discoloration associated with elevated bilirubin levels. The methodology for the study involved several stages: designing the device, calibrating the color sensor, and conducting field tests in clinical settings. The TCS3200 sensor was used to capture the skin color from various body areas of newborns, including the forehead, chest, abdomen, and limbs. The collected data were correlated with serum bilirubin levels, and the severity of jaundice was classified using the Kramer scale, a clinical tool that assesses the spread of yellow pigmentation across the body. The results showed that the jaundice detection tool achieved a high degree of accuracy, with its readings closely mirroring laboratory-based bilirubin measurements. This was particularly evident in cases of moderate to severe jaundice, where the tool provided precise early detection. The non-invasive nature of the tool, combined with its portability, makes it an ideal solution for use in healthcare facilities with limited access to laboratory equipment. Additionally, the tool can be operated by healthcare workers with minimal training, which enhances its potential for deployment in rural or underserved areas. The study concludes that the developed jaundice detection tool has significant potential to improve neonatal care by enabling early diagnosis and timely intervention, thereby reducing the risk of severe jaundice-related complications. Future research will focus on scaling up the testing of the device across a larger and more diverse population of newborns, as well as pursuing product certification to facilitate broader implementation in healthcare settings.

Abstract

The prevalence of type 2 Diabetes Mellitus (DM) continues to increase globally, including in Indonesia, causing a decrease in quality of life and the risk of serious complications. The Chronic Disease Management Program (Prolanis) is designed to improve disease management, but its utilization has not been optimal. Self-efficacy is a key factor in encouraging patient participation in health services. Objectives: This study aims to analyze the relationship between self-efficacy and the utilization of Prolanis health services in patients with type 2 diabetes mellitus. Specifically, the objectives of this study include identifying the level of self-efficacy of type 2 DM patients who are members of the Prolanis group and assessing the level of utilization of Prolanis health services by type 2 DM patients. Methods: The study was conducted using a cross-sectional approach at the Lamongan Health Center from March to May 2023. The study sample was 125 DM patients in the Prolanis group who were selected by accidental sampling. The research instrument used a general self-efficacy questionnaire and a questionnaire on utilizing the Prolanis health service system compiled by researchers from the results of a literature review and data analysis using the Spearman Rank test. The study results showed that the majority of respondents, 68%, had high self-efficacy, and 51.2% made good use of Prolanis health services. The results of the Spearman Rank test are p = 0.000 and r = 0.763, meaning that there is a relationship between self-efficacy and the utilization of Prolanis health services for type 2 DM patients. The correlation strength is strong and positive. The conclusion shows that high self-efficacy in type 2 DM patients tends to be better at utilizing Prolanis health services than low self-efficacy.
