Abstract

The digital transformation of healthcare through Electronic Medical Records (EMRs) has improved the efficiency and continuity of clinical information management but has also raised significant concerns regarding data security and privacy. The increasing frequency of cyberattacks and the limitations of conventional data protection mechanisms, particularly in developing countries such as Indonesia, highlight the need for more reliable security solutions. This study explores the potential of blockchain technology to enhance EMR security through a Systematic Literature Review (SLR) following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) framework. A total of 13 peer-reviewed articles published within the last five years were systematically reviewed. The findings indicate that blockchain can serve as an additional security layer by improving data integrity and auditability through immutable ledgers, cryptographic hashing, and distributed consensus mechanisms. Data confidentiality is strengthened through patient-centered decentralized access control and advanced cryptographic techniques, while data availability is supported by hybrid architectures using off-chain storage. However, blockchain implementation still faces challenges, including scalability, cryptographic key management, system integration, and regulatory constraints. Overall, blockchain demonstrates strong potential to enhance security and trust within the EMR ecosystem, although it is better positioned as a complementary technology rather than a standalone solution.

Abstract

Prevalence of acute respiratory infections (ARI) in Indonesia among toddlers in 2024 was 52.7%, with West Jakarta Administrative City ranking second highest in DKI Jakarta Province. ARI is an upper respiratory tract infection influenced by infectious agents and various individual and environmental risk factors. Differences in risk factors have the potential to cause differences in ARI incidence between regions, requiring spatial analysis to identify the geographical correlation of disease incidence. This study aims to analyze the spatial correlation of ARI incidence in toddlers with risk factors between sub districts in the Kebon Jeruk Health Center working area in 2025. The study was conducted from August - September 2025, with a population of 16,344 children with ISPA and a sample of 109 children. Analyzed using Moran's I Global Index and Local Indicators of Spatial Association (LISA) using the GeoDa program. The global analysis revealed no spatial autocorrelation between the occurrence of ISPA in children and all the risk factors studied. However, local spatial analysis identified significant spatial autocorrelation between ISPA cases and the following risk factors incomplete immunization (p-value = 0,015) in Kebon Jeruk subdistrict, smoking habits of family members (p-value = 0,015) in Kedoya Selatan and Sukabumi Selatan subdistrict, and mother education level (p-value = 0,006) in Kelapa dua subdistrict.

Abstract

ABSTRAK Keberhasilan klaim JKN-BPJS sangat bergantung pada akurasi dokumentasi klinis dan kodefikasi diagnosis serta prosedur. RSIJ Pondok Kopi masih menghadapi kendala dalam pemanfaatan data klaim untuk evaluasi mutu dan optimalisasi nilai klaim. Kegiatan ini bertujuan meningkatkan kemampuan PMIK dalam mengolah dan menganalisis data klaim INA-CBG’s untuk mendukung akurasi kodefikasi dan dokumentasi. Program dilaksanakan melalui sosialisasi, pelatihan teknis Excel PivotTable, dan pendampingan penyusunan dashboard analisis klaim. Evaluasi menggunakan pre-test dan post-test serta observasi produk analisis. Pelatihan meningkatkan skor pengetahuan tim sebesar 22 poin, terutama pada materi MCC/CC. Tim berhasil menghasilkan dua laporan dashboard internal terkait capture rate MCC/CC dan distribusi LOS (AMLOS/GMLOS). Peningkatan kompetensi PMIK dalam analisis data klaim mampu memperbaiki akurasi kodefikasi dan mendorong pemanfaatan data klaim sebagai alat manajemen mutu dan optimalisasi nilai klaim rumah sakit. Kata Kunci: Data Klaim, Dokumentasi Klinis, INA-CBG’s, Klaim BPJS, Kodefikasi.  ABSTRACT The success of JKN–BPJS claims strongly depends on the accuracy of clinical documentation and clinical coding of diagnoses and procedures. RSIJ Pondok Kopi still faces challenges in utilizing claim data to evaluate service quality and optimize reimbursement values. This community service activity aims to improve the capacity of Health Information Management professionals (PMIK) in processing and analyzing INA-CBG’s claim data to support accurate coding and clinical documentation. The program was carried out through socialization, technical training using Excel PivotTable, and mentoring in developing analytical dashboards. Evaluation was conducted using pre-test and post-test assessments, as well as observation of the analytical products. The training improved staff knowledge by 22 points, with the highest increase in MCC/CC competence. The team successfully produced two internal dashboard reports related to MCC/CC capture rate and LOS distribution (AMLOS/GMLOS). Improving PMIK competency in claim data analysis enhances the accuracy of clinical coding and encourages the use of claim data as a tool for quality management and reimbursement optimization in hospitals. Keywords: Claim Data, Clinical Coding, Clinical Documentation, INA-CBG’s, BPJS Claim.

Abstract

Background: The Mobile JKN application enables BPJS Kesehatan participants to access health services, including online outpatient registration. At Jampangkulon Hospital, 13,021 of 28,896 outpatient visits (44.92%) were registered through Mobile JKN between January and June 2025. Despite increasing use several system related issues persist underscoring the need to evaluate the system and information quality supporting the application. Objective: This study aimed to assess the association of system quality and information quality on the utilization of the Mobile JKN. Methods: The study adopted a cross-sectional analytical design and involved 388 Mobile JKN users selected through accidental sampling. The data were collected using a validated and reliable questionnaire with a four-point Likert scale and analysed through multiple linear regression. The Mobile JKN use was evaluated using five indicators: frequency, duration, intensity, nature, and pattern. Results: The result showed that system quality (b= 0.204; 95% CI: 0.11–0.29; p < 0.001) and information quality (b= 0.547; 95% CI: 0.47–0.63; p < 0.001) were positively and significantly associated with application use. Conclusion: The model explained 71.2% of the variance in application use (R² = 0.712). Suggestion: Hospital Manajemen and BPJS Kesehatan should continue improving the system to encourage wider adoption.

Abstract

Abstract: Electronic Medical Records (EMR) is a computerized recording of patient illnesses and problems. This is stated in PMK No. 24 of 2022 which states that Medical Records need to be organized electronically. However, in its implementation, various problems were found  such as format incompatibility, network system disruptions, unavailability of features that meet user needs, and displays that are not easy to understand. As well as the unavailability of SOPs that serve as guidelines for users, and lack of training in the Use of EMR. This study aims to see the effect of Technology and Training Aspects on the Use of EMR at Patria IKKT General Hospital. The type of research used is quantitative research and uses a cross-sectional design. The population in this study were Professional Care Providers (PPA) and Medical Supporters in outpatient care and the sample in this study was 53 respondents with a total sampling technique. Data collection used a questionnaire with a Likert scale. The data analysis technique used multiple linear analysis tests. The results of the study explain that there is a significant influence on the System Quality variable on the Use of RME with a p-value of 0.002 <0.05, the Information Quality variable has a significant effect on the Use of RME with a p-value of 0.000 <0.05 and the Training variable has a significant effect on the Use of RME with a p-value of 0.018 <0.05. While the Service Quality variable does not have a significant effect on the Use of RME with a p-value of 0.132> 0.05. Keyword: Electronic Medical Records, HOT-Fit, System Quality, Information Quality, Service Quality, Training, System Users Abstrak: Rekam Medis Elektronik (RME) adalah pencatatan penyakit dan permasalahan pasien yang terkomputerisasi. Hal tersebut tertuang dalam PMK No 24 Tahun 2022 yang menyatakan bahwa Rekam Medis perlu diselenggarakan secara elektronik. Namun dalam pelaksanaannya masih kurang optimal, terdapat berbagai permasalahan yang ditemukan seperti ketidaksesuaian format, gangguan sistem jaringan, ketidaksediaan fitur yang memadai kebutuhan pengguna, dan tampilan tidak mudah dipahami. Serta belum tersedianya SOP yang menjadi panduan bagi pengguna, dan kurangnya pelatihan Penggunaan RME. Penelitian ini bertujuan melihat pengaruh dari Aspek Teknologi dan Pelatihan Terhadap Penggunaan RME di RS Umum Patria IKKT. Jenis Penelitian yang digunakan ialah penelitian kuantitatif dan menggunakan desain cross sectional. Populasi dalam penelitian ini adalah Profesional Pemberi Asuhan (PPA) dan Penunjang Medis di rawat jalan dan sampel dalam penelitian ini sebanyak 53 responden dengan teknik total sampling. Pengumpulan data menggunakan kuesioner dengan skala likert. Teknik analisis data menggunakan uji analisis linier berganda. Hasil penelitian menjelaskan bahwa terdapat pengaruh signifikan pada variabel Kualitas Sistem terhadap Penggunaan RME dengan nilai p–value 0.002 < 0.05, Variabel Kualitas Informasi berpengaruh signifikan terhadap Penggunaan RME dengan nilai p–value 0.000 < 0.05 dan variabel Pelatihan berpengaruh signifikan terhadap Penggunaan RME dengan nilai p –value 0.018 < 0.05. Abstract

Abstract: Timeliness of coding is crucial and requires attention, as failure to do so will delay the coding process and even impact financing claims. Initial observations indicate that 38.88% of coding is inaccurate. This study aims to analyze the effect of clinical documentation quality on the timeliness of clinical coding in the Casemix Installation of Tangerang Regency General Hospital. The study was conducted from August to October 2025 using a quantitative approach with a cross-sectional case study. A sample of 96 medical records was obtained using a random sampling technique. Data collection was carried out by reviewing medical records. The analysis technique used binary logistic regression. The results of the study showed that there was a 52.08% inaccuracy in the timeliness of coding and only 12.5% ??met the 7 criteria of clinical documentation quality. The Nagelkerke R Square value of 0.346 was obtained, which means that the quality of clinical documentation (Independent Variable) contributed 34.6% in explaining the timeliness of clinical coding (Dependent Variable), while 65.4% was explained by other factors outside the research model such as hospital information systems, doctor workloads and other internal policies. The Wald test showed a significant influence between the quality of clinical documentation on the timeliness of clinical coding with a p-value of 0.000 (p = <0.05). It is necessary to update the SIMRS coding system and optimal socialization in filling in the completeness and timeliness of clinical documentation.   Keywords: Medical Record Data. Timeliness of Coding, Quality of Clinical Documentation Abstrak: Ketepatan waktu kodefikasi merupakan hal penting dan perlu mendapatkan perhatian, jika tidak akan menyebabkan keterlambatan proses pengkodean salah satunya diakibatkan oleh kualitas dokumentasi, bahkan berdampak pada klaim pembiayaan. Hasil observasi awal menunjukkan terdapat 38,88% ketidaktepatan waktu kodefikasi. Penelitian ini bertujuan untuk menganalisis pengaruh kualitas dokumentasi klinis terhadap ketepatan waktu kodefikasi klinis di Instalasi Casemix RSU Kabupaten Tangerang. Penelitian dilakukan pada bulan Agustus - Oktober 2025 menggunakan pendekatan kuantitatif dengan kasus potong lintang (Cross- Sectional). Didapatkan Sampel sebanyak 96 rekam medis dengan teknik random sampling. Pengumpulan data dilakukan dengan cara telaah rekam medis. Teknik analisis menggunakan regresi logistik biner. Hasil Penelitian menunjukkan terdapat ketidaktepatan waktu kodefikasi sebesar 52,08% dan hanya terdapat 12,5% yang memenuhi kriteria 7 kriteria kualitas dokumentasi klinis, Diperoleh nilai Nagelkerke R Square sebesar 0,346, dapat diartikan kualitas dokumentasi klinis (Variabel Independen) memiliki konstribusi sebesar 34,6% dalam menjelaskan ketepatan waktu kodefikasi klinis (Variabel Dependen), sedangkan 65,4% dijelaskan oleh faktor lain diluar model penelitian seperti sistem informasi rumah sakit, beban kerja dokter dan kebijakan internal lainnya. Uji wald menunjukkan adanya pengaruh signifikan antara kualitas dokumentasi klinis terhadap ketepatan waktu kodefikasi klinis dengan nilai p-value 0,000 (p = < 0,05). Perlu dilakukan update sistem SIMRS kodefikasi dan sosialisasi optimal dalam pengisian kelengkapan dan ketepatan waktu dokumentasi klinis.   Kata Kunci: Data Rekam Medis, Ketepatan Waktu Kodefikasi, Kualitas Dokumentasi Klinis

Abstract

Abstract: Electronic Medical Records (EMR) is a form of health information service that is carried out and recorded computerized. In 2024, at Soeharto Heerdjan Hospital, obstacles encountered in filling out the Integrated Patient Progress Notes (CPPT) were its completeness, where 25.6% of patient data documents were incomplete. The purpose of this study was to determine the relationship between nurses' knowledge of electronic medical records and the completeness of the Integrated Patient Progress Notes (CPPT) at Soeharto Heerdjan Hospital. This study was conducted at Soeharto Heerdjan Hospital from February 2024 to June 2025 using a quantitative method with a cross-sectional research design with a sample of 58 inpatient nurses at Soeharto Heerdjan Hospital. The instruments in this study were the CPPT completeness observation sheet and the nurses' knowledge questionnaire about EMR. The data collection technique in this study was non-probability sampling with an accidental sampling method with univariate and bivariate analysis using chi-square. The analysis of the completeness of the CPPT showed that 53.4% (31 respondents) completed the form completely, and 46.6% (27 respondents) incompletely. Regarding nurses' knowledge of EMR, 33 nurses (56.9%) had sufficient knowledge, and 25 nurses (43.1%) had good knowledge. Bivariate analysis using the Chi-Square test revealed a correlation between nurses' knowledge of EMR and the completeness of the CPPT (p-value 0.000). To improve nurses' knowledge of EMR use at Soeharto Heerdjan Hospital, routine technical training on the use of EMR and nursing documentation is recommended at least twice a year to achieve the completeness of the CPPT. Abstrak: Rekam Medis Elektronik merupakan salah satu bentuk layanan informasi kesehatan yang dilakukan dan tercatat secara komputerisasi. Tahun 2024 di RS Soeharto Heerdjan kendala yang terjadi dalam pengisian CPPT terdapat pada kelengkapannya dimana terdapat 25,6% dokumen data pasien yang belum terisi. Tujuan penelitian ini adalah mengetahui hubungan antara pengetahuan perawat tentang rekam medis elektronik dengan kelengkapan Catatan Perkembangan Pasien Terintegrasi (CPPT) di RS Soeharto Heerdjan. Penelitian ini dilakukan di RS Soeharto Heerdjan pada bulan Februari 2024-Juni 2025 dengan metode kuantitatif dengan desain penelitian cross sectional dengan jumlah sampel 58 perawat rawat inap RS Soeharto Heerdjan. Instrumen dalam penelitian ini adalah lembar observasi kelengkapan CPPT dan Kuesioner pengetahuan perawat tentang RME. Teknik pengumpulan data pada penelitian ini yaitu non-probability sampling dengan metode accidental sampling dengan analisi univariat dan bivariat menggunakan chi-square. Hasil analisis kelengkapan CPPT didapatkan sebanyak 53,4% (31 responden) terisi lengkap dan sebanyak 46,6% (27 responden) tidak terisi lengkap. Pada variabel pengetahuan perawat tentang RME, sebanyak 33 perawat (56,9%) memiliki pengetahuan cukup dan 25 perawat (43,1%) memiliki pengetahuan baik. Berdasarkan hasil uji bivariat menggunakan uji Chi-Square terdapat hubungan antara pengetahuan perawat tentang RME dengan kelengkapan pengisian CPPT (p- value 0.000). Dalam upaya untuk meningkatkan pengetahuan tentang penggunaan RME pada perawat di RS Soeharto Heerdjan sehingga kelengkapan CPPT dapat tercapai diharapkan untuk menyelenggarakan pelatihan teknis rutin minimal dua kali dalam setahun tentang penggunaan Rekam Medis Elektronik (RME) dan pendokumentasian keperawatan. Abstract

Abstract: Mental disorders are one of the major health problems that continue to increase in Indonesia and require serious attention. According to Riskesdas (2018), the prevalence of severe mental disorders increased from 1.7 per 1,000 households in 2013 to 7 per 1,000 households in 2018, equivalent to around 450,000 people with severe mental illness, and in 2019 there were still 5,218 cases of severe mental illness under restraint. At Soeharto Heerdjan Hospital, the number of inpatients with mental disorders has shown an increasing trend from 2019 to 2023, rising from 3,068 patients in 2019 to 4,036 patients in 2023, and reaching 4,781 patients in 2024. This study aims to analyze the profile of inpatients with mental disorders at Soeharto Heerdjan Hospital in 2024 based on demographic characteristics, environmental factors, and clinical profiles. This research employed a descriptive quantitative design with a sample of 400 patients drawn from a total population of 4,781 patients. Data were collected through medical record reviews and analyzed using univariate analysis. The results showed that the majority of patients were male (50.2%) and the largest age group was 20–29 years (25.8%). Most patients had no formal education (35.5%) and were covered by BPJS health insurance (89.3%). In terms of environmental factors, the most dominant cause was family conflict (27.8%), followed by socioeconomic factors (20.3%) and the loss of parents/family members (20.0%). The most common diagnosis was paranoid schizophrenia (F20.0) at 40.5%, while the most frequent medical intervention was behavioral therapy (94.23) at 55.3%. The length of stay was most commonly 14 days (12.5%). Keywords: Patient Profile, Mental Disorders, Inpatient Care   Abstrak: Gangguan jiwa merupakan salah satu masalah kesehatan yang terus meningkat di Indonesia dan memerlukan perhatian serius. Menurut Riskesdas (2018), prevalensi gangguan jiwa berat meningkat dari 1,7 per 1.000 rumah tangga pada 2013 menjadi 7 per 1.000 rumah tangga pada 2018, setara dengan sekitar 450 ribu ODGJ berat, dan pada 2019 masih tercatat 5.218 ODGJ berat dalam kondisi pasung. Pada rumah sakit Soeharto Heerdjan, jumlah pasien rawat inap dengan gangguan jiwa mengalami peningkatan dari tahun 2019 hingga 2023, yaitu dari 3068 pasien pada 2019 naik menjadi 4036 pasien pada 2023, hingga mencapai 4.781 pasien pada tahun 2024. Penelitian ini bertujuan untuk menganalisis profil pasien rawat inap dengan gangguan jiwa di Rumah Sakit Soeharto Heerdjan tahun 2024 berdasarkan karakteristik demografis, faktor lingkungan dan profil klinis. Metode penelitian yang digunakan adalah deskriptif kuantitatif dengan sampel sebanyak 400 pasien dari total populasi 4.781 pasien. Data diperoleh melalui telaah rekam medis dan dianalisis secara univariat. Hasil penelitian menunjukkan bahwa mayoritas pasien berjenis kelamin laki-laki (50,2%) dengan kelompok usia terbanyak 20–29 tahun (25,8%). Sebagian besar pasien tidak menempuh pendidikan formal (35,5%) dan menggunakan jaminan kesehatan BPJS (89,3%). Berdasarkan faktor lingkungan, penyebab dominan adalah konflik keluarga (27,8%), disusul faktor sosial ekonomi (20,3%) dan kehilangan orang tua/keluarga (20,0%). Diagnosis terbanyak adalah skizofrenia paranoid (F20.0) sebesar 40,5% dengan tindakan medis dominan berupa terapi perilaku (94.23) sebesar 55,3%. Lama rawat terbanyak adalah 14 hari (12,5%).
