Abstract

Background:The rising prevalence of Type 2 Diabetes Mellitus (T2D) has become a significant global health burden, with profound implications for healthcare systems and affected individuals. Strengthening primary healthcare services by integrating health and community-based interventions is critical for improving health outcomes. This rapid systematic review aimed to evaluate the effectiveness of community-based interventions in enhancing health outcomes for patients with T2D. Objectives:To evaluate the effectiveness of community-based interventions in managing T2D, focusing on their impact on health outcomes Methods:This systematic review adhered to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Data collection involved five major databases: Scopus, Cochrane Library, PubMed, Web of Science, and Clinical Key, covering publications from January 2013 to May 2024. Eligible studies included randomized controlled trials, mixed-methods research, clinical trials, prospective studies, experimental repeated-measures designs, and quasi-experimental designs. The risk of bias in the included studies was assessed using the RevMan 5.4.1 software. Article selection, risk of bias evaluation, data extraction, and synthesis were conducted independently by the authors, with discrepancies resolved through discussion and consensus. Results:Out of 4.463 identified articles, 13 met the eligibility criteria for inclusion in the review. The community-based intervention strategies identified in these studies involved three primary approaches: family-centered interventions, peer support, and community volunteer programs. Conclusion:This systematic review concludes that community-based interventions are effective in improving health outcomes for individuals with T2D. Integrating and strengthening health and community-based collaborations within the broader healthcare system is crucial for achieving optimal health outcomes. Nurses are encouraged to collaborate with community-based networks to bridge the gap between healthcare providers and communities, thereby enhancing public health outcomes. Abstract

This research explores the application of a modified deep learning model for electroencephalography (EEG) signal classification in the context of schizophrenia diagnosis. This study aims to utilize the temporal and spatial characteristics of EEG data to improve classification accuracy. Four popular convolutional neural network (CNN) architectures, namely LeNet-5, AlexNet, VGG16, and ResNet-18, are adapted to handle 1D EEG signals. In addition, a hybrid architecture of CNN-gated recurrent unit (GRU) and CNN-long short-term memory (LSTM) is proposed to capture spatial and temporal dynamics. The model was evaluated on a dataset consisting of EEG recordings from 14 patients with paranoid schizophrenia and 14 healthy controls. The results show high accuracy and F1 scores for all modified models, with CNN-LSTM and CNN-GRU achieving the highest performance with scores of 0.96 and 0.97, respectively. Receiver operating characteristic (ROC) curves demonstrate the model's ability to distinguish between healthy controls and schizophrenia patients. The proposed model offers a promising approach for automated schizophrenia diagnosis based on EEG signals, potentially assisting clinicians in early detection and intervention. Future work will focus on larger data sets and explore transfer learning techniques to improve the generalization ability of the model.

Abstract

Pendahuluan: Stroke merupakan salah satu penyebab utama kecacatan dan kematian di dunia, dengan prevalensi yang signifikan di Indonesia. Defisit perawatan diri pada klien stroke menjadi tantangan besar dalam proses pemulihan, sehingga diperlukan intervensi yang efektif untuk meningkatkan kemampuan perawatan diri. Kombinasi dukungan perawatan diri dan terapi akupunktur diharapkan dapat memberikan manfaat holistik bagi klien stroke. Tujuan: Penelitian ini bertujuan untuk menganalisis penerapan dukungan perawatan diri dan akupunktur pada klien stroke dengan defisit perawatan diri, serta mengevaluasi dampaknya terhadap kemandirian klien dan pengetahuan caregiver. Metode: Penelitian ini menggunakan pendekatan studi kasus deskriptif dengan melibatkan dua klien stroke dan dua caregiver. Intervensi yang diberikan meliputi dukungan perawatan diri dan terapi akupunktur selama tiga minggu dengan enam kali kunjungan. Data dikumpulkan melalui asuhan keperawatan dan analisis Traditional Chinese Medicine (TCM). Hasil: Hasil penelitian menunjukkan peningkatan signifikan dalam kemampuan perawatan diri klien, termasuk makan, mandi, berpakaian, dan menggunakan toilet. Terapi akupunktur juga membantu meningkatkan kekuatan otot dan keseimbangan energi tubuh. Caregiver mengalami peningkatan pengetahuan dan keterampilan dalam merawat klien stroke, serta menunjukkan perilaku yang lebih sesuai dengan anjuran. Kesimpulan: Kombinasi dukungan perawatan diri dan terapi akupunktur efektif dalam meningkatkan kemandirian klien stroke dan pengetahuan caregiver. Pendekatan holistik ini dapat menjadi alternatif yang bermanfaat dalam perawatan klien stroke dengan defisit perawatan diri. Penting untuk memberikan dukungan psikologis kepada klien stroke dan caregiver, serta melakukan penelitian lebih lanjut dengan sampel yang lebih besar dan durasi yang lebih panjang untuk mengkonfirmasi temuan ini.

Abstract

Background: Type 2 diabetes mellitus (T2DM) is a global health problem with an increasing prevalence. Most related studies focus on Diabetes Self-Management (DSM) in urban populations or general contexts, leaving a gap in understanding the specific factors affecting DSM among rural T2DM patients. Therefore, there is limited understanding of the factors that influence DSM in rural T2DM patients.Purpose: This study aimed to investigate DSM in rural T2DM patients and its associated factorsMethods: This study employed a cross-sectional design. The samples involved 146 patients with T2DM from the community health center in Ogan Komering Ulu Regency, Indonesia, selected using a simple random sampling technique. Diabetes Knowledge Questionnaire (DKQ), Diabetes Management Self-Efficacy Scale (DMSES), Hensarling Diabetes Family Support Scale (HDFSS), and Diabetes Self-Management Instrument (DSMI) were used for data collection. The t-test, ANOVA, Pearson correlation, and multiple linear regression were used to analyze the data.Results: The study found a significant relationship between gender (p=0.001), knowledge (p=0.001), self-efficacy (p=0.001), and family support (p=0.032) with DSM in rural T2DM patients. However, the multiple regression analysis revealed that self-efficacy (β=0.392, p=0.001), gender (β=0.283, p=0.001), and duration of DM (β=-0.189, p=0.012) significantly affected  DSM in patients with T2DM in rural areas, explaining  28 percent of the variance  (R2=0.280).Conclusion: The results indicated a significant relationship between gender, knowledge, self-efficacy, and family support with DSM in T2DM patients in rural areas, with self-efficacy being the most dominant factor affecting DSM. This study recommends that community nurses can play a pivotal role in designing and implementing interventions enhancing self-efficacy. Additionally, the involvement of PROLANIS, collaborating with key community figures, is crucial to increasing community participation and support for effective diabetes self-management.

Abstract

Introduction: Family health is a crucial aspect of community life, with mothers playing a key role in managing minor health complaints such as headaches, muscle pain, and digestive issues. Traditional methods like Family Medicinal Plants (TOGA) and acupressure offer effective, low-cost solutions. Objective: This community service program aimed to educate and train mothers in the Majelis Taklim on self-care management using TOGA and acupressure techniques to address minor health complaints. Method: The program employed a participatory approach, conducted in three stages: preparation (March–May 2024), implementation (June–October 2024), and evaluation (November 2024). Activities included TOGA education, acupressure training, safety discussions, holistic lifestyle counseling, and community forums. Quantitative (pre-test/post-test) and qualitative (observation, satisfaction surveys) evaluations were used to measure outcomes. Result: The program significantly improved participants' knowledge and skills, with 70% reporting high effectiveness of TOGA for minor ailments. Daily TOGA usage increased to 45%, and 65% of participants shifted from chemical drugs to natural remedies. Challenges included material availability (40%) and processing techniques (30%). Participant satisfaction reached 85%, and 60% disseminated knowledge to their communities. Conclusion: The program successfully enhanced mothers' capacity in self-health management using TOGA and acupressure. Continued support, such as mentoring groups and community TOGA gardens, is recommended for sustainability.

Abstract

Komplikasi Diabetes Mellitus dibagi menjadi 2 yaitu mikrovaskular dan makrovaskular, komplikasi makrovaskular meliputi penyakit kardiovaskular, stroke, dan penyakit pembuluh darah perifer. Penyakit pembuluh darah perifer dapat menyebabkan memar atau luka yang tak kunjung sembuh, gangren sehingga dapat menyebabkan tindakan diamputasi, mati rasa atau baal, dan nyeri. Tujuan dari penelitian literature review ini adalah untuk menganalisis intervensi Buerger Allen Exercise (BAE) dalam memperbaiki sirkulasi darah perifer pasien DM Tipe 2. Metode penelitian yang digunakan adalah Literature Review dengan desain Narrative Review. Pencarian artikel dilakukan berdasarkan data base online terdiri dari Google Scholar, PubMed, dan Proquest. Dari 5 artikel yang dibahas dalam Literature Review ini ditemukan bahwa pemberian intervensi berupa BAE pada ke 4 artikel diatas memiliki kesamaan dalam pemberian dosis pada kelompok perlakuan yakni diberikan intervensi sebanyak 2x/sehari pada pagi dan sore hari. Variasi pemberian dosis intervensi BAE sebanyak 2x/sehari pagi dan sore hari selama 15-20 menit dalam 5 hari berturut-turut, sedangkan 1 jurnal lainnya berbeda yakni dosis pada kelompok perlakuan sebanyak 5x/sehari selama 3-5 menit dalam 4 hari berturut-turut. Pemberian intervensi BAE dapat di evaluasi dengan pengukuran Ankle Brachial Index (ABI) dengan alat ukur doppler, sphmomanometer, dan stetoskop dan sphygmomanometer digital. Penerapan intervensi BAE paling efektif dilaksanakan sebanyak 2x/sehari (pagi 09.00, sore 15.00) selama 15 menit dalam 5 hari berturut-turut. Pengukuran ABI dapat dilakukan menggunakan alat sederhana sphmomanometer dan stetoskop atau sphygmomanometer digital.

Abstract

Gestational diabetes mellitus (GDM) is a global health concern requiring effective self-care, yet barriers such as limited social support and education persist. Peer teaching has emerged as a potential strategy to enhance self-care in GDM patients, though evidence of its effectiveness remains scarce. This quasi-experimental study aimed to evaluate the impact of peer teaching on diabetes self-care behaviors among GDM patients. The study involved 80 pregnant women with GDM, divided into an intervention group (receiving peer teaching) and a control group (receiving standard care). The intervention consisted of weekly 90-minute peer-led sessions over four weeks, focusing on practical self-care strategies. Self-care behaviors were assessed using the Diabetes Self-Management Instrument (DSMI) at baseline and post-intervention. Results revealed significant improvements in all DSMI subscales (self-integration, self-regulation, interaction with healthcare providers, self-monitoring, and adherence to therapy) in the intervention group (p < 0.001), with no comparable changes observed in the control group. The findings demonstrate that peer teaching effectively enhances self-care practices in GDM patients, particularly in glucose monitoring and treatment adherence. These results support integrating peer-led education into routine antenatal care for GDM, especially in resource-limited settings. Future research should explore long-term outcomes and scalability to strengthen implementation strategies

Abstract

Penderita asma biasanya mengalami sesak napas dan batuk. Serangan asma biasanya terjadi secara tiba-tiba ditandai dengan suara napas yang berbunyi wheezing dan mengi. Bunyi mengi biasanya terdengar ketika penderita menghembuskan napas nya. Teknik pernapasan Buteyko merupakan salah satu alternatif pencegahan kekambuhan asma. Teknik pernapasan Buteyko dapat membantu mengurangi kesulitan bernapas dengan cara hiperventilasi. Tujuan Penelitian : Menggambarkan latihan pernapasan buteyko dalam asuhan keperawatan klien asma bronkial dengan pola napas tidak efektif. Metode Penelitian : Penelitian ini dengan desain studi kasus. Studi kasus ini dilaksanakan pada bulan bulan februari sampai dengan mei 2021. Subjek 2 orang yang menderita asma bronchial berumur 55 – 65 yang berobat ke puskesmas tanjung agung dan telah di diagnosa oleh dokter mengalami asma bronchial yang menunjukan tanda dan gejala dispnea, penggunaan otot bantu pernapasan, fase ekspirasi memanjang, dan pola napas abnormal. Hasil Penelitian : pada klien I sebelum dilakukan teknik pernapasan buteyko didapatkan hasil dispnea sedang, penggunaan otot bantu meningkat, pemanjangan fase ekspirasi meningkat, frekuensi napas sedang, dan nilai control pause 18 detik. Pada klien II sebelum dilakukan teknik pernapasan buteyko didapatkan hasil dispnea sedang, penggunaan otot bantu napas sedang, pemanjangan fase ekspirasi sedang, ortopnea sedang, frekuensi napas cukup membaik dan nilai control pause 17 detik. Kesimpulan : Teknik pernapasan buteyko berhasil untuk memperbaiki pola napas pada 2 penderita asma yang mengalami gangguan pola napas. Saran : Perlu dilakukan kajian lebih mendalam tentang teknik pernapasan buteyko dengan metoda yang lebih kompleks pada subjek yang lebih banyak
