Abstract

Hemodialysis is a routine medical intervention for a patient with chronic kidney disease. The duration of hemodialysis treatment makes the patient's behavior change to be not adherence to the recommended diet, including restrictions on fluid intake. The objective of this research is to determine the Relationship duration of hemodialysis treatment with Adherence to fluid restriction of Chronic Kidney Disease Patients. This cross-sectional study involving 44 respondents, taken by consecutive sampling techniques. Research has been conducted in the hemodialysis room at the Reksodiwiryo hospital in Padang from May-June 2019. Data collection was carried out by looking at medical records and questionnaires regarding fluid intake compliance in patients with chronic kidney disease. The data obtained were analyzed univariate with frequency distribution and bivariate with the Kendall Tau test. The results showed that there were 26 (59.1%) non-adherence patients, and 18 (40.9%) patients were adherence to limiting fluid intake. Old patients were 17 people (38.6%) and new patients were 27 (61.4%). The Kendall Tau bivariate test showed that the length of hemodialysis has a significant correlation with adherence fluid restriction (p = 0.014), with a negative correlation (r -0.375), it's meaning that the longer the hemodialysis, the lower the fluid restriction compliance. It is expected that hemodialysis nurses regularly evaluate the compliance of hemodialysis patients and routinely carry out educational and motivational programs as well as fluid diet counseling at each hemodialysis session. Keywords: compliance, fluid, hemodialysis, restriction

Abstract

Relapse and rehospitalization are common occurrences in patients with coronary heart disease (CHD), and these can be mitigated through effective self-care practices. Positive illness perceptions play a crucial role in promoting good self-care. This study aimed to investigate the connection between illness perception and self-care among patients with coronary heart disease at the Cardiac Polyclinic of RSUP Dr. M. Djamil Padang. Conducted from June to July 2023, this quantitative research utilized a cross-sectional approach and involved 242 patients from the cardiac polyclinic at Dr. M. Djamil Padang, using purposive sampling. The instruments used were the Self-Care Coronary Heart Disease Inventory (SC-CHDI) and the Brief Illness Perception Questionnaire (B-IPQ). The mean values for illness perception and self-care were 52.27 (SD=9.76) and 62.31 (SD=11.35), respectively. Data analysis used the Spearman Rank correlation test, yielding a p-value of 0.014 (r=0.157). The findings revealed a positive correlation, indicating a relationship between illness perception and self-care in patients with coronary heart disease. This implies that a more optimistic perception of the disease is associated with improved self-care among CHD patients. It is advisable for individuals with coronary heart disease to enhance their perception by engaging in informational support activities, thereby fostering optimism, and ultimately improving self-care behavior.

Abstract

Decreased consciousness is a clinical condition that requires rapid and accurate assessment. One method used is the application of nail bed pressure pain stimulation to elicit a response from the patient. This study aims to determine the consistency of responses from patients with decreased consciousness when given pain stimulation based on the Glasgow Coma Scale (GCS) score in the emergency department of Dr. M. Djamil Hospital in Padang. This was a quantitative descriptive study with 70 respondents selected using purposive sampling. Consciousness levels were measured twice. Data were collected in July 2025 using the GCS assessment format, then analyzed descriptively and cross-tabulated using frequency distribution, percentages, means, standard deviations, and correlations. The nail bed pressure technique produced an average GCS score of 6.01 ± 0.625 on the first assessment and 6.04 ± 0.711 on the second assessment. Most scores were in the range of 6–7. The nail bed pressure technique provided an inconsistent picture of changes in GCS responses in patients with decreased consciousness.

Abstract

Intradialytic hypotension (IDH) is one of the most common complications of hemodialysis therapy experienced by hemodialysis patients, with a prevalence of 8–40%. IDH carries important clinical consequences, including reduced dialysis adequacy and interruption of treatment sessions. Several factors are known increase the occurrence of IDH. This study aimed to identify risk factors influencing the occurrence of intradialytic hypotension in patients with chronic kidney disease undergoing hemodialysis. This study used a quantitative method with a cross-sectional design. The study was conducted in the hemodialysis unit of a government hospital in Padang City. The sampling technique was purposive sampling with a sample size of 104 respondents with inclusion and exclusion criteria, between April and May 2025. Data analysis used the Chi-Square test and the Mann-Whitney U test. The results showed that gender, dialysate temperature, and meals during hemodialysis were not associated with intradialytic hypotension (p > 0.05). However, hemoglobin and Interdialytic Weight Gain (IDWG) were related to the occurrence of intradialytic hypotension (p < 0.05). Based on the results of this study, nurses can monitor and evaluate anemia status and IDWG, which can influence the incidence of intradialytic hypotension. The role of nurses in improving dietary compliance and fluid restriction is crucial in preventing intradialytic hypotension.

Abstract

Cardiac catheterization is performed to reduce mortality risk from vascular stenosis. Following the procedure, femoral sheath removal often resulted in mild to severe pain. This study aimed to describe nursing care through the application of an ice gel pack as a nonpharmacological method to reduce pain during femoral sheath removal in post–cardiac catheterization patients at RSUP Dr. M. Djamil Padang. A case study design was applied, which included assessment, nursing diagnoses, intervention, implementation, and evaluation. The ice gel pack was applied over the sheath site for 20 minutes before sheath removal. Evaluation showed that pain intensity decreased from a scale of 5 to 3 after the intervention. The application of an ice gel pack effectively reduced pain and is recommended for inclusion in hospital standard procedures.
